Field of technology
The invention relates to medicine and pharmacology and concerns the use of tyrosyl-alanyl-glycyl-phenylalanyl-leucyl-arginine hexapeptide in the treatment of wounds or burns of the skin as a medicine for external use.
State of the art
According to WHO, 180 thousand people die every year due to thermal injury in the world. At the same time, burns are one of the main causes of long-term hospitalization and permanent disability, often accompanied by social stigma and significantly worsening the quality of life [1]. The principle of unity of general and local treatment underlying the treatment of burnt patients emphasizes the special role of the latter, aimed at high-quality healing of burn wounds and early restoration of the skin [2]. Thus, the development of new effective local anti-burn agents undoubtedly remains an urgent healthcare task.
A whole complex of pathophysiological mechanisms is associated with thermal injury, in particular hypoxia and lipid peroxidation (LPO), decreased tissue perfusion in the stasis zone, aggregation and hyperactivation of neutrophils, a cascade of inflammatory reactions and severe pain. Obviously, new drugs must also have a complex mechanism of action corresponding to the pathogenetic picture of the injury. In this regard, the clinical development of a new anti-burn drug based on a synthetic regulatory peptide, which has a wide spectrum of biological activity, is of significant interest.
[5].As a component of the stress-limiting system, hexapeptide exhibits antioxidant properties [3], inhibits lipid peroxidation [4], and improves peripheral microcirculation [5].
[6].Being a mediator of the opioid antinociceptive system, it practically does not penetrate the blood-brain barrier. Moreover, its analgesic effect is realized mainly at the peripheral/tissue level, which allows us to consider this drug as having some properties of a topical anesthetic [6].
[7-9].At the same time, the hexapeptide demonstrated serious potential for tissue regeneration. Thus, a number of researchers have shown that it stimulates cellular mitotic activity through interaction with opiate receptors, and also stimulates wound reparative processes [7-9].
В. [14].Currently, the hexapeptide in the form of the drug Dalargin is approved for medical use in Russia for the treatment of peptic ulcers and pancreatitis as a means of reducing the acidity of gastric juice and the external secretory activity of the pancreas [10]. However, the effectiveness of this drug in the treatment of peptic ulcers is also largely explained by its reparative properties in relation to the mucous membrane of the stomach and duodenum [11-13]. The effectiveness of Dalargin as part of complex therapy for Crohn's disease (1 mg intramuscularly 2 times a day) (patent RU 2363455, application No. 2007142745/14 dated November 19, 2007) also indicates the presence of a systemic reparative effect. The wound-healing potential of Dalargin was proposed for use in the treatment of corneal injuries in the work of Maksakova E.V. [14].
It is known that decision making at the cellular level: apoptosis or DNA synthesis and mitosis largely depends on the oxidative status or redox state [15], which is especially important for reducing the stasis zone of a burn wound. It is obvious that the antioxidant properties, the presence of antiradical and nitroxidergic activity [16] provide Dalargin with additional potential for burn wound regeneration.
An invention in the field of cosmetology was published in which Dalargin was used as part of an anti-inflammatory cream to relieve acne symptoms (RF patent No. 2045949, A61K 7/48, 1995). Another patent (No. 2221547, application No. 2002134728/15 dated December 24, 2002) claimed the immunomodulatory effect of Dalargin as part of a cosmetic product.
There is data from an experiment in which it was demonstrated in vitro that Dalargin, when added to peripheral blood samples of patients with burns (n=15), normalized the increased functional and metabolic activity of neutrophils, reducing the increased activity of NADPH oxidase, alkaline phosphatase (ALP), myeloperoxidase, and nuclear chromatin , and also normalized the increased content of cationic proteins in neutrophils and the total number of activated neutrophils. It is important to note that in the blood of healthy people (n=27), Dalargin, on the contrary, increased the total number of activated neutrophils, as well as NADPH oxidase activity. The above effects were mediated through opiate receptors and were characterized by a dose dependence with maximization of the effect at a molar concentration of 5x10'8 M [17]. However, this effect has not been studied in an in vivo burn model.
In general, any violation of tissue integrity causes activation of neutrophils, which enhances the production of reactive oxygen radicals, lipid peroxidation and a whole cascade of reactions that damage tissues not affected by the pathological process [18]. Therefore, Dalargin’s ability to normalize the hyperactivation of neutrophils is a pathogenetically important link for the implementation of its anti-burn effect.

Currently, data from experiments on the effectiveness of parenteral administration of Dalargin for burns have been published, but none of them have studied the anti-burn effect when using the drug in the form of a local application. One study compared Dalargin with delta-sleep-inducing peptide (DSIP) in the treatment of burns, with both drugs administered intraperitoneally to experimental animals (RU patent No. 2070054, publication date: 12/10/1996). In another study, Dalargin (SC at a dose of 0.3 mg) demonstrated a significant acceleration of skin repair, as well as a decrease in the severity of productive inflammation, in a model of third-degree thermal burn in rats [9]. Patent RU No. 2196603 discloses the use of Dalargin (1-2 mg in 1-2 ml intravenous drip) as part of infusion therapy for the treatment of burn disease.
In contrast to the above works, the present invention reveals the potential of the drug Dalargin in the form of a dosage form for external use.
In addition, there is data on the use of Dalargin for a number of other diseases. [19] showed the effectiveness of Dalargin as a means of correcting the cytokine profile in patients with coronary heart disease and metabolic syndrome. At the same time, the present invention, while declaring the effectiveness of Dalargin in the treatment of burns, is fundamentally different from the listed works, which are related to the treatment of other diseases.
Summarizing the above, we can conclude that Dalargin is a very promising drug for clinical development, the complex action of which, eliminating pathophysiological processes (oxidative stress, lipid peroxidation, hyperinflammatory and nociceptive reaction) and enhancing reparative processes, provides undeniable advantages of this drug in the local treatment of wounds and burns. skin.

Data from an experiment to study the effectiveness of the hexapeptide tyrosyl-D-alanyl-glycylphenylalanyl-leucyl-arginine in a burn model in rats.
[20].The effectiveness of local application of the hexapeptide tyrosyl-D-alanyl-glycyl-phenylalanyl-leucyl-arginine in the treatment of thermal burns was assessed by modeling a II-IIIA degree burn in 30 nonlinear male rats. The animals were kept in vivar conditions (ambient temperature 19-23°C) in accordance with the standards of good laboratory practice of the Eurasian Economic Union, as well as current guidelines [20].
2.Flat-bottomed glass vessels of the appropriate diameter, 2/3 filled with liquid paraffin, were used as a thermal agent. Before application to the skin, the containers were preheated in a water bath. The animals were randomized into three groups (n=10 in each group): control and two experimental ones. As a comparison, another stimulator of tissue repair, Dexpanthenol (ointment for external use 5%) was chosen. All rats received burns of a normalized area of ~40 cm 2 .
After thermal injury was applied during the first 20 minutes, animals in the control group were treated with an aseptic bandage with further therapy with iodopirone and levomekol (group I); in the experimental groups, the study drugs Dexpanthenol (group II) or a 0.02-0.5% solution of hexapeptide tyrosyl-D-alanyl-glycyl-phenylalanyl-leucyl-arginine (group III, which consisted of 5 animals receiving 0.02 % solution and 5 animals receiving a 0.5% solution).
During the experiment, the general condition of the animals was assessed, a macroscopic assessment was given of the severity of tissue edema in the burn area, hyperemia, infiltration, the nature of the wound discharge, and epithelization of the wound defect. These symptoms were assessed in points: the maximum severity of a symptom was assessed as 3 points, its absence - 0 points. An objective assessment of the healing process was carried out based on the results of planimetry. All observations were carried out on days 1, 3, 5, 7 and 10 of the experiment.
The healing rate was determined by the formula V=(S1-S2)/t (where S1 is the value of the wound area at the previous measurement; S2 is the value of the wound area at the subsequent measurement; t is the number of days between the previous and subsequent measurements.
The reaction of the leukocyte pool was also assessed in the experiment - as the ratio of the absolute number of lymphocytes and segmented neutrophils in venous blood.
Results. On the 1st day of the study, the general condition of all experimental animals was assessed as moderate, which was due to the amount of thermal injury inflicted. The animals moved little within the cage, slept most of the time and responded weakly to external stimuli.
By the 3rd day, an increase in their activity and mobility was noted, the animals showed a defensive reaction when removed from the cage, appetite appeared, etc. There were no differences in the general condition of the animals between the experimental groups. In table Figure 1 shows changes in swelling of the burn wound and adjacent tissues. Analysis of the results obtained indicates that the initial edema as a reaction to thermal injury and the timing of its disappearance were significantly shorter in the group receiving tyrosyl-D-alanyl-glycyl-phenylalanyl-leucylarginine hexapeptide, both in comparison with the control group and the group , receiving dexpanthenol.
Similar dynamics were noted when assessing hyperemia (Table 2). The use of the drugs already on the 1st day reduced the severity of redness of the skin. Moreover, the hexapeptide tyrosyl-Dalanyl-glycyl-phenylalanyl-leucyl-arginine is significantly superior to dexpanthenol in reducing the dynamics of hyperemia.
3).chemistry of burn wound healing in animals of group III (tyrosyl-D-alanyl-glycylphenylalanyl-leucyl-arginine hexapeptide). It was characterized by a significant decrease in the area of the burn already on the 3rd day of the experiment, a decrease in the depth of the burn and, as a consequence, a more favorable course of the wound process. The results in groups II and III also showed the advantages of the hexapeptide tyrosyl-Dalanyl-glycyl-phenylalanyl-leucyl-arginine in the dynamics of reducing the area of the burn lesion (Table 3).

The average healing rate is shown in Fig. 1. Statistically significant differences were found only between the control group and the group receiving the tyrosyl-O-alanyl-glycyl-phenylalanyl-leucyl-arginine hexapeptide.
Dynamics of changes in the lymphocyte-neutrophil index.
Taking into account that the physiological feature of rats as a biological model is the predominance of a monocytic pool of leukocytes in the peripheral blood (normally in animals, lymphocytes make up 65-77% of all leukocytes), changes in leukograms in experimental groups were studied by assessing the lymphocyte-neutrophil index (the ratio of the number lymphocytes to segmented neutrophils).
The analysis showed that on the 1st day of observation there was a pronounced decrease in the lymphocyte-neutrophil index, which indicated a natural reaction caused by thermal injury. The dynamics of changes in the lymphocyte-neutrophil index, shown in Fig. 2 demonstrates statistically significant differences in this indicator between group I (control) and group III (tyrosyl-B-alanyl-glycyl-phenylalanyl-leucyl-arginine hexapeptide). In group III, this indicator returned to normal by the 7th day, maintaining an increasing trend in other groups of animals.
Thus, local application of the hexapeptide tyrosyl-O-alanyl-glycyl-phenylalanyl-leucylarginine immediately after thermal injury can reduce swelling and hyperemia. At the same time, there is a pronounced effectiveness in such indicators as wound surface area, average wound healing rate, as well as the dynamics of the lymphocyte-neutrophil index.
The results obtained in the preclinical experiment indicate the high effectiveness of the hexapeptide tyrosyl-P-alanyl-glycyl-phenylalanyl-leucyl-arginine for both first aid and further treatment of moderate superficial burns. In addition, the results obtained suggest that tyrosyl-O-alanyl-glycyl-phenylalanyl-leucylarginine hexapeptide can also be used to prepare wound surfaces of deep burns for subsequent skin transplantation, as well as in the treatment of trophic ulcers in weakened patients with low regenerative potential.

Data from an experiment to study the effectiveness of the hexapeptide tyrosyl-P-alanyl-glycylphenylalanyl-leucyl-arginine in a model of planar wounds in rats.
The study was conducted on male white Wistar rats. All animals were randomly divided into three groups of 13 individuals each:
I) контрольная группа (без лечения);I) control group (no treatment);
II) comparison group (Bepanten, cream for external use, containing dexpanthenol 5%) and
III) experimental group (tyrosyl-D-alanyl-glycyl-phenylalanyl-leucyl-arginine hexapeptide).
The experimental group consisted of five animals that received a 0.02% solution and five animals that received a 0.5% solution); the text provides average statistical data in this group.
Linear wounds were a longitudinal incision of the skin and subcutaneous fat along the midline of the back, about 4 cm long. After the incision, the edges of the wound were brought together by placing three sutures at an equal distance from each other. Starting from the day of surgery, the animals were treated with the study drugs, and the control group received a placebo. In each group, three rats were assigned to perform tensiometric testing - with euthanasia on the 7th day of the experiment. Immediately after complete healing was achieved (on the 25-26th day), samples were taken for histological examination.
The compared drugs were applied once a day, daily, for 20 days. The wound-healing effect was assessed according to the nature of the clinical course (the presence of purulent inflammation, the dynamics of fusion of the wound edges) on the 4th, 7th, 15th and 20th days of the experiment.
In fig. Figure 3 shows the results of visual control, clearly demonstrating more intensive healing of a linear wound in the experimental group (tyrosyl-alanyl-glycylphenylalanyl-leucyl-arginine hexapeptide) compared to both the control and active comparison groups.
As is known, the 15-20th day, corresponding to the third phase of healing, is characterized by epithelization and scar formation. However, in the control group, even on the 20th day of observation, complete healing was not observed, the scab remained, and complete epithelization in this group occurred only on the 25-26th day.
In the control group, according to microscopic examination, in the intact zone, collagen fibers were loosely located and characterized by greater thickness compared to the area of healing of a linear wound, where collagen fibers were much thinner and a large number of capillary vessels were preserved, i.e. all signs of immaturity of the formed scar were present. In this case, incomplete regeneration of the epithelium was noted in the area of consolidation of the wound edges. Focal-diffuse inflammatory infiltrates were noted in the structure of the scar. Granulation tissue extended deep into the dermis almost to the border with the subcutaneous fatty tissue
In the group receiving the hexapeptide tyrosyl-alanyl-glycyl-phenylalanyl-leucyl-arginine, the scar ended at the depth of the hair follicles, emptying of blood vessels was noted, and a predominance of fibrous elements over cellular elements was noted in the scar tissue, which indicated the stage of completion of scar formation.
Interim histological results (between the control group and the group receiving tyrosyl-D-alanyl-glycyl-phenylalanyl-leucyl-arginine hexapeptide) were found for the Bepanthen cream treatment group.
Tensiometric measurement data showed a significant superiority of the hexapeptide tyrosyl-Dalanyl-glycyl-phenylalanyl-leucyl-arginine compared to the control group - more than 2 times.

Data from an experiment to study the effectiveness of tyrosyl-D-alanyl-glycylphenylalanyl-leucyl-arginine hexapeptide applied in the form of 0.02 and 0.5% gel on a burn model in rats.
2.The effectiveness of topical application of tyrosyl-D-alanyl-glycyl-phenylalanyl-leucyl-arginine hexapeptide in the treatment of thermal burns was assessed based on a II-IIIA degree burn model in 40 nonlinear male rats. The animals were kept in vivar conditions in accordance with the standards of good laboratory practice of the Eurasian Economic Union, as well as current guidelines [20]. The role of a thermal agent was performed by flat-bottomed glass vessels of the appropriate diameter, 2/3 filled with liquid paraffin, preheated in a water bath before application to the skin. The animals were randomized into four groups (n=10 in each group): I - control and three experimental ones. As a comparison, a widely used drug for tissue repair was chosen - Bepanten (a cream for external use containing 5% dexpanthenol). The burn surface area was normalized to ~40 cm2 .
As a control, we used an aseptic dressing applied during the first 20 minutes after the burn, with further therapy with iodopirone and levomekol (group I). Group II received Bepanten as therapy. Group III received tyrosyl-D-alanyl-glycyl-phenylalanyl-leucyl-arginine hexapeptide as a 0.02% gel for topical use, and group IV received tyrosyl-D-alanyl-glycyl-phenylalanyl-leucyl-arginine hexapeptide as a 0.5% gel for topical use. external use.
During the experiment, in addition to monitoring the general condition of the animals, a planimetric assessment of the healing process was given on the 1st, 3rd, 5th, 7th and 10th days of the experiment. The healing rate was determined by the formula V=(S1-S2)/t (where S1 is the value of the wound area at the previous measurement; S2 is the value of the wound area at the subsequent measurement; t is the number of days between the previous and subsequent measurements.
Results.
On the 1st day of the study, the general condition of all experimental animals corresponded to moderate severity. An improvement in condition and an increase in activity were observed on the 3rd day of the experiment. At this stage, there were no differences in the general condition of the animals between the experimental groups.
4).In table Figure 4 shows the results of planimetry. There was a pronounced statistically significant positive dynamics of healing of burn wounds in animals of groups III and IV (tyrosyl-Dalanyl-glycyl-phenylalanyl-leucyl-arginine hexapeptide 0.02 and 0.5%, respectively). It was characterized by a significant (both in comparison with the control group and the group receiving Bepanten) reduction in the area of the burn already on the 3rd day of the experiment, a decrease in the area and depth of the burn and, as a consequence, a more favorable course of the wound process. No statistically significant differences were found between groups III and IV, although group IV tended to have a more pronounced reduction in burn area than group III (Table 4).

The average healing rate is shown in Fig. 4. Statistically significant differences were detected only between control group I and groups receiving tyrosyl-D-alanyl-glycylphenylalanyl-leucyl-arginine hexapeptide in the form of a gel (both 0.02 and 0.5%). Despite better rates of wound healing in group IV, no significant differences were found in comparison with group III, which is explained by the small sample of the study. The results confirm the pronounced effectiveness of tyrosyl-D-alanyl-glycyl-phenylalanyl-leucyl-arginine hexapeptide in the form of 0.02 or 0.5% gel in reducing both burn surface area and the average rate of healing of burn wounds.
0,02-0,5%.The results obtained in a preclinical experiment indicate the high effectiveness of the hexapeptide tyrosyl-D-alanyl-glycyl-phenylalanyl-leucyl-arginine both for first aid and for further treatment of moderate superficial burns. The results obtained suggest that tyrosyl-D-alanyl-glycyl-phenylalanyl-leucyl-arginine hexapeptide, in the dosage form of a gel for external use or a solution for external use, can be used to prepare the wound surfaces of deep burns for subsequent skin transplantation, as well as in therapy of trophic ulcers in patients with low regenerative potential. The therapeutic concentration range for tyrosyl-D-alanyl-glycyl-phenylalanyl-leucylarginine hexapeptide in dosage forms: solution for external use or gel for external use is 0.02-0.5%.
