(Heemels, Nature (2016) 539:179).Neurodegenerative diseases are age-related disorders that are becoming increasingly common, in part due to the increase in the elderly population (Heemels, Nature (2016) 539:179).
For example, amyotrophic lateral sclerosis (ALS), also known as motor neuron disease, Lou Gehrig's disease, or Charcot's disease, is estimated to affect 30,000 Americans and more than 400,000 people worldwide at any given time. Approximately 5,000 Americans are diagnosed with ALS each year. The disease causes the steady death of motor neurons, resulting in progressive paralysis that kills its victims within an average of one to five years. Most people diagnosed with ALS live 3-5 years after the first signs of the disease appear. Approximately 10% of people with ALS live for at least 10 years. The variable rate of disease progression makes prognosis difficult to predict and treatments difficult to develop.
Only two drugs (riluzole and edaravone) have been approved by the FDA for the treatment of ALS, and although both slow the progression of the disease in some patients and can prolong life by several months, neither can treat or cure the disease.
Some inherited forms of ALS are caused by genetic mutations. The genetic change modifies an abundant enzyme in cells called copper-zinc superoxide dismutase (Cu-Zn superoxide dismutase, now commonly called SOD1). This enzyme serves to protect cells from metabolic wastes that can cause damage if not neutralized.
Однако высокая аффинность меди CuATSM делает это средство неэффективным средством доставки в обход системы распределения, которая естественным образом ограничивает транспорт меди в центральную нервную систему (ЦНС).CuATSM was shown to provide protection in mice with high expression of superoxide dismutase SOD1 G93A according to stringent criteria established in the field. However, the high affinity of CuATSM copper makes it an ineffective means of delivery by bypassing the distribution system that naturally limits copper transport into the central nervous system (CNS).
Therefore, there is a need in the art for improved therapeutic agents that can treat neurological diseases and/or disorders associated with copper deficiency.

Detailed description
Provided herein are methods of treating a neurodegenerative disease in a subject in need thereof, the method comprising administering to the subject a therapeutically effective amount of CuPTSM. In an embodiment, the neurodegenerative disease is amyotrophic lateral sclerosis (ALS), frontotemporal dementia (FTD), Parkinson's disease, Huntington's disease, and Alzheimer's disease. In an embodiment, the neurodegenerative disease is ALS.
As discussed above, CuATSM has been used clinically for the treatment of ALS, but has significant drawbacks that severely limit its use as a therapeutic agent. Additionally, CuATSM has a compact, symmetrical structure, which contributes to another limitation associated with this compound: its compact structure allows the ATSM ligand component to quickly form extremely stable crystals, which poses several challenges in the preparation and formulation of this compound, hindering its ability to be a suitable pharmacological agent. means. For example, the metal-free component of the ATSM ligand crystallizes within seconds during its synthesis in all common refluxing solvents; thus, it is difficult to subsequently add copper at the final stage of synthesis, especially on the industrial scale required to obtain the large quantities required for clinical use.
(Int. J. Rad. Appl. To address these challenges, this paper proposes CuPTSM as an effective therapeutic agent for the treatment of neurodegenerative diseases, including ALS. CuPTSM is an asymmetric bis(thiosemicarbazone) copper(II) complex in which the bis(thiosemicarbazone) ligand is coordinated to the copper center in a k 4 -type manner. This compound was first synthesized in 1987 (Int. J. Rad. Appl. Instr., part B, 1987, 14, 59), and was used as a tracer for positron emission tomography. CuPTSM differs from CuATSM in the ligand backbone symmetry (see structures below). Surprisingly, it was found that despite the lower in vivo drug potency of CuPTSM compared to CuATSM, CuPTSM was more effective in treating ALS symptoms at significantly lower doses.

The term sporadic refers to a neurodegenerative disease, such as ALS, that is not inherited. Sporadic ALS accounts for about 90% of cases in which the patient is the only family member with the disease. The cause of sporadic ALS is not fully understood but may be due to a combination of environmental and genetic risk factors.
The term familial refers to a neurodegenerative disease, such as ALS, that runs in families. Familial ALS accounts for about 10% of cases where more than one person in a family has ALS, and sometimes family members also have frontotemporal dementia. People with familial ALS often begin to show symptoms at an earlier age than those with sporadic ALS. Familial ALS is most often inherited in an autosomal dominant manner.
Goodman and Gillman's The Pharmaceutical Basis of Therapeutics (Alfred Goodman Gilman, Louis S. Goodman, and Alfred Gilman, eds., 6th ed. 1980). Half-life is used in the usual sense, as in Goodman and Gillman's The Pharmaceutical Basis of Therapeutics (Alfred Goodman Gilman, Louis S. Goodman, and Alfred Gilman, eds., 6th ed. 1980). Briefly, this term encompasses a quantitative measure of drug elimination time. Elimination of most drugs is exponential (i.e., follows first-order kinetics) because drug concentrations typically do not approach those needed to saturate the elimination process. The rate of an exponential process can be expressed by its rate constant k, which expresses the fractional change per unit time, or by its half-life, t1/2, the time required for 50% completion of the process. The units of these two constants are time-1 and time, respectively. Rate constant
CuPTSM.- 4 045139 first order and the half-life of the reaction are simply related (kxt1/2=0.693) and can be replaced accordingly. Because first-order elimination kinetics dictate that a constant proportion of drug is lost per unit time, the plot of logarithmic drug concentration versus time is linear throughout the time after the initial distribution phase (i.e., after drug absorption and distribution is complete). The half-life of a drug can be accurately determined using this graph. The half-life of absorption is defined as the amount of time required for half the dose of CuPTSM to be absorbed.
Methods of treatment.
The methods described herein involve administering a therapeutic amount of CuPTSM to a subject in need thereof. A therapeutically effective amount is an amount of CuPTSM that, when administered to a patient, alone effectively treats the neurodegenerative disease. An amount that appears to be a therapeutically effective amount in a given case for a particular subject may not be effective for 100% of subjects receiving similar treatment for the disease or condition in question, even if such dose is considered a therapeutically effective amount by qualified physicians. The amount of CuPTSM corresponding to a therapeutically effective amount depends greatly on the type of disease, stage of the disease, age of the patient receiving treatment, and other facts.
CuPTSM.In one aspect, provided herein is a method of treating or preventing ALS in a subject in need thereof, comprising administering to the subject a therapeutically effective amount of CuPTSM.
In another aspect, the present document provides the use of CuPTSM for the production of a medicament for the treatment of ALS in a subject.
In various embodiments, depending on the effective amount of CuPTSM used, CuPTSM can effectively treat ALS.

In other embodiments, provided herein is a method of treating ALS in a subject in need thereof, wherein the subject is administered a low dose of CuPTSM. The term low dose as used herein refers to a dose that is below the therapeutically effective amount of the reference CuATSM compound when administered for the treatment of ALS. In some embodiments, the term low dose refers to a dose that is one or more orders of magnitude lower than a therapeutically effective amount of the reference CuATSM compound when administered to treat ALS. In some embodiments, the term low dose refers to a dose that is one-half, one-third, one-fourth, one-fifth, one-sixth, one-seventh, one-eighth, or less of the therapeutically effective amount of the reference CuATSM compound when administered for the treatment of ALS. In some embodiments, the low dose of CuPTSM is within the doses or dose ranges described herein. Although the amount of CuPTSM should result in effective treatment of ALS, it is desirable that the amount is not excessively toxic to the patient (ie, the amount is preferably within the toxicity limits established by the diagnostic and treatment protocol). In some embodiments, either to prevent excessive toxicity or to provide more effective treatment for ALS, or both, there is a limit on the total dose administered. Generally, this document refers to quantities per day; however, this document also covers half-day and two-day or three-day cycles.
In some embodiments, the subject in need thereof has not previously received treatment. In some embodiments, the subject in need thereof has previously received an ALS treatment other than CuPTSM that was inadequate (e.g., as assessed by the subject and/or physician), was ineffective, and/or did not result in significant improvement in one or more parameters, or symptoms associated with ALS and/or did not produce a biological effect that correlates with the underlying pathology causing ALS symptoms.
SOD 1.In some embodiments, the subject in need thereof is a human, and the human has a genetic mutation associated with ALS, wherein the genetic mutation associated with ALS includes a mutation in the SOD 1 gene.
Pharmaceutical compositions.
The formulation of therapeutic compositions and their subsequent administration (dosing) is within the competence of specialists in this field. Dosage depends on the severity and susceptibility of the disease state being treated, with treatment lasting from several days to several months or until sufficient relief of the disease state is achieved. Optimal dosing regimens can be calculated based on measurements of drug accumulation in the patient's body.
Optimal dosages, dosing techniques, and repetition rates can be easily determined by a person of ordinary skill. Optimal doses may vary depending on the relative potency of CuPTSM and can usually be estimated based on EC50s that have been found to be effective in in vitro and in vivo animal models. Typically the dose ranges from 0.01 mcg to 100 g/kg body weight and may be administered once or more per day, week, month or year, or even once every 2 to 20 years. Those skilled in the art can readily estimate the frequency of dosage repetition based on the measured retention time and drug concentration in body fluids or tissues. After successful treatment, it may be desirable for the patient to undergo maintenance therapy to prevent recurrence of the disease state, with CuPTSM administered in maintenance doses ranging from 0.01 mcg to 100 g/kg body weight one or more times daily, up to once every 20 years .

Examples
CuATSM.Example 1: Pharmacokinetics of CuPTSM compared to CuATSM.
The SOD1 G93A transgenic mouse model of ALS has been the most widely used animal model since the 1990s. The mice were genetically modified to overexpress a mutant form of the human Cu/Zn superoxide dismutase 1 (SOD1) gene, carrying the ALS-associated glycine to alanine mutation at amino acid position 93 (G93A). The current clinical strategy for treating ALS with CuATSM is to escalate the human dose until ALS patients achieve plasma levels equivalent to those in mice treated with 30 mg/kg/day.
To compare the effects of CuPTSM and CuATSM in vivo, SOD1 G93A mice were administered a single bolus of 30 mg/kg CuPTSM or 30 mg/kg CuATSM in an oral suspension of 0.5% methylcellulose/0.4% TWEEN80/0.9% saline. Measurements of half-life, half-life, and Cmax were performed in both plasma and spinal cord samples to measure drug exposure to CuPTSM or CuATSM within 6 hours of drug administration. A decrease in CuPTSM plasma concentration was observed over time (Figure 1A), with an absorption half-life of 0.54 h, an elimination half-life of 2.44 h, and a plasma Cmax of 1565 ng/mL. Plasma concentrations of CuATSM stabilized and remained constant 6 hours after administration (Figure 1B), with an absorption half-life of 0.24 hours, an elimination half-life of 13.7 hours, and a plasma Cmax of 198 ng/mL.
Similar measurements were recorded for CuPTSM and CuATSM concentrations in the spinal cord. As with plasma concentrations in mice administered 30 mg/kg CuPTSM, concentrations decreased over time (Figure 2A) and CuPTSM was found to have an absorption half-life of 1.0 h, an elimination half-life of 1.9 h, and C max 7.27 ng/ml. Similar to plasma concentrations, CuATSM concentrations in the spinal cord remained constant even 6 h after administration (Fig. 2B). In the spinal cord, CuATSM had an absorption half-life of 0.65 hours, an elimination half-life of 13.8 and a Cmax of 2 ng/mL.
3В).Using data and parameters obtained from the above experiments using a single bolus of 6 mg/kg CuPTSM, 30 mg/kg CuATSM, or 100 mg/kg CuATSM, plasma and spinal cord concentrations were plotted against time (hours) for illustrations of pharmacokinetic modeling over time. Linear kinetics were assumed. Previous studies using CuPTSM have shown that although 30 mg/kg CuATSM is well tolerated in mice, CuPTSM at the same dosage is toxic to mice. Plasma concentrations of 30 mg/kg CuATSM and 100 mg/kg CuATSM did not return to 0 ng/mL before the next dose. In contrast, plasma concentration levels of CuPTSM increased after mice were administered 6 mg/kg, and plasma levels returned to 0 ng/ml before the next dose (Fig. 3A). Similar results were obtained when concentrations were measured in spinal cord samples (Fig. 3B).
Эффективность CuPTSM у мышей SOD1G93A.Example 2: Efficacy of CuPTSM in SOD1 G93A mice.
To test the effectiveness of CuPTSM compared to CuATSM, mice were tested for latency to paresis and overall lifespan. Mice were given 6 mg/kg CuPTSM (n=32), 30 mg/kg CuATSM (n=32) and 100 mg/kg CuATSM (n=32) once daily to assess disease progression and detect drug-induced changes. in SOD1 G93A mice compared with their respective vehicle-only control mice. Neurological assessment to check the occurrence of paresis for each mouse was performed throughout the study according to the protocol described in Hatzipetros et al. (2015, J. Visualized Experiments. doi:10.3791/53257), incorporated herein by reference in its entirety. Briefly, mice were scored from NS 0 (before symptom onset) to NS 4 (humane endpoint) based on a combination of the tail suspension test, the walking test, and the righting reflex test. The occurrence of paresis was measured
