Description
Endotracheal tubes have been state of the art in anesthesia for many years.
Except for neonatal anesthesia, they have a tracheal sealing balloon or cuff and an extraoral control balloon that allows the pressure of the trachea, i.e. the sealing cuff, to be controlled.
The sealing cuff itself has been designed as a low pressure cuff for several years, whereby the tracheal pressure is below the capillary pressure of the tracheal mucosa.
This is associated with the disadvantage that, particularly during the inspiration phase, respiratory gases are released extraorally into the room air, which leads, for example, to an increase in the concentration of certain respiratory gas components, possibly causing long-term damage to personnel.
The essence of the invention therefore consists in applying a higher pressure to a preferred low pressure cuff during the inspiration phase, so that a backflow of respiratory gases is eliminated.
The task is solved by integrating a second cuff into the low pressure cuff that is ventilated extraorally, which communicates optimally with the tube lumen via at least one perforation.
A further task is to rationalize tube production so that a minimized purchase price and an optimized price-performance ratio is guaranteed.
The latter has already been described by the same applicant, here in particular as a method for automated tube production, whereby the same materials, such as PVC, are preferably welded using ultrasound, for example. Thus, first of all, with the preformed and perforated inner tube, the inner tracheal cuff is welded onto it in such a way that it can be ventilated depending on breathing and is fixed in such a way that the ventilation lumen for the outer cuff is not affected.
This means that the outer cuff can be pressurized separately and acts primarily as a low-pressure cuff. During inspiration, i.e. when pressure is built up by the ventilation device, the inner cuff is inflated, which inevitably leads to an increase in pressure in the outer low-pressure cuff. This prevents a leak of respiratory gas to the extraoral region during inspiration, which does not occur during expiration when the outer cuff pressure has returned to normal.
The invention will now be explained in more detail with reference to claims 1-10 and figures 1 and 2.
Dabei zeigtThis shows
Figure 1 shows a schematic detailed view of a double cuff tube (1). The tube tube (7) is surrounded by the tube outer tube (8), which forms a unit with the outer cuff (3) manufactured using a blow molding process (8), so that a low-pressure cuff can be applied through the tube tube recess (4).
The outer tube (8) is preferably welded, glued or similar in the area of the tube eye (9) and cut to length so that
daß eine atraumatische Tubusaugenöffnung (9) entsteht.that an atraumatic tube eye opening (9) is created.
The inner cuff (2) was also preferably welded in the area of the perforation(s) (5) before the outer tube (8) was pulled on, so that it (2) can only be ventilated through these (5). This process is optimized by the overall size of the perforation(s) (5).
These can be round, slit-shaped, a combination of round and slit-shaped or otherwise designed. The breathing-dependent process is thus such that in the inspiration phase the inner cuff (2) is inflated, which, due to its increase in volume, causes an inspiratory increase in pressure in the outer cuff (3). The normal outer cuff pressure is restored in the expiration phase, so that damage to the tracheal mucosa caused by excessive cuff pressure is also counteracted.
It should be noted that the perforation(s) (5) must be selected in such a way that cuff(2) material cannot be drawn in during the expiration phase and that tube stability is not endangered.
B., eine Luer-Spritze abdichtend zu adaptieren, deren Luftvolumen o. dgl. bei Spritzenaktivierung unter Dehnung von Schlitz (12)Figure 2 shows a schematic sectional view of the cuff (3) inlet and outlet valve (10) with its tube holder (14), syringe holder (11) and sealing part (13), which is preferably slotted through (12). This makes it possible, for example, to adapt a Luer syringe in a sealing manner, the air volume or the like of which increases when the syringe is activated by stretching the slot (12).
geeignet ist, Cuff (3) zu blocken oder zu entlüften.suitable for blocking or deflating cuff (3).
Auf die Darstellung weiterer Ventilvarianten wurde verzichtet. The representation of further valve variants was omitted.
