Abstract

The invention relates to a document enabling a medical staff member to have access, after breaking a closed, non-reconstructible system, to personal and medical information regarding the bearer identified by a photograph printed on the outside. It consists of two flaps initially folded along a fold 25; hence four sides: two external sides, front and back, showing the photograph, the number of the card and general information, and two internal sides bearing personal information 10 to 19 and medical information 20, these two being folded one A over the other A', after printing, and are sealed around a 1-mm margin against the edge 24. The breakable corner 1 can be broken along the precut line 2 and the metal wire 23 connected to it pulled to cut the card around three sides, the fourth of which acts as a hinge 22 to the back of the card which opens like a page of a book and gives access to the information 10 to 20.

Description

The present invention relates to a closed, rigid, unalterable, waterproof, space-saving document system (powerful in "credit card" format), written in 2 languages (carrier language + English), intended
- to be worn by the person who had it established jointly with his doctor.

- to be opened only by a person from the medical profession in case of emergency or necessity (*)
It identifies the person by photo printed on the face of the card, as well as the nationality of the person (Figure 1).

On the back of the card are printed
- a reference code number which will allow, in the event of loss, if it is returned to the organization which established it, to transmit it to its owner;
- statements specifying that any opening of the closed, irrecoverable system by an unauthorized person, in circumstances other than those necessitating it, is an act of malicious intent.

 On the internal sides, the personal and medical information is given as specified in Figure 2 and its legend.

* by pulling the metal wire connected to it along a line of weakness that cuts

along three sides of the back of the card.

 The fourth side, endowed with a line of weakness not cut by the wire, serves as a hinge on the back of the card which opens like a flap and allows access to confidential personal and medical information carried on the two internal sides.

 To date, there is no such system known in France. People, if necessary, have only a letter from their doctor or one or more paper cards (blood donor, pace maker door, hemophiliac card, etc.).

(*) The document is made up of two sections, therefore four sides: 2 exterior with general and rude and 2 internal with personal and medical information.

 This document has the advantage of being closed, semi-rigid, waterproof, unalterable (except thinners and fire), of having an irreconstructible opening system, of being bilingual, written in clear, of requiring no decoding device and therefore to be readable anywhere.

 The surface allows printing up to 6,000 (six thousand) characters, small but legible, as currently printed on so-called credit cards. The sides of this document may have magnetic support surfaces intended to store information readable with a reader.

 It makes it possible to identify the person by external photo, before opening to return it to the organization which established it. This one, only, knows the coordinates of the carrier and will transmit the document to him in reco ond ".

 It is, by the way it is established (a precise questionnaire) a summary, a precise medical report, and specifies the family, medical contacts and the wishes of the wearer, certified by his signature.

 Reduced in size, the card may be larger than the format of a credit card but not larger than the format of a national identity card. This document is intended to be worn by its holder or in certain circumstances as a so-called risky professional activity (road, pilots, etc ...) on the belt, on an armband, in a pocket or in the wallet. A sticker on the car or suitcase indicates that the person is carrying such a document.

Claims (3)

 1) Closed document constituting a confidential medical card characterized in that it is made up of two parts (25) therefore four sides, two exterior: front and back, bearing the photo, the number of the card and the general information, and two internal with personal information in lines 10 to 19 and medical (20), folded over one another (A on A ', fig. na 4), after printing and welded 1 mm from the edge.  rigid, is provided with a breakable angle (1) along the pre-cut line (2) to which is attached a metal wire (23) which will be used, by traction, to cut the card along a line of weakness (21) in relief at 1 rrm of the edge, along the weld, on three dimensions, the fourth serving as a hinge on the back of the card which opens as a flap and allows access to the data.
