Description
The invention relates to a catheter for arterial vessels and Arteries according to the preamble of claim 1.
Catheters of this type are used in the removal of a stenosis (Constipation) in an artery with the help of a so-called bal lon dilation used (DE 32 35 974 A1). This will help eliminate the stenosis a small one inserted into the artery by means of the catheter The balloon is inflated and deflated several times at the narrowed point tet. When the balloon is inflated, the plaque breaks up, so that in the Ideally, the narrowed cross section of the artery is eliminated.
When dilating the balloon, however, it must be taken into account that by breaking the plaque a wound surface is created with the result that the blood flowing through the artery on this Make the formation of a clot. Basically There is a risk that the clot will overflow Clot in turn causes occlusion of the artery. In sol cases, it is then necessary to bypass however, this can only be done by surgery.
Until the operation, there must be a small amount of blood flow through the Constipation or stenosis are maintained through so the heart muscle is not damaged. For this purpose, Per fusion catheter. This perfusion catheter is best hen from a hose passed through the blockage, which is provided with openings on its surface. Through the Blood can pass through the tube and from the orifices one side of the constipation flow to the other side.
In practice, however, it has been shown that the perfusion catheter is disadvantageous because there is a risk that the Holes of the perfusion catheter also caused by the formation of Clots can be closed. The hose of the Perfusi on catheter with its openings forms for that through the arte flowing blood namely a foreign body onto which the blood is in reacts to form clots.
Knowing this possible danger of closing the opening against the perfusion catheter and thus the interruption of the blood stream from one side of the constipation to the other side the patient has been given intravenous medication so far, which have an inhibitory effect on the formation of clots practice to block the openings if possible prevent the perfusion catheter.
In the case of intravenous administration, the medication in question arrives ment in the entire bloodstream of the human body, making it a relatively large amount of the inhibitory medication is required provisions to provide the desired effect on the desired To achieve place.
Otherwise there is a risk of intravenous administration that in other places in the bloodstream of the human body - not just at the desired location of the stenosis in question - natural coagulation is inhibited. So if at another If a wound appears in the bloodstream, the the formation of a clot necessary for healing nsels prevented or inhibited.
The intravenous application also comes into play in practice Use when there is no constipation due to overflow stops the clot, because in any case there is in the Bal lon dilation the requirement to form the clot in ver keep reasonable boundaries to harmful cross-sectional union avoid arterial conditions. In any case, it is he required to inhibit relatively large amounts of clot formation to enter the drug into the bloodstream, leading to the described harmful side effects.
The invention is based on the object of a catheter create with which the drug supply is controlled can be.
The problem is solved in the preamble of the patent Claims 1 catheter characterized by the features of the nenden part of claim 1.
The invention provides a catheter which is internal half of his catheter tube an externally accessible Zu sentence channel owns. This additional channel penetrates the catheter hose in the immediate application area. Through this over surprising measure creates the opportunity that is required medical means directly and directly, i.e. locally to the desired place, and in a relatively high con concentration and dosage. One connected to the dilatation balloon dene pressure line points in the area of the parallel to it Additional channel a balloon-like expandable point that the Quantity of transport of medical devices in the application area can regulate moderately. This results in a number of significant benefits.
So it is due to the high dosage with the local application in much improved way possible, a disadvantageous Gerin to prevent island formation. This also applies to the case of on Set of a perfusion catheter, as by the invention intended local and extremely high dose can be prevented that the openings of the perfusion catheter are clotted close education.
A major advantage of the invention is that Extremely high doses in a targeted manner and locally at the desired location can be applied, and thus maximum effectiveness is achieved that the locally supplied amount in Ver further dilute the bloodstream so far that the above not mentioned harmful side effects in other places there are more to fear.
In order to achieve the advantages described, it is single Lich required the catheter according to the invention with a Additional channel and a balloon-like in the blood circulation expandable place, through which the outside desired drug is delivered locally within the artery that can.
If the catheter than with a pressure line leading into the balloon device is formed, the invention provides that the wall the pressure line in an area in front of and in front of the balloon the opening of the additional channel softer than the rest of the wall is formed. The pressure can therefore be in the range mentioned line when a selectable pressure is exceeded within the Expand the pressure line like a balloon and thereby the additional channel squeeze until it locks. By this measure is the possibility of influencing the desired Permeability of the additional channel or the supply of the ent talking drug created through the additional channel.
In a further embodiment of the invention is the catheter trained as a perfusion catheter, the catheter tube in End area is provided with openings. The opening of the additional ka nals is then located locally in front of or in that with the others ren openings provided end portion of the catheter tube, so that it is possible to use desired drugs to prevent the Formation of clots locally directly at the required place to apply.
The catheter according to the invention protects with its novel Build up the angioplasty (enlarged) vascular site, whereby the Principle of the new catheter is essentially based on local application by means of the specially provided Zu sentence channel substances or medication directly at the interven to bring the outcome of the intervention already to protect directly during the intervention.
The catheter is designed so that on the one hand locally extreme high doses can be applied and thus a maximum ef effectiveness can be achieved, and that on the other hand it results in a systemic effect in the patient does not arise, which acts as a syste mixing effect may not be tolerated by the patient en and could lead to serious side effects. Furthermore the catheter is designed so that the drug intervention maintained throughout the local treatment period can be and therefore no interruption of the intervention local treatment or the introduction of further intervention ka theder is necessary.
The location and purpose of the new catheter are all artificial vessels, especially the coronary arteries, renal arteries arm, leg and arteries of the head and neck empire. The catheter can be used with all vessels interventions by, e.g. B. catheter dilatation, atherectomy (Peeling of arteries) with atherectomy catheters or through Laser angioplasty (vascular modulation).

In connection with leukocyte immigration (immigration of white blood cells) and the inhibition of free radicals to think especially of substances that act as radical scavengers or ver prevention of radical formation can apply, such as superoxide dismuta se or catalysis. In particular, is related to blood clotting others on thombinantagonists such as hirudin or other low molecular weight lare, synthetic substances, or heparinoids, or thromboxa natogonists, or prostacyclins, or platelets (thrombozy ten) to think receptor antagonists.
B. Molsidomin, Thromboxane, Pro xazykline, Substanz P o. ä. von Bedeutung. In connection with the inhibition or prevention of the prolifera tion of constituents of the vessel wall is in particular not low heparin and heparinoids, as well as heparin in general, ACE inhibitors or similar. For substances that the faster healing of the resulting wounds from the interven tion can also be non-coagulant with heparin or heparins and heparinoids in general and to ACE Think inhibitor. For substances that affect the vascular tone nitrates and calcium channel blockers as well as nitrate-like substances such as B. Molsidomine, Thromboxane, Pro xacycline, substance P or the like is of importance.
Thus, the new catheter is used overall to prevent or Minimize damage in acute complications of the interventions as well as to prevent or minimize chronic complications tion, or undesirable further developments and effects, the cause of which comes from the actual intervention.
Es zeigen:Based on the embodiments shown in the drawing the invention will be described in more detail below for a better understanding explained. Show it:

Fig. 4 is a detailed view of a portion of the balloon catheter of FIG. 3.
In Fig. 1 a variant of a perfusion catheter with additional channel 18 and without pressure line 20 is shown. In Fig. 2, Fig. 3 and Fig. 4 variants are shown, the balloon catheter according to the invention.
The illustration of FIG. 1 is a schematic cross-sectional view of a perfusion catheter with a catheter tube 14 which is inserted into an artery 10th The artery 10 has a stenosis 12 through which the end region of the catheter tube 14 is passed, which is provided with openings 16 on its surface in this end region. The arrow A indicates the direction of the blood flowing through the artery 10 .
As can be seen in Fig. 1, the openings 16 of the catheter tube 14 are both on the left and on the right side of the stenosis 12 , so that the blood can penetrate the area of the stenosis 12 through these openings. By arrow B it is indicated that the catheter tube 14 of the Perfusionska theter is provided in a manner known per se with the usual control wires etc.
Within the catheter tube 14 extends a Zusatzka channel 18 , which penetrates the catheter tube 14 in the region of the stenosis 12 with its opening 20 . Medicines can then be discharged through the opening 20 into the bloodstream or into the artery 10 via this additional channel 18 .
In FIG. 2, a balloon catheter is shown having a balloon 22. Because of the openings 16, this balloon catheter is to be regarded as a catheter with an additional perfusion device. A pressure line 24 is used to inflate the balloon 22 or to deflate it. Furthermore, the catheter comprises one or more control wires 26 in a manner known per se.
Here too, according to the invention, an additional channel 18 with the opening 20 is again provided within the catheter tube 14 , wherein medication can be administered locally through the additional channel 18 . Because of the formation of the balloon catheter with the per fusion openings 16 , the blood flow is maintained in any case, so that the balloon 22 for a long time - z. B. 5 minutes - can be inflated.
Fig. 3 shows a balloon catheter without a perfusion device. Here, the blood flow through the artery is interrupted when the balloon 28 is inflated. Within the catheter tube 14 , the pressure line 24 extends in order to be able to inflate or deflate the balloon 28 . When using this Ka theter the balloon 28 is successively inflated and deflated. It is important that after the deflation of the balloon 30, the wall surface of the artery is treated immediately, that is, a means is added to prevent the formation of a clot.
This aspect is also taken into account in the catheter according to FIG. 3, like that through the additional channel 18 , through the opening 20 of which the medicament in question can be supplied locally.
Both the catheter according to FIG. 2 and that according to FIG. 3 are constructed with respect to the pressure line 24 in such a way that part of the wall of the pressure line 24 within a region C according to FIGS. 3 and 4 is softer and therefore more flexible than the rest of the wall Pressure line 24 is formed.
When the pressure is supplied to inflate the balloon 28 , the pressure line 24 will then expand to a blocking balloon 30 at the location mentioned, with the result that medication supply via the additional channel 18 is interrupted at this location. Conversely, if the balloon 28 is vented via the pressure line 24 , the path via the additional channel 18 is released again.
This variant takes into account the fact that infusion pumps are generally used continuously for the medicament in question. When the balloon 28 is inflated, and thus when the blood builds up in the artery, the blocking balloon 30 is also formed at the same time, so that the supply of the medication via the additional channel 18 is interrupted in the desired manner. Only when the balloon 28 is vented, and when the blocking balloon 30 also regresses, can the medication be supplied in the desired manner again via the additional channel 18 .
