Die Erfindung betrifft ein Stabinstrument für laparoskopisches Operieren, insbesondere zur Anwendung bei der minimal invasiven operativen Therapie der Gastro-ösophagealen Refluxerkrankung.The invention relates to a rod instrument for laparoscopic surgery, in particular for use in the minimally invasive surgical treatment of gastroesophageal reflux disease.
Bei manchen Menschen kann es zu einem das normale Maß überschreitenden chronischen Rückfluß von saurem Magensaft aus dem Magen in die Speiseröhre (Ösophagus) kommen. Neben konservativen Behandlungsmethoden mittels Protonenpumpenhemmern etc. gibt es die Möglichkeit einer operativen Therapie. Diese wäre z.B. bei therapieresistenten Fällen bezüglich der medikamentösen Therapie indiziert.The constant reflux of gastric juice can have pathological consequences, such as so-called reflux esophagitis. Die Grundlage der operativen Therapie ist die Verstärkung des gastro-ösophagealen Überganges. Dazu wird als Verstärkung des unteren Ösophagussphinkters in diesem Bereich ein kleiner Anteil des Magens ( Magenfundusanteil) manschettenförmig um den fraglichen Übergang gelegt und fixiert. Dieser operative Eingriff nennt sich Fundoplicatio. Je nach Schule wird die Manschette zirkulär (360°) oder hemizirkulär (180°) gelegt. Dazu wird allerdings während der gesamten OP eine Hand eines Assistenten benötigt, da der Retraktor intraabdominell nicht fixiert werden kann und somit ständig in Position gehalten werden muß. d.h., für die Operation werden neben dem Operateur insgesamt 2 Assistenten benötigt.1.
