Abstract
Prothèse de hanche à fixation par expansion et vis ou clous permettant le renforcement d'un cotyle humain avant la mise en place du fémur artificiel, caractérisée en ce que la prothèse est constituée, d'une part, d'un élément en forme de calotte (1) sensiblement hémisphérique présentant à sa partie supérieure une ouverture centrale (2) et, sur sa surface bombée, des trous (3) pour la pose des vis ou clous de fixation tandis que la base de la calotte (1) présente une gorge circulaire (4) bordée à sa partie inférieure par un rebord (5) puis les trous (6), disposés de manière symétrique, par exemple à 90 degré les uns des autres, sont prolongés dans des fentes (7) débouchant en arrondi sur la gorge circulaire (4) afin de permettre à la calotte (1) d'être souple et de se déformer légèrement par resserrement des fentes (7).
Description
La présente invention a pour objet une prothese de hanche à fixation par expansion et vis ou clous qui permet soit le renforcement d'un cotyle humain avant la aise en place du fémur artificiel, soit le remplacement d'un cotyle artificiel use.
On connalt neja des cotyles artificiels que l'on met en place dans la partie du bassin d'un patient afin de renforcer la cavite cotyloldienne mais ces protheses sont très difficilement remplaçables et également d'une pose delicate, entralnant de ce fait des risques d'usure d'où des difficultes pour le patient de vivre et de marcher normalement.
Dans le cas présent, l'élément de prothese permet une mise en place correcte par une fixation simple et peut etre remplacé en cas de nécessité.
Cet élément remplaçant le cotyle humain est réalisé en matière compatible et de par sa conception est très facile å mettre en place en raccourcissant ainsi la duree de l'intervention délicate et parfois difficile du fait de l'age du patient.
Conformement à l'invention, la prothèse de hanche a fixation par expansion et vis ou clous permettant le renforcement d'un cotyle humain avant la nise en place du fémur artificiel est caracterisee en ce que la prothese est constituee d'un élément en forme de calotte sensiblement hémisphérique presentant, à sa partie superieure, une ouverture centrale et, sur sa surface bombée, des trous pour la pose des vis ou clous de fixation tandis que la base de la calotte présente une gorge circulaire bordée a sa partie inférieure par un rebord puis les trous, disposés de maniere symétrique, par exemple a 90' les uns des autres, sont prolongés dans des fentes débouchant en arrondi sur la gorge circulaire afin de permettre à la calotte d'etre souple et de se deformer légèrement par resserrement des fentes.
Suivant une autre caracteristique de l'invention, la prothese de hanche est caracterisee en ce que, au moment de la pose, la partie inférieure de la calotte est légèrement resserre soit par un fil en forme de toron place dans la gorge, soit à l'aide d'une pince comprenant quatre branches formant levier.
Diverses autres caractéristiques de l'invention ressortent d'ailleurs de la description détaillée qui suit.
Des formes de réalisation de l'objet de l'lnven- tion sont représentées, à titre d'exemples non limitatifs, aux dessins annexes.
La fig. 1 est une élévation de face d'une pre hère forme de réalisation de l'élément formant la prothèse et destiné a contenir un noyau évidé intérieu- rement qui reçoit la tete hémisphérique de la prothese terminale du femur.
La fig. 2 est un plan vu de dessus correspondant à la fig. 1.
La fig. 3 est une élévation latérale d'une deuxième forme de réalisation de l'élémentformant prothèse pour un cotyle humain.
La fig. 4 est une élévation montrant la mise en place de la prothèse de cotyle humain à l'aide d'une pince.
La fig. 5 est une vue en plan d'un clou utilisé pour la fixation de la prothèse de cotyle.
La fig. 6 est une vue de face du clou de la fig. 5.
La fig. 7 montre la prothese de cotyle selon la fig. 3 en place dans une hanche humaine.
A la fig. 1, on a représenté une première forme de réalisation d'une prothese de hanche à fixation par expansion et vis ou clous dans laquelle l'élément principal de la prothèse est constitué par une calotte 1 sensiblement hémisphérique, présentant, à sa partie supérieure, une ouverture centrale 2 et, sur sa surface bombée, des trous 3 pour la pose de vis ou clous, ce qui sera expliqué plus loin, tandis que la base de la calotte 1 présente une gorge circulaire 4 bordee à sa partie inférieure par un rebord 5.
I1 est prévu également des trous 6, dans le cas présent disposes à 90 les uns des autres, et qui sont prolongés par des fentes 7 debouchant en arrondi sur la gorge circulaire 4.
Ainsi, l'élément formant prothèse en forme de calotte 1 est souple et peut se déformer légèrement par resserrement des fentes 7, ce qui permet une pose aisée de cette prothèse dans le cotyle humain (voir fig. 7) préalablement mis en forme.
Apres relachement de l'élément formant prothèse 1, c'est-à-dire après avoir retiré la pince de serrage 8 (voir fig. 4) dont les branches 9, au nombre de quatre, enserrent la base de l'élément de la prothese 1, celui-c: s ' étend légèrement pour prendre la position définitive dans le cotyle humain.
Comice cet élément de prothese 1 est évide interieurement, il est alors aisé au praticien de mettre en place à travers les trous 3, des vis 10 du type représente a la fig. 5 qui sont des elements expansifs connus en eux-memes. La pointe 10a est légèrement élargie à l'aide de la pièce filetee 11 se vissant dans la tete 10b de la vis 10, ce qui permet le retrait de la vis 10 si on désire changer la prothèse 1.
11a désigne les saignées permettant l'utilisa- tion d'un tournevis.
Dans certains cas, cette vis est simplement un clou largement utilisé dans l'ostéosynthèse humaine. Le nombre de ces fixations, qui peuvent etre démontées, est variable suivant l'état du cotyle humain. Dans certains cas, à la place de la pince 8 on peut avoir un lien enserrant la base de l'élément 1 formant prothese, lien, en général en forme de toron, qui est libere après la mise en place de la prothese dans le cotyle humain par exemple un fil synthétique qui est coupé et qui est normalement loge dans la gorge circulaire 4.
La face extérieure de l'élément formant prothèse 1 peut entre revetue d'une fine couche de métal poreux avec une H.A.P. (Hydroxylapatite). Cet élément I est réalisé en alliage métallique conforme aux législations et aux normes en vigueur dans le domaine chirurgical.
Lorsque l'élément 1 est en place, le praticien inclut, dans la cavité intérieure de l'élément 1, un noyau d'un diametre légèrement supérieur au diamètre de la cavité interne de l'élément de prothèse 1 en assurant ainsi la prothèse de cotyle 1, une expansion des panneaux délimités par les fentes 7 et les trous 6 afin d'obtenir une fixation complémentaire de l'ensemble de la prothese dans le cotyle humain. Le praticien peut alors mettre convenablement en place la prothèse de fémur.
A la fig. 3, l'élément extérieur représenté est identique à celui représenté à la fig. 1 et la calotte 20 comporte egalement les fentes 7, les trous 6, la gorge 4, le rebord 5 et les trous 3 dont la mise en place des clous et des vis, mais en plus, le rebord peripherique inferieur 5 est muni d'un prolongement 21 en demi-cercle relie a une patte d'appui 22 qui vient se fixer d'une manière connue en soi sur le bord de la hanche du patient au-dessous du cotyle (voir fig. 7) en obtenant ainsi une sécurité supplémentaire de la fixa tion de la calotte 20 surtout dans le cas où l'os de hanche du patient et, en particulier, la zone du cotyle, est faible, risquant d'engendrer un mauvais maintien de l'ensemble de la prothese.
Comme dans le cas précédent, la calotte 20 est en alliage compatible revetu ou non de B.A.P (Hydroxylapatite).
Claims (5)
1. Prothèse de hanche à fixation par expansion et vie ou clous permettant le renforcement d'un cotyle humain avant la mise en place du fémur artificiel, caractérisée en ce que la prothèse est constituée, d'une part, d'un élément en forme de calotte (1) sensiblement hémisphérique présentant à sa partie supérieure une ouverture centrale (2) et, sur sa surface bombée, des trous (3) pour la pose des vis ou clous de fixation tandis que la base de la calotte (1) présente une gorge circulaire (4) bordee à sa partie inférieure par un rebord (5) puis les trous (6), disposés de manière symétrique, par exemple à 90' les uns des autres, sont prolongés dans des fentes (7) débouchant en arrondi sur la gorge circulaire (4) afin de permettre à la calotte (1) d'etre souple et de se déformer légèrement par resserrement des fentes (7).
2. Prothese de hanche suivant la revendication 1, caractérisée en ce que, au voient de la pose, la partie inférieure de la calotte (1) est légèrement reserrée soit par un fil en forme de toron place dans la gorge (4), soit à l'aide d'une pince comprenant quatre branches formant levier.

Prothese de hanche suivant l'une des revendications 1 ou 2, caractérisée en ce que sur le rebord (5) est prévu un prolongement (21) en demi-cercle supportant une patte d'appui (22) permettant la fixation définitive de la calotte (20) sur la hanche du patient au moyen de vis ou clous utilisés dans l'ostéosynthèse humaine.
4. Prothèse de hanche suivant l'une des revendications précédentes, caractérisée en ce que les éléments (1 ou 20) en forme de calotte qui sont réalisés en général en alliage métallique conforme aux législations et aux normes en vigueur dans le domaine chirurgical, reçoivent intérieurement chacune un noyau d'un diamètre légèrement superieur au diamètre de la cavité interne de l'élément (1 ou 20), noyau servant à la mise en place de la prothèse de fémur.
5. Prothèse de hanche suivant l'une des revendications precedentes, caractérisée en ce que l'élément de base (1 ou 20) est revetu d'une fine couche de metal poreux avec ou sans B.A.P. (Hydroxylapatite).
