Abstract

Obturateur pour cavités osseuses, réalisé en un matériau biocompatible résorbable. Selon l'invention, ledit obturateur comporte un élément (110) en matériau souple apte à s'expanser latéralement de manière à produire une obturation forcée cavités osseuses. Application à la pose, avec ciment chirurgical, de prothèses ou d'implants en chirurgie orthopédique.

Description

La présente invention concerne un obturateur biocompatible, biodégradable, souple et déformable.
Cette invention a rapport notamment à la chirurgie orthopédique. Lors d'une pose de prothèse ou implant, fixés par du ciment chirurgical, un obturateur est généralement placé à un endroit précis de la cavité osseuse pour:
- Eviter que le ciment ne pénètre plus profondément que nécessaire dans le
canal médullaire de l'os.
- Rendre plus homogène la répartition du ciment autour de la prothèse, et
obtenir une meilleure et plus durable fixation de l'implant.
Actuellement la mise en place des obturateurs biocompatibles et souples nécessite:
- Un nombre important d'obturateur de différents diamètres.
- L'utilisation préalable de gabarits.
- Au mieux, trois temps opératoires.
- Une attention supplémentaire et contraignante dans une phase opératoire
critique ; ce qui pour certain chirurgien n'est pas en rapport avec le gain
escompté.
L'intervention vise à fournir un obturateur se déformant sous l'action de son ancillaire sur un ou plusieurs de ses composants. Cette déformation se fait par expansion de l'obturateur, proportionnellement à la contrainte exercée par l'ancillaire lors de la mise en opiace du bouchon.
L'expansion complète de l'obturateur est obtenue une fois celui-ci positionné dans la cavité osseuse, à l'emplacement déterminé par le praticien.
L'obturateur ainsi déformé occupe la totalité de la cavité osseuse, à I'endroit choisi par l'utilisateur. En effet, l'expansion permet la mise en adéquation de l'obturateur ainsi déformé, et des formes irrégulières de la section osseuse à l'endroit retenu par le chirurgien.
L'invention évite l'utilisation de gabarits ou encore d'un ancillaire à force pour enfoncer et mettre en place l'obturateur.
L'invention évite les risques de fuite et migration de l'obturateur dans le canal médullaire, lors de la mise en pression du ciment. Les contraintes mécaniques de l'obturateur expand étant supérieures à celles de la pression du ciment.
L'invention autorise un seul temps opératoire. L'obturateur est composé d'un ou plusieurs bio-matériaux résorbables permettant par action de l'ancillaire, sa déformation, sa mise en adéquation avec la cavité osseuse, et sa tenue mécanique une fois son expansion réalisée.
La réalisation d'un obturateur autre que sphérique est possible, en fonction que l'expansion est obtenue par compression, par changement d'état mécanique, par gonflement, par remplissage, par élasticité, par mémoire de forme, par compression préalable, par écrasement, par changement de volume dû à une réaction chimique, par rigidification mécanique ou chimique de l'obturateur.
L'invention peut être réalisée à base de collagène, d'alginate, de dextran, de caséine ou autre matériau biocompatible et dégradable, ou à base d'une combinaison de ces produits.

Claims (6)

REVENDICATIONS:
1- Obturateur notamment d'un canal osseux en chirurgie orthopédique,
caractérisé en ce qu'il est: - Biocompatible
- Biodégradable
- Souple
- Déformable en expansion dans la cavité
intra médullaire, sous l'action de son
ancillaire.
2- Obturateur conforme à la revendication 1, caractérisé en ce que
l'expansion est obtenue sous l'action de l'ancillaire sur un ou plusieurs des
composants de l'obturateur.
3 - Obturateur conforme aux revendications I et 2, caractérisé en ce que son
ou ses composants sont des matériaux aux caractéristiques mécaniques,
ou/et chimiques différentes.
4 - Obturateur conforme aux revendication 1, 2 et 3, en ce que les matériaux
sont à base de collagène, d'alginate, de dextran, de caséine, ou autre
matériau biocompatible et dégradable, ou à base d'une combinaison de
ces produits.
5 - Obturateur conforme aux revendications 1, 2, 3 et 4, en ce que l'expansion
soit obtenue par compression, par changement d'état mécanique, par
gonflement, par remplissage, par élasticité, par mémoire de forme, par
compression préalable, par écrasement, par modification de volume dû à
une réaction chimique, par rigidification mécanique ou chimique de
l'obturateur.
6- Obturateur conforme aux revendications 1, 2, 3, 4 et 5, en ce que son
expansion complète soit obtenue une fois l'obturateur à la position retenue
par le chirurgien.
