Abstract
L'objet de l'invention concerne une prothèse discale comportant : - une première (2) et une deuxième plaques (3) destinées à être fixées à des vertèbres cervicales voisines, - et des moyens d'articulation (7) interposés entre les deux plaques placées en position superposée, comportant : - des moyens autorisant des mouvements de flexion-extension dans le plan sagittal (S) selon un débattement angulaire limité, - des moyens autorisant des mouvements d'inflexion latérale dans un plan perpendiculaire au plan sagittal (S) selon un débattement angulaire limité, - des moyens autorisant des mouvements de rotation relative entre les première (2) et deuxième (3) plaques selon un débattement angulaire limité, - des moyens d'assemblage (17) avec la première (2) et la deuxième (3) plaques de manière à constituer une prothèse monobloc.
Description
PROTHESE DISCALE POUR VERTEBRES CERVICALES A DEBATTEMENT CONTROLE L'objet de l'invention concerne une prothèse discale pour vertèbres cervicales destinée à être substituée au disque fibro-cartilagineux assurant la liaison entre les vertèbres cervicales de la colonne vertébrale. Il est connu qu'un disque intervertébral peut subir des altérations telles que tassement, déformation, déplacement ou usure et plus généralement une dégénérescence associée à des contraintes mécaniques qui lui sont appliquées et conduisent à une destruction anatomique et fonctionnelle du disque et du segment vertébral. Cette altération discale modifie le comportement mécanique du disque et aboutit à une diminution de la hauteur de l'espace intersomatique, laquelle entraîne une perturbation de l'ensemble fonctionnel articulaire. Il en résulte une instabilité induisant, en particulier, une réaction arthrosique, source de douleurs et de processus ostéophytique. Dans le même sens, il peut apparaître une hernie discale nécessitant 1 ' ablation du disque cervical. Il a donc été proposé de remplacer le disque déficient par un disque artificiel dont différents types de réalisation ont été envisagés. Ainsi, il est connu, par exemple, notamment par le brevet US 5 562 738, une prothèse discale pour vertèbres lombaires comportant une première et une deuxième plaques de fixation aux vertèbres voisines, réalisées en un matériau métallique tel qu'en titane. Entre les plaques, est interposée une rotule d'articulation comportant un premier insert monté sur l'une des plaques et composé d'une calotte sphérique coopérant avec une cupule sphérique d'un deuxième insert monté sur l'autre plaque. Les inserts sont réalisés en un matériau céramique biocompatible présentant des caractéristiques tribologiques améliorées notamment en ce qui concerne sa résistance à l'usure. Toutefois, une telle prothèse discale pour vertèbres lombaires ne se trouve pas adaptée pour remplacer le disque des vertèbres cervicales dans la mesure où une telle prothèse ne permet pas aux vertèbres cervicales de retrouver leur mobilité naturelle. Il s'avère également que la rotule d'articulation présente une forme de réalisation relativement difficile à mener à bien et sensible aux phénomènes de brisure ou de fêlure, réduisant ainsi la durée de vie de la prothèse.
De même, le brevet US 5 895 428 décrit une prothèse discale comportant une première et une deuxième plaques conformées pour constituer une articulation du type à rotule. Les plaques sont maintenues entre une demi-sphère et une vis de blocage vissée dans la demi-sphère. II s'avère qu'un tel montage est susceptible d'entraîner un blocage de l'articulation à rotule, de sorte qu'une telle prothèse ne permet pas aux vertèbres de retrouver leur mobilité naturelle. L'objet de la présente invention vise donc à remédier aux inconvénients de l'art antérieur en proposant une prothèse discale pour vertèbres cervicales conçue pour présenter une durée de vie relativement longue en étant pratiquement insensible aux phénomènes d'usure et de brisure, tout en étant adaptée pour autoriser une mobilité physiologique entre les deux vertèbres cervicales instrumentées. Pour atteindre un tel objectif, la prothèse selon l'invention pour vertèbres cervicales comporte : - une première et une deuxième plaques destinées à être fixées à des vertèbres cervicales voisines, - et des moyens d'articulation interposés entre les deux plaques placées en position superposée. Selon l'invention, les moyens d'articulation comportent : - des moyens autorisant des mouvements de flexion-extension dans le plan sagittal selon un débattement angulaire limité par des moyens de butée en flexion-extension, - des moyens autorisant des mouvements d'inflexion latérale dans un plan perpendiculaire au plan sagittal selon un débattement angulaire limité par des moyens de butée en inflexion latérale, - des moyens autorisant des mouvements de rotation relative entre les première et deuxième plaques selon un débattement angulaire limité par des moyens de butée en rotation relative, - des moyens d'assemblage avec la première et la deuxième plaques de manière à constituer une prothèse monobloc. Selon une caractéristique de l'invention, les moyens d'articulation comportent :
- un alésage de profil partiellement sphérique s 'établissant à l'intérieur d'une chambre aménagée dans une plaque, - et une portée de profil complémentaire à l'alésage aménagée dans l'autre plaque et montée dans l'alésage pour être verrouillée dans ce dernier. Selon une première variante de réalisation, les moyens autorisant des mouvements de flexion-extension comportent un axe s 'étendant dans le plan sagittal en faisant saillie de part et d'autre de la portée, dans des dégagements aménagés dans la deuxième plaque en s'ouvrant dans l'alésage sphérique. De préférence, les dégagements possèdent un diamètre déterminé permettant de définir le débattement angulaire des mouvements de rotation relative entre les première et deuxième plaques. Selon une deuxième variante de réalisation, les moyens de butée en rotation relative sont constitués par une forme géométrique femelle coopérant avec une forme géométrique mâle complémentaire, l'une des formes géométriques étant aménagée sur la première plaque tandis que l'autre forme géométrique est réalisée sur le deuxième plaque. Avantageusement, les moyens de butée en inflexion latérale sont constitués par le profil des plaques venant en contact l'une de l'autre. Avantageusement, la portée est aménagée dans un premier insert monté sur la première plaque et réalisé sous la forme d'un téton et en ce que l'alésage est aménagé dans un deuxième insert monté sur la deuxième plaque et réalisé sous la forme d'une bague. De préférence, les inserts sont réalisés en céramique ou en métal. Diverses autres caractéristiques ressortent de la description faite ci-dessous en référence aux dessins amiexés qui montrent, à titre d'exemples non limitatifs, des formes de réalisation de l'objet de l'invention. La Figure 1 est une vue en perspective montrant un premier exemple de réalisation d'une prothèse conforme à l'invention. La Figure 2 est une vue en coupe élévation prise sensiblement dans le plan sagittal selon les lignes II-II de la Fig. 1. La Figure 3 est une vue de dessus de la prothèse illustrée à la Fig. 1.
La Figure 4 est une vue frontale en élévation de la prothèse illustrée à la Fig. 1. La Figure 5 est une vue en coupe élévation frontale prise sensiblement selon les lignes V-V de la Fig. 2. La Figure 6 est une vue en perspective partiellement en coupe montrant un deuxième exemple de réalisation d'une prothèse conforme à l'invention. La Figure 7 est une vue en coupe élévation prise dans le plan sagittal de la prothèse illustrée à la Fig. 6. La Figure 8 est une vue de dessus de la prothèse illustrée à la Fig. 6. La Figure 9 est une vue en coupe frontale réalisée sensiblement selon les lignes IX-IX- de la Fig. 7. Tel que cela ressort plus précisément des figures, l'objet de l'invention concerne une prothèse discale 1 destinée à être implantée à la place d'un disque entre deux vertèbres cervicales adjacentes. La prothèse cervicale 1 selon l'invention comporte une première plaque 2 dite supérieure dans l'exemple illustré et une deuxième plaque 3 dite inférieure. Les plaques 2 et 3 sont destinées à être fixées à des vertèbres cervicales voisines et présentent chacune une face externe respectivement 2 , 3χ, de dimensions sensiblement similaires adaptées pour épouser le contour des surfaces articulaires associées. Conformément à l'invention, les plaques 2 et 3 présentent un profil anatomique permettant de s'adapter à l'espace intervertébral. Ainsi, tel que cela ressort plus précisément de la Fig. 2, la première plaque 2 présente dans le plan sagittal S, un profil convexe tandis que dans ce même plan sagittal S, la deuxième plaque 3 présente un profil sensiblement plan. Par ailleurs, comme cela ressort plus précisément de la Fig. 4, la deuxième plaque 3 possède dans le plan frontal F, un profil convexe tandis que la première plaque 2 possède un profil plan. De préférence, les faces externes 2\, 3ι des plaques 2 et 3 sont équipées d'éléments d'ancrage osseux
4 dans les vertèbres. Dans l'exemple illustré sur les Fig. 1 à 3, les faces externes 2\ et
3ι comportent en tant qu'éléments d'ancrage 4, des crans parallèles entre eux et au plan frontal F. Avantageusement, et tel que cela ressort plus précisément de la Fig. 3, chaque plaque 2, 3 présente une forme générale trapézoïdale en possédant un bord postérieur
respectivement 2a, 3a s'étendant à l'opposé et parallèlement au plan frontal F et à un bord antérieur 2b, 3b respectivement. Le bord antérieur 2b, 3b de chaque plaque 2, 3 est relié aux bords postérieurs 2a, 3a respectifs à l'aide de deux bords latéraux 2c, 3c divergents à partir du bord antérieur 2b, 3b. De préférence, chaque bord latéral 2c, 3c se trouve raccordé d'une part au bord antérieur 2b, 3b respectif par un congé de raccordement 2d, 3d et d'autre part au bord postérieur 2a, 3a par l'intermédiaire d'un congé de raccordement 2e, 3e. Conformément à l'invention des moyens d'articulation 7 sont interposés entre les deux plaques 2, 3 placées en position superposée. Conformément à l'invention, les moyens d'articulation 7 comportent : - des moyens autorisant des mouvements de flexion-extension dans le plan sagittal S selon un débattement angulaire limité par des moyens de butée en flexion-extension 9, - des moyens autorisant des mouvements d'inflexion latérale dans un plan frontal F perpendiculaire au plan sagittal S selon un débattement angulaire limité par des moyens de butée en inflexion latérale 12, - des moyens autorisant des mouvements de rotation relative entre les première 2 et deuxième 3 plaques selon un débattement angulaire limité par des moyens de butée en rotation relative, - des moyens d'assemblage 17 avec la première 2 et la deuxième 3 plaques de manière à constituer une prothèse monobloc. Tel que cela ressort de la description qui précède, les moyens d'articulation 7 permettent de contrôler dans le plan frontal F, dans le plan sagittal S et dans le plan transversal T les mouvements relatifs entre les première 2 et deuxième 3 plaques selon des mouvements limités correspondant à la mobilité physiologique naturelle entre deux vertèbres cervicales. Bien entendu, les moyens d'articulation 7 permettent que les plaques puissent avoir des mouvements selon les diverses combinaisons de ces différents plans. Les moyens d'articulation 7 sont constitués en particulier, par un alésage 20 de profil partiellement sphérique, s'établissant à l'intérieur d'une chambre 21 aménagée dans l'exemple illustré dans la deuxième plaque 3. Cet alésage est destiné à coopérer avec une portée 23 de profil complémentaire à l'alésage 20. La portée 23 de profil
partiellement sphérique est montée dans l'alésage 20 de manière à être verrouillée ensemble. La portée 23 est aménagée dans un premier insert 25 monté sur la face interne de la première plaque 2 et réalisé sous la forme d'un téton. En effet, tel que cela ressort des figures, cet insert 25 comporte une embase de section circulaire engagée partiellement dans un logement s'ouvrant sur la face interne de la plaque 2. Cette embase se prolonge par la portée 23 de profil partiellement sphérique. L'alésage 20 est aménagé dans un deuxième insert 26 monté dans la chambre 21 de la deuxième plaque 3 et réalisé sous la forme d'une bague. Les inserts 25, 26 sont réalisés en métal ou en céramique. L'alésage 20 et la portée 23 constituent des moyens autorisant les mouvements de flexion-extension dans le plan sagittal S, les mouvements d'inflexion latérale dans le plan frontal F, et les mouvements de rotation relative dans le plan transversal T. Les moyens d'assemblage 17 entre la première et la deuxième plaques 2, 3 sont réalisés par l'insertion de la portée 23 de profil sphérique dans l'alésage 20. Selon la première variante de réalisation illustrée aux fig. 1 à 5, les moyens d'articulation 7 comportent un axe 31 traversant de part en part la portée 23 en s'étendant dans le plan sagittal S. L'axe 31 fait saillie de part et d'autre de la portée 23 en étant engagé dans des dégagements 32 aménagés dans la bague 26 en s'ouvrant dans l'alésage 20. Les deux dégagements 32 qui s'étendent de façon diamétralement opposée présentent une hauteur, c'est-à-dire une mesure dans le plan sagittal S adaptée pour permettre les mouvements de flexion-extension dans le plan sagittal de la plaque 2 relativement à la plaque 3. Les moyens de butée 9 en flexion-extension sont constitués par le profil des plaques 2, 3 venant en contact l'une de l'autre. Selon une autre caractéristique avantageuse de réalisation, les dégagements 32 possèdent un diamètre déterminé, c'est-à-dire une mesure dans le plan transversal T déterminée pour définir le débattement angulaire des mouvements de rotation relative entre les plaques 2, 3. En effet, les dégagements 32 limitent la rotation de l'axe 31 selon le plan transversal T. Les dégagements 32 constituent ainsi les moyens de butée en rotation relative entre les plaques 2, 3. Selon une autre caractéristique avantageuse de réalisation, l'axe 31 autorise les mouvements d'inflexion latérale dans le plan frontal F perpendiculaire au plan sagittal S, de la plaque 2 relativement à la plaque 3. Les moyens de butée 12 en
inflexion latérale sont constitués par le profil des plaques venant en contact l'une de l'autre. Ainsi, les plaques 2, 3 viennent en contact l'une de l'autre selon les mouvements d'inflexion latérale selon leur face interne respectivement 22 et 32 s'étendant en vis-à-vis l'une de l'autre. Les Figures 6 à 9 illustrent une deuxième variante de réalisation d'une prothèse 1 conforme à l'invention. Cette prothèse comporte des moyens d'articulation 7 identiques à ceux décrits ci-dessus et comportent à cet effet les mêmes références. Dans cette variante de réalisation, l'alésage 20 fait partie d'une bague 38 présentant une fente 39 permettant le montage d'une portée 23 par déformation élastique. Selon cette deuxième variante de réalisation, les moyens de butée en rotation relative sont constitués par une forme géométrique femelle 40 coopérant avec une forme géométrique mâle complémentaire 41. L'une des formes géométriques est aménagée sur une plaque tandis que l'autre forme géométrique est réalisée sur l'autre plaque. Dans l'exemple illustré, la forme géométrique femelle est réalisée sur l'insert 25 monté solidaire de la première plaque 2 tandis que la forme géométrique mâle complémentaire 41 s'étend à partir du fond de la chambre 21 de la deuxième plaque 3. Ces formes géométriques femelle 40 et mâle 41 sont dites complémentaires dans le sens où elles autorisent des mouvements de flexion-extension dans le plan sagittal S, des mouvements d'inflexion latérale dans le plan frontal F, et des mouvements de rotation relative entre les première et deuxième plaques. Ces formes géométriques 40 et 41 sont adaptées pour constituer des moyens de butée en rotation relative entre les plaques 2 et 3 pour un débattement angulaire limité. Les moyens de butée en flexion-extension sont constitués par les faces internes
22, 32 des plaques tel que cela ressort plus précisément de la Fig. 7. De même, les moyens de butée en inflexion latérale 12 sont constitués par les faces internes respectivement 22 et 32 des plaques 2, 3 tel que cela ressort clairement de la Fig. 9. L'invention n'est pas limitée aux exemples décrits et représentés car diverses modifications peuvent y être apportées sans sortir de son cadre.
Claims
REVENDICATIONS 1 - Prothèse discale pour vertèbres cervicales comportant : - une première (2) et une deuxième plaques (3) destinées à être fixées à des vertèbres cervicales voisines, - et des moyens d'articulation (7) interposés entre les deux plaques placées en position supeφosée, caractérisée en ce que les moyens d'articulation (7) comportent : - des moyens autorisant des mouvements de flexion-extension dans le plan sagittal (S) selon un débattement angulaire limité par des moyens de butée en flexion-extension (9), - des moyens autorisant des mouvements d'inflexion latérale dans un plan perpendiculaire au plan sagittal (S) selon un débattement angulaire limité par des moyens de butée en inflexion latérale (12), - des moyens autorisant des mouvements de rotation relative entre les première (2) et deuxième (3) plaques selon un débattement angulaire limité par des moyens de butée en rotation relative, - des moyens d'assemblage (17) avec la première (2) et la deuxième (3) plaques de manière à constituer une prothèse monobloc. 2 - Prothèse discale selon la revendication 1 , caractérisée en ce que les moyens d'articulation (7) comportent : - un alésage (20) de profil partiellement sphérique s'établissant à l'intérieur d'une chambre (21) aménagée dans une plaque, - et une portée (23) de profil complémentaire à l'alésage (20) aménagée dans l'autre plaque et montée dans l'alésage (20) pour être verrouillée dans ce dernier. 3 - Prothèse discale selon les revendications 1 et 2, caractérisée en ce que les moyens autorisant des mouvements de flexion-extension comportent un axe (31) s'étendant dans le plan sagittal (S) en faisant saillie de part et d'autre de la portée, dans des dégagements (32) aménagés dans la deuxième plaque en s'ouvrant dans l'alésage sphérique. 4 - Prothèse discale selon la revendication 3, caractérisée en ce que les dégagements (32) possèdent un diamètre déterminé permettant de définir le
débattement angulaire des mouvements de rotation relative entre les première et deuxième plaques. 5 - Prothèse discale selon la revendication 1 , caractérisée en ce que les moyens de butée en rotation relative sont constitués par une forme géométrique femelle (40) coopérant avec une forme géométrique mâle complémentaire (41), l'une des formes géométriques étant aménagée sur la première plaque tandis que l'autre forme géométrique est réalisée sur le deuxième plaque. 6 - Prothèse discale selon la revendication 1 , caractérisée en ce que les moyens de butée en inflexion latérale (12) sont constitués par le profil des plaques venant en contact l'une de l'autre. 7 - Prothèse selon la revendication 2, caractérisée en ce que la portée (23) est aménagée dans un premier insert (25) monté sur la première plaque (2) et réalisé sous la forme d'un téton et en ce que l'alésage (20) est aménagé dans un deuxième insert (26) monté sur la deuxième plaque (3) et réalisé sous la forme d'une bague. 8 - Prothèse discale selon la revendication 7, caractérisée en ce que les inserts
(25, 26) sont réalisés en céramique ou en métal.
