Description

The present invention generally relates to the field of adaptive intraoral protections (PIBs) for protecting individuals against the consequences of craniofacial impact.

By "adaptable" we mean the PIBs able to be shaped in the mouth.

The present invention relates in particular, but not exclusively, to adaptable PIBs by thermo-adaptation to hot water.

The present invention serves, more particularly, but not exclusively, when practicing sports at risk of oral, maxillofacial, cerebral and cervical trauma. It can target other areas such as health for the prevention of oral trauma that can occur during endotracheal intubation.

Sport is one of the main causes of oral trauma. The American Dental Association and the Academy of Sport Odontology have compiled a list of sports and recreational activities at risk for which they recommend the use of a well-adapted GDP, such as, for example, martial arts, baseball, basketball, boxing, cycling, football, gymnastics, weightlifting, handball, hockey, wrestling, parachuting, rugby, skateboarding, skiing, equestrian sports, squash, surfing, volleyball or even water polo.

From its first role as a mucosal protector, GDP has evolved to become first of all a protection of the oral and maxillary structures, then his role extended to the prevention of concussions and cervical injuries during the practice of risky sports.

  • de réduire le risque de lésion des tissus mous (la langue, les lèvres et les joues) ;
  • de réduire le risque de lésion des dents antérieures maxillaires ;
  • de réduire le risque de lésion buccodentaire par choc violent inter-arcades ;
  • de réduire le risque de commotion cérébrale ;
  • de réduire le risque d'atteinte cervicale.
One of the goals of a GDP is to:
  • reduce the risk of soft tissue injury (tongue, lips and cheeks);
  • reduce the risk of injury to maxillary anterior teeth;
  • reduce the risk of oral injury by violent inter-arcade shock;
  • reduce the risk of concussion;
  • to reduce the risk of cervical involvement.

  • d'occuper les espaces édentés ;
  • de permettre un engrènement des arcades dentaires mandibulaire et maxillaire ;
  • d'être confortable ;
  • de ne pas présenter de saillie, ni de rugosité ;
  • d'être rétentive ;
  • d'absorber et dissiper l'énergie d'impact ;
  • de permettre la phonation ;
  • de permettre la ventilation buccale mâchoires serrées.
GDP has moreover, among others, for properties:
  • to occupy edentulous spaces;
  • to allow a meshing of the mandibular and maxillary dental arches;
  • to be comfortable;
  • not to show protrusion or roughness;
  • to be retentive;
  • absorb and dissipate impact energy;
  • to allow phonation;
  • to allow oral ventilation tight jaws.

Retention is the ability of GDP to stay in place when the user opens the mouth.

The term "meshing" means the penetration of the opposing teeth into the occlusal rim of the trough to obtain the wedging of the mandibular and maxillary arches.

On entend par « phonation » la capacité de l'utilisateur à pouvoir s'exprimer oralement."Phonation" means the ability of the user to express himself orally.

  • Type I : modèle standard, uni ou bi-maxillaires. Il s'agit d'une PIB prête à l'emploi. Une telle PIB présente cependant comme inconvénients :
  • de ne pas permettre l'engrènement des arcades dentaires mandibulaire et maxillaire ;
  • de ne pas être rétentive : elle ne tient en place que lorsque le sportif est mâchoires serrées ;
  • de perdre jusqu'à 99% d'épaisseur lors de l'adaptation en bouche, ce qui limite l'absorption et la dissipation de l'énergie d'impact ;
Three types of GDP are currently proposed:
  • Type I: standard model, uni or bimaxillary. It is a ready-to-use GDP. Such a GDP, however, has the following disadvantages:
  • not to allow the meshing of the mandibular and maxillary dental arches;
  • not to be retentive: it only holds in place when the athlete is tight jaws;
  • to interfere with the athlete's ventilation and, moreover, there is a risk that the protection escapes during the effort and gets stuck in the athlete's airway.
  • Type II: adaptable model, uni or bimaxillary, This is a PIB able to be formatted in the mouth of the user. Such a GDP may, however, have the following disadvantages:
  • to lose up to 99% thickness during adaptation in the mouth, which limits the absorption and dissipation of impact energy;
  • to limit mouth ventilation tight jaws.
  • Type III: customized model. This is a GDP performed after taking impressions of the mandibular and maxillary dental arches of the user. The (tailor-made) method of preparation must allow the production of a high-quality GDP in accordance with the essential criteria of objectives and properties recognized for this prevention device. However, depending on

opérateurs, certaines de ces PIB peuvent présenter une qualité inférieure à des modèles de type Il en terme d'épaisseur de matériau et d'aptitude de ventilation mâchoires serrées.operators, some of these PIBs may have lower quality than Type II models in terms of material thickness and tight jaw ventilation ability.

The preservation of the material thicknesses of the PIB is necessary for a correct absorption and dissipation of the energy related to the impacts.

In addition, ventilatory studies have shown that all types of PIBs lead to greater air resistance and significantly reduce the athlete's ability to ventilate tight jaws.

This reduction in tight jaw ventilation results in hypoxia, hypercapnia and the logical result is a decrease in performance.

This reduction in tight jaw ventilation results in an oral opening movement essential to reduce hypoxia and hypercapnia caused by tight jaw strain. But, this movement causes two major consequences:
  • increased risk of trauma after impact on a dislocated mandible of the facial mass, called "mandible projectile", such as dental lesions by inter-arcade shock, a predominantly postural KO or a mandibular fracture consecutive to the percussion of the posterior zone of the temporal eminence by the condylar process.
  • an increased risk of concussion and cervical spine trauma by shaking the muscular guying of the craniofacial and cervical complex and thus a disruption in the maintenance of the head and neck.

The aim is therefore to propose an adaptable PIB to guarantee the user's tight oral jaw ventilation and the preservation of the thicknesses of the PIB after the intra-oral shaping procedure.

Among the many patents on GDP, we know the international patent application WO94 / 27691 in the name of EZ GARD INDUSTRIES, which was published on December 8, 1994, under priority of a US application US08 / 066.468 filed May 1993. This application discloses a composite material protection, comprising a maxillary gutter of a thermo-adaptable material and a skeleton of a non-thermo-adaptable material for absorbing and dissipating the impact forces. The structure of this GDP type II allows that the posterior contact (ie at the bottom of the jaw) on the non-thermo-adaptable material retains an anterior free space (ie in front of the mouth, at the level of the incisors) to ventilate and speak. However, the free space obtained after the shaping of the PIB may be insufficient to allow satisfactory ventilation when the user squeezes the jaws. Moreover, the presence of the only non-thermo-adaptable material facing the lateral sectors makes it impossible to imprint the mandibular dental arch during the shaping procedure and therefore does not allow meshing between the mandibular dental arches and maxillary.

However, the athlete needs on the one hand a sufficient ventilation by performing his jaws tight effort, on the other hand that his mandible is well meshed and secured to the maxillary dental arch to avoid the phenomenon of "mandible projectile".

We also know the international patent application WO98 / 34574 filed in February 1997 on behalf of WIPSS PRODUCTS INC under priority of US application US 5,636,379 filed in August 1995 which also discloses a composite material protection. One of the disadvantages of this product is that the small thickness of thermo-adaptable material around the skeleton of non-thermo-adaptable material does not allow sufficient meshing between the mandibular and maxillary dental arches during the formatting procedure and therefore neither a sufficient wedging of the mandible nor a sufficient retention of the GDP.

The invention aims to remedy these drawbacks by proposing a PIB which, after the shaping procedure, allows tight jaw ventilation to be effective regardless of the dento-maxillo-facial typology of the sportsman, while maintaining minimum thicknesses of material. for optimal GDP participation in the prevention of oral, maxillofacial, cerebral and cervical injuries.

For this purpose, it is therefore proposed, according to a first aspect of the invention, intra-oral protection (PIB), able to be shaped in the mouth, and comprising:
  • an adaptable structure, consisting of an adaptable material, having an upper gutter for receiving the maxillary dental arch, and a lower gutter for receiving the mandibular dental arch,
  • a skeleton, at least partially covered by the adaptable structure, made of a non-adaptable material,
  • the adaptable structure and the skeleton being arranged to form
  • an incisivo-canine protection block at least at the level of the canines and upper incisors,
  • and an occlusal margin at each lateral dental area, ie at the level of the upper and lower premolars and molars on each side,
and which further comprises a removable pallet, made of a non-adaptable material, comprising a first zone intended to bear on the skeleton, preferably at the level of the incisivo-canine block, and not necessarily in direct contact, said zone of the pallet being dimensioned so that, during the adjustment phase of the GDP, the joint presence of said pallet and said skeleton makes it possible to guarantee both a ventilatory space and a sufficient presence of material at the level of the incisivo-canine block.

The term "adaptable material" means that the material becomes malleable during the process of adjusting the PIB.

By "support" is meant that the pallet cooperates with the skeleton to allow both a sufficient thickness of material at the incisivo-canine block and a ventilatory space sufficient for good ventilation of the user. Therefore, there may be a layer of thermo-adaptable material between the pallet and the skeleton (which implies a non-direct contact support) without diminishing the effects of the interaction between the pallet and the skeleton.

The adaptation of the GDP uses, for example, a thermo-adaptation technique by softening the material with hot water to allow adaptation to the mouth of the user.

In the absence of the skeleton, when the user squeezes the jaws during the shaping procedure, the teeth sink into the adaptable structure without being able to control the thicknesses. If the thicknesses of materials are no longer sufficient, GDP no longer plays its role correctly. The maintenance of minimum thickness, reproducible for all athletes, promotes the absorption and dissipation of impact energy by the composite material of the protection. Moreover, in the absence of the pallet, the crushing of the mandibular and maxillary gutters during the adaptation of the PIB does not allow to preserve a previous space sufficient for an effective ventilation of the user.

The term "gutter" means the volume of adaptable material for receiving the upper and lower dental arches. Thus, before adjustment of the GDP, the upper and lower gutters have regular surfaces, and after adjustment of the GDP, the upper and lower gutters present the fingerprints of the teeth.

The gutters therefore initially have sufficient thicknesses so that the adaptable material of the adaptable structure can fill a hole left by a missing tooth, and that all the teeth are well covered by the adaptable material to be effectively protected, guarantee the retention of the GDP and secure the mandibular dental arch to the maxillary dental arch.

This first area of the palette can have different shapes. It can, for example, be flat, but also plano-convex or biconvex.

The first zone of the pallet thus has a length L, a width l and a height h. Once the PIB in the mouth, we can define the length L as the dimension extending from the outside towards the inside of the mouth, the width l like the dimension extending from one canine to the other, and the height h is the thickness of the pallet, ie the dimension to define the opening of the jaws, the space left free between the maxillary teeth and the mandibular teeth.

Advantageously, the first zone of the pallet has a cross section of a surface approximately equal to or greater than 30 millimeters squared.

The ventilatory space thus created is sufficient and necessary for good ventilation of the user, especially during an effort. Below this value, the user can not ventilate enough and is then led to open the mouth, which causes the disadvantages mentioned above.

The complementary function between the pallet and the non-adaptable skeleton makes it possible to respond in an effective and reproducible way to the two main problems encountered in the existing protections, namely:
  • guarantee the conservation of minimum thicknesses for the GDP to efficiently absorb and dissipate impact energy,
  • and keep enough space for the user's tight jaw ventilation.

The present invention thus meets the recognized essential properties for this device:
  • occupy toothless spaces.
  • allow meshing of the mandibular and maxillary dental arches.
  • to be comfortable.
  • do not show protrusion or roughness.
  • to be retentive.
  • absorb and dissipate impact energy.
  • allow phonation.
  • allow oral ventilation tight jaws.

Advantageously, the pallet has at least one arm having a free end, for example two arms, comprising a lug, preferably at the free end, said lug being able to fit into a slot provided for this purpose in the adaptable structure , preferably at an occlusal bead, to maintain the position of the PIB during the adaptation phase, and said pin can come into contact with the skeleton.

The stop created by this bead facilitates the positioning of the lugs in their slot during the establishment of the pallet. In addition, the joint presence of the arcuate bead and lugs makes it possible to maintain a good positioning of the pallet during the adaptation phase, the correct positioning of the pallet being necessary for the adaptable material of the gutters does not obstruct not the ventilatory space when adapting the GDP.

According to an exemplary embodiment, the PIB has a window, for example delimited by the skeleton and the lower gutter, in which the pallet can be housed.

Indeed, before formatting, the pallet is housed in a window reserved for it to maintain a sufficient ventilation space, or once the adaptable structure softened to adapt to the mouth of a user, the pallet creates or increases a space by sinking into softened adaptable material. In the case where the PIB initially has a window, several embodiments are possible.

According to a first embodiment, the lower gutter has a discontinuity at the incisivo-canine block. This discontinuity thus forms a window in which is positioned the pallet which is then supported on the skeleton, but not necessarily in direct contact with the skeleton. The presence of the pallet thus makes it possible to prevent this space from becoming clogged at least partially during the adjustment of the GDP by a user.

According to a second embodiment, the lower groove is continuous all along the mandibular dental arch; the skeleton and the lower channel form between them, at their interface, a window in which the pallet can be inserted. Thus, the mandibular teeth of the incisivo-canine block can also be protected and the anterior ventilatory space preserved during the adaptation phase.

According to another exemplary embodiment, the adaptable structure comprises an upper vestibular flap delimiting the upper trough intended to receive the maxillary dental arch.

According to another exemplary embodiment, the adaptable structure comprises a lower vestibular flap delimiting the lower gutter intended to receive the mandibular dental arch.

Advantageously, the skeleton comprises a fixing arch, for example at the incisivo-canine block. This hoop can be used, for example, to attach the GDP to a helmet grid in American football.

Advantageously, the skeleton comprises frontal reinforcements. These frontal reinforcements, preferably located at the upper level of the incisivo-canine block, allow in particular to maintain the position and thickness of the upper vestibular flap.

It can also be provided that the skeleton comprises lateral reinforcements, preferably located at the upper and / or lower level of the outer edges of the lateral sectors.

The frontal and lateral reinforcements thus make it possible to guide the correct positioning of the maxillary and mandibular dental arches of the user in the upper (maxillary) and inferior (mandibular) grooves of the adaptable structure during the phase of adaptation of the PIB to the morphology of the individual.

According to another embodiment, the skeleton comprises at least one anchor hole, preferably located in one of the lateral sectors, to ensure the maintenance of the skeleton in the adaptable structure. Thus, during the manufacture of PIBs, generally by overmolding, the holes in the skeleton allow a better integration of the skeleton in the adaptable material of the adaptable structure.

According to yet another embodiment, the front reinforcements are connected to each other by a junction element, thus delimiting spaces. These spaces make it possible, like the anchoring holes of the lateral sectors, to reinforce the maintenance of the skeleton in the adaptable structure at the level of the incisivo-canine block, as well as the shape of the upper vestibular flap.

The present invention further comprises a method of use.

A first preliminary step is to make malleable the adaptable structure, for example, by placing the GDP in hot water. During this stage, the pallet is positioned at its location to maintain the GDP and keep the minimum dimensions of the ventilatory space.

Once the adaptable structure has become malleable, a second step is to place the GDP in the mouth of the user who squeezes the jaws and who can press GDP with his fingers and tongue so that the GDP takes the form of intraoral reliefs.

Then, a third step is to remove GDP from the mouth. Preferably, the user waits a few seconds before removing the PIB from his mouth so that the adaptable structure no longer deforms.

The PIB can be reshaped at will at any time if the user considers it necessary, by repeating the previous steps.

Finally, when the adaptable structure is in shape, a fourth step is to remove the pallet of GDP. The user can then place the GDP in his mouth to check the adaptation, and, if necessary, repeat the previous steps.

In the first embodiment in which the ventilatory space is mainly formed by a discontinuity of the lower gutter, the pallet is removed by disengaging the lugs of the free ends of the arms of the slots provided for this purpose in the adaptable structure.

In the second embodiment in which the ventilatory space is formed between the lower trough and the skeleton, the lugs of the free ends of the arms are first disengaged from the slots provided for this purpose in the adaptable structure, then the arms are tightened. one to the other by elastic deformation, and the pallet is then withdrawn through the ventilatory space.

Thus, in this second embodiment, the pallet may have a more indented geometry at the junction between the arms to facilitate their elastic deformation.

La PIB selon l'invention comporte une structure adaptable 1 et un squelette 2.The PIB according to the invention comprises an adaptable structure 1 and a skeleton 2.

A PIB generally has a shape that resembles that of the dental arch, i.e. it has an arched area that constitutes the incisivo-canine block, and two lateral sectors corresponding to the positions of premolars and molars.

The adaptable structure 1 is made of an adaptable material, i.e. a material which must allow the user to adapt the PIB to its oral morphology, for example, by a thermo-adaptation technique with hot water.

In the first embodiment described, the adaptable structure 1 has an upper gutter 122 for receiving the maxillary dental arch, and a lower gutter 121 for receiving the mandibular dental arch. The shape of the maxillary 122 and mandibular 121 gutters of the adaptable structure 1 makes it possible to fill potential edentulous spaces during the phase of adaptation of the PIB to the oral morphology of the user.

The adaptable structure 1 has an incisivo-canine block 11 connecting two lateral sectors 12 (a left and a right). When the GDP is in the mouth, the incisivo-canine block 11 is located at the level of the canines and the incisors upper (maxillary) and inferior (mandibular), and the two lateral sectors 12 are located on each side, at the levels of the premolars and upper molars (maxillary) and lower (mandibular).

The adaptable structure 1 further comprises an upper vestibular flap 111, which delimits the upper gutter 122 for receiving the maxillary dental arch.

Advantageously, the upper vestibular flap 111 has a notch 112 facilitating the position of the brake of the upper lip.

The adaptable structure 1 may, according to a second embodiment shown Figure 1 (b) , also include a lower vestibular flap 114, which then delimits the lower groove 121 for receiving the mandibular dental arch.

According to a first exemplary embodiment, the adaptable structure 1 has a window 113 delimited by the skeleton 2 and the discontinuous lower gutter 121 (or even the lower vestibular flap 114 if it is the second embodiment), in which come to lodge a pallet 3 intended to maintain a sufficient ventilatory space during the shaping of the PIB, while allowing to maintain minimum thicknesses.

La forme de la structure adaptable 1 est alors conforme à la morphologie buccodentaire standardisée (profondeur des volets vestibulaires supérieur (maxillaire) 111, et inférieur (mandibulaire) 114 s'il existe, position de l'échancrure 112, largeur et longueur de l'arcade, etc.).The shape of the adaptable structure 1 is then in accordance with the standardized oral morphology (depth of the vestibular flap upper (maxillary) 111, and lower (mandibular) 114 if it exists, position of the notch 112, width and length of the arcade, etc.).

It is preferably composed of a non-modifiable (or modelable) texture material by the technique recommended for adapting the adaptable structure 1 to the oral morphology of the user (thermo technique). -adaptation to hot water or other techniques).

Thus, during the adaptation in the mouth after softening of the adaptable structure 1, the skeleton 2 makes it possible to guarantee the conservation, in the areas at risk of trauma, of the minimum thicknesses of material necessary for the absorption and the dissipation of the energy of traumatic impact.

The skeleton 2 also has an incisivo-canine block 21 and two lateral sectors 22 (a left and a right). As for the adaptable structure 1, when the PIB is in the mouth, the incisivo-canine block 21 is located at the level of the canines and upper (maxillary) and lower (mandibular) incisors, the lateral sectors 22 being located on each side, at the levels of premolars and upper (maxillary) and lower (mandibular) molars.

According to the present embodiment, the skeleton 2 has several reinforcements.

At the incisivo-canine block 21, the skeleton 2 has three frontal reinforcements 211, but not necessarily exclusively. The shape and the position of these frontal reinforcements 211 allow, first of all, that the PIB keeps a sufficient thickness at the level of the incisors and the canines, then, to contribute to the conservation of the shape of the upper vestibular flap 111 when the material adaptable structure was softened, finally to guide the correct positioning of the maxillary incisors during the adaptation procedure.

According to an exemplary embodiment, the front reinforcements 211 of the incisivo-canine block 21 are connected to each other by a connecting element 212 for reinforcing the joining of the front reinforcements 211 and maintaining the upper vestibular flap 111.

These upper lateral reinforcements 222 and lower 223 may be a continuous band of material or pads. In the example shown on the figure 6 , the skeleton 2 comprises upper lateral reinforcements 222 in the form of a continuous strip, and lower lateral reinforcements (223a, 223b, 223c, 223d) in the form of pads.

The skeleton 2 also comprises anchor holes 221 in the lateral sectors 22, or even spaces 214 in the incisivo-canine block 21 in the case where a joining element 212 is present. Indeed, according to a preferred embodiment, the skeleton 2 is made, for example by molding, and the adaptable structure 1 is obtained by overmolding. The presence of the anchoring holes 221 in the lateral sectors 22 of the skeleton 2, or in some cases spaces 214 at the level of the incisivo-canine block, allows a better integration of the skeleton 2 in the adaptable material of the adaptable structure 1.

According to yet another embodiment, the skeleton 2 also has a hoop 213 at the incisivo-canine block 21 which can be used to attach the GDP to a helmet grid in American football for example.

The PIB further comprises a removable pallet 3 preferably made of a rigid material of non-modifiable (or modelable) texture by the technique recommended for adapting the adaptable structure to the oral morphology of the user (thermo-adaptation technique with hot water or other techniques).

The adjustment of the pallet 3 with the skeleton 2 contributes to the guidance of the maxillary dental arch of the user in the upper (maxillary) groove 122 of the adaptable structure 1 during the phase of adaptation of the PIB to the morphology the subject.

The pallet 3 has mainly a first zone 33 adapted to cooperate with the skeleton 2 to preserve a sufficient ventilation space while allowing the preservation of the thicknesses of the protection.

Said zone 33 is dimensioned so that, during the phase of adaptation of the PIB, the positioning of this zone 33 at the level of the incisivo-canine block, in the space 113 when it exists, and in cooperation with the skeleton 2, makes it possible to preserve, or even create or enlarge, an anterior ventilatory space of sufficient dimensions during the phase of adaptation of the PIB to the oral morphology of the user, so that the latter can benefit from a ventilation effective in tight jaw position, for example during a sports activity. This first zone 33 presents, in particular, in the zone of intersection between the pallet 3 and the incisivo-canine block (when the pallet 3 is in place for the adaptation of the PIB), a cross section 331 defined by a width l and a minimum height h ( figure, 8a), 8b), 10 and 11 ). The dimensions of the ventilatory space are thus guaranteed by the dimensions of this smaller cross section 331 of this first zone 33.

Advantageously, this cross section 331 should be designed to maintain, after the GDP shaping procedure, an anterior ventilatory space of a section equal to or greater than 30 millimeters squared.

  • hauteur h : 2 mm,
  • largeur l : 15 mm.
For example, this cross section 331 is designed to pass, after shaping, a template of the following dimensions:
  • height h: 2 mm,
  • width l: 15 mm.

The pallet 3 has, according to a preferred embodiment, two arms 34, a left arm 341 and a right arm 342, each terminated by a lug (343, 344), each lug (343, 344) being adapted to be inserted in slots provided for this purpose in the lateral sectors 12 of the adaptable structure 1 and to bear on the lateral sectors 22 of the skeleton 2 (not necessarily in direct contact) to maintain the pallet 3 in position during the phase of adjustment of GDP.

The junction 346 makes it possible, for example, to facilitate insertion and extraction. of the pallet when the space 113 is formed at the interface between the lower continuous trough 121 and the skeleton 2 (for example, when the PIB is produced according to the second embodiment).

The pallet 3 also has a bead 32 in an arc which is preferably precisely fitted to the rim of the incisivo-canine block 11 of the adaptable structure 1 to facilitate the positioning of the lugs (343, 344) in the slots during the formatting in the mouth, and thus maintain the position of the zone 33 for example during the adaptation phase.

The GDP can finally be realized according to the methods conventionally used. In particular, the skeleton, made for example by molding, is positioned in a mold in which the material of the adaptable structure is injected. The adaptable structure can therefore ultimately have small holes corresponding to the pads present in the mold to maintain the skeleton in place during the injection of the material of the adaptable structure.

Naturally, various alternative embodiments are possible, without departing from the scope of the invention.
