Abstract

Elle comprend: - une calotte sphérique (2) mince dont la face intérieure est solidaire d'un tronc de cône (3) ouvert en direction de ladite collerette, ce tronc de cône laissant libre à sa périphérie une jupe annulaire (2a) prolongeant la calotte; - des picots (4) s'étendant parallèlement à l'axe géométrique de la calotte et régulièrement répartis sur une circonférence coaxiale à la calotte (2); - une quille aplatie (5) à extrémité (5f) arrondie faisant suite de manière continue au tronc de cône (3) de la calotte sphérique (2) grâce à une collerette de liaison (5b) audit tronc de cône.

Description

La présente invention est relative à une prothèse destinée à permettre la reconstitution de la surface de La tête fémorale. Cette prothèse est destinée à traiter par exemple les cas de nécroses idiopathiques de la tête fémorale, invalidantes et étendues, mais avec un cartilage cotyloldien intact.
Ces nécroses, qui se traduisent par un défaut de l'os sous-jacent au cartilage, entrafnent des douleurs souvent insupportables et la destruction du cartilage non soutenu par de l'os.
Chez des patients agés, on remédie à ces nécroses par la pose d'une prothèse totale de hanche, mais chez les sujets jeunes ou très jeunes, on doit tout tenter pour retarder la pose d'une prothèse totale, de telle sorte qu'on utilise d'autres solutions.
Hormis les solutions strictement conservatrices telles que les greffes pédiculées, les ostéotomies de dérotation qui donnent des résultats aléatoires, il existe des solutions consistant en une arthroplastie économique de surface appelée cupule ajustée ou céphalique. Toutefois, ces solutions ne donnent actuellement pas entière satisfaction, bien que la pose de ces cupules se base sur une économie de moyens sans résection mutilatrice de la tête et du col du fémur.
Les perfectionnements qui font l'objet de la présente invention visent à remédier aux inconvénients précités et à permettre la réaLisa- tion d'une prothèse de réhabilitation de la tête fémorale qui assure
- une limitation au strict minimum du creusement ou de la coupe de la tête fémorale ;
- une continuité parfaite entre la surface extérieure de la cupule et le cartilage voisin resté intact ;
- une stabilité dans le temps au basculement et à la rotation ;
- la préservation de la vascularisation de la tête fémorale ;
- la stabilité à l'enfoncement, c'est-à-dire au tassement, grâce à une transmission très physiologique des forces au moignon sain du col fémoral ;
- une présentation bien orientée dans les deux plans frontal et sagittal ;
- une bonne congruence avec le cotyle ;
- la conservation de la mobilité de l'articulation réparée ;
A cet effet, la prothèse suivant l'invention comprend
- une calotte sphérique mince dont la face intérieure est solidaire d'un tronc de cône ouvert en direction de ladite calotte, ce tronc de cône laissant libre à sa périphérie une jupe prolongeant la calotte parallèlement à l'axe géométrique ;
- des picots s'étendant parallèlement à l'axe géométrique de la calotte et régulièrement répartis sur une circonférence coaxiale à la périphérie de la calotte ;
- une quille aplatie à extrémité arrondie faisant suite de manière continue au tronc de cône de la calotte sphérique, grâce à une collerette de liai son audit tronc de c8ne.
Le dessin annexé, donné à titre d'exemple, permettra de mieux comprendre L'invention, les caractéristiques qu'elle présente et les avantages qu'elle est susceptible de procurer

Fig. 3 et 4 sont des coupes suivant III-III, respectivement
IV-IV (fig. 2) sur lesquelles on a silhouetté en traits mixtes la tête fémorale.
La prothèse suivant l'invention référencée 1 en fig. 1 est composée d'une calotte sphérique 2 relativement mince, c'est-à-dire de l'ordre de 2 à 3 mm, dont la face intérieure est solidaire d'un tronc de cône massif 3 ouvert en direction de la calotte, de telle sorte que son extrémité libre constitue une face circulaire 3a comportant un trou central taraudé 3b (fig. 3 et 4). On observe que la base du cône, c'est-à-dire sa liaison avec la face interne de la calotte sphérique, présente un diamètre inférieur à celui de cette dernière, de manière que celle-ci forme une jupe annulaire 2a autour dudit tronc de cône. Le tronc de cône 3 est pourvu de quatre lamages 3c, 3d, 3e et 3f disposés à égale distance l'un de l'autre sur une circonférence co-axiale au pourtour de la base du tronc de cône et qui sont évidemment disposés de 900 les uns par rapport aux autres. Le centre de chacun des lamages est pourvu d'un trou taraudé référencé 3cul, 3e1 en fig. 3 puisqu'on ne voit que ceux-ci sur les dessins. Chacun des trous taraudés ménagés au centre des lamages reçoit l'extrémité filetée d'un picot 4 dont l'extrémité opposée est pointue et qui s'étend parallèlement à l'axe géométrique de la calotte 2.
Le troisième élément de la prothèse suivant l'invention est réaLisé sous la forme d'une quille 5 comportant un embout fileté 5a se vissant dans le trou taraudé 3b du cône 3. L'embout 5a est issu d'une collerette circulaire 5b dont la face qui porte l'embout Sa est de diamètre strictement identique à celui de la face extrême 3a du cône 3. A partir de la collerette 5b, la quille 5 est réalisée sous la forme d'une palette 5c reliée à la collerette par deux congés 5d, Se de grand rayon.
On note qu'en élévation, la palette présente la forme générale d'un trapèze isocèle dont la petite base 5f est arrondie. En outre, la palette est traversée par une multitude de trous 59.
Le filetage de l'embout 5a et celui du trou 3d sont tels que lorsque les faces en vis-à-vis de la quille 5 et du tronc de c8ne sont en appui l'une contre l'autre, la palette est orientée dans le plan médian des picots.
La calotte sphérique 2 et le tronc de cône 3 peuvent être prévus soit en acier, soit en céramique, tandis que les picots et la quille sont toujours réalisés en acier.
Bien entendu, on réalise toute une série de prothèses 1 permettant à la calotte sphérique de s'adapter à toutes les dimensions des têtes fémorales.
On observe que la présence de la jupe 2a permet de réaliser une jonction parfaite avec le cartilage de la tête fémorale au-delå de La lésion cartilagineuse qui dépasse généralement la zone nécrosée.
Le tronc de cône 3 peut présenter un aspect de surface à aspérités pour permettre un meilleur accrochage du ciment. De toute manière, les trous 59 favorisent la repousse osseuse dans le cas d'une pose sans ciment.
On observe enfin que la liaison entre la base du tronc de cône 3 et la collerette 5b de la quille 5 est absolument lisse.
Il doit d'ailleurs être entendu que la description qui précède n'a été donnée qu'à titre d'exemple et qu'elle ne limite nullement le domaine de l'invention dont on ne sortirait pas en remplaçant les détails d'exécution décrits par tous autres équivalents.

Claims (6)

REVENDICATIONS
1. Prothèse (1) de réhabilitation de la tête fémorale réalisée sous la forme d'une cupule ajustée, caractérisée en ce qu'elle comprend :
- une calotte sphérique (2) mince dont la face intérieure est solidaire d'un tronc de cône (3) ouvert en direction de ladite collerette, ce tronc de cône laissant libre à sa périphérie une jupe annulaire (2a) prolongeant la calotte ;
- des picots (4) s'étendant parallèlement à l'axe géométrique de la calotte et régulièrement répartis sur une circonférence coaxiale à la calotte (2) ;
- une quille aplatie (5) à extrémité (5f) arrondie faisant suite de manière continue au tronc de cône (3) de la calotte sphérique (2) grace à une collerette de liaison (5b) audit tronc de cgne.
2. Prothèse suivant la revendication 1, carcatérisée en ce que la face extérieure de la calotte (2) est parfaitement polie.
3. Prothèse suivant la revendication 1, caractérisée en ce que l'extrémité libre (3a) du tronc de cône (3) constitue une face circulaire de dimensions exactement identiques à celles de la collerette (5b) de la quille (5).
4. Prothèse suivant la revendication 1, caractérisée en ce qu'elle comprend quatre picots (4) répartis à 900 Les uns par rapport aux autres.
5. Prothèse suivant La revendication 4, caractérisée en ce que la partie plate ou palette (5c) de la quille (5) affecte en élévation une forme trapézoldale à petite base arrondie (5f) et en ce qu'elle est orientée dans le plan médian des picots (4).
6. Prothèse suivant la revendication 5, caractérisée en ce que La partie plate de la quille (5) comporte une multiplicité de trous traversants (5g)
