Claims (17)

Ü687 4S2 Patentansprüche
1. Gelenkpfannenprothese, die von einem Becher (I)7 z.B. aus Metall, der erste Verankerungsmittel (3, 4), die von der äusseren Oberfläche (2) des Bechers vorstehend ausgebildet sind und durch die durchlaufende Durchbrüche (6) zur Aufnahme der Befestigungsschrauben (4) des Bechers in dem Knochengelenkpfannenhohlraum ausgebildet sind, wobei der Becher (1) aussen eine im allgemeinen konvexe Form und innen einen im wesentlichen konkaven Lagerraum (7) besitzt, und einem Einsatz (17) gebildet wird, der zum Beispiel aus Kunststoff besteht, mit Mitteln (19) zur Verbindung mit dem Becher versehen ist und der aussen eine dem Lagerraum (7), indem der Einsatz (17) aufgenommen ist, entsprechende Form aufweist und innen eine halbkugelförmige Stützfläche (20) besitzt, die dazu bestimmt ist, den Kopf einer Oberschenkelknochenprothese aufzunehmen, wobei die genannte Prothese elastische Mittel (16) zur Erzeugung einer Vorspannung zwischen der Gelenkpfanne, dem Becher (1) und dem Einsatz (17) besitzt, die von wenigstens einem Spalt (16) gebildet werden, der durch die Wand des Bechers (1) hindurch von der Nachbarschaft von dessen Pol (2a) aus bis zu dessen unterem umlaufenden Rand (12) gemäss einem Meridian ausgebildet sind, um die Ausdehnung und das Zusammenziehen des Bechers (1) gegenüber einer mittleren Ruhestellung zu erlauben, wobei der oder die Meridianspalt (e) (16) nur in einer Becherhälfte (1) gegenüber der anderen Becherhälfte, die die ersten Verankerungsmittel (3, 4) in der Gelenkpfanne besitzt, ausgebildet sind, dadurch gekennzeichnet, dass die ersten Verankerungsmittel von einem bogenförmigen Hocker (3) gebildet werden, der konzentrisch zu dem Becher (1) ist und über die äussere Oberfläche des Bechers (1) vorspringt.
2. Gelenkpfannenprothese nach Anspruch 1, dadurch gekennzeichnet, dass der genannte Hocker (3) einen Kreisbogen

bildet, der sich über ungefähr 120° erstreckt, um z.B. drei Befestigungsschrauben (4) aufzunehmen.

Gelenkpfannenprothese nach Anspruch 1 oder 2, dadurch gekennzeichnet, dass der innere Lagerraum (7) des Bechers (1) von dem unteren umlaufenden Rand (12) ausgehend einen zylindrischen Teil (8) besitzt, der sich durch einen kugelförmigen Teil (9) fortsetzt, der in der Nähe des Pols (2a) des Bechers (1) durch eine ebene Fläche (10), die parallel zur äquatorialen Ebene (11) des Bechers (1) ist, abgebrochen ist.

Gelenkpfannenprothese nach einem der vorangehenden Ansprüche, dadurch gekennzeichnet, dass der Becher (1) in Höhe seines unteren umlaufenden Rands einen kreisförmigen Kragen (12) besitzt, der dazu bestimmt ist, sich gegen einen entsprechenden, am Rand der Gelenkpfanne ausgebildeten Lagerraum abzustützen.

Gelenkpfannenprothese nach Anspruch 4, dadurch gekennzeichnet, dass der Becher (1) ferner unter dem Kragen (12) wenigstens einen nach unten vorstehenden Nocken (13) besitzt, der in der Lage ist, sich in eine Ausnehmung (19) entsprechender Form zu lagern, die in einem umlaufenden Rand (18), der die Basis des Einsatzes (17) der Gelenkpfannenprothese umläuft, ausgebildet ist.
6. Gelenkpfannenprothese nach Anspruch 5, dadurch gekennzeichnet, dass die Grosse jeder Ausnehmung (19) und des zugeordneten Nockens (13) wesentlich unterschiedlich sind, um ein Klemmen zwischen dem Becher (1) und dem Einsatz (17) zu erhalten.
7. Verwendung einer Gelenkpfannenprothese (1, 17) nach einem der vorangehenden Ansprüche, mit elastischen Vorspannungsmitteln (16), um die Abnutzung zwischen dem Becher (1) und dem Einsatz (17) zu verringern.
8. Verwendung einer Gelenkpfannenprothese (1, 17) nach einem der Ansprüche 1 bis 6, mit elastischen Vorspannungsmitteln (16), um das plastische Fliessen des Einsatzes (17) und das Lösen von plastischen Abfallteilchen zu verringern.
9. Verwendung einer Gelenkpfannenprothese (1, 17) nach einem der Ansprüche 3 bis 5, mit einem kreisförmigen Kragen (12) in Höhe der äquatorialen Ebene (11) des Bechers (1), um dessen Schwenken und die Migration des Bechers (1) zum Boden des Gelenkpfannenhohlraums zu verringern.
10. Verwendung einer Gelenkpfannenprothese (I7 17) nach einem der Ansprüche 1 bis 6, um Mikrobewegungen des Bechers (1) in der Gelenkpfanne zu verringern und das Neuwachsen des Knochens zu begünstigen.
11. Hilfsweise Instrumentierung zum Einsetzen der Gelenkpfannenprothese nach einem der Ansprüche 1 bis 6, im wesentlichen mit einem Formstückträger (25) und einem Bohrformstück (24), das von einem massiven Teil mit wenigstens einem Loch (31, 37), das von seiner unteren Fläche (29) aus ausgebildet ist, gebildet wird, wobei das erste Loch (37) in der äusseren kugelförmigen Fläche (24a) des Bezugsformstückes (24) ausmündet, um eine Bohrung, die den ersten Verankerungsmitteln (3) des Gelenkpfannenbechers (1) entspricht, zu höhlen, und die mit Hilfe einer Fräse (38), die durch das genannte erste Loch (37) geht, gebildet wird, dadurch gekennzeichnet, dass das Bohrbezugsformstück (24) eine äussere zylindrisch-kugelförmige Fläche (24a, 24b) aufweist und ein zweites Loch (31) besitzt, das axial das Formstück (24) durchläuft, um in der Gelenkpfanne eine zentrische gelenkpfannenförmige Öffnung zu bohren.
12. Hilfsweise Instrumentierung nach Anspruch 11, dadurch gekennzeichnet, dass das Bohrbezugsformstück ein drittes Loch (37) besitzt, das auf der äusseren kugelförmigen Fläche (24a)

des Formstückes (24) ausmündet und von dem ersten Loch (37) durch einen Kreisbogen getrennt ist, der dem Hocker (3) des Gelenkpfannenbechers (1) entspricht.
13. Hilfsweise Instrumentierung nach Anspruch 11 oder 12, dadurch gekennzeichnet, dass das Bohrbezugsformstück (24) ein weiteres Loch (33) besitzt, das einem Stössel (34) zugeordnet ist und dazu bestimmt ist, eine Kuppe (32) zur Befestigung des Bohrbezugsformstückträgers (25) aufzunehmen und zurückzuhalten.
14. Hilfsweise Instrumentierung nach Anspruch 4 und einem der Ansprüche 11 bis 13, dadurch gekennzeichnet, dass sie eine kreisförmige Fräse (39) besitzt, die in der Lage ist, einen kreisförmigen Lagerraum in dem koaxialen Knochen auszuhöhlen, um den kreisförmigen Kragen (12) des Gelenkpfannenbechers (1) aufzunehmen, wobei die genannte Fräse (39) von einem Fräsenträger (41), der mit einem axial beweglichen Kolben (44) versehen ist, dessen Stange (48) in der Lage ist, axial die kreisförmige Fräse (39) zu durchlaufen und dessen Kopf (47) sich in dem genannten zweiten zentralen Loch (31) des Bohrformstückes (24) lagert, getragen wird, und dass der zylindrische Teil (24b) des Bohrformstückes (24) als Führung für die gezahnte Krone (40) der Fräse (39) dient.

Hilfsweise Instrumentierung nach einem der Ansprüche 11 bis 14 oder zum Einsetzen der Gelenkpfannenprothese nach Anspruch 5 oder 6, mit einem Becherträger (15), der einen Stiel (15b), der sich durch eine Stange (15c, 15d) bis zu einer Zentrierkuppe (15a) hin verlängert, und ein Mittel (55, 56) zur Blockierung der Rotation um die Achse (57) der genannten Stange eines Bechers (1), der auf die genannte Kuppe aufgesetzt ist, besitzt, dadurch gekennzeichnet, dass das Drehblockiermittel eine Hülse (55), die gleitend axial auf die genannte Stange aufgesetzt ist, und eine Klaue (56), die an einen freien Ende (59) ausgekerbt ist, zum Einsatz eines Nockens (13) des Umfangs (12) des Bechers (1), und

vorzugsweise beweglich durch den Kopf (55a) der Hülse (55) und quer zur Achse (57) der genannten Stange eingesetzt ist, besitzt, und dadurch, dass die genannte Kuppe (15a) dazu bestimmt ist, auf ein zentrales Gewinde (14) des Bechers (1) aufgeschraubt zu werden.
16. Hilfsweise Instrumentierung nach Anspruch 15, dadurch gekennzeichnet, dass sie ein elastisches Mittel (61) zwischen dem Stiel (15b) und/oder der Stange (15c, 15d) des Becherträgers (15) und der gleitenden Hülse (55) besitzt, um letztere zur Zentrierkuppe (15a) hin zu beaufschlagen.
17. Hilfsweise Instrumentierung nach Anspruch 16, dadurch gekennzeichnet, dass das elastische Mittel eine Kompressionsfeder (61) ist, die zwischen die Stange (15d) und die Hülse (55) eingesetzt ist und sich an einem Ende auf einer radialen Schulter (62) der Stange (15c) und am anderen Ende gegen die genannte Klaue (56) abstützt, um gleichzeitig letztere in ihrer Stellung durch den Kopf (55a) der Hülse (55) hindurch zu blockieren und letztere zu der Kuppe (15a) hin zu beaufschlagen.
