Description
Drainage pipes - often simply called rails - out elastic material are long in medicine known and serve to keep open narrowed organs and enables the transfer of body or rinsing fluids from one cavity to the other, or to the outside. - A very frequent application for such drainage pipes the splinting of the ureter. In this ureter splint urine is passed from the renal pelvis into the bladder or the Splinting also bridges the urethra and directs the urine from the renal pelvis through the ureter, bladder, urethra outward. - There have been a lot of drainage pipes for splinting proven, the tip of which does not come from the Kidney pelvis slides out, but keeps itself there. - To the These drainage pipes have to be inserted because of their winding shape as a "pig-tail" (pig tail) seemed be referred to, be brought into a stretched shape. The The curvature of the tip and the end of the drainage pipe is characterized by balanced a relatively stiff stylet. This stylet still has a second task. It is used to thread an auxiliary splint. in the As a rule, one end of the drainage tube is in the kidney pelvis, the other end in the bubble. The placement of such a rail done with the help of a cystoscope. The surgeon does that Cystoscope through the urethra into the bladder, optically searches for the Opening of the ureter and pushes the tip of the drainage tube through the ureter mouth into the ureter and then the drain pipe up so far and with it kidney until the drainage tube tip is in the kidney pelvis is coming. For this insertion into the ureter, the must their own curvature stretched round curved drainpipe tip be what a stiff stylet does. To the drain pipe, which is slightly longer than the ureter, so that the Curvature in the drainage tube tip area in the kidney pelvis and the Curvature in the end of the drainpipe in the bladder is required a second tube, an "auxiliary rail", which after Threading the drainage tube on the stylet also on the Mandrin is pushed on and allowed to drain the end of the drainpipe to advance through the cystoscope shaft into the bladder. With the Mandrin is also eliminated for the insertion process numerous drainage pipes existing curvature of the Drainage pipe end, which is to prevent the end of the Drains completely into the ureter.
With the commercially available systems, it is also necessary for the Insertion process at the beginning of both the end of the drainage tube as well also the end of the auxiliary rail with strong clamps against the Press the stylet and fix it for insertion. This relative large and heavy clamps hinder the surgeon in his Work and often bend the mostly made of metal Stylet. - After using the drainage tube when pushing up of a cystoscope (bladder mirror), in the lower part of the The ureter is inserted, the end of the disappears Drainage tube in the cystoscope shaft. At this point the at the end of the drainage pipe, the one firm connection between stylet and drainage tube created had and with it the firm connection between drainage pipe and stylet lifted. From this point on, the Drain rail only forward towards the kidney pelvis be pushed. With every attempt, the drain rail pulling back or rotating, the drain pipe detaches from the Guide stylet and often remains before the drainpipe tip Renal pelvis has reached back in the ureter and in the bladder, and must now be in its own with a pair of pliers Operation can be removed. - These rotary movements and Pulling back the drainage tube are common with the irregular and narrowed ureter courses often do not bypass. It is therefore the task of the auxiliary track to become one To contribute to the stiffening of the stylet where it is not Drainage rail is surrounded and continue to pull out Mandrins to support the drainage tube, that is, to ensure that when pulling out the stylet, the drainpipe is not itself is pulled out again.
From DE 33 39 179 A1 it is known to manufacture the drainage tube (called ureteral splint there and usually also referred to as ureter splint) and an associated tubular rail feed piece with one another from the same material. Drainage pipe and rail feed are pulled over a rigid stylet. Perforations are provided at the transition from the area of the rail feed to the area of the drainage pipe. After inserting the drainage pipe into the intended end position, the drainage pipe is separated from the rail feed by a tear-off process in the area of the perforations. The object and solution of the invention are not affected. The same applies to an arrangement according to DE 86 14 013 U1. Here, too, a hose with a predetermined breaking point actuated by tearing is provided in the form of perforations, with a wire forming the stylet being located within a drainage pipe (called a guide rail there). From DE 35 11 448 A1 a jamming of a stylet within a drainage tube can be achieved by spreading apart the inside of the drainage tube tip provided inside. According to the embodiment according to FIG. 4 of DE 35 11 448 A1 there is a releasable jamming of an auxiliary rail with the stylet. From this Ausfüh tion, the present invention is based on the Oberbe handles of claims 1, 2 and 4 explained below. A disadvantage of the subject of 35 11 448 A1 is that handling, in particular solving the displacement of the relevant end of the stylet, is difficult. In addition, it is not possible to clearly determine from the outside whether this spread has been set sufficiently, too weakly or too strongly by the attending doctor.
From US 4,684,369 is a medical instrument with a needle to pierce the bladder wall and Discharge of liquid a screw connection between the needle and a drain hose. Task and solution of the present invention is not affected by this either.
In contrast, the object of the invention is the detachable connection of the drainage pipe to the man to improve in that the difficult Insert the drainage tube into the ureter Withdrawing as well as rotating movements of the drainage pipe are easily and easily possible.
Such a connection consists in a first execution rungsform of the invention according to claim 1 in the Ver screwing an external thread of the stylet tip with a counter thread either the inside wall of the drainage tube itself or a tube inserted in it.
Another embodiment of the invention is the subject of Claim 2. According to this, the thread is in the drainage pipe end provided, which is related to the Claim 1 mentioned tip of the drain pipe set lies. In this embodiment, the So the stylet has no screw thread in the tip area; but instead in its middle section over an external ge winch, which has a counter thread in the inner lumen of the drainage opposite pipe end area and ver can screw.
To unscrew the stylet from the drain pipe easy and safe to design and a shift ben or twisting between drainage pipe and auxiliary To prevent rail is the execution according to claim 3 provided.
Another embodiment of the invention is the subject of Claim 4. Here, unwanted twists of the This prevents mandrins within the drainage tube that the angularly profiled stylet tip in one appropriately profiled inner lumen of the top range ches of the drainage pipe. This connection but allows you to rotate the stylet deliberately to the drainage pipe via the aforementioned profiles transfer; if this should be necessary. Further is a screw connection in the form of a tooth the drainage pipe and the auxiliary rail on the Draina tubular and the auxiliary rail tip provided.
According to claim 5 can in all the aforementioned Ausfüh forms the releasable jamming of the person concerned The stylet ends with the auxiliary rail through a clamping ring in this end area, which in the clamped state Auxiliary splint firmly pressed with the stylet.
The following will refer to the above explained claims the function of this execution possibilities of the invention explained.
The drainage tube is inserted according to claim 1 as follows: First, over the stylet, - which is a little longer is together as the drain pipe and auxiliary rail - the auxiliary rail plugged in. Thereafter, the stylet is in accordance with claim 1 with its external thread in the tip area into the counter thread screwed into the inner lumen of the drainpipe tip. The top of the Auxiliary rail will go as far as the end of the drain pipe advanced until the end of the drainage pipe according to claim 3 with the tip of the auxiliary rail due to their profiles with each other interlock. This position of the drain pipe, auxiliary splint and stylet one another on the auxiliary rail in the end area fixed clamping ring or a clamp attached here. According to claim 2, the screwing of drainage pipe and Auxiliary rail in the same way. Only this is done here Screwing by means of the stylet not in the tip area but in the end area of the drainage pipe.
According to claim 4, the drainage pipe is attached to the Auxiliary rail by means of a screwing possibility caused by corresponding recesses and projections at the end of the drain pipe and the auxiliary rail tip. Here is before introducing the First threaded the auxiliary splint onto the stylet and then the drain pipe is plugged on. The stylet is now in so far inserted the drain pipe until its angular profile Tip fully into the correspondingly shaped inner lumen section of the Drain pipe tip has added. Then be Drainage pipe end and auxiliary rail tip by rotating movements connected to each other and with the help of the clamping ring Auxiliary rail end pressed onto the stylet. This functional Unity is introduced as described. After correct The placement of the drainage tube tip in the renal pelvis is the Clamping ring released. While the angular profile of the stylet tip the Drainage pipe holds in place by rotating movement the auxiliary rail loosened the screw connection to the drain pipe and then with removal of the stylet and then leave the drainage tube in the body of the auxiliary splint.
The aforementioned jamming of the auxiliary rail end with the stylet with a clamping ring and loosening it Jamming also occurs when handling the execution Rungsbeispiele according to claim 1 and 2 instead.
The subject matter of the invention is explained with reference to exemplary embodiments shown in FIGS. 1, 2 and 3.

Fig. 3 shows a longitudinal section through the drainage tube with stylet and auxiliary rail with a screw connection of the walls of the drainage tube and auxiliary rail.
Fig. 1 shows a longitudinal section through the drainage pipe 5, the mandrel 4 and the auxiliary rail 9. The screw mandrel 4 consists of a hard elastic material and has an external thread 2 in the tip area, which can be screwed into or out of the counter thread of the drainage pipe 5 . With this screwing in or out, the stylet 4 is firmly connected to the drainage tube 5 or separated again. - These rotary movements are facilitated with the help of the handpiece 10 . To facilitate the pushing up of the drainage tube 5 through the urethra and bladder, the auxiliary splint 9 is pushed over the stylet 4 and, like the stylet, is removed again after the correct placement of the drainage tube. So that the auxiliary rail 9 cannot rotate with respect to the drainage tube 5 during the rotational movements of the stylet 4 , the auxiliary rail 9 has protrusions 8 which engage in corresponding recesses 6 of the drainage tube end 7 and lead to a toothing of the auxiliary rail 9 with the drainage tube 5 . A clamping ring 16 , which can also be replaced by an ordinary clamp, presses the auxiliary rail firmly onto the stylet and thus ensures that the teeth cannot come off prematurely.
Alle übrigen Elemente entsprechen der Fig. 1.In Fig. 2 is a longitudinal section through the drainage pipe 5, the mandrel 4 and the auxiliary rail 9 is reproduced with a screw in the region of the drainage tube end 7. In this exemplary embodiment, the screw connection does not take place in the tip area of the drain pipe 1 , but in the area of the drain pipe end 7 . The external thread 11 on the stylet shaft 4 is opposed by a counter thread 12 in the region of the drain pipe end. All other elements correspond to FIG. 1.
In Fig. 3 a longitudinal section through the drainage pipe 5, the mandrel 4 and the auxiliary rail 9 is reproduced with a screw connection of the walls of the drainage tube end and of the auxiliary bar tip.
