Abstract

Superficial wounds, esp. dental wounds, are covered and protected by a prod. contg. calcium peroxide. The calcium peroxide may be mixed with calcium hydroxide, in a proportion up to total replacement of hydroxide. The peroxide supplies nascent oxygen which compensates for the lack of physiological oxygen and aids survival of dental pulp affected by ischemic gangrene. The calcium peroxide may also be mixed with calcium oxide, and has the advantage of giving a foamed solid, facilitating penetration of probes etc.. The calcium peroxide may be further mixed with sodium peroxide, which modifies the rate of reaction and renders it safer. The prod. is better tolerated and more effective than conventional materials used for dental wounds, such as calcium-or sodium peroxide.

Description

The present invention relates to a product for covering and protecting the surface of superficial wounds, and in particular dental wounds.

We know the physiological action of calcium hydroxide (slaked lime) applied in medicine, and especially in dentistry to cover and protect the surface of superficial wounds and in particular dental pulp and to allow healing.

Calcium hydroxide normally has a caustic action by alkalinity. However, this action does not manifest itself in contact with living tissues because alkalinity is neutralized by carbon dioxide (Co2 or carbon dioxide), constantly produced by the activity of tissues (basal metabolism) which absorbs the oxygen provided by arterial blood and which produces C02 evacuated by venous blood.

Figure img00010001

On contact with this CO2, the calcium hydroxide transforms into neutral calcium carbonate and water according to the reaction
Figure img00010001

OH ions, caustic alkalinity factors have locally disappeared.

C02 being so absorbed, there results a relative deficiency of C02 in the balance of the ratio (O2 F C02), which is a rigorous physiological constant. The balance thus broken is immediately restored by an increase in the basal metabolism which consumes oxygen supplied by the blood. The disappeared COa is reformed and can thus continue its neutralizing action on the hydroxide.

However, in certain pathological cases, resulting in a slowing down or even a stopping of the blood circulation, there is ischemia, that is to say local asphyxia or anoxia by deficiency of oxygen. In this case, the calcium hydroxide retains its causticity and causes painful irritation.

Figure img00020001
Another product known in dentistry is sodium peroxide or oxylith, used in cases of mortification and gangrene of the dental pulp to destroy cadaveric debris by caustic soda. Due to its chemical reaction with dental water, sodium peroxide gives sodium hydroxide and oxygen depending on the reaction
Figure img00020001

But this reaction, very exothermic, is very strong and sometimes explosive, with projection of very hot caustic soda droplets.

The present invention aims to remedy in particular the drawbacks of the products defined above.

To this end, the product which it concerns contains calcium peroxide.

According to a first embodiment, the calcium peroxide is incorporated into calcium hydroxide. This incorporation into the calcium hydroxide can be carried out in proportions which can go as far as the total replacement of the hydroxide with calcium peroxide.

This association is interesting because calcium peroxide produces a release of "nascent" oxygen, compensating for a possible deficiency in physiological oxygen.

This results in a possibility of survival of the pulp which is otherwise doomed to ischemic gangrene.

Figure img00020002
Indeed, in contact with water, calcium peroxide gives calcium hydroxide and oxygen depending on the reaction
Figure img00020002

This reaction is spontaneously limited for a given temperature, in this case 370C, and the release of oxygen stops as soon as its partial pressure exceeds that of the oxygen normally present in the tissues.

Under these conditions, the basal metabolism resumes its activity, the causticity of the hydroxide is neutralized and there does not appear any morbid reaction, in particular no pain.

Figure img00030001
According to a second possibility, the calcium peroxide is incorporated into calcium oxide CaO. Calcium oxide (quicklime) is used in dentistry in the case of dental canals inaccessible to probes and other instruments (narrow, bent or branched canals ...). In these cases, the phenomenon of volumetric expansion (expansion) is exploited, which accompanies the hydration of the oxide, with the formation of calcium hydroxide depending on the reaction.
Figure img00030001

This expansion allows the penetration of the product into areas inaccessible to the instruments. However, this expansion is limited by the rigid walls (ivory or dentin) of the canal. This limitation results in an expansion or condensation of chalky or marmoreal type, that is to say very hard. This hardness is an obstacle for the practitioner who needs to have access to the canal to, for example, place a tenon there. prosthetic.

The incorporation of calcium peroxide produces a release of oxygen distributed in the hydro-calcic mass, and gives it a heterogeneous, micro-cavitary structure.

We have a calcium foam, like sea foam silico magns denne, which has a frothy consistency, which facilitates the penetration of probes and other instruments in the canal.

Alternatively, calcium peroxide is incorporated into its sodium counterpart sodium peroxide.

By the incorporation of calcium peroxide whose hydration reaction is very slow, unlike that of sodium peroxide, the mass of sodium peroxide is dispersed in that of calcium peroxide, so that the liveliness of the reaction hydration is considerably attenuated and is no longer explosive.
