Title: Anti-Varus Shoe

Description

The invention relates to a shoe, in particular for children or toddlers, with a shaft that encloses a foot from above, according to the preamble of claim 1.

In small children, correction of a so-called sickle foot is often necessary. A sickle foot is a foot that has the following deformities to a greater or lesser extent, although some deformities may be completely absent: The forefoot area is slightly to strongly adducted medially, the heel of the foot is in a neutral to valgus position, with the heel bone opposite

one ankle bone may be displaced medially, and there is a slight to severe internal rotation of the foot or possibly the entire leg.

These deformities must be corrected at the same time for successful treatment. In a primary correction phase, a sickle foot splint is used to press and fix the foot in a corrected position in a so-called 3-point correction. In the case of particularly severe deformities or if the sickle foot splint is unsuccessful, surgery may be required.

Treatment is carried out at a child's early age, as the foot skeleton is still deformable or malleable during this phase of child development. However, the problem arises that after the primary correction phase has ended, a possible regression of the deformity back to clubfoot, i.e. a recurrence of the clubfoot, can be observed due to growth spurts and the further deformability of the child's foot skeleton up to the stage of learning to walk.

It is the object of the present invention to provide a shoe of the above-mentioned type which stabilizes the success of the treatment of a primary regression of a clubfoot position and ensures this up to and including the child's learning to walk phase.

This object is achieved by a shoe of the above-mentioned type with the features characterized in claim 1.

For this purpose, the invention provides that a first pad is arranged integrated into the shaft on a lateral side of the foot adjacent to its cuboid and a second pad is arranged on a medial side of the foot adjacent to its metatarsophalangeal joint.

This has the advantage that in a secondary correction phase of a sickle foot, the first pad holds the foot behind the base five, i.e. at the tuberosity of the metatarsal bone, and at the same time forms a hypomochlion, i.e. a pivot point, on the cuboid or cuboid bone. In this way, treatment success is also stabilized and secured postoperatively or after completion of the corrective treatment for treating a sickle foot. The first pad provides a further corrective lever-like force with a hypomochlion on the cuboid and the second pad forms a counter-bearing and prevents the formation of a hallux valgus due to partially excessive pressure by redistributing pressure. This supportive measure is also included directly in the shoe according to the invention without any additional device measures, so that the treatment in this second phase of the correction is almost unnoticed, at least externally, and therefore no acceptance problems arise, especially in children and toddlers. Because the pads are firmly integrated into the shaft of the shoe, in addition to the abduction position of the last, so-called soft tissue correction, a so-called skeletal correction is also carried out after completion of a primary repositioning of the deformed foot, for example surgical or by means of a splint.

Preferably further designs of the shoe are described in the subclaims.

The shoe is preferably an anti-varus shoe with an integrated abduction of the foot of 6 degrees in particular.

A long-lasting and constant correction effect without pressure points is achieved by making the pads from a soft, particularly resilient material, particularly silicone.

Because the second pad runs forward to a joint gap and beyond it in a thin, padded flap, the child's foot in particular is guided into a corrected position with a soft, precisely placed pad.

In a particularly preferred embodiment, the first pad runs in a straight line in cross section on a side facing away from the foot and is curved on a side facing the foot, with a curvature of the curved formation initially increasing from the base 5 towards the heel, reaching a peak at a point forming the hypomochlion and then decreasing again.

In a further particularly preferred embodiment, the second pad runs in a straight line in cross section on a side facing away from the foot and is formed in an arched shape with two bumps on a side facing the foot, so that it essentially follows a contour of the metatarsophalangeal joint of the big toe.

The invention is explained in more detail below with reference to the attached drawing. This shows a schematic sectional view of the arrangement of pads in a shaft of a shoe according to the invention in relation to a foot skeleton.

The preferred embodiment of a shoe 100 according to the invention shown in the figure comprises a shaft 10 with an upper leather 12 and a lining 14. The shaft 10 encloses a foot 16 from above, which is shown in a skeleton representation for better illustration. In the shaft 10, in the preferred embodiment 100, a first pad 18 and a second pad 20 are arranged between the upper leather 12 and the lining 14 of the shaft 10. These pads 18 and 20 are thus firmly integrated into the shaft 10.

The first pad is arranged laterally with respect to the foot 16 at the level of the cuboid or cube bone 22 and is designed in such a way that a hypomochlion, i.e. a pivot point, is formed on the cuboid 22. A forefoot area 24 and a heel area 26 are bent in a rotating manner around this pivot point, i.e. abducted or bent laterally, in such a way that the sickle-foot-like deformation of the foot 16 is counteracted. The pivot point or hypomochlion is indicated by 28 and is located essentially at a vertex of an arched side of the first pad 18 facing the foot 16. This simultaneously supports the foot 16 behind the base 30 and thus prevents the heel area 26 from slipping laterally. The hypomochlion 28 is thus formed behind the base 30 and next to the cuboid 22 with respect to the foot 16.

The second pad 20 is provided medially with respect to the foot 16 at the level of a metatarsophalangeal joint 32 behind a metatarsal thickening 34. This second pad 20 runs forward with respect to the foot 16 to a joint gap 36 and beyond this joint gap 36 in a thin padded flap 38.

The pads 18 and 20 are preferably made of soft but particularly resilient material, such as silicone. This guarantees a long-lasting and constant corrective effect without causing pressure points.

As a result, the shoe according to the invention ensures both a slight regression of the foot, which is still flexible in childhood, and also causes an active abduction of the forefoot area 24 while simultaneously supporting the heel. The bony structures of the foot 16 are thus taken into account in the treatment and, in this second or secondary stage after completion of the primary regression, a further regression with dynamic correction of the foot deformity is achieved.

The integration of the pads 18 and 20 into the shaft 10 according to the invention is particularly advantageously combined with an anti-varus shoe. This is a shoe which, like a sickle foot orthosis, also has a corrective effect with, for example, 6 degrees of abduction, i.e. bending of the foot laterally, around the hypomochlion 28. The shoes 100 are cut so inconspicuously that their corrective effect is hardly noticeable. These shoes are available for toddlers and children in sizes 20 to 32, so that a corresponding slight correction of the foot 16 up to preschool age can be carried out without any problems. This successfully prevents a recurrence of sickle foot. A toe offset of around 5 degrees in the forefoot area ensures easy rolling. A stable and at the same time light outsole can be subsequently sanded down. The pads 18 and 20 also ensure a more even correction pressure. A reinforced heel guide also ensures a secure hold and a correct fit. Additional neutral half insoles are removable and can be sanded down. A large entry opening makes it easier to put on the shoes 100.

It is particularly advantageous in a shoe 100 that consists of an outer shoe or upper leather 12 and an inner lining 14 if the two pads 18, 20 are arranged between the inner lining 14 and the outer shoe 12, each pad then being non-slip and secured against displacement in a pocket-like interior, which is obtained by gluing the inner lining all around to the outer shoe at the edge of the pad or using other connecting means, whereby gluing the pad in is also possible. The pad can also be attached via a pressure

button connection or a Velcro connection on the inner wall side of the outer shoe or on the wall surface of the inner lining facing the outer shoe.

In order to be able to hold the heel bone 26' from the medial side in order to achieve a safe forefoot correction in the shoe, it is advantageous if, as indicated in the drawing at 40' by the dashed line, the large width of the shoe is reduced on the medial side in the area of the heel bone 26', since otherwise the heel could evade the correction pressure medially. The shoe 100 is therefore provided with a recess 40 in the heel area.

On the other hand, if a normal shoe 100 is assumed, it is also possible to fill the gap 50 between the shaft of the shoe and the heel bone 26' with a pad, which preferably consists of an incompressible plastic, such as silicone rubber, in order to prevent the heel from deviating medially from the correction pressure.

Claims (10)

9 Claims 1. Shoe (100), in particular for children or toddlers, with a shaft enclosing a foot (16) from above (10), characterized, that integrated into the shaft (10) a first pad (18) is arranged on a lateral side of the foot (16) adjacent to its cuboid (22) and a second pad (20) is arranged on a medial side of the foot (16) adjacent to its big toe joint (32). 2. Shoe (100) according to claim 1,
thereby recognized, that this is an anti-varus shoe (100) with an integrated abduction of the foot (16) of in particular 6 degrees. 3. Shoe (100) according to one of the preceding claims, characterized in that the second pad (20) is straight in cross-section on a side facing away from the foot (16) and is arched with two bumps on a side facing the foot (16) in such a way that it essentially follows a contour of the metatarsophalangeal joint (32). 7. Shoe (100) according to one of claims 1 to 6, characterized in that the pads (18, 20) are arranged in a shoe (100) consisting of an outer shoe or the upper leather (12) and an inner lining (14) between the inner lining (14) and the outer shoe (12). 8. Shoe (100) according to one of claims 1 to 7, characterized in that for the correction of the forefoot and the support of the heel bone (26') the shoe width in the heel area is reduced medially up to approximately the heel bone. 9.
