La présente invention concerne le domaine médical et de la vie courante . Il s'agit d'un pessaire vaginal destiné à soutenir l'utérus et la face postérieure de la vessie pour s'opposer à la descente de ces organes chez la femme présentant un prolapsus
Le pessaire traditionnel (anneau semi rigide) est posé par le médecin . Cette mise en place est souvent pénible, en raison du volume de l'appareil et de la nécessité d'une abduction importante des cuisses . Le pessaire est abandonné en place plusieurs mois ; il induit de façon quasi constante une infection vaginale chronique . In addition, the permanent presence of the traditional pessary considerably interferes with sexual intercourse.
Le dispositif selon l'invention permet de remédier à ces inconvénients par sa mise en place personnelle, atraumatique et son usage unique, exclusivement diurne . Il comporte en effet
1 - Une mince enveloppe inextensible prenant lorsqu'elle est gonflée la forme d'un anneau rigide circulaire et pouvant être repliée sur elle même lorsqu'elle est vide . Cette enveloppe présente sur sa face inférieure une valve à sens unique pour le gonflage à l'air, et, toujours sur sa face inférieure, une zone de renforcement sur laquelle est fixé un fil relativement rigide dont l'extrémité est pourvue d'un petit renflement sphérique
2 - Un dispositif pour la mise en place de l'anneau, constitué par une canule incurvée en demi-cercle, souple mais inextensible, permettant d'accéder sans effort et sans danger au fond vaginal .Cette canule présente à son extrémité profonde arrondie un embout qui s'adapte à la valve de gonflage de l'enveloppe, et, à son extrémité superficielle une partie creuse destinée à recevoir l'embout d'une seringue
L'utilisatrice coiffe l'extrémité profonde de la canule d'un pessaire replié, maintenu compact par une crème adhésive et lubrifiante, et branche la seringue ouverte à l'extrémité superficielle . L'utilisatrice se place en position allongée et pousse l'extrémité de la canule chargée du pessaire jusqu'au fond du vagin, réduisant, ainsi le prolapsus, puis elle exprime à fond l'air contenu dans la seringue . Le pessaire est éjecté automatiquement ou par légère traction sur la canule L fil s'allonge dans le vagin ; sa portion distale se replie dans la région vulvaire ; son extrémité renflée non piquante est facilement repérable au doigt .Quand la femme tire sur le fil, celui-ci déchire l'enveloppe . Le fil reste cependant solidaire de l'enveloppe et permet de ramener à l'extérieur le pessaire ainsi dégonflé . Le pessaire est jetable ; la canule et la seringue peuvent servir à de nouvelles mise en place
Selon une forme de réalisation préférentielle, le pessaire est recouvert d'une couche de ouate imprégnée de produit aromatique ou médicamenteux (antibiotique, antifungique, hormonal)
Selon une variante d'utilisation, la canule recourbée constitue à elle seule un dispositif pour l'injection vaginale d'un produit mis en place dans la seringue .Sa forme courbe présente un grand intérêt chez les femmes agées, malhabiles et dont la flexion du tronc en avant est gênée par l'arthrose vertébrale
Selon une forme de réalisation s'établit une gamme de différentes tailles pour le pessaire : diamètre extérieur de 5,6, 7, 8, 9, ou 10 centimètres

La figure 2 représente, en coupe, le dispositif de mise en place selon l'invention, chargé de l'enveloppe à l'état dégonflé
Le dispositif représenté sur la figure 1 comporte une enveloppe gonflable {1) annelée, circulaire ayant des dimensions de 5 à 10 centimètres pour le diamètre extérieur, et de 3 à 8 centimètres pour le diamètre intérieur selon le modèle, et un diamètre de 2 centimètres à la coupe quelque soit le modèle .A sa face inférieure une valve (2) et à proximité de celle-ci une zone renforcée (3) sur laquelle est inséré un fil (4) rigide d'environ 15 centimètres de long terminé par une sphère repère (5)
Le dispositif représenté sur la figure 2 comporte l'enveloppe (1) repliée sur elle même, maintenue compacte par une crème adhésive (6)
Le pessaire est implanté sur la canule (7) ayant des dimensions de l'ordre de 1,5 centimètres de diamètre et de 30 centimètres de long incurvée en demi-cercle
En son centre un canal (8) d1environ 0,3 centimètres de diamètre se termine à une extrémité par une saillie (9) implantée dans la val- ve et à l'autre extrémité par une excavation (10) recevant l'embout d'une seringue . The user covers the deep end of the cannula with a folded pessary, kept compact by an adhesive and lubricating cream, and connects the open syringe to the surface end. The user lies down and pushes the end of the cannula loaded with the pessary to the bottom of the vagina, reducing the prolapse, then it fully expresses the air contained in the syringe. The pessary is ejected automatically or by light traction on the cannula. The wire stretches out in the vagina; its distal portion folds in the vulvar region; its swollen, non-pungent end is easy to locate with your finger. When the woman pulls on the wire, the latter tears the envelope. The wire remains however integral with the envelope and allows the pessary thus deflated to be brought outside. The pessary is disposable; the cannula and syringe can be used for new placement
According to a preferred embodiment, the pessary is covered with a layer of cotton wool impregnated with aromatic or medicinal product (antibiotic, antifungal, hormonal)
According to a variant of use, the curved cannula alone constitutes a device for vaginal injection of a product placed in the syringe. Its curved shape is of great interest in elderly women, unskilled and whose flexion of the forward trunk is hampered by spinal osteoarthritis
