Abstract
Un dispositivo de ayuda (1) para personas con discapacidad motora, particularmente adecuado para permitir la marcha de los amputados transfemorales, que comprende: - una prótesis (2) para un miembro amputado; y - una órtesis (3) adecuada para ser utilizada en otra parte del cuerpo, en el que dicha prótesis (2) y dicha órtesis (3) están operativamente conectadas entre sí, y en el que dicho dispositivo de ayuda (1) comprende además una unidad de control (8) en comunicación con dicha prótesis (2) y dicha órtesis (3), caracterizado por que dicho dispositivo de ayuda comprende además un sistema sensorial, estando dicho sistema sensorial en comunicación con dicha unidad de control (8).

primitiva motora seleccionada ejecutándose, las articulaciones de la prótesis y la órtesis se accionan preferentemente a través de una estrategia de control de impedancia. Para la órtesis está disponible una estrategia alternativa de ʺcontrol del par de la articulación por impedancia ceroʺ. Este modo se utiliza para proporcionar el par de asistencia con una impedancia de salida mínima, cuando sea necesario. Por último, pero no menos importante, la salida d) del algoritmo de fusión multisensorial, el "estado del sistema amputado-dispositivo", se introduce en el bloque de la unidad de retroalimentación eferente. En la presente realización, como se ha comentado previamente, el estado se codifica en una estimulación temporalmente discreta vibrotáctil de un sitio funcional del amputado (por ejemplo, muñón del amputado).
Por lo tanto, la unidad de control 8 del dispositivo de ayuda 1 se basa en primitivas motoras como elementos fundamentales, dotando por consiguiente al dispositivo con un comportamiento semiautónomo para planificar el movimiento de las articulaciones de la prótesis y la acción de asistencia del módulo de órtesis. El dispositivo es capaz de desarrollar habilidades cognitivas de alto nivel, interconectadas con el amputado a través de una interacción bidireccional.
Además, la unidad de control 8 evalúa la condición de estrés psicofisiológico posible del amputado y en línea adapta la estrategia de asistencia, así como el patrón de marcha. La unidad de control 8 también proporciona al usuario una retroalimentación eferente aumentada en el estado amputado-dispositivo, favoreciendo así la aparición de una sensación de propiedad del cuerpo (eficiencia cognitiva).
Algunos de los componentes introducidos hasta ahora se muestran con más detalle en las Figuras 3A y 3B.
La implementación específica del dispositivo 1 mostrado en las Figuras 3A y 3B comprende una prótesis 2 con dos grados de libertad activos, el módulo 32 para la pelvis con dos grados de libertad activos para la asistencia de la flexión-extensión en cada cadera, y el módulo 31 ortopédico del miembro -concretamente, una órtesis de rodillatobillo-pie -con dos grados de libertad activos para ayudar a la flexión-extensión de las articulaciones de la rodilla y el tobillo del miembro sano.
La prótesis 2 está conectada al módulo 31 para la pelvis por medio de un acoplamiento mecánico pasivo que se detallará más adelante.
La prótesis 2 también está conectada a un encaje del muslo 20 por un tubo convencional 200 para la aplicación protésica.
El módulo 32 para la pelvis comprende una estructura 320 en forma de C que contiene -para cada articulación de la cadera -un accionador de rotación, en particular, un motor, para ayudar a la flexión-extensión de la cadera. Estos dos accionadores están representados esquemáticamente en la Figura 3A y 3B y se indican como 555 y 556, respectivamente.
Como se ha mencionado previamente, el módulo 32 para la pelvis también está dotado de dos grados de libertad pasivos para cada articulación de la cadera, a saber, intra-extra rotación y aducción-abducción. La estructura 320 en forma de C del módulo 32 para la pelvis también contiene dos articulaciones pasivas para la intra-extra rotación y la abducción-aducción.
La estructura 320 en forma de C también está conectada a las carcasas 325 ortopédicas para la pelvis adaptada, fijadas entre sí por correas 326. Las carcasas 325 y las correas 326 permiten el acoplamiento mecánico de la estructura 320, y en general, del módulo 32 para la pelvis, con la pelvis del usuario.
Para el miembro amputado, el accionador 555 de flexión-extensión de la cadera transfiere el par de asistencia al muñón por medio de una barra rígida 50 conectada a la prótesis 2 por medio del acoplamiento mecánico pasivo mencionado anteriormente. Para el miembro sano, el accionador 556 de flexión-extensión de la cadera transfiere el par de asistencia al muslo por medio de una barra rígida 57 conectada a un manguito ortopédico 550. La barra 57, que es paralela al muslo del miembro sano, se conecta entonces a la órtesis 31 de rodilla-tobillo-pie.
La órtesis 31 de rodilla-tobillo-pie comprende dos unidades de accionamiento de rotación -indicadas como 53 y 54 para mantener la coherencia con la Figura 1 -para la ayuda de la flexión-extensión de la rodilla y el tobillo, respectivamente. Estas dos unidades 53 y 54 de accionamiento de rotación – en base a los motores de rotación -se acoplan mediante un enlace 58 de barra rígida que se interrelaciona con la pierna del miembro sano por medio de un manguito ortopédico 551 adicional. Por último, el motor del tobillo de la unidad 54 mueve un enlace rígido que está acoplado mecánicamente al zapato del miembro sano y que se indica como 310 para mantener la coherencia con la Figura 1.
En el ejemplo de las Figuras 3A y 3B, el conjunto de baterías 6 y la unidad de control 8 del dispositivo 1 se contienen en una mochila 60 que es llevada por el usuario por medio de correas.
El peso del dispositivo 1 se transfiere parcialmente a la espalda del amputado por medio de correas 328.

Por último, otras correas 327, que pasan por debajo de la ingle, evitan que el sistema se deslice hacia arriba como consecuencia de la interacción de dispositivo 1 con el terreno.
Naturalmente, las realizaciones variantes pueden proporcionar diferentes medios para dicha transferencia principal a la espalda y/o para evitar dicho deslizamiento.
Las Figuras 4A y 4B hacen referencia a una realización diferente del dispositivo de acuerdo con la invención, globalmente indicado como 10, en el que se proporciona únicamente un módulo 311 ortopédico para el muslo, es decir, el módulo ortopédico no se extiende ni a la rodilla y ni a la pierna.
Los componentes restantes, los grados de libertad y los modos de operación del dispositivo 10 pueden ser idénticos a los que ya se han descrito en conjunción con las Figuras 1 a 3B.
Se apreciará que el dispositivo de ayuda descrito anteriormente proporcione un sistema de múltiples grados de libertad con un reemplazo de miembros inferiores y las capacidades de asistencia y permite al usuario utilizar la ayuda de robot sobre una jornada completa.
Actualmente se entenderá mejor que el dispositivo de ayuda de la invención, particularmente, en las realizaciones descritas anteriormente, mejora la eficiencia del amputado en un punto de vista tanto metabólico como cognitivo.
En lo que se respecta a la eficiencia metabólica, el dispositivo permite la disminución de la carga cardiovascular y muscular en el amputado, para permitirle utilizar la ayuda robótica en una jornada completa. Esto se consigue a través de la presencia de la prótesis transfemoral activa, que persigue el reemplazo funcional de la función de propulsión del miembro amputado a través de las articulaciones de la rodilla y del tobillo activas, y el aumento de potencia utilizable del dispositivo ortopédico en el miembro contralateral, que soporta la pierna sana "débil" en el difícil cometido de compensar el déficit de eficiencia introducido por la prótesis activa.
En lo que se refiere a la eficiencia cognitiva, gracias a la unidad de control y a la unidad de retroalimentación eferente, el dispositivo asegura la carga cognitiva más baja para el amputado. Esto se alcanza principalmente mediante el reparto del esfuerzo cognitivo del control entre el usuario y el robot. El dispositivo se comporta en gran medida como un par de piernas robóticas semiautónomas, inteligentes y bioinspiradas, es decir, las piernas protésicas y ortopédicas. La pierna protésica reemplaza funcionalmente la pierna amputada biológica. El dispositivo ortopédico actúa en paralelo a la pierna sana del amputado. En ambos casos, un enfoque de control bioinspirado en base a las primitivas motoras asegura que la pierna protésica se comporte como una pierna normal y que el dispositivo ortopédico coopere naturalmente con el miembro sano, con muy poca necesidad de control consciente por parte del usuario.
En particular, el dispositivo es capaz de inferir con la intención motora del amputado -y después utilizarla para controlar tanto la prótesis como la órtesis -mediante el procesamiento de la información procedente de la interfaz humano-robot. Dicha interfaz humano-robot (que conecta las intenciones de movimiento del amputado con el movimiento real de los módulos robóticos) se basa en un seguimiento completo del propio movimiento del usuario obtenido a través de un aparato de detección generalizado, miniaturizado, distribuido para seguir toda la cinemática relevante y los datos dinámicos a partir de: el propio robot, el miembro contralateral del usuario, las caderas y la parte superior del cuerpo; la interacción del dispositivo y los pies del amputado con el suelo; y las fuerzas de interacción en la interfaz física entre el amputado y el dispositivo ortopédico. Las intenciones motoras del usuario se deducen -y luego se utilizan para controlar el robot -a partir del movimiento del resto del cuerpo, por lo que se requiere que el amputado haga poco o ningún esfuerzo consciente para controlar el dispositivo y cooperar con este. La intención del movimiento de alto nivel detectada se utiliza para controlar la tarea funcional global del dispositivo (por ejemplo, avanzar, retroceder, detenerse, subir un escalón). Aparte de este comando, la prótesis se comporta de forma autónoma con respecto al control de las articulaciones accionadas individuales.
Mediante el procesamiento de la información procedente de la interfaz humano-robot, el dispositivo es capaz de detectar cuándo el amputado va a tropezar o está cerca de caerse. Después de haber detectado este riesgo, el sistema de control del dispositivo puede ayudar al amputado a ejecutar la acción de recuperación apropiada. Gracias a esta habilidad, los amputados transfemorales que vayan a utilizar el dispositivo no se sienten frustrados por pensar continuamente acerca de andar y moverse con la prótesis.
Mediante el seguimiento de algunos de los parámetros fisiológicos, como la temperatura de la piel, la conductancia y frecuencia cardíaca, el dispositivo es capaz de evaluar la posible condición de estrés psicofisiológico del amputado. La idea es que si el dispositivo se da cuenta de que el amputado se encuentra bajo estrés (físico o mental), puede actuar sobre las estrategias de control al cambiar el tipo de asistencia (por ejemplo, aumento de la cantidad de la asistencia del par) o los parámetros de la marcha (por ejemplo, reducción de la cadencia de la marcha). Finalmente, el dispositivo es capaz de proporcionar al usuario una retroalimentación eferente aumentada en el estado amputadodispositivo. Proporcionando al amputado una retroalimentación en el estado del sistema se contribuye a que perciba el dispositivo como parte de su propio cuerpo, es decir, favorece el sentido de propiedad del cuerpo.
