Verzögerte oder ausbleibende Konsolidierung von Frakturen sowie die Rekonstruktion knöcherner Defekte stellen sowohl aus chirurgischer als auch aus ökonomischer Sicht eine große Herausforderung dar. Da für au.tologe Transplantationen die Ressourcen begrenzt sind und ein nicht zu vernachlässigendes Risiko bei Zweiteingriffen zur Gewinnung von körpereigenem Knochenmaterial besteht, wird seit jeher nach Alternativen zur autologen Spongiosatransplantation gesucht (1). Die Vorläufer reifer Osteoblasten finden sich in Knochenmark und können aus diesem erfolgreich isoliert und angereichert werden. Bekanntermaßen stellt plazentares Restblut aus der Nabelschnur Neugeborener eine Alternative zu Knochenmark als Stammzellpool der Hämatopoese dar (2). Bislang wurde Existenz von Osteoblasten oder deren Vorläufer im Nabelschnurblut nicht beschrieben.Delayed or non-existent consolidation of fractures and the reconstruction of bone defects represent a major challenge from both a surgical and an economic point of view. Im Gegensatz zu den begrenzten Ressourcen der autologen Spongiosaplastik sind durch die Existenz von Nabelschnurblut-Banken die Ressourcen für eine Transplantation von Osteoblasten und Osteoblastenvorläufern nahezu unbegrenzt vorhanden. Durch die große Zahl an vorhandenen asservierten Nabelschnurblut-Transplantaten ist es weiterhin möglich, HLA-angepaßte Osteoblasten und Osteoblastenvorläufer einzusetzen, um etwaige Probleme bezüglich der Gewebeverträglichkeit zu minimieren. Desweiteren bestehen durch den Ersatz autologer Transplantationen von Spongiosa durch die allogene Transplantation von Osteoblasten und deren Vorläufern aus Nabelschnurblut ökonomische Vorteile, da ein Zweiteingriff zur Entnahme der Spongiosa entfällt.
