Description
SPECIFICATION
MINIMAL INVASIVE, COMPRESSIVE, LOCKING HIP SCREW WITH AN ANTI-ROTATION SLIDING BLADE
Technological Scope :
This invention is related to the minimal invasive, compressive, locking, antirotator sliding blade holder hip screw which provides fusion by fixing bones in case of single or multiple fractures of femoral neck bones.
Common Status of The Technique:
At the present time, many surgical treatment methods have been applied to femoral neck fractures. Non-union, union detention or avascular necrosis complication rates are at high levels. The minimal invasive, locking hip screw aiming to be able to reach the desired results, allowing sliding compression, inhibiting femoral head rotation, has been developed.
This invention is related to the antirotator blade holder femoral head hip screw being able to overcome the disadvantages determined above, providing bone fusion by bone fixation, being able to make compression.

Its applications are for femoral neck fractures (subcapital, transcervical, bazicervical)_ and intertrochanteric stabil fractures (type I and type II ).
The invention is the compressive, antirotator blade holder cannulated femur head hip screw (MIS A CHS); and its feature is that operation incision is small.
The invention is the compressive, antirotator blade holder cannulated femur head hip screw (MIS A CHS); and its feature is that application period is too short and easy.
Usage of scopy is the least for reduction control and placing the guide kirschner wire (1). Infection risk is too low.

The interlocking screw can be easily applied from superio lateral to infero-medial with a guide. There is no need for scopy.
Compression is" achieved in 3 different ways:
• With the main screw, the hip screw (MIS A CHS) (primary)
• With the compression screw (secondary)
• With slide effect (tertiary)
The invention is the compressive, antirotator blade holder cannulated femur head hip screw (MIS A CHS); and its feature is providing positive effect to union because of not being preventing slide compression in the fracture region (because of not being Z- effect) and migration of the screw (MIS A CHS) to the joint (cut out problem) is minimized.
The screw prevents the head's rotation on the threaded part with the blade, in MIS A CHS system.
Its application:
1. Patient is lay to an orthopaedic operation table.
2. Following close reduction, operation region is prepared. 3. A 2.4mm threaded pointed wire that will prevent the head rotation is sent from trochanteric major to the head in the way that won't obstruct the main hip screw (MIS A CHS) to be placed.
4. A threaded pointed wire having diameter 2.4mm is placed from proximal lateral femur, from the middle-bottom part of the neck to the middle of the head till 8- 10mm to the subcondral distance; and fracture reduction and the hip screw (MIS A CHS) position are controlled by scopy. 5. 2cm incision is made for the sieve that will be placed over the guide wire and the portal is widened by reaching to the bone with a pair of scissors, the sleeve is placed.
6. The length of the hip screw is measured with the same length screw or the measuring gauge. 7. Lateral cortex neck and head are holed with an adjustable and special length marked drill over the guide wire. Bone raspings from drilling are put with a special cannula to the fracture region by reaching there from the hole opened.
8. It's placed with the screw holder through the hole opened over the guide wire, and primary compression is applied, when desired it's controlled by scopy. 9. Rotation preventer wire is removed and compression is renewed.
10. Guide wire is removed.
11. From proximal-lateral to distal-medial is drilled with a 4mm drill for the locking cortex bone screw by the help of the external guide.
12. Length measurement is made for the cortex bone screw to be used. 13. Appropriate cortex bone screw is placed for locking and compression.
14. It's nailed into its place with the antirotator blade impacter.
15. Secondary compression is made by placing the threaded compression screw.
16. The sieve is removed and the cut is stitched. Operation is been ended.
17. If the reduction has been anatomic and the compression has. been performed; at the earliest period after operation, hip exercixes are started and load is given as it can be tolerated. So tertiary compression is provided with slide compression.

1- The invention is the minimal invasive, compressive, locking, antirotator sliding blade holder hip screw (MIS A CHS); and its feature is being consisted of that kirchner guide wire (1), minimal invasive, compressive, locking, antirotator sliding blade holder hip screw (2), spongios threaded part of MIS A CHS (2-1), hole through the hip screw (2- 2), cortex bone screw socket for compression (2-3), tapered neck of the hip screw (2- 4), gradually thicken last part of the hip screw (2-5), slide socket of the antirotator blade up to the end of the hip screw (2-6), threaded socket of the compression screw (2-7), step inhibiting the hip screw to sink into the bone (2-8), antirotator blade (3), propulsive screw socket of the antirotator blade (3-1), sliding part of the antirotator blade (3-2), back part of the antirotator blade (3-3), threaded compression screw (4), sliding propulsor of the antirotator sliding blade (5), hip screw (MIS A CHS) stabilizer (6), cannulated sled bearing inserter of the cannulated hip screw (MIS A CHS) (7), guide for the cortex bone screw ( 8 ) and cortex bone screw ( 9 ).
2- The antirotator blade (3) is mentioned in Demand 1; and its feature is characterized with antirotation feature by being pushed from the slide -socket (2-6) of the antirotator blade upto the end of the hip screw on the hip screw (MIS A CHS) (2).
3- The cortex bone screw (9) is mentioned in Demand 1; and its feature is characterized with that it's sended with the guide for the cortex bone screw (8) to the cortex bone screw socket for compression (2-3) on the hip screw (MIS A CHS) (2), and that the threaded—compression screw (4) is screwed and .so.-, broken femur head can be compressed.
