Description
ihrer Sübtypen, Sonderformen, sowie zur topischentheir subtypes, special forms, as well as topical
The invention relates to a new externally applicable khellin preparation suitable for application to the skin for the phototherapeutic treatment of psoriasis, its subtypes, special forms and eczema.
Psoriasis is one of the most common skin diseases with a serious psychosocial burden for the affected patients. The clinical picture is characterized by sharply defined erythematous plaques of various sizes and shapes covered with silvery-white scales, sometimes itchy. Different genetic predispositions together with trigger factors such as bacterial infections, stress and medication lead to a wide spectrum of clinical manifestations. The disease usually has a chronic, recurrent course.
Zudem ist die Behandlung vonThe treatment of psoriasis aims at remission with as long-lasting freedom from symptoms as possible. Moderate to severe psoriasis is usually treated with oral or local photochemotherapy with 8-methoxypsoralen (8-MOP) and subsequent UVA irradiation (= PUVA therapy). Due to the accompanying nausea, oral therapy often cannot be used. In addition, the treatment of
Gegenwärtig gibt es jedoch kein zufriedenstellendes Medikament zur Behandlung der Schuppenflechte.accompanied by very high phototoxicity and requires extreme safety precautions and intensive cooperation from the patient. Important data also suggest that PUVA therapy provokes the occurrence of malignant tumors of the skin, especially squamous cell carcinomas. PUVA phototherapy is therefore generally only used from the 4th to 5th decade of life. A possible alternative to oral PUVA therapy is bath PUVA therapy with 8-methoxypsoralen or trimethylpsoralen. The carcinogenicity of this photochemotherapy appears to be more favorable than that of oral PUVA therapy. Reliable data are not yet available. Bath PUVA therapies require a considerable amount of time, cause very great logistical difficulties, since the bath unit and radiation unit must be in direct proximity, and are difficult to control therapeutically, since they are associated with very pronounced photosensitivity and can therefore easily cause phototoxic reactions. However, there is currently no satisfactory drug for treating psoriasis.
Eczema occurs either endogenously, such as atopic eczema, or through the acute or chronic impact of specific (allergen) or non-specific (mechanical irritation, pressure, chemicals, frequent cleaning of the skin) stimuli on the skin. The skin reacts in turn with an inflammatory infiltrate of lymphocytes and other mononuclear cells and possibly also increased growth (proliferation). To analyze the effectiveness of KUVA therapy, we have examined the effectiveness of the treatment for palmoplantar eczema as an example.
The invention is therefore based on the object of providing a drug for the treatment of psoriasis that overcomes the disadvantages of previous preparations. The drug should be particularly suitable for the phototherapeutic treatment of psoriasis. In particular, the drug should be in a form that is acceptable to the patient and causes remission of the symptoms after a suitable period of time. Furthermore, the drug should be safe, easy to use and have few side effects. It should also be suitable for long-term therapy and be able to be administered satisfactorily to young patients.
Surprisingly, it has now been found that the furanochromone khellin and its derivatives, which lead to monoadducts, can be used topically for the photochemotherapeutic treatment of psoriasis, especially when formulated as a preparation suitable for application to the skin, without causing harmful side effects.
The invention therefore relates to a preparation for the topical treatment of psoriasis, its special forms and subtypes, hand eczema, and a preparation that can be spread or applied to the skin by spreading measures, which contains khellin and its derivatives, which lead to monoadducts, in a dermatologically compatible base that can absorb hydrophobic/lipophilic substances.
Im Gegensatz zu 8-Methoxypsoralen, einem weit verbreiteten Photosensibilisator, der zur PUVA-Photochemotherapie eingesetzt wird, hat Khellin nur eine sehr geringe Fähigkeit, Diaddukte auszubilden.4,9-Dimethoxy-7-methyl-5H-furo[3,2-g][l] benzopyran-5-one, or khellin, is a furanochromone that has been used for phototherapeutic purposes for over 3000 years. The substance is extracted from the Ammei fruits of the shrub Ammi visnaga, which is native to the Mediterranean region. In the late 1940s, khellin was used orally as a vasodilator and was prescribed for many years as an antihypertensive drug and vasodilator for bronchial asthma. No long-term effects were observed. In contrast to 8-methoxypsoralen, a widely used photosensitizer used for PUVA photochemotherapy, khellin has only a very low ability to form diadducts.
In dermatology, Khellin tablets are used for oral Khellin UVA phototherapy of vitiligo (white spot disease) (Dermatologica, 165, 136-140, 1982). In addition, recent case studies show that Khellin in a cream base in combination with subsequent UVA irradiation can be used with moderate success for alopecia areata and vitiligo. For these indications, the results are said to be comparable to those of oral PUVA treatment. No precise comparison is available.
Bei einer Photochemotherapie, die unter Zuhilfenahme derIt was also shown that when 100 mg of khellin was administered orally and then exposed to the sun, 80% of 10 patients with plaque psoriasis responded to the treatment and 6 out of 8 of these patients were symptom-free for three years (Dermatologica, 167, 109-110, 1983). However, khellin has not yet been used for the topical treatment of psoriasis. Photochemotherapy, which is carried out with the aid of
Sun exposure makes it very difficult to separate the effect of sun therapy from that of photochemotherapy, since the sun's rays contain large amounts of UVB radiation, which in itself can lead to the healing of psoriasis.
Experience with conventional treatment of psoriasis with 8-MOP and subsequent UVA irradiation has suggested that phototoxic irritation or treatment close to the phototoxic threshold is necessary in order to treat psoriasis as successfully as possible using photochemotherapy (PUVA). However, this does not apply to the treatment of vitiligo. Since the treatment described for vitiligo with khellin and subsequent UVA irradiation does not lead to clinically visible phototoxic reactions, it has so far seemed unlikely that topical therapy with khellin and subsequent UVA irradiation would be suitable for the treatment of psoriasis.
In Kombination mit einer anschließenden UVA-oder UVAl-Bestrahlung eignet sich diese Zubereitung mit gutem Erfolg zur topischen Verwendung in der phototherapeutischen Behandlung derIt has now surprisingly been shown that psoriasis, including palmoplantar psoriasis, as well as eczema can be successfully treated with khellin. The prerequisite is that khellin or its derivatives, which lead to monoadducts, are applied to the skin in an external base that can absorb hydrophobic/lipophilic substances and the skin is then irradiated with UVA or UVAl. In combination with subsequent UVA or UVAl irradiation, this preparation is suitable for topical use in the phototherapeutic treatment of
Schuppenflechte. Psoriasis. The preparation was well tolerated and did not lead to any negative local or systemic reactions when used with subsequent phototherapy.
When used with phototherapy (photochemotherapy), the khellin preparation according to the invention shows very good effectiveness in the various forms of psoriasis, palmoplantar psoriasis and hand and foot eczema. It has a very good local anti-inflammatory effect and leads to a rapid regression of the skin symptoms of psoriasis. The remission was confirmed histologically.
Weiterhin führt eine Khellin-UVA-Therapie nicht zu den bekannten, oben teilweise erwähnten, systemischen Nebenwirkungen der oralen Phototherapie mit 8-MOP (es ist keine Übelkeit beobachtet worden) und ist logistisch wesentlichKhellin has the advantage that, according to current knowledge, no phototoxic reactions occur and that it has only a low photomutagenic potential, especially in comparison with 8-MOP. One of the reasons for the low photomutagenic potential may be the very low ability of khellin to form diadducts, which are seen in connection with the carcinogenic properties of photosensitizers. No significant mutagenicity was observed in green algae and only a low photomutagenicity in bacteria. Animal experiments showed that UVA phototherapy with khellin has a significantly lower carcinogenic risk than the previously common photochemotherapy with 8-MOP. Furthermore, khellin UVA therapy does not lead to the known systemic side effects of oral phototherapy with 8-MOP, some of which have been mentioned above (no nausea was observed) and is logistically much easier.
einfacher und zeitsparender als die bisher bekannte Alternative, die Bade-PUVA-Therapie.simpler and more time-saving than the previously known alternative, bath PUVA therapy.
The khellin preparation according to the invention contains khellin and a dermatologically compatible base. Es traten aber keine phototoxischen Reaktionen oder ungewöhnliche Hyperpigmentierungen derThe Khellin preparation according to the invention is suitable for use in the phototherapeutic/photochemotherapeutic treatment of all forms of psoriasis, in particular the chronic moderate and severe form, including palmoplantar psoriasis and hand and foot eczema. The treatment is easy to carry out. Normal pigmentation of the skin can be observed. However, no phototoxic reactions or unusual hyperpigmentation of the skin occurred.

Durchschnittlich werden 20-30 Behandlungen benötigt. The amount of Khellin to be applied at any one time depends on the patient's condition, the severity of the disease, the duration of the treatment, etc. and is determined by the attending physician. The usual dose is 1-10 J/cm 2 , and the treatment is carried out about 4 times a week. On average, 20-30 treatments are required.
The invention is explained in more detail with reference to the following examples.
The psoriasis plaques are treated with the Khellin preparation, the Khellin preparation is left to work for one hour. Six patients (3 women, 3 men; average age 55 years, 38-76 years) had suffered from psoriasis for over 10 years. Histological examinations revealed thickening of the epidermis with elongation of the rete ridges and Munro's microabscesses. Numerous pro-inflammatory lymphocytes and neutrophil granulocytes were found in the dermis, thus confirming the diagnosis of psoriasis.
Treatment was carried out with the inventive Khellin cream in combination with UVA radiation. On average, the patients received 23.2 treatments. After treatment with the inventive Khellin UVA phototherapy, a large-scale normalization of the entire structure of the epidermis was seen in all patients with a reduction in pro-inflammatory lymphocytes. These therapeutic successes are comparable to the results that can be achieved with conventional PUVA therapy with 8-MOP.

The effectiveness of phototherapy using the Khellin cream according to the invention was analyzed in comparison with bath PUVA and UVB-311nm phototherapy, two recognized effective treatment methods for psoriasis. In particularly severe forms of psoriasis, UVB-311nm phototherapy was carried out in combination with a topical therapy such as anthralin. Such a combination therapy usually requires inpatient treatment, since treatment with anthralin cannot be carried out at home.
Die UVB-311nm-Phototherapie nutzt im UVB-Anteil die Wellenlänge 311nm, da diese imBath PUVA therapy is based on the principle of photosensitization through 8-MOP and subsequent UVA irradiation. Im Vergleich hierzu zeigen die Bade-PUVA-TherapieA comparison of cream-PUVA therapy, patients (n=32) with bath PUVA (n=40) and UVB-311nm phototherapy (n=144) alone or in combination with external therapy is shown in the course. Starting from a mean PASI score of 22.9 before starting therapy with cream-KUVA, a rapid, significant and sustained improvement in the clinical condition occurred after one week (14.9), 3 weeks (7.3), 4 weeks (6.9) and 5 weeks (4.7). In comparison, bath PUVA therapy
It should be noted that in the cream-KUVA group the severity of the psoriasis before the start of therapy was greater (mean PASI score 22.9) than in the group treated with UVB-311nm (mean PASI score 18.4). In summary, cream-KUVA therapy proves to be at least as fast and as efficient a form of treatment for psoriasis as the two established forms of therapy.

The effectiveness of phototherapy using the Khellin cream according to the invention was investigated in comparison with bath PUVA phototherapy for psoriasis on the hands and feet, including the pustular forms. It was found that patients responded more quickly on average with the Khellin cream phototherapy according to the invention and required a lower number of treatments overall. The cumulative UVA dose was higher on average with the Khellin cream phototherapy according to the invention, due to the possibility of a faster increase due to the lack of phototoxic reactions during therapy. The proportion of complete remissions, i.e. complete healing, was comparable in both treatment methods.
