Abstract
Description
Tige humérale pour prothèse d'épaule. Humeral stem for shoulder prosthesis.
There are essentially two main types of humeral stems, depending on how they are anchored in the humerus, with or without cement.
These rods are applied, either for degenerative treatments, or for trauma treatments. However, no prosthesis meets these two indications.
One of the problems which the invention proposes to solve is to create a humeral prosthesis, which, in particular at the level of its stem, responds to the indications of degenerative treatment and trauma treatment.
Another problem which the invention proposes to solve is to equip the rod with a system allowing the positioning and the maintenance of bone fragments, in particular in the case where it is necessary to join the pieces of an exploded shoulder.
There are known femoral stems having an overflow at their proximal part, a reinsertion wing equipped with holes for bringing together and maintaining the bone fragments, in combination with suture threads. However, it appears that the fin completely overflows from the profile of the humeral rod, thereby creating a projection which risks crossing the gutter of the humerus.
The object of the invention is to remedy these drawbacks in a simple, safe and effective manner.
The problem which the invention proposes to solve is to avoid any problem of intraoperative fractures, taking into account the presence of the fin, while retaining its holes, for bringing together the bone fragments of the posterior and anterior planes. .
Such a problem is solved in that the fin is connected to the body of the rod by a longitudinal and transverse profile capable of ensuring the partial integration of said fin in the general volume defined by the section of the body of the rod.
It appears that this proximal external geometry of the humeral rod makes it possible to bring the bone fragments into contact, one with respect to the other, while ensuring bone regrowth as quickly as possible.
Advantageously, the longitudinal profile is formed by grooves formed on either side of the fin and extending in a progressively decreasing manner with the body of the rod. The transverse profile consists of cut sides determined so as to reduce the cross-section of the body of the rod at the level of the fin.
According to another characteristic, the upper part of each of the grooves is connected at the level of the proximal part by a stepped profile.
The upper end of the fin and of the stepped profile constitute an inclined section whose edges are cut.
To resolve the problem posed of ensuring perfect stability of the rod after impaction in the medullary canal, the latter has at its proximal part, a support flange of ovoid shape.
To solve the problem posed of making a synthesis between the internal and external compartment, the rod is pierced transversely, right through, at its proximal part, in particular in the middle part of the methaphyse. This hole allows the passage of a grommet.
The invention is set out below in more detail using the appended drawings, in which:
Figure 1 is a partial perspective view of the proximal part of the humeral rod according to the invention.
Figure 2 is a front view of the rod according to the invention.
Figures 3 and 4 are side views corresponding to Figure 2.
Figure 5 is a partial sectional view of the proximal part of the humeral rod.
Figure 6 is a cross-sectional view taken along line 6.6. in Figure 5.
Figure 7 shows the impaction of the humeral stem.
The humeral rod generally designated by (1), has in a known manner, a reinsertion wing (la). This fin has a series of holes (1b) for positioning and maintaining the bone fragments.
According to the invention, the fin (1a) is connected to the body of the rod (1), by longitudinal and transverse profiles, capable of ensuring the partial integration of said fin in the general volume defined by the section of the body. of the stem.
As shown in particular in Figures 1 and 4, the longitudinal profile is constituted by grooves (tic) and (nest) formed on either side of the fin (la). These grooves (1c) and (1d), connect progressively decreasing with the body of the rod. The upper part of each of the grooves (tic) and (nest) is connected at the proximal part by a stepped profile (here) (1d1). The upper end of the stepped profile (here) (ide) and the upper end of the fin (la) constitute an inclined face (1e), the edges of which are bevelled.
According to another characteristic, the proximal part of the rod (1) has, at its metaphysis, a transverse hole (If), to allow the positioning and the maintenance of the bone fragments, in combination with the holes (lob) of the 'fin (the). The proximal part of the stem has a support flange (1g), of ovoid shape.
Obviously, as shown in Figure 5, the upper part of the rod has any arrangement (1h) allowing the attachment of a humeral head (2).
The advantages emerge clearly from the description, in particular it underlines and-one recalls the integration of the fin avoiding any problem of intraoperative fracture, while retaining the holes for bringing together the bone fragments of the posterior and anterior planes.
