Description
Eine solche Behandlungsweise ist aber nicht nur selbst sehr schmerzhaft, sondern ist auch von der großen Gefahr begleitet, daß dabei die Pulpa abstirbt und dadurch eine Dauerbeeinträchtigung desAbout the processes that take place in the treatment of teeth as well as the subsequent triggering of any pain sensations and the corresponding Measures to be taken to enable painless tooth drilling have hitherto prevailed different opinions. One view was that any sensitivity of the tooth should be eliminated would have to ensure painless tooth drilling. From this point of view goes the proposal to use a gas mixture of air and a freezing agent, e.g. B. carbonic acid and Ethyl chloride to conduct onto the tooth and thereby freeze it. Such a treatment is not only very painful in itself, but is also accompanied by the great danger that the pulp will die and thereby a permanent impairment of the
ao Zahnes herbeigeführt wird.ao tooth is brought about.
Another view is that the overheating of the drill and the mechanical Effect of the drilling chips that accumulate at the drilling site, which causes overheating still increase, would be the main cause of pain triggering. The proposal is based on this, through one to the drill head and the Drilling point brought stream of an appropriately cooled agent for this purpose, a hot run of the drill and a cooling of the tooth as well as keeping the drilling site free to achieve by drilling chips. However, even such a procedure does not lead to the desired effect of painless drilling, since a dissipation of the drilling heat and a Keeping away the drilling chips as well as a more or less slight cooling of the tooth a vibration of the albumin substance in the dentin tubes, so the real cause able to prevent the triggering of pain.
In fact, contrary to previous views, it was recognized that the one just mentioned Appearance is the actual cause of the sensation of pain when drilling a tooth. In accordance with this new finding, the invention is based contrary to the aforementioned Proposals based on a fundamentally different and completely new principle, namely Avoid vibrations of the albumin substance during drilling. this will according to the invention achieved by not applying a cooling agent directly to the tooth or the drilling site is performed, but by the fact that the drill head is cooled of the drill shaft is indirectly cooled by means of liquid carbon dioxide and thus by the von outflowing cold is enabled on the outer, adjacent to the drill head At the end of each dentin tube, a plug of coagulated, so to speak Albumin substance and thus to form an isolation zone, which the transmission or Triggering of a shock and over-
This movement is carried on the odontoblasts and through this on the nerve substance of the pulp, the albumin substance resting in the interior of the dentin tubes and thus the development of a pain sensation. tie. As the drilling process progresses, this insulating layer that was initially created is removed step by step by means of appropriate chip removal, but at the same time and in parallel with this ίο another such insulating layer is formed again inwards, i.e. the presence of such a layer is constantly ensured, since the deep-frozen drill keeps flowing out again and again cold which communicates itself to the albumin substance adjacent to it, coagulates it and thus in turn allows it to act in the same sense as an insulator against vibrations of the inner or underlying albumin substance.
Accordingly, the method according to the invention is to cool the dental drill by means of liquid carbonic acid in such a way that the carbonic acid in the liquid state under the evaporation pressure of, for. B. 50 to 60 atm. is guided into a cavity arranged on the shaft of the drill and is partially relaxed and gasified there, whereupon the carbonic acid gas that is produced, possibly still under pressure, is led into the open through a discharge line.
Auf der Zeichnung ist eine Ausführungsform des neuen Bohrerhandstückes beispielsweise ■ dargestellt, und zwar zeigtTo properly carry out the method according to the invention, a drill handpiece be used, inside of which a supply line for the carbon dioxide to the drill, a cavity arranged around part of the drill shaft and a discharge line for the gasified carbonic acid is arranged. Devices can be placed in the cavity or on the drill shank be arranged such. B. ribs, grooves, spherical polished masses, etc., which favor the transfer of cold and increase the cold capacity. The one not admitting the carbonic acid-exposed part of the drill in the handpiece is also advantageously used how to isolate the discharge pipe of the carbonic acid. Appropriately used as an isolating agent poorly conductive materials, such as B. ebonite, fiber, pressed material o. The like. Selected. Around To enable the carbon dioxide to be switched on and off quickly, it is advantageous inside of the handpiece to arrange a valve on the supply line of the carbonic acid, which is of can be operated externally. Fig. 6 the connection of the new handpiece to a carbon dioxide bottle.
A lateral, detachable hose connection 2 for liquid carbon dioxide is arranged on the drill handpiece 1, to which a flexible line 3 is connected, which takes the liquid carbon dioxide from a steel bottle 4 which is turned upside down for this purpose. After the normal shut-off valve of the carbon dioxide bottle, a pipe rupture valve 5 is installed in front of the line. The carbonic acid is fed through a line i a on the handpiece to a rotatable piston valve 6, passes through its narrow opening 7 into the cavity 8 arranged around the drill shank 9, where it changes into the gaseous state in a partially relaxing manner and through a larger bore of the valve flows through line i 6 into the open. The flexible line 3 and the adjoining pipe i ° are under the evaporation pressure of the liquid carbonic acid of 50 to 60 atm, which prevails at room temperature. The drill chuck 12 is surrounded by a heat-insulating sleeve 13, and the piston valve 6 and the discharge line i 6 themselves consist of insulating material in order to prevent the discharge of cold to the handpiece as far as possible.
The piston valve 6 has over a quarter of its circumference a groove 10 into which a Stop pin 11 engages. Is the part of the piston protruding from the handpiece by hand If the drill is turned a quarter in the direction of rotation, the flow of carbonic acid is interrupted or vice versa open. In order to be able to comfortably grip the valve piston, which is at least strongly cooled, is Ribbed front part.
What was said above for a handpiece also applies to a contra-angle handpiece, whereby the cavity, in which the carbonic acid acts on the drill, in which the angle attachment can be arranged.
Due to the strong cooling of the drill shaft by the gasifying liquid carbonic acid the drill tip is rapidly and constantly cooled to temperatures below about -10 to -80 ° C, in such a way that the areas of the dentin touched by the drill are exposed to strong cold set and thereby the aforementioned insulation layers made of coagulated albumin substance are generated, which cause a vibration of the inner albumin substance and iao thus prevent the cause of the pain triggering.
