Abstract
Description
- L'invention concerne un raccord pour tubulure de nutrition entérale.
- Ainsi qu'on le sait la nutrition entérale d'un patient consiste à amener dans l'estomac ou l'intestin au moyen d'une sonde suivant l'oesophage, une composition nutritive conditionnée dans un récipient situé à distance et auquel la sonde est connectée au moyen d'une tubulure dite tubulure de nutrition entérale.
- Il existe à ce jour plusieurs types de récipients munis de goulots de formats prédéterminés différents, choisis par les préparateurs des compositions nutritives, et à chacun de ces goulots, donc à chacun de ces types de récipients, correpond actuellement une tubulure spécifique munie en une extrémité d'un raccord adapté à coopérer avec ledit goulot. En pratique une chambre de goutte à goutte est interposée entre ce raccord et la partie flexible de la tubulure.
- L'invention vise, à des fins de standardisation, un raccord du type précité qui soit adapté à coopérer avec plusieurs formats prédéterminés de goulot. Une telle standardisation a pour intérêt de réduire les coûts de fabrication, et de nécessiter un mondre nombre de tubulures à garder en stock auprès de chaque lieu d'utilisation : il en résulte un moindre volume de stockage et un plus fabile coût global de tubulures à garder en stock.
- L'invention propose à cet effet un dispositif pour le raccordement d'une tubulure de nutrition entérale à un récipient contenant une composition nutritive et muni d'un goulot, du type comportant une paroi transversale d'obturation et une jupe formant avec cette paroi un bouchon adapté à se fixer sur ce goulot, caractérisé en ce qu'il est indépendant de la tubulure, en ce que la paroi transversale d'obturation comporte une zone d'épaisseur réduite adaptée à être perforée par un embout perforateur monté à une extrémité de cette tubulure, et en ce que ladite jupe fait partie d'un jeu d'au moins deux jupes cylindriques élastiques coaxiales solidaires de la paroi transversale d'obturation et pouvant s'engager chacune, intérieurement ou extérieurement, sur des goulots de récipients ayant au moins deux formats prédéterminés différents, l'une des jupes comporte intérieurement une première paroi cylindrique adjacente à la paroi transversale d'obturation et bordée à distance de celle-ci par une nervure en saillie radialement interne, en sorte de pouvoir s'adapter autour d'un premier format de goulot, présentant un rebord en saillie radialement externe, et une seconde paroi cylindrique de plus grand diamètre raccordée à la première par une portée transversale d'appui et bordée à distance de celle-ci par une seconde nervure en saillie radialement interne, en sorte de pouvoir s'adapter autour d'un second format de goulot, de plus grand diamètre que le premier, et présentant un rebord en saillie radialement externe, cette même jupe comportant extérieurement une paroi globalement cylindrique adaptée à pénétrer dans le goulot d'un biberon.
- On appréciera que le fait que ce raccord soit indépendant du reste de la tubulure en permet une plus grande facilité de fabrication et de stockage stérile tout en permettant la mise en oeuvre de tubulures munies de perforateurs utilisables dans d'autres applications, ce qui en réduit le prix de revient.
- Selon des dispositions préférées de l'invention, éventuellement combinables :
- ledit jeu de jupes comporte au moins une première jupe munie extérieurement de nervures de frottement, et adaptée à s'engager intérieurement dans un goulot de récipient selon un premier format, et une seconde jupe adaptée à s'engager extérieurement sur un goulot de récipient selon un second format,
- les première et seconde jupes sont disposées d'un même côté de la paroi transversale d'obturation,
- le premier format correspond à un goulot de format standard du type adapté à recevoir des capsules métalliques standards, et le second format correspond au format standard des goulots de flaçon pour perfusion,
- cette jupe est associée, radialement à l'intérieur de celle-ci, à une seconde jupe adaptée à s'engager dans le goulot lisse d'un récipient en forme de poche. - Ces dernières dispositions permettent de s'adapter ainsi à trois formats courants de goulots de récipient.
- De manière avantageuse, en outre,
- cette paroi globalement cylindrique a un diamètre qui diminue en s'éloignant de la paroi transversale d'obturation,
- cette paroi transversale d'obturation est limitée extérieurement par une paroi cylindrique, adaptée à traverser la bague de serrage dudit biberon, raccordée à la paroi extérieure de la jupe par une bride annulaire ménagée radialement en saillie externe et adaptée à être serrée axialement entre le goulot du biberon et la portée annulaire transversale de la bague de serrage. - Des objets, caractéristiques et avantages de l'invention ressortent de la description qui suit, donnée à titre d'exemple illustratif non limitatif, en regard des dessins annexés sur lesquels :
- - la figure 1 est une vue en coupe axiale diamétrale d'un raccord selon l'invention, selon la ligne I-I de la figure 2,
- - la figure 2 en est une vue de dessous selon la flèche II de la figure 1,
- - la figure 3 est une version agrandie de la figure 1 montrant divers goulots de récipient susceptibles de recevoir le raccord; et
- - la figure 4 est une vue latérale avec arrachement partiel d'une extrémité de tubulure de nutrition entérale munie d'un perforateur adapté à coopérer avec le raccord.
- Le raccord 1 représenté à titre d'exemple aux figures 1 à 3 comporte un jeu de plusieurs jupes coaxiales, élastiques, ici au nombre de deux (références 2 et 3), raccordées à une paroi transversale d'obturation 4 comportant une zone 5 d'épaisseur réduite, ici disposée dans l'axe X-X, cette zone est adaptée à être perforée par un embout perforateur, par exemple représenté sous la référence 6 à la figure 4, monté à une extrémité d'une tubulure 7 que ce raccord 1 est destiné à raccorder au goulot d'un récipient (non représenté) contenant une composition nutritive. Cette zone 5 est ici constituée par un perçage partiellement obturé (par exemple de diamètre égal à 4 mm).
- Les jupes 2 et 3 de ce raccord sont conformées de manière à pouvoir chacune s'engager, intérieurement ou extérieurement, sur des goulots 8A, 8B, 8C et 8D ayant des formats prédéterminés différents.
- Dans l'exemple considéré les jupes 2 et 3 sont disposées d'un même côté de la paroi transversale d'obturation 4, la jupe 2 étant située à l'intérieur de la jupe 3.
- La jupe 2, ou jupe interne, est munie extérieurement de nervures 10 de frottement décalées axialement grâce auxquelles cette jupe interne est adaptée à s'engager et se retenir intérieurement dans le goulot 8C et à s'y maintenir. Ce goulot 8C est par exemple un goulot cylindrique plastique à parois lisses dont est muni à ce jour un type de récipient conformé en poche souple.
- Le diamètre extérieur de cette paroi est par exemple de 14,2 mm avec des nervures venant en saillie sur 0,5 mm.
- La jupe 3, ou jupe externe, comporte intérieurement une première paroi cyindrique 11, adjacente à la paroi d'obturation 4 et bordée à distance de celle-ci par une nervure 12 en saillie interne grâce à laquelle cette première paroi peut venir coiffer un gouloit 8B muni d'un rebord 13 sous lequel vient s'engager ladite nervure 12. Le goulot 8B correpond de préférence au format standard des bouteilles adaptées à être fermées par une capsule métallique. Le diamètre interne de cette première paroi est dans ce cas de 26 mm.
- Cette jupe 3 comporte en outre, intérieurement, une seconde paroi cylindrique 14 de plus grand diamètre que la première, racordée à la première par une portée transversale d'appui 15, et bordée à distance de celle-ci par une seconde nervure 16 en saillie interne grâce à laquelle cette seconde paroi peut venir coiffer un goulot 8A muni d'un rebord 17 sous lequel vient se placer ladite nervure 16. Ce goulot 8A correspond de préférence au format normalisé des goulots de flaçons de perfusion. Il existe en effet des préparations nutritives conditionnées dans des récipients munis de tels goulots. Le diamètre interne de cette seconde paroi cylindrique est par exemple dans ce cas égal à 30,5 mm.
- De manière avantageuse cette jupe externe comporte extérieurement une paroi 18 globalement cylindrique adaptée à pénétrer dans le goulot 8D d'un récipient, par exemple un biberon. Pour faciliter la pénétration du raccord dans le biberon cette paroi externe 18 est légèrement effilée avec, dans l'exemple considéré, un diamètre qui diminue de 34 mm à proximité de la paroi transversale 4 à 33 mm à 23 mm de celle-ci.
- En combinaison avec cette paroi externe 18 est avantageusement prévue une paroi cylindrique 19 bordant radialement la paroi transversale d'obturation 4 tandis qu'une bride 20 en saillie radialement externe est ménagée longitudinalement entre les parois 18 et 19. Cette bride 20 est adaptée à être pressée axialement contre le goulot 8D par la portée transversale 21A d'une bague de serrage 21 associé à ce biberon, cernant latéralement la paroi transversale d'obturation 4.
- Il va de soi que la description qui précède n'a été proposée qu'à titre d'exemple illustratif non limitatif et que de nombreuses variantes peuvent être proposées sans sortir du cadre de l'invention. Ainsi par exemple, en fonction des formats des goulots sur lesquels ce raccord doit venir s'adapter, on peut prévoir des jupes complémentaires disposées à l'extérieur des jupes 2 et 3, ou du côté opposé par rapport à la paroi transversale d'obturation.
- Comme il ressort de la figure 4, le perforateur 8 ménagé à un extrémité de la tubulure en est généralement séparé par une chambre 22 de goutte à goutte, muni d'un embout de prise d'air 23. Dans l'embout perforateur 8 sont ménagés deux canaux longitudinaux 24 et 25 respectivement reliés à l'intérieur de la chambre 22 et l'embout 23.
- La matière constitutive du raccord est de tout type approprié compatible avec les exigences médicales.
- La paroi de la jupe interne équipée des nervures 10 est avantageusement tronconique tandis que, le cas échéant, la jupe externe peut comporter des perforations.
