Abstract
Description
The present invention relates to an endotracheal tube or a tracheotomy balloon tube for anesthesia, in particular for the laser treatment of pathological tissues of the upper respiratory tract.
There is already known an endotracheal tube for anesthesia comprising a tubular body used to convey the ventilation and anesthesia gas and provided in the vicinity of its distal end with a balloon capable of being inflated from the outside, by the through an auxiliary channel, so as to seal with the trachea and thus allow efficient ventilation. Such a tube, as well as its inflatable balloon, is made of natural or synthetic organic materials, in order to present sufficient flexibility so as not to traumatize the respiratory tracts into which it is introduced.
This known endotracheal tube gives complete satisfaction in the case of a conventional intervention. However, in the case of a laser treatment, which has certain advantages over traditional operating techniques, difficulties appear.
In fact, the material constituting such a known endotracheal tube generally absorbs infrared energy well, so that if anesthesia is delivered by this tube during an intervention by means of a laser, there is a risk of deterioration, and even the destruction of said tube which is intended to deliver the gases ensuring the breathing of the patient. It can therefore result in serious consequences for the latter, especially since the organic materials used for the manufacture of endo-tracheal tubes are not only combustible, but can also give off caustic or toxic vapors.
It may happen that anesthesia cannot be performed by natural means or that, the patient being already tracheotomized, one does not use an endotracheal tube, but a tracheostomy tube carrying an anesthesia balloon, or that the if the patient is already tracheotomized, the cannula with which he is fitted is replaced by a flexible cannula provided with a balloon allowing anesthesia.
The drawbacks of using the laser beam when anesthesia is delivered by tracheostomy tube, and not by endotracheal tube, are of course the same.
In an attempt to remedy these drawbacks, it has already been thought of protecting the endotracheal tube or the tracheotomy tube by means of helical windings of a strip of a material reflecting infrared energy, such as aluminum, these windings being for example fixed on the tube by means of an adhesive. Such a solution can be considered acceptable for the protection of the tube itself, but has no effect on protecting the balloon equipping the tube and which has the function of ensuring sealing and thus allowing effective ventilation. However, it is precisely the ball which is the most vulnerable part of said tube, because it is made of a very thin thickness of a tensioned elastomer
We also thought of making the endo-tracheal tubes out of a metallic material. But then, we obtain rigid tubes whose introduction is traumatic for the respiratory tract. -Moreover, because of the specular polish of such an instrument, there is a risk of reflections of the laser beam on healthy tissue which should not be treated.
The object of the present invention is to remedy these drawbacks.
To this end, according to the invention, the endo-tracheal or tracheotomic tube for anesthesia comprising a tubular body serving to convey ventilation and anesthesia gases and provided in the vicinity of its distal end with a sealing balloon capable of 'being inflated, is remarkable in that it comprises between said balloon and the proximal end of the tube, at some distance from said balloon, a flexible heat shield.
Thus, thanks to the invention, it is possible to obtain a flexible tube in which the balloon, which is the most fragile part, is protected by the heat shield.
Such a heat shield can be in the form of a disc coaxial with the tube. It can be fixed on it or be integral with it. It may be in one piece or, on the contrary, consist of several sectors integral with each other in the vicinity of their articulated part and separated from each other by slots whose edges overlap.
Preferably, the heat shield consists of a flexible material similar to that of the tube and containing a dispersion of a finely divided metallic powder such as a powder of aluminum, silver, gold, etc. These particles metals absorb and reflect the energy received from the impact of the laser beam used to treat pathological tissue.
Only the heat shield can contain the finely divided metal powder dispersion. However, it is advantageous for the entire tube to have such a metallic dispersion, so that its resistance to infrared energy is improved.
In the case of an endotracheal tube, the distance separating the balloon from the shield is chosen approximately equal to 2 or 3 cm so as to allow the vocal cords to be crossed in two stages. In the case of a tracheostomic tube, the the problem of the passage of the vocal cords obviously does not arise. Under these conditions, it is possible to place the thermal protection shield a few millimeters from the balloon allowing the seal between the trachea and the tube. The diameter of the shield is chosen, in both cases, slightly less than the diameter of the trachea to be intubated.
The figures of the appended drawing will make it clear how the invention can be implemented.
Figure 1 is a perspective view of an endo-tracheal tube according to the invention.
Figure 2 is an enlarged front view of an alternative embodiment of the heat shield.
The endotracheal or tracheotomy tube according to the invention, shown in FIG. 1, comprises in a known manner a main duct 1 used to convey the ventilation and anesthesia gases and an auxiliary channel 2, which is incorporated into the duct 1, and used for inflating a balloon 5, capable of ensuring sealing with the tissues of the trachea.
Conformément à l'invention, le tube comporte, en avant du ballon gonflable 5, un bouclier thermique souple 4.In accordance with the invention, the tube comprises, in front of the inflatable balloon 5, a flexible heat shield 4.
Sur la figure 1, ce bouclier thermique est formé par un disque solidaire dudit tube, orthogonalement à l'axe de celui-ci.In Figure 1, this heat shield is formed by a disc integral with said tube, orthogonally to the axis thereof.
On the other hand, in Figures 2 and 3, the embodiment of the heat shield shown consists of several sectors 4 secured at their base to the duct 1 and partially overlapping, in the manner of petals. The sectors 4 are easily deformed when crossing narrow passages, but resiliently return to their natural position when the tube is in place in the trachea.
The tube can be manufactured in different materials: natural or artificial elastomers, such as natural rubber, synthetic polyisoprenes, polychloroprenes, polybutadienes-styrenes; polybutadienes, either pure or in combination, may possibly be suitable. It is the same for plasticized polyvinyl chlorides, polyurethanes.
mais il est préférable d'utiliser des polysiloxanes, tout particulièrement résistant à l'élévation de température et dont la dégradation thermique n'entraîne pas la formation de vapeurs caustiques ou toxiques. but it is preferable to use polysiloxanes, very particularly resistant to the rise in temperature and whose thermal degradation does not lead to the formation of caustic or toxic vapors.
In practice, it is sufficient to protect the balloon 5 against the laser, that the silicone tube is fitted above the balloon, with a shield 4 of diameter slightly smaller than the diameter of the trachea to be intubated and of a thickness of about 6 to 8/10 mm, which is for example made of silicone loaded with aluminum powder.
Experimental tests have shown that a proportion of 1% of aluminum by weight makes it possible to guarantee resistance to 100 pulses of 5/10 of a second to the laser beam.
However, it is often advantageous to produce the entire elastomer tube loaded with metallic powder, used for the manufacture of shield 4.
