Definitions
- the invention relates to N-terminally modified cyclic tripeptide mimetics which, as inhibitors of the serine protease plasmin, are suitable for inhibiting fibrinolysis and thereby reducing blood loss during operations.
- Plasmin is a trypsin-like serine protease that cleaves peptide and protein substrates C-terminal of the basic amino acids arginine or lysine. Plasmin is formed from the zymogen plasminogen by catalytic action of the plasminogen activators urokinase or tPA.
- the plasmin substrates include various extracellular matrix and basement membrane proteins, such as fibronectin, laminin, type IV collagen or fibrin, but also numerous zymogens, such as the pro forma of the matrix metalloproteases or the plasminogen activator urokinase. In the blood, plasmin is primarily responsible for fibrinolysis by cleaving fibrin into soluble products.
- Plasmin is also believed to play an important role in rheumatoid arthritis by being involved in the breakdown of articular cartilage and matrix proteins ( Li et al., Am. J. Pathology 2005, 166, 783-792 ). Plasmin has also been described as an important activator of many inactive progenitors of proteases and of angiogenesis or other growth factors that promote tumor formation and metastasis. Also described was a function of the plasmin in proliferative vitreoretinopathy (PVR) via a plasma-catalyzed activation of PDFG-C (platelet-derived growth factor C, Lei et al., Investigative Ophthalmology & Visual Science, January 2008, 49, 42-48 ).
- PVR proliferative vitreoretinopathy
- the PVR can cause retinal detachment in the eye and is one of the major complications of retinal surgery in the eye.
- thrombolysis therapy for heart attacks ( Marder et al., Stroke. 2010, 41, Suppl. 1 S45-S49 ).
- thrombolytic therapy with plasminogen activators urokinase, tPA, streptokinase
- bleeding complications that require a suitable antidote, such as an efficient plasmin inhibitor, can also occur with the use of the plasmid.
- aminocarboxylic acids such as ⁇ -aminocaproic acid, p-aminomethylbenzoic acid or tranexamic acid (trans-4-aminomethylcyclohexanecarboxylic acid)
- ⁇ -aminocaproic acid p-aminomethylbenzoic acid
- tranexamic acid trans-4-aminomethylcyclohexanecarboxylic acid
- a major use of aprotinin has been to reduce blood loss, particularly in cardiopulmonary bypass surgery (CPB), significantly reducing the need for perioperative blood transfusions ( Sodha et al., Expert Rev. Cardiovasc. Ther. 2006, 4, 151-160 ).
- Aprotinin is also used as an additive in fibrin livers.
- the use of aprotinin has several disadvantages. Because it is isolated from bovine organs, there is a general risk of pathogenic contamination and allergic reactions. The risk of anaphylactic shock is relatively low ( ⁇ 0.1%) when aprotinin is given for the first time, but increases to 4-5% after repeated dosing within 200 days. It has recently been reported that the administration of aprotinin induces an increased number of side effects in direct comparison to ⁇ -aminocaproic acid or tranexamic acid ( Mangano et al., New Engl. J. Med. 2006, 354, 353-365 ).
- aprotinin resulted in a doubling of the number of kidney lesions, necessitating dialysis. Similarly, the administration of aprotinin increased the risk of heart attack and stroke compared to the control groups. As a result, aprotinin was largely withdrawn from the market in 2008.
- thrombin inhibitor inhibits melagatran plasmin with a K i value of 0.7 ⁇ M ( Gustafsson et al., Thromb. Haemost. 1998, 79, 110-118 ), while the structurally closely related compounds H317 / 86 has an inhibition constant of 0.22 ⁇ M.
- both compounds inhibit the protease thrombin significantly stronger with K i values ⁇ 2 nM.
- plasmin As described in the introduction, plasmin, with about 70 other representatives, belongs to the group of trypsin-like serine proteases.
- the large number of structurally similar proteases makes it difficult to develop selective substrate-analogous inhibitors.
- Many of these serine proteases have characteristically formed pockets that are responsible for the specific binding of certain amino acid residues of substrates and contribute to the affinity of substrate-analogous inhibitors.
- the amino acid at position 99 (numbering based on the sequence of the chymotrypsinogen) affects the size of the S2 pocket (definition of binding sites based on Schechter and Berger, Biochem. Biophys. Res. Comm.
- plasmin probably also has a low substrate specificity and cleaves many different substrates.
- the ring structure is obtained by acylation by means of a suitable, substituted piperazine linker derivative between the side chains of the d-4-aminophenylalanine in the P3 position and the 4-aminophenylalanine in the P2 position of substrate-analogous inhibitors of trypsin-like serine proteases (definition of the P2 and P3 amino acids in FIG Based on Schechter and Berger, Biochem. Biophys. Res. Comm. 27, 157-162 ).

Abstract
Description
The invention relates to N-terminally modified cyclic tripeptide mimetics which, as inhibitors of the serine protease plasmin, are suitable for inhibiting fibrinolysis and thereby reducing blood loss during operations.
Plasmin is a trypsin-like serine protease that cleaves peptide and protein substrates C-terminal of the basic amino acids arginine or lysine. Plasmin is formed from the zymogen plasminogen by catalytic action of the plasminogen activators urokinase or tPA. The plasmin substrates include various extracellular matrix and basement membrane proteins, such as fibronectin, laminin, type IV collagen or fibrin, but also numerous zymogens, such as the pro forma of the matrix metalloproteases or the plasminogen activator urokinase. In the blood, plasmin is primarily responsible for fibrinolysis by cleaving fibrin into soluble products.
Under certain pathological conditions, spontaneous activation of fibrinolysis may occur. In the case of such hyperplasminemia not only is the wound-closing fibrin degraded, but also anticoagulant fibrinogen degradation products are formed. This can cause severe haemostatic disorders.
As antifibrinolytic agents, synthetic aminocarboxylic acids, such as ε-aminocaproic acid, p-aminomethylbenzoic acid or tranexamic acid (trans-4-aminomethylcyclohexanecarboxylic acid), are used in the clinic. These compounds block binding of the zymogen plasminogen to fibrin and thereby inhibit its activation to plasmin. Therefore, these compounds are not direct inhibitors of plasmin and can not inhibit already formed plasmin.
