Abstract

The fixing consists of a nail (A) which passes through the bone fragments of the neck and greater trochanter of the femur and is attached to a plate fastened to the femur's outer surface. The nail has a transverse hole (1) for a fixing screw (11) and a threaded hole (9) in its end connected to the plate to receive a retaining screw (7) for a right-angled guide bracket (D) used to locate the entry point for the transverse fixing screw.The upper portion of the bracket has a lengthwise slot (10) for a hollow guide dowel (11), the inner diameter of which is sufficient to receive the fixing screw.

PERTROCHANTERIENNES DE L'EXTREMITE SUPERIEURE DU FEMUR
Le dispositif présenté vise à améliorer la fixation des fractures instables à foyer comminutif de la région trochantérienne du fémur par clou-plaque (I) (figure 1). Ces types de fractures sont devenus aujourd'hui plus fréquents du fait du nombre croissant des personnes très âgées avec os fragile. Le clou-plaque est un dispositif rigide dont l'extrémité proximale du clou (A) risque de perforer la tête fémorale, puis de traverser l'articulation de la hanche. Cette complication survient parce que la partie plaque (B) du clou-plaque (I) est fixée sur la diaphyse fémorale par des vis. II n'y a donc aucune possibilité pour que le clou soit chassé vers l'extérieur du fémur lorsque le foyer de fracture s'impacte. Pour pallier à cette complication, la présente invention consiste à verrouiller le clou selon le mode de l'enchevillement par une vis (II) qui traverse perpendiculairement le col du fémur et le clou (A).
