Abstract
Description
The invention relates to an electrocardiograph with electro the bioelectric amplifier and ECG recording part, like a monitor in particular.
Electrocardiographs for recording cardiac action voltages will u. a. in operating theaters, introductory rooms, waking up clear and in intensive care units of hospitals in large Number of pieces required. It is common during prep phase and the operation of the patient's heart Help monitor the electrocardiograph. Here are stationary or trained as portable handheld devices Cardiographs used with a monitor for recording of the electrocardiogram.
Die Muskelkontraktion zeigt also die richtige Lage der Punktionskanüle an.For example in plexus anesthesia (brachial plexus, plexus lumbalis, plexus lumbosakralis) as well as in the peri peripheral nerve blocks become localization and location control of the puncture cannula electronic nerve stimulato ren used. It is a small, battery powered pulse generator using the puncture cannula be electrically connected. As long as the tip of the metals a puncture cannula lies outside the vascular nerve sheath, the impulses of the nerve stimulator cannot be transmitted to the ner ven act. But as soon as the tip of the puncture cannula Penetrating the vascular sheath, the electrical Im pulse on the nerves and then lead to visible muscle contractions z. B. in the hand area. The muscle contraction shows the correct position of the puncture cannula.
The catheterization of the central veins can lead to incorrect positions the venous catheter is coming. For a position check in the right front courage of the heart, it is known to use one electrically conductive liquid, e.g. B. a saline solution, filled catheter to use that the EKG potentials conducts what is seen on the monitor of the electrocardiograph is cash. When the catheter is inserted, it is replaced Puncture advanced in the atrium of the heart, except for the monitor, which is characteristic of the atrium P wave appears. The catheter is then returned pulled until the P wave cant disappears. After slight further withdrawal of the catheter the correct position of the catheter tip in the upper cavity vein reached. An incorrect position of the catheter (e.g. in the head or in the right or left arm vein) is using this method not recognizable.
The invention is primarily based on the object Electrocardiograph, which is preferred in the clinical field Use comes in such a way that it is suitable for this device an extended range of use results.
This object is achieved in that the Electrocardiograph with at least one external tap for the amplified electrical measurement signals are provided. This Tap is conveniently from an outside, for. B. at the Front plate, arranged on the housing of the electrocardiograph Plug connection, such as. B. a socket or the like. It It is also recommended that the tap be operated manually Adjustment device, such as in particular a resistor switch or the like. To assign, with the help of which Setting the strength and / or the tension on the tap removed electrical output pulses can be reached.
The electrocardiograph according to the invention therefore has one or also several taps for the when recording the EKG Drawing or reproduction used electrical measurement signals based on the measured and amplified cardiac action tensions arise. These measurement signals can be tapped at the Device taken as electrical impulses and for various processes in the medical field are made usable. The electrocardiograph according to the invention receives with the called tap the additional function of an electrical pulse generator, whose pulses from the amplified measurement signals of Electrocardiographs are formed. In the simplest way the arrangement meet so that in the electrocardiograph any existing bioelectric amplifier another forming the tap or leading to the tap Receives output at which the from the electrocardiograph on the Electrodes picked up and amplified to the measurement signals Heart action voltages can be tapped if necessary.
With the design of the electrocardiograph according to the invention can, for example, the function of the usual Plexus anesthesia common nerve stimulators men, so that the use of the usual stimula gates unnecessary. In this case, the puncture of the Plexus used puncture cannula with a cable to the ab grip connected to the electrocardiograph, so that the off gait impulses are transmitted via the puncture cannula. The Plexus anesthesia or other peripheral nerve block can be done in the same way as with the usual ner ven stimulators are performed.
Furthermore, the electrocardio designed according to the invention graph also useful for checking the position of a central vein Catheters are used, the tap via an elec tric cable to the with electrically conductive liquid filled venous catheters or to an electrically conductive guide wire of the catheter is connected. The one on the tap electrical pulses picked up by the electrocardiograph the increased measurement signals of the electrocardiograph, if necessary, matched to the desired pulse strength, for Determination of the position of the central venous catheter, since the impulses when placing the catheter are characteristic Ab effects of the recorded electrocardiogram, which appear on the monitor of the electrocardiograph and miss position of the catheter can be clearly seen when the Catheter tip is under one of the EKG electrodes, on the typical misalignments (neck - towards the head; right or left arm). This results in a considerable simplification in the localization of the center len venous catheter and the possibility of timely Cor rectification.
In hospitals, especially in operating rooms and up Intensive care units, there is often a need to Stabilization of cardiac activity of the patient's temporary heart use pacemakers. The invention can also be used for this use appropriate electrocardiograph with its tap, which thereby outputs the pacemaker function taking pulse generator that forms via a pacemaker probe or via a venous catheter that works with an electrical conductive guidewire is attached to the heart chamber is laid. Above all, for the pacemaker function the pulses corresponding to the P waves of the EKG are used will. For example, in the AV block, the P waves corresponding increased impulses via the pacemaker probe or via the current-carrying stylet of a central venous catheter ters transferred to the chamber muscle.
In the clinical area, artificial ventilation is often a problem Patients necessary. Different types of ventilation are available for this devices available. These processing devices work with Ventilation rates that are independent of the heart rate of the patient to be ventilated. That means that Compression phase and the decompression phase of ventilation device or the inhalation phase and the exhalation phase not to the Cardiac activity are coordinated, often the compression or inhalation phase coincides with the decontraction phase (Diastole) of the heart so that the ventilator against the current cardiac activity is working, d. H. the filling of the heart difficult. This leads above all to ventilation of old patients ten whose chest elasticity is reduced, and Pa suffered severe hemo with decompensated heart failure dynamic loads during contraction (systole) of the heart an increase in intrathoracic pressure, ver caused by an inspiration (compression phase of loading breathing apparatus) is facilitated.
According to the invention, the electrocardiograph can also be used for control ventilator. This can be done in the way that happen at the tap of the electrocardiograph a control part of the ventilator is connected, wel ches the ventilation frequency or the ventilation phases to the Heart rate or heart activity co-ordinates with Common mode of the compression or inhalation phase with the con traction phase of the heart. This is how ventilation works to match the respective heart activity by using the Pulse tap of the electrocardiograph ent the ventilator is controlled according to the cardiac activity.
The invention is described below in connection with that in the Drawing illustrated embodiment explained in more detail.
The drawing shows a trained as a portable handheld device th electrocardiograph 1 , which is equipped with an EKG recording part in the form of a monitor 2 as well as with the function and control buttons and display devices normally present, which need not be discussed in more detail here. The device 1 has, as usual, a plug or socket connection 3 for a cable 4 , which establishes the electrical connection to the electrodes 5 , which are applied to the body of the patient 6 , to the cardiac activity of the patient in the form of Record and record an ECG. The electrocardiograph 1 has, as usual, an electrical amplifier which amplifies the bioelectrical potentials which are removed with the aid of the electrodes 5 and which occur with every action of the heart muscle for recording. The electrocardiograph has a line selector in the usual way, which connects the bioelectric potentials to the electrocardiographic leads so that they are amplified by the bioelectric amplifier for recording.
In this respect, the electrocardiograph is of conventional training. According to the invention, it has at least one additional tap for the amplified measurement signals which result from the electrical potentials derived from the body of the patient 6 with the aid of the electrodes 5 . This separate tap is net in the drawing at 7 on the device housing. It is from a on the outside of the device housing angeordne th connector 8 , z. B. in the form of a Buchsenanschlus ses, via which the amplified th in the electrocardiograph measurement signals can be tapped and can be performed via a further cable 9 to a medical device.
On tap 7 is next to the connector 8 and an adjusting device 10 with control buttons 11 and 12 angeord net, by their manual operation, the measurement signals abgenom on tap 7 can be set in their amperage and voltage level. Electrical resistance devices or other electrical elements of the type customary for these purposes can be used for the setting device.
The entire tap 7 with the connector, the operating buttons or other manually operated switching elements and the adjusting device are advantageously combined in a module that is detachably arranged on or in the housing as a plug-in or plug-in module.
The tap 7 can be electrically connected to the electrical output of the amplifier that is already present in the electrocardiograph. However, it can also be the output of a separate electrical amplifier which amplifies the potentials conducted via the electrodes 5 , so that they can be removed from the tap 7 .
Various preferred applications are indicated in the drawing. For example, the electrocardiograph 1 with the tap 7 can additionally function as a nerve stimulator, as is used in plexus anesthesia and peripheral nerve blocks. In this case, the cable 9 is connected to the connector 8 of the tap 7 , with which a connecting cable 15 is electrically connected via a socket 13 and a plug part 14 , which in turn is electrically connected to the puncture cannula 16, with the aid of which, for. B. the plexus anesthesia is carried out in the usual manner. The picked up on the tap 7 amplified measurement signals of the electrocardiograph are thus brought to the pulse generation at the metal tip of the puncture cannula 16 to the effect ge to indicate the successful puncture by visible muscle contractions in the hand area or foot area of the patient 6 at the puncture of the puncture cannula 16 . If necessary, the cable connected to the tap can also have an additional line connection 17 as a neutral conductor, which is applied to the body of the patient 6 with a separate electrode 18 , as shown.
Furthermore, the separate tap 7 of the electrocardiograph 1 can be used to control the catheterization of the central vein. In this case, the catheter 19 is connected to the plug connection 8 of the tap 7 via the cable 9 via a plug 20 . The catheter can with an electrically conductive liquid, e.g. B. a saline solution, or, as usual, have a metal guidewire to transmit the electrical impulses taken off at the tap 7 to the catheter tip. When the catheter 19 is placed, it is advanced to the atrium of the heart 21 after the puncture. If the catheter is out of position, 2 typical changes in the recorded ECG appear on the monitor, which indicate the out of position. The catheter 19 can then be withdrawn to attempt again. If the catheter is in the correct position, this can be seen on the EKG recorded on the screen 2 . This enables good control when placing the central venous catheter. In this mode of operation, the electrodes 5 are applied to the body of the patient 6 in such a way that they are located in the region of the misalignment typical of the placement of the central venous catheter.
The tap 7 can be used for temporary stabilization of the heart activity in the manner of a pacemaker. In this case, the part 19 which is electrically connected to the tap 7 forms the pacemaker probe which is placed on the heart chamber, as is customary in pacemakers. In the pacemaker function, the measurement signals picked up and amplified via the electrode lines of the cable 4 , together with the pulses represented by the P-waves of the EKG, form the pacemaker pulses which trigger the heart actions presented by the R-waves of the EKG, as is known in pacemakers is.
The drawing also schematically shows a respirator 22 , which can be controlled via a connecting cable 23 and in the example shown, via the cable 9 electrically connected to the tap 7 . The electrical pulses tapped via the tap 7 are in this case control pulses which control this via a control part of the conventional type arranged on the ventilator 22 in such a way that the ventilation phases or the ventilation frequency is matched to the cardiac activity or the cardiac frequency, in such a way that the compression or inhalation phase of the ventilator 22 is synchronized with the con traction phase of the heart and vice versa.
With the embodiment of the electrocardiograph 1 according to the invention, its area of application is thus increased beyond the pure ECG recording, without this being associated with a considerable additional outlay. The tapped on the tap 7 elec trical impulses are in all cases the recorded on the electrode cable 4 and amplified in the electrocardiograph pulses.
