Abstract

Prothèse totale métatarso-phalangienne de resurfaçage, comprenant un composant métatarsien (100) et un composant phalangien (200). Selon l'invention, ledit composant métatarsien (100) comporte un bouclier (110) de frottement ayant sensiblement la forme d'une portion de sphère ouverte latéralement, et destiné à être fixé à l'extrémité distale du métatarse (10) sans résection des os sésamoïdes, et en ce que ledit composant phalangien (200) comprend une vis (210) destinée à être introduite axialement dans la phalange (20), et dont la tête (211) est munie d'une cupule (220) formant surface articulaire pour le bouclier (110) de frottement du composant métatarsien (100). Description

PROTHESE TOTALE METATARSO-PHALANGIENNE DE
RESURFACAGE SANS CIMENT
La présente invention concerne une prothèse totale métatarso-phalangienne de resurfaçage.
L'invention trouve une application particulièrement avantageuse dans le domaine des prothèses de resurfaçage des os concernant l'articulation métatarso-phalangienne.
Dans le domaine de l'orthopédie, il existe des prothèses totales métatarso-phalangiennes, dites de resurfaçage, au sens où elles respectent l'anatomie et la cinématique de l'articulation. Ces prothèses de type connu sont constituées de deux parties, à savoir un composant métatarsien et un composant phalangien. Le composant métatarsien comporte généralement un bouclier de frottement qui, pour être fixé de façon stable à l'extrémité distale du métatarse, doit avoir la forme la plus enveloppante possible, ce qui nécessite des résections relativement importantes, en particulier des os sésamoïdes. Or, la destruction de ces os, qui participent à la sensation tactile de la marche, a pour effet d'induire chez les patients des causes d'instabilité et d mauvaise appréhension du terrain. De plus, il faut souligner qu'avec ce cygne de prothèses, i'appui lors de la marche se fait directement sur le composant métatarsien. d'où un facteur d'inconfort et à'instabilité supplémentaire.
Enfin, la pose des prothèses actuelleent connues provoque généralement des tensions articulaires favorisant la douleur, ceci du fait de l'allongement de l'articulation consécutif au volume occupé par les composants métatarsien et phalangien entre la métatarse et la phalange.
Aussi, le problème technique à résoudre par l'objet de la présente invention est de proposer une prothèse totale métatarso-phalangienne de resurfaçage, comprenant un composant métatarsien et un composant phalangien, prothèse qui permettrait, d'une part, de préserver les os sésamoïdes, et, d'autre part, d'ajuster les tensions ligamentaires de l'articulation.
La solution au problème technique posé consiste, selon la présente invention, en ce que ledit composant métatarsien comporte un bouclier de frottement ayant sensiblement la forme d'une portion de sphère ouverte latéralement, et destiné à être fixé à l'extrémité distale du métatarse sans résection des os sésamoides, et en ce que ledit composant phalangien comprend une vis destinée à être introduite axialement dans la phalange, et dont la tête est munie d'une cupule formant surface articulaire pour le bouclier de frottement du composant métatarsien.
Ainsi, par une forme partiellement enveloppante du bouclier métatarsien, la prothèse, objet de l'invention permet d'éviter la résection des os sésamoïdes avec tous les effets bénéfiques qu'il peut en résulter pour le confort de marche des patients.
D'autre part, la vis du composant phalangien, associée à une ostéotomie de raccourcissement de la phalange, rend possible le réglage des tensions articulaires afin d'éliminer les douleurs dues à une surtension des ligaments et d'obtenir ainsi une articulation équilibrée.
Avantageusement, le composant métatarsien est posé sans ciment, ce qui évite les risques de réaction au ciment, de nécrose de l'os, et de descellement lié au vieillissement du ciment.
On notera également que le vissage assure au composant phalangien une bonne stabilité en compression, favorisant la repousse osseuse.
La description qui va suivre en regard des dessins annexés, donnés à titre d'exemples non limitatifs, fera bien comprendre en quoi consiste l'invention et comment elle peut être réalisée.
La figure 1 est une vue de côté d'une articulation métatarso-phalangienne munie d'une prothèse totale de resurfaçage conforme à l'invention.
La figure 2a est une vue de face du composant métatarsien de la prothèse de la figure 1.
Les figures 2b et 2c sont respectivement une vue de gauche et une vue de dessus du composant métatarsien de la figure 2a.
La figure 3a est une vue de côté de la vis du composant phalangien de la prothèse de la figure 1.
La figure 3b est une vue de gauche de la vis de la figure 3a.
La figure 4 est une coupe d'une cupule du composant phalangien de la prothèse de la figure 1.
La figure 1 montre une vue de côté une prothèse métatarso-phalangienne de resurfaçage, comprenant un composant métatarsien 100 et un composant phalangien 200.
Comme l'illustrent les figures 1, 2a, 2b et 2c, ledit composant métatarsien 100 comporte un bouclier 110 de frottement ayant sensiblement la forme d'une portion de sphère ouverte latéralement, et destiné à être fixé à l'extrémité distale du métatarse 10 sans résection des os sésamoides 11. Dans le mode de réalisation montré aux figures 1 et 2a à 2c, le bouclier 110 de frottement a une forme relativement peu enveloppante, inscrite dans une demi-sphère.
La mise en place du composant métatarsien 100 sur le métatarse 10 exige, par exemple, trois coupes planes de l'extrémité distale de l'os sur lesquelles viennent en appui trois faces planes conjuguées 111, 112, 113, usinées sur la partie interne du bouclier 110.
Bien entendu, la stabilité du bouclier de frottement tel qu'il est représenté sur les figures 1, 2a, 2b et 2c, est inférieure à celle d'un bouclier plus enveloppant qui nécessiterait la résection des os sésamoides 11. Aussi, de façon à compenser cette moindre rétention, l'invention prévoit que le composant métatarsien 100 comporte au moins une, par exemple deux quilles 120 d'ancrage lisses et de forme effilée. Cette forme particulière donnée aux quilles d'ancrage participe également à la stabilisation à long terme du composant métatarsien 100 puisqu'elle a pour effet de préserver le stock osseux concerné. L'os étant présent en plus grande quantité, le vieillissement sera moins rapide. La repousse osseuse peut être encore favorisée du fait que les quilles 120 d'ancrage portent un revêtement d'hydroxy-apatite. Les quilles elles-même et le bouclier de frottement peuvent être réalisés en acier inoxydable par exemple.
La stabilité du composant métatarsien 100 étant ainsi assurée, sa pose ne nécessite pas l'utilisation de ciment, avec tous les avantages mentionnés précédemment.
Il apparaît sur la figure 1 que le composant phalangien 200 comprend une vis 210, représentée en détail sur les figures 3a et 3b, destinée à être introduite axialement dans la phalange 20. La tête 211 de la vis 210 est munie d'une cupule 220 formant surface articulaire pour le bouclier 110 de frottement du composant métatarsien 100. Ladite cupule 220 du composant phalangien peut être en polyéthylène, la vis 210 étant en acier inoxydable. Ainsi que le montrent les figures 3a, 3b et 4, la cupule 220 est fixée sur la tête 211 de la vis 210 par emmanchement d'un pion 221 dans un logement 212 aménagé dans la tête 211.
Dans le mode de mise en oeuvre de l'invention illustré sur la figure 2, la phalange 20 a subi une ostéotomie de raccourcissement qui permet de régler les tensions ligamentaires de l'articulation en ajustant la pénétration de la partie distale 22 de la phalange 20 par une position plus ou moins grande de l'extrémité filetée 213 de la vis 210 dans ladite partie distale 22, la tête 211 de ladite vis étant engagée dans la partie proximale 21 de la phalange 20.

Claims (5)

REVENDICATIONS
1 - Prothèse totale métatarso-phalangienne de resurfaçage, comprenant un composant métatarsien (100) et un composant phalangien (200), caractérisée en ce que ledit composant métatarsien (100) comporte un bouclier (110) de frottement ayant sensiblement la forme d'une portion de sphère ouverte latéralement, et destiné à être fixé à l'extrémité distale du métatarse (10) sans résection des os sésamoïdes, et en ce que ledit composant phalangien (200) comprend une vis (210) destinée à être introduite axialement dans la phalange (20), et dont la tête (211) est munie d'une cupule (220) formant surface articulaire pour le bouclier (110) de frottement du composant métatarsien (100).
2 - Prothèse selon la revendication 1, caractérisée en ce que le composant métatarsien (100) comporte au moins une quille (120) d'ancrage lisse et de forme effilée.
3 - Prothèse selon la revendication 2, caractérisée en ce que ladite quille (120) d'ancrage porte un revêtement d'hydroxy-apatite.
4 - Prothèse selon l'une des revendications 2 ou 3, caractérisée en ce que le composant métatarsien (100) et la vis (210) du composant phalangien (200) sont en acier inoxydable.
5 - Prothèse selon l'une quelconque des revendications 2 à 4, caractérisée en ce que la cupule (220) du composant phalangien (200) est en polyéthylène.
