Abstract

Die Erfindung betrifft eine für eine Gelenkprothese (1) bestimmte, in einer Knochenstruktur (6) fixierbare Gelenkpfanne (2) mit einem Pfannen-Einsatz (3). Die Gelenkpfanne (2) hat eine in ihrer Grundform zylindrische Außenkontur. An einem einer Öffnung der Gelenkpfanne (2) abgewandten Abschnitt ist die Außenkontur kreisrohrförmig als umlaufender Rand ausgeführt und bildet dadurch einen einer lediglich beispielhaft dargestellten Knochenstruktur (6) zugewandten Vorsprung (7). Der Vorsprung (7) ist konzentrisch zu dem Pfannen-Einsatz (3) und wird zur Fixierung der Gelenkpfanne (2) in eine nutenförmige Aussparung (9) der Knochenstruktur (6) eingesetzt, welche den Pfannen-Einsatz (3) umfangsseitig konzentrisch mit einem geringen radialen Abstand einschließt. Auf diese Weise bildet der Vorsprung (7) eine Ringhülse um den Pfannen-Einsatz (3), wodurch die Belastbarkeit der aufgrund des Substanzverlusts infolge der Vertiefung (8) in der Knochenstruktur (6) für die Gelenkpfanne (2) geschwächten Knochenstruktur (6) ausgeglichen wird, indem sowohl radiale Spreizkräfte aufgrund der Fixierung der Gelenkpfanne (2) als auch äußeren Krafteinwirkungen aufgenommen werden und die Knochenstruktur (6) nicht weiter belastet wird.

Description

Gelenkpfanne für eine Gelenkprothese
Die Erfindung betrifft eine in einer Knochenstruktur fixierbare, einen Pfannen-Einsatz zur Aufnahme eines zumindest abschnittsweise sphärischen Gelenkkörpers aufweisende Gelenkpfanne für eine Gelenkprothese.
Gelenkprothesen mit in die Knochensubstanz einschraubbaren Verankerungsteilen sind bereits in verschiedenartigen Ausführungen und Anwendungen, insbesondere bei Hüftgelenks-Prothesen bekannt. Dabei wird die Gelenkpfanne mit der verjüngten Seite voran in eine operativ vorbereitete Vertiefung im Knochen eingesetzt.
In der Praxis werden üblicherweise konisch geformte oder sphärische Gelenkpfannen eingesetzt, die jeweils eine metallische Außenschale und einen in der Schale angeordneten Einsatz oder ein Inlay aufweisen, die üblicherweise aus Keramik, Kunststoff oder Metall hergestellt sind.
Ein Vorteil bei der Verwendung der sphärischen Pfannen liegt darin, dass beim Vorfräsen der sphärischen Vertiefung weniger Knochensubstanz des Beckenknochens entfernt werden muss als bei einer konisch geformten Pfanne. Auch das Fräsen selbst ist bei den sphärischen Pfannen einfacher, da die Orientierung nur geringen Einfluss hat.
Neben der Standardausführung der sphärischen Pfannen, welche einer Halbkugel entspricht, sind auch Pressfitausführungen im Einsatz. Die Pressfitpfannen sind am Pol leicht abgeflacht und im Durchmesser etwas größer als das vorgefräste halbkugelförmige Bett im Knochen. Dadurch wird der Presssitz der Pfanne im vorgefrästen Knochen gewährleistet. Eine raue Oberflächenbeschichtung der gebräuchlichen Schalen aus Metallen wie zum Beispiel Reintitan erleichtert das Anwachsen der Knochenzellen an das Implantat. Dies soll eine optimale Stabilisierung sicherstellen. Im Gegensatz zu den Schraubpfannen werden die sphärischen Pfannen ohne Gewinde und vor allem die Pressfitpfannen in einer linearen Bewegung in das vorgefräste Bett eingeschlagen oder eingepresst.
Aus der EP 0 169 978 B1 ist eine aus einem inneren Pfannenkörper und einer Außenschale bestehende, für eine zementfreie Verankerung geeignete Endoprothese für eine Hüftgelenkspfanne bekannt, bei der der Pfannenkörper mit einem konischen Mantel und die Außenschale mit einem an den Mantel angepassten Hohlraum versehen ist, wobei die Außenschale und der Pfannenkörper über eine Verriegelungsstruktur ineinander gehalten sind. Auf dem Umfang der Außenschale sind mehrere Schlitze gleichmäßig verteilt, wobei die Oberfläche der Außenschale mit einer aus Vorsprüngen bestehenden Struktur versehen ist. Die Vorsprünge sind stiftförmig ausgebildet und auf die Oberfläche der Spreizpfanne aufgesetzt.
Aus der EP 0 242 633 B1 ist eine aus einem inneren Pfannenkörper und einer Außenschale bestehende, für eine zementfreie Verankerung geeignete Endoprothese für eine Hüftgelenkspfanne bekannt, welche im zum Äquator gelegenen Bereich der Außenschalenoberfläche eine aus mehreren peripheren Reihen angeordneten Vorsprüngen bestehende Struktur aufweist. Die Vorsprünge sind dornenförmig geformt und auf konzentrischen Kreisen auf der Oberfläche der Spreizpfanne angeordnet.
Die DE 35 18246 A1 bezieht sich auf eine Hüftgelenkspfannen-Prothese mit einer kegeligen Außenform zum Einsatz in eine entsprechend ausgeformte Bohrung in einem Knochen, wobei die Konizität der kegeligen Form für einen dauerhaften Eingriff mit dem umgebenden Knochen ausgelegt ist.
Bei der aus der DE 2925 089 A1 bekannten Gelenkprothese sind ein innerer Napf und ein äußerer Ring vorgesehen, wobei der Napf einer ringförmigen Bandagierungsbelastung ausgesetzt ist, die auf den Napf durch den äußeren Ring entweder direkt oder mittels eines eventuell zwischengeschalteten Zwischenringstücks ausgeübt wird, welches zwischen dem Napf und dem Ring angeordnet ist.
Die DE 3347 983 C2 betrifft eine Gelenkpfannen-Prothese, mit einer an einem Knochen fixierbaren Schale und mit einem in die Schale einsetzbaren, aus zwei Lagersegmenten zusammengesetzten Einsatz, welche die Gelenkkugel ohne jegliche Teilung einschließen, wobei ein geschlitzter Ring elastisch aufweitbar ist, sodass er über die Gelenkkugel geschoben werden kann. Die DE 102006 019620 B4 bezieht sich auf eine künstliche Gelenkpfanne mit einer Anzahl von radialen Durchgangslöchern für den Durchlass jeweils einer Knochenschraube zur Verankerung der Pfanne im angrenzenden Knochenbereich.
Aus der WO 1985 / 02 535 A1 ist bereits eine künstliche Gelenkpfanne für ein Hüftgelenk bekannt mit einer eine kugelkalottenförmige Innenfläche aufweisenden Schale und mit einem hohlzylindrischen Verankerungsteil, der einerseits an der Schale anliegt, andererseits an seinem der Schale abgewandten freien Ende offen ist.
Die US 11 478 358 B2 beschreibt ein Implantat mit einer Schale mit einem Körper, der eine dem Knochen zugewandte Aussparung definiert, wobei die dem Knochen zugewandte Aussparung eine Ringoberfläche und eine konvexe Oberfläche umfasst. Die Ringoberfläche hat ein Profil, das dem äußeren Knochenring entspricht, und kann Halteelemente, beispielsweise Vorsprünge oder Aussparungen, aufweisen, die in den Knochen eingreifen.
Als nachteilig beim Stand der Technik erweist sich, dass die Knochen- Implantate die Knochensubstanz infolge der erforderlichen Vertiefungen schwächen, sodass ein erhöhtes Risiko bezüglich möglicher Frakturen entsteht. Dies gilt insbesondere für Revisionsoperationen, die in der Regel mit einem erneuten Knochensubstanzverlust verbunden sind.
Nachteilig an Spreizpfannen für Hüftgelenksprothesen ist insbesondere, dass die Vorsprünge auf der Spreizpfanne so angeordnet sind, dass Kräfte, die entgegen der Implantationsrichtung der Spreizpfanne im Knochengewebe wirken, dazu führen können, dass sich die Spreizpfanne aus dem Knochengewebe löst.
Besonders nachteilig ist die Wirkrichtung der Fixierkräfte, die bei den meisten in der Praxis eingesetzten Fixationsoptionen im Wesentlichen radial nach außen gerichtet sind, sodass Spreizkräfte entstehen. Diese Spreizkräfte können eine Ursache für eine nicht selten auftretende Verminderung der Haltekraft der Gelenkpfanne in der Knochenstruktur sein.
Der Erfindung liegt die Aufgabe zugrunde, eine Möglichkeit zu schaffen, die unvermeidliche Schwächung der Knochensubstanz durch die Gestaltung der Gelenkpfanne weitgehend auszugleichen, sodass die Belastbarkeit der Knochenstruktur nicht reduziert, sondern verbessert ist. Insbesondere soll die Einleitung radialer Spreizkräfte in die die Gelenkpfanne umgebende Knochenstruktur vermieden werden. Diese Aufgabe wird erfindungsgemäß mit einer Gelenkpfanne gemäß den Merkmalen des Anspruchs 1 gelöst. Die weitere Ausgestaltung der Erfindung ist den Unteransprüchen zu entnehmen.
Erfindungsgemäß ist also eine Gelenkpfanne vorgesehen, bei der die Gelenkpfanne zumindest einen der Knochenstruktur zugewandten, zum Eingriff in eine zu der Vertiefung für den Pfannen-Einsatz in der Knochenstruktur radial beabstandete, nutenförmige Aussparung der Knochenstruktur bestimmten Vorsprung hat, welcher den Pfannen-Einsatz umfangsseitig, insbesondere konzentrisch, mit einem geringen Abstand einschließt, wobei der Vorsprung zumindest abschnittsweise eine konvexe Außenfläche, begrenzt durch eine äußere ringförmig geschlossene Kontur, sowie eine konkave Innenfläche, begrenzt durch eine innere ringförmig geschlossenen Kontur, aufweist, sodass der Vorsprung mit der Außenfläche und der Innenfläche in die nutenförmige Aussparung formschlüssig einsetzbar ist und die Außenfläche sowie die Innenfläche jeweils gegen eine die nutenförmige Aussparung begrenzende Wandfläche anliegen.
Die Erfindung geht von der Erkenntnis aus, dass die Belastbarkeit der aufgrund des Substanzverlusts für die Aufnahme der Gelenkpfanne geschwächten Knochenstruktur dadurch wiederhergestellt werden kann, dass ein oder mehrere Vorsprünge der Gelenkpfanne nach dem Prinzip einer Ringklammer oder einer Manschette den Pfannen- Einsatz einschließen. Insbesondere soll die Einleitung radialer Spreizkräfte in die die Gelenkpfanne umgebende Knochenstruktur vermieden werden.
Der erfindungsgemäße Effekt beruht darauf, dass die Belastbarkeit des die Gelenkpfanne aufnehmenden Knochens, beispielsweise des Beckenknochens, verbessert wird, indem die Kräfte, die quer zur Achse wirken, nämlich einerseits die Spreizkräfte aufgrund der eingesetzten Gelenkpfanne und andererseits aufgrund der äußeren Krafteinwirkung, insbesondere infolge der Gelenkbelastung, durch den Vorsprung zuverlässig aufgenommen werden können und somit zusätzliche Krafteinwirkungen in die Knochenstruktur vermieden werden. Beispielsweise werden auch Biegekräfte des Knochens aufgenommen, indem Dehnungen in der Ebene der Knochenfläche von dem zumindest einen Vorsprung aufgenommen werden. Erstmals kann die Prothese somit auch Zugkräfte quer zur Symmetrieachse der Gelenkpfanne aufnehmen. Dadurch entsteht eine Verklammerung in der Fläche und der eingeschlossene, den Schaleneinsatz tragende Bereich bleibt frei von unerwünschten Krafteinwirkungen. Zugleich werden auch die erzielbaren Fixierkräfte der Gelenkpfanne gesteigert, sodass diese auch bei starker Belastung zuverlässig fixiert bleibt und sich nicht aus der Knochenstruktur herauslöst, weil die Haltekräfte in dem Knochen erheblich gesteigert sind. Es hat sich gezeigt, dass die Fixierung sogar das Fortschreiten einer Fraktur der Knochenstruktur verhindern kann und die Gelenkpfanne auch bei Auftreten einer solchen Fraktur sicher fixiert bleibt. Aufgrund der im Vergleich zum Stand der Technik wesentlich erhöhten Haltekräfte kann die zu entfernende Knochenstruktur auf ein Minimum beschränkt werden.
Dabei hat es sich bereits als besonders vorteilhaft erwiesen, wenn der Vorsprung ringförmig geschlossen ist und den Pfannen-Einsatz umfangsseitig einschließt. Hierdurch ist die Wirkung der umfangsseitigen Verklammerung richtungsunabhängig, wodurch zugleich die Fixierung des Vorsprungs in der nutenförmigen Aussparung vereinfacht wird. In der Praxis führen bereits die entstehenden Klemmkräfte zu einer ausreichenden Fixierung.
Darüber hinaus ist es besonders Erfolg versprechend, wenn der Vorsprung einen kreisringförmigen Querschnitt mit konstanter Wandstärke aufweist und die Gelenkpfanne konzentrisch einschließt. Durch die rotationssymmetrische Gestaltung des kreisrohrförmigen oder kegelförmigen Vorsprungs sind die erreichbaren Haltekräfte unabhängig von der Winkelstellung, sodass die Gelenkpfanne in beliebigen Winkelstellungen eingesetzt werden kann und dadurch die Durchführung der Operation erleichtert wird. Zum Einbringen der nutenförmigen Aussparung kann in besonders einfacher Weise ein Kronenbohrer, vorzugsweise in Verbindung mit einem Tiefenanschlag, eingesetzt werden, sodass der mit der Verwendung von Fräswerkzeugen verbundene Aufwand entfällt.
Bei einer anderen, ebenfalls besonders zweckmäßigen Abwandlung der Erfindung sind mehrere Vorsprünge innerhalb eines ringförmigen Bereichs begrenzt durch die äußere Kontur sowie durch die innere Kontur in Umfangsrichtung zueinander beabstandet angeordnet. Dadurch können mehrere, vorzugsweise geometrisch übereinstimmende Vorsprünge zur Fixierung genutzt werden, um so die Fixierkräfte weiter zu steigern. Hierzu können die Vorsprünge stirnseitige Abwinkelungen oder Abbiegungen aufweisen, die bei einer Drehung der Gelenkpfanne in die Knochenstruktur eindringen. Weiterhin können die Vorsprünge erhöhte Klemmkräfte übertragen, wenn zumindest einzelne der Vorsprünge beispielsweise einstellbar sind, um diese innerhalb der Aussparung gegenüber den begrenzenden Wandflächen zu verspannen.
Besonders bevorzugt sind mehrere zueinander beabstandete Vorsprünge gleichverteilt am Umfang angeordnet, sodass die Haltekräfte gleichmäßig über den gesamten Umfang in die Knochenstruktur eingeleitet werden. Dabei können die benachbarten Vorsprünge eine unterschiedliche Geometrie und auch eine unterschiedliche Länge aufweisen. Besonders vorteilhaft ist es hingegen, wenn die axiale Erstreckung des Vorsprungs in Umfangsrichtung konstant ist und/oder die axiale Erstreckung benachbarter, insbesondere aller Vorsprünge übereinstimmt, sodass die Gelenkpfanne eine durch die nutenförmige Aussparung vorgegebene Tiefenposition innerhalb der Knochenstruktur sicher erreicht und diese dauerhaft eingehalten wird.
Darüber hinaus ist gemäß einer weiteren sinnvollen Abwandlung der Erfindung der zumindest eine Vorsprung in axialer Richtung parallel zu der Symmetrieachse der Gelenkpfanne in unterschiedlichen Positionen, insbesondere stufenlos festlegbar. Hierdurch kann mittels des Vorsprungs die Tiefenposition der Gelenkpfanne eingestellt werden, sodass eine optimale Positionierung erzielt werden kann. Indem bei mehreren am Umfang verteilten Vorsprüngen die axiale Tiefenposition unterschiedlich eingestellt wird, kann darüber hinaus auch bedarfsweise ein gewünschter Neigungswinkel der Gelenkpfanne eingestellt werden.
Bei der praktischen Erprobung hat es sich bereits als zweckmäßig erwiesen, wenn der radiale Abstand des Vorsprungs gegenüber dem Pfannen-Einsatz zwischen 0,5 mm und 10 mm, die Tiefe der nutenförmigen Aussparung zwischen 5 mm und 35 mm und der Abstand zwischen der konvexen Außenfläche und der konkaven Innenfläche zwischen 1 mm und 5 mm beträgt.
Eine weitere, besonders vorteilhafte Ausführungsform der Erfindung wird dann erreicht, wenn der zumindest eine Vorsprung an seiner Außenfläche und/oder seiner Innenfläche Einwachsöffnungen hat. Diese auch als Einwachsbuchten bezeichneten Einwachsöffnungen fördern das Einwachsen der umgebenden Knochensubstanz und bilden bereits nach kurzer Zeit Hinterschneidungen mit der Gelenkpfanne, die dadurch zuverlässig fixiert wird.
Als besonders Erfolg versprechend hat sich auch bereits eine Variante der Erfindung erwiesen, bei welcher die Gelenkpfanne einen Halterahmen mit einer Durchlassweite geringer als der Großkreisdurchmesser des Pfanneneinsatzes aufweist. Dadurch wird ein in den Pfanneneinsatz eingesetzter sphärischer Gelenkkörper durch den Halterahmen gesichert, sodass eine Luxation des Gelenks verhindert wird.
Vorzugsweise ist die Durchlassweite entgegen einer elastischen Rückstellkraft des Halterahmens auf den Großkreisdurchmesser erweiterbar, sodass der Gelenkkörper nach dem Prinzip eines Schnappverschlusses in dem Pfanneneinsatz gesichert ist. Indem die Gelenkpfanne eine der Knochenstruktur innerhalb der Vertiefung zugewandte Unterseite aufweist, die in der implantierten Position beabstandet zu der Knochenstruktur in die Vertiefung eingesetzt ist, wird ein Spalt erzeugt, sodass die Fixierkräfte ausschließlich oder zumindest weitgehend mittels des Vorsprungs in die Aussparung der Knochenstruktur eingeleitet werden.
Die Erfindung lässt verschiedene Ausführungsformen zu. Zur weiteren Verdeutlichung ihres Grundprinzips ist eine davon in der Zeichnung dargestellt und wird nachfolgend beschrieben. Diese zeigt in

Eine Gelenkprothese 1 mit einer erfindungsgemäßen Gelenkpfanne 2, die einen Pfannen- Einsatz 3 zur Aufnahme eines nicht gezeigten Gelenkkörpers einschließt, wird nachstehend anhand der Figuren 1 bis 13 näher erläutert. Die Gelenkpfanne 2 hat eine in ihrer Grundform zylindrische Außenkontur 4. An einem einer Öffnung 5 der Gelenkpfanne 2 abgewandten Abschnitt ist die Außenkontur 4 kreisrohrförmig als umlaufender Rand ausgeführt und bildet dadurch einen einer lediglich beispielhaft dargestellten Knochenstruktur 6 zugewandten Vorsprung 7. Der Vorsprung 7 ist konzentrisch zu dem Pfannen-Einsatz 3, wie insbesondere in den Figuren 4 und 5 zu erkennen. Der Vorsprung 7 dient der Fixierung der Gelenkpfanne 2 in einer für den Pfannen-Einsatz 3 bestimmten Vertiefung 8 und wird hierzu in der Knochenstruktur 6 in eine die Vertiefung 8 radial einschließende, nutenförmige Aussparung 9 der Knochenstruktur 6 eingesetzt, wodurch der Vorsprung 7 den Pfannen-Einsatz 3 konzentrisch mit einem geringen radialen Abstand einschließt. Hierzu ist der Vorsprung 7 mit seiner konvexen Außenfläche 10 und seiner konkaven Innenfläche 11 gegen eine jeweilige Wandfläche der nutenförmigen Aussparung 9 kraftschlüssig anlegbar. Dadurch bildet der Vorsprung 7 eine Spannhülse um den Pfannen-Einsatz 3, wodurch die Belastbarkeit der aufgrund des Substanzverlusts infolge der Vertiefung 8 in der Knochenstruktur 6 für die Gelenkpfanne 2 geschwächten Knochenstruktur 6 wiederhergestellt wird, indem sowohl radiale Spreizkräfte aufgrund der Fixierung der Gelenkpfanne 2 als auch äußere Krafteinwirkungen aufgenommen werden und die Knochenstruktur 6 nicht weiter belastet wird. Wie insbesondere in den Figuren 1, 2 und 5 zu erkennen, hat der Vorsprung 7 an seiner Außenkontur 4 mehrere Einwachsöffnungen 12, in welche die umgebende Knochensubstanz 6 einwächst und dadurch eine die Gelenkpfanne 2 fixierende Hinterschneidung bildet.
Wie insbesondere in den Figuren 6 und 13 zu erkennen, ist eine Unterseite 13 der Gelenkpfanne 2 in der Knochenstruktur 6 innerhalb der Vertiefung 8 in der implantierten Position mit einem Abstand A zu der Knochenstruktur 6 fixiert, um so die Krafteinleitung auf den Vorsprung 7 zu konzentrieren.
In Figur 11 ist ein Halterahmen 14 dargestellt, der in den Figuren 9, 10, 12 und 13 in einer montierten Position erkennbar ist. Der Halterahmen 14 hat eine geringere Durchlassweite D als der Großkreisdurchmesser des Pfanneneinsatzes. Dadurch wird ein in den Pfanneneinsatz eingesetzter sphärischer Gelenkkörper durch den Halterahmen gesichert, sodass eine Luxation des Gelenks verhindert wird. Der Halterahmen 14 weist seinerseits zwei Vorsprünge 7‘ auf, die mit den Vorsprüngen 7 der Gelenkpfanne 2 konturbündig verbunden werden, wie in den Figuren 9 und 10 erkennbar.
In den Figuren 14 und 15 ist noch eine weitere Variante einer weiteren erfindungsgemäßen Gelenkpfanne 2 mit einem exzentrisch ausgeführten Pfanneneinsatz 3 in einer perspektivischen Darstellung und in einer geschnittenen Seitenansicht dargestellt. Die Gelenkpfanne 2 hat eine in ihrer Grundform zylindrische Außenkontur 4 mit einem kreisringförmigen Rand als Vorsprung 7, der konzentrisch zu der Außenkontur 4 des Pfannen-Einsatzes 3 angeordnet ist. Eine Aufnahme 16 für eine nicht gezeigte Gelenkkugel ist nicht koaxial, sondern gegenüber der Außenkontur 4 mit einem Versatz V einer Mittelachse 17 der Aufnahme 16 gegenüber einer Mittelachse 15 der Gelenkpfanne 2 exzentrisch angeordnet, sodass sich in verschiedenen Umfangsabschnitten unterschiedliche Randabstände R, r ergeben. Durch eine Änderung der Winkelposition des Vorsprungs 7 innerhalb der beispielsweise in der Figur 13 gezeigten nutenförmigen Aussparung 9 kann so in einfacher Weise die Mittelachse 17 der Aufnahme 16 optimal zu der Knochenstruktur 6 ausgerichtet und positioniert werden. Claims

PATENTANSPRÜCH E
1. Eine in einer Knochenstruktur (6) fixierbare, einen Pfannen-Einsatz (3) zur Aufnahme eines zumindest abschnittsweise sphärischen Gelenkkörpers aufweisende Gelenkpfanne (2) für eine Gelenkprothese (1), wobei die Gelenkpfanne (2) zumindest einen der Knochenstruktur (6) zugewandten, zum Eingriff in eine zu der Vertiefung (8) für den Pfannen- Einsatz (3) in der Knochenstruktur (6) radial beabstandete, nutenförmige Aussparung (9) der Knochenstruktur (6) bestimmten Vorsprung (7) hat, welcher den Pfannen-Einsatz (3) umfangsseitig insbesondere konzentrisch einschließt, und wobei der Vorsprung (7) zumindest abschnittsweise eine konvexe Außenfläche (10) sowie eine konkave Innenfläche (11) aufweist, sodass der Vorsprung (7) mit der Außenfläche (10) und der Innenfläche (11) in die nutenförmige Aussparung (9) formschlüssig einsetzbar ist.
2. Gelenkpfanne (2) nach Anspruch 1, dadurch gekennzeichnet, dass der Vorsprung (7) ringförmig geschlossen ist und den Pfannen-Einsatz (3) umfangsseitig einschließt.
3. Gelenkpfanne (2) nach Anspruch 1 oder 2, dadurch gekennzeichnet, dass der Vorsprung (7) rohrförmig, insbesondere kreisrohrförmig, ausgeführt ist und den Pfannen- Einsatz (3) konzentrisch einschließt.
4. Gelenkpfanne (2) nach zumindest einem der vorhergehenden Ansprüche, dadurch gekennzeichnet, dass mehrere Vorsprünge (7) innerhalb eines ringförmigen Bereichs begrenzt durch die äußere Kontur sowie durch die innere Kontur in Umfangsrichtung zueinander beabstandet angeordnet sind.
5. Gelenkpfanne (2) nach Anspruch 3, dadurch gekennzeichnet, dass mehrere zueinander beabstandete Vorsprünge (7) gleichverteilt am Umfang angeordnet sind.
6. Gelenkpfanne (2) nach zumindest einem der vorhergehenden Ansprüche, dadurch gekennzeichnet, dass die axiale Erstreckung des Vorsprungs (7) in Umfangsrichtung konstant ist und/oder die axiale Erstreckung benachbarter, insbesondere aller Vorsprünge übereinstimmt.
7. Gelenkpfanne (2) nach zumindest einem der vorhergehenden Ansprüche, dadurch gekennzeichnet, dass der zumindest eine Vorsprung (7) in axialer Richtung parallel zu der Symmetrieachse der Gelenkpfanne (2) in unterschiedlichen Positionen, insbesondere stufenlos festlegbar ist.

Gelenkpfanne (2) nach zumindest einem der vorhergehenden Ansprüche, dadurch gekennzeichnet, dass der radiale Abstand des Vorsprungs (7) gegenüber dem Pfannen- Einsatz (3) zwischen 0,5 mm und 10 mm beträgt und/oder der Abstand zwischen der konvexen Außenfläche (10) und der konkaven Innenfläche (11) zwischen 1 mm und 5 mm beträgt.
9. Gelenkpfanne (2) nach zumindest einem der vorhergehenden Ansprüche, dadurch gekennzeichnet, dass die Gelenkpfanne (2) und/oder der zumindest eine Vorsprung (7) an seiner Außenfläche (10) und/oder seiner Innenfläche (11) Einwachsöffnungen (12) aufweist.

Gelenkpfanne (2) nach zumindest einem der vorhergehenden Ansprüche, dadurch gekennzeichnet, dass die Gelenkpfanne (2) einen Halterahmen (14) mit einer insbesondere entgegen einer elastischen Rückstellkraft veränderbaren Durchlassweite (D) geringer als der Großkreisdurchmesser des Pfannen-Einsatzes (3) aufweist.

Gelenkpfanne (2) nach zumindest einem der vorhergehenden Ansprüche, dadurch gekennzeichnet, dass die Gelenkpfanne (2) einer der Knochenstruktur (6) innerhalb der Vertiefung (8) zugewandte Unterseite (13) aufweist, die in der implantierten Position beabstandet zu der Knochenstruktur (6) einsetzbar ist.

Gelenkpfanne (2) nach zumindest einem der vorhergehenden Ansprüche, dadurch gekennzeichnet, dass eine Aufnahme (16) für eine Gelenkkugel gegenüber der Außenkontur (4) mit einem Versatz (V) einer Mittelachse (17) der Aufnahme (16) gegenüber einer Mittelachse (15) der Gelenkpfanne (2) exzentrisch angeordnet ist.
13. Gelenkpfanne (2) nach zumindest einem der vorhergehenden Ansprüche, dadurch gekennzeichnet, dass der Pfannen-Einsatz (3) und der Vorsprung (7) einteilig und/oder einstückig, insbesondere aus einem Kunststoff, hergestellt sind.
