Abstract

The wedge to position a patient's body more conveniently on the operating table for certain types of operations, e.g. abdominal, thoracic or gynaecological, creating a hyperextension in certain parts of the body, consists of an airtight chamber containing a pre-shaped foam block. The chamber is connected through a tube with a valve to a compressed air supply which allows it to be progressively and partially inflated and deflated by remote control during the operation, according to the surgeon's requirements. After use, the chamber returns to its initial shape once the air is released from it.

Description

The invention relates to a device for wedging or hyperextending a body in a specific location for a patient placed on a table called the operating table.

By the term "Billot" we will designate a more or less rigid body which serves as a wedge or hyperextensor and by the term "cylinder" we will designate a perfectly cylindrical cylinder or not with a possible "flat" along its length or part of this one or any other anatomical form undergoing surgery requiring this type of support (hand surgery on external approach for example)
Certain abdominal, thoracic, gynecological or vascular interventions are difficult at first and in order to better expose the site where the surgeon must intervene, it is easier for his assistants to have the patient in hyperextension in a very specific place depending on the case. hyperextension is generally carried out by means of a block made of rolled fields or a block of more or less malleable material, rubber for example, sufficiently soft to not traumatize the naked body of the patient but rigid enough to obtain an effect bridge arch style of the posterior or lateral face of the abdomen for example. In principle, it is put in place before the intervention, when the patient is positioned on the operating table and is just before or at the start of his anesthesia.

Once this is done, the surgical team can then, after the usual skin preparation, place on the patient the sterile drapes provided for this purpose and which will isolate the practitioner and his assistants also dressed in sterile clothing, furniture and other accessories, disinfected certainly but not providing the necessary guarantees to avoid infection.

The patient is therefore very largely covered and the fields which hang on each side no longer leave the dressers and other assistants the possibility of accessing any part of the table top or of the patient without making the latter take a risk. very damaging infectious.

During the incision by the practitioner the circular extension, abdominal for example, which results from the effect of the block facilitates the access of the tissues and other viscera following and especially allows him to better approach the deep planes where it is supposed to intervene since these are better exposed.

The intervention ended, it is at this time that begins another difficulty, that of the closing of the different abdominal plans, because the patient is always in hyperextension, and we saw it previously it is not possible without risk major for the patient to go and remove this block wedged under the patient.

The object of the invention is to remedy this drawback.

This new type of chopping block is made up of a cylinder, with or without a so-called secant part of the circle for ease of laying flat on a plane such as an operating table for example and which may prove necessary in certain positions or types of intervention, but which for practical reasons we will continue to call cylinder here.

This cylinder is made up of an envelope of flexible material such as airtight canvas and responsible for maintaining a constant or adjustable volume of air during the intervention.

This cylinder characterized by the fact that it is filled with a preformed cellular foam (round or preformed cylinder as the case may be) foam such as plastic foam for example, the use of which is multiple, namely
- Occupy the empty space represented by the cylinder
-To ensure by its compression a mobility of the forms controlled until allowing a flattening of the cylinder, flattening induced by the natural compression of the patient or by a separate gesture complementary or not of the hospital staff.

- Ensure by its natural foam function, after compression, an automatic cylindrical reshaping by the simultaneous filling of all the cells that make up the foam, filling which could also be achieved by means of a compressor or the installation of compressed air from the operating room for example. if you want to speed up the process or get an initial pressure higher than atmospheric pressure.

The cylinder is further characterized in that it is provided with a flexible tube provided with a shutter or tap system. This sufficiently long tube would then allow the staff present and "not sterile" to have access to the obturator despite the presence of fields.

What in the course of the phases of use of this log
MOUSS-AIR, would be done as described below
The tube being uncorked, the block inflates alone to its nominal volume by the foam board which resumes its shape inside the cylinder, this process can be accelerated or overinflated if necessary.

The tube is then obstructed and the block placed under the patient at the desired location so as to obtain the desired hyperextension effect.

At the appropriate time, the partial uncorking of the tubing which will then be closed would eventually make it possible to obtain all the intermediate positions desired depending on the patient's own anatomy.
The complete permanent unblocking of the tubing then makes it possible to obtain an inert cylinder therefore a hyperextension canceled during the intervention which will then facilitate the suturing work of the surgeon of the different planes for example which will appear to him in a natural and / or anatomical this time.

Finally, it will be noted that, and in particular in the so-called lateral decubitus position, in addition to its role of self-inflation of the cylinder, the foam plays a role of wedge and or it will remain filled with air in the case of the blocked tubing and will remain inflated and in front and behind the patient, thus making it possible to significantly reduce the rolling effect that is encountered with conventional logs.
