Abstract
Description
The present invention relates to a cup for acetabular implantation and to hip prostheses comprising it.
Total hip prostheses include two cooperating parts: a lower part inserted into the femur and an upper part fixed in the pelvis. These two parts are shaped to create between them a contact surface reproducing the hip joint.
The object of the invention is to improve the upper part of such a hip prosthesis. This part, called cup, comprises in known manner a metal member which is fixed in the acetabulum and a member of synthetic material or insert, which is housed in the metal member.
The metallic organ is generally hemispherical.
The internal cavity of the synthetic material organ cooperates with a ceramic element provided on the femur prosthesis.
To date there is a problem of fixing the metal cups in the acetabulum.
To ensure stability, so-called pilot screws are used which pass through the upper part of the cup, which is provided for this purpose with a corresponding number of orifices.
The surgeon directs and places the piloted screws, generally using a cardan screwdriver.
Certain known metal cups have the shape of a hemisphere whose outer surface is entirely smooth, so that the fixation in the acetabulum is only achieved by screws. It was found that this fixation was not always sufficient. This is why, it has already been proposed in the prior art to provide the base of the cup with an external thread, which allows it to be screwed into the bone.
The pitch of the external thread is then relatively large in order to have a stable fixing.
The present invention provides a metal cup for acetabular implantation, the upper surface of which is generally spherical in shape and is provided with a plurality of holes for the passage of screws, said cup being characterized in that the upper spherical surface is interrupted before the equatorial plane and is extended by a conical part allowing a forceful anchoring in the acetabular cavity.
- la partie conique se situe pratiquement dans le prolongement de la surface sphérique, la génératrice du cône étant sensiblement tangente à cette surface;

In the drawings in which the same reference symbols designate similar parts, the metal cup of the present invention, which bears the reference 1, is more particularly illustrated in FIGS. 1, 2, 3 and 9, while the monobloc insert, cooperating with this cup 1 and designated by the reference 8, is shown in Figures 4, 5, 6 and 8.
The cup 1, which is made of metallic material, has a body 4.
The body 4 of the cup 1 comprises an upper external surface 2 of generally hemispherical shape, provided with a plurality of orifices 3.
The base 5 of the body 4 has a conical part 6 extending the spherical part 7.
The conical part 6 can be smooth as illustrated in FIG. 9 or be provided with a series of shallow ridges 10.
The base 5 ends with a heel ll intended to ensure cooperation during the introduction of the one-piece insert 8 into the internal part 9 of the metal cup 1.
The conical part 6 comprises notches 12 distributed uniformly at the periphery.
The heel 11 is discontinuous and several windows 16 are provided between the heel parts 11.
In an embodiment not shown, the heel 11 could be produced continuously and in this case there would be no provision for the windows 16.
The one-piece insert 8 is made of a synthetic material. These include plastic of the polyethylene type.
This one-piece insert 8 has on its external surface a groove 18 intended to come to fit in an internal rib 17 of the metal cup 1.
The one-piece insert 8 comprises a bead 13. The bead 13 may be provided with perforated parts 14 situated opposite the tenons 15.
Figure 7 shows the adaptation of the one-piece insert 8 in the metal cup 1 of the present invention.
To obtain a secure fixing without the possibility of rotation of the bead 13, the pins 15 are arranged opposite the windows 16 of the heel 11; this ensures a latching effect of the one-piece insert 8 in the metal cup 1 due to the cooperation of the shapes.
La partie conique 6 prolonge la surface sphérique 7.The inventive feature lies in the shape and structure of the metal cup 1, which allow stable and resistant anchoring or wedging in the acetabulum as well as a very good adaptation of the insert 8, made of polyethylene for example
The conical part 6 extends the spherical surface 7.
The extent of this conical part can represent up to one third of the total external surface of the cup 1. The angle of the cone is chosen so that the conical part 6 extends the spherical surface 7 along a plane substantially tangent to it. this. The angle at the top of the cone can be variable, for example from 10 to 300 approximately.
The technique for applying the cup 1 of the present invention is described below.
First, the surgeon prepares the acetabulum for the strawberry. He introduces into this cavity a test cup with a diameter equal to that of the cutter, which makes it possible to check the good peripheral bone / cup contact.
The surgeon then forcibly anchors the cup 1 of the invention, the dimension of which is somewhat greater than that of the milled cavity to ensure perfect stability.
The surgeon can then orient and install the screws using the holes 3 and for this purpose, he generally uses a cardan screwdriver.
Two to three screws are sufficient to complete the stability of the acetabulum. Finally, the surgeon snaps the one-piece insert 8 into the metal cup 1.
The metal cup of the present invention ensures, thanks to its force anchoring which is due to its particular geometry, namely the combination of a conical part and a spherical part, an almost perfect adaptation over the entire surface of the acetabulum.
The invention is not limited to the embodiments shown and described in detail and various modifications can be made thereto without departing from its scope.
