Bezeichnung:Designation:
Commercially available occlusion balloon catheters with circular fixation of both balloon ends to the catheter shaft can have the disadvantage that they open and close eccentrically to the catheter shaft in an uncontrolled and non-reproducible manner. If the opening of the balloon channel on the catheter shaft is closed like a valve by the skin of the eccentrically deployed balloon, balloon deflation is no longer possible. In such cases, a balloon inserted into the patient's body (e.g. vena spermatica occlusion balloon) could only be deflated and removed again by percutaneous needle puncture.
Bei der perkutanen transkatheteralen Embolisationsbehandlung von Gefäßaussackungen (Aneurysmen) durch Metall Spiralen oderIn the percutaneous transcatheteral embolization treatment of vascular aneurysms using metal spirals or
In the case of liquid embolism (£ "Oj.-C^aViacr^XaJtj) the difficulty arises of filling the existing vascular bulge as completely as possible with the embolism without allowing any of the applied material to flow into the vessel. This is easier to do in aneurysms with a narrow "neck" than in aneurysms that have a broad-based contact with the vessel. In the latter case, catheter treatment often has to be dispensed with due to the risk of embolism being carried away.
The invention is based on the object of proposing a balloon catheter with special design features which, with reliable reproducible inflation and deflation, enables the complete filling of broad-based aneurysms with embolic material, without the risk of embolic material being carried away.
To solve this problem, a device with the features specified in the characterizing part of claim 1 is proposed.
The balloon catheter is positioned so that the balloon can be inflated in the area of the aneurysmal vessel segment. The part of the balloon envelope that is directly attached to the catheter shaft is oriented towards the aneurysm neck. Under fluoroscopy, this alignment can be made easier by a radiopaque marking on the catheter shaft. When a longitudinal strip of the balloon envelope is attached to the catheter shaft, the balloon opens eccentrically, in a predictable and reproducible manner. Because the balloon channel opens opposite the fixed part of the balloon envelope, the balloon envelope cannot lie on the opening of the balloon channel. Accidental closure of the balloon channel, which would hinder or prevent balloon deflation, is thus ruled out.
Ober einen Arbeitskanal, dessen öffnung auf dem den fixiei— ten Ballonanteil tragenden und dem Aneurysmahals zugewandtenWhen the eccentrically deployable balloon is inflated, the vessel segment and the aneurysm neck are closed. Using a working channel, the opening of which is on the side that carries the fixed balloon part and faces the aneurysm neck.
Embolization material such as cyanoacrylate or metal coils can be transported into the aneurysm. Since the aneurysm neck is closed, the aneurysmal bulge can be completely filled without the risk of the embolization being carried into the actual vessel lumen.
During the introduction of the embolic agent, the expanded balloon blocks the blood flow in the vessel. This can be problematic, especially when treating cerebral aneurysms.
According to a special design of the eccentrically deployable balloon catheter, it has a second point on the balloon circumference and over the entire length of the balloon, at which the balloon is limited in its complete radial expansion by being fixed to the catheter shaft. This can be achieved, for example, by a non-stretchable thread that runs on the outside of the balloon envelope or is cast into it, or by a septum running inside the balloon. Preferably, the second balloon section, which is restricted in its deployment, is located exactly opposite the balloon strip described above that is directly attached to the catheter shaft. When such a balloon is deployed, the aneurysm neck is closed for embolization treatment, while the remaining cross-section of the vessel remains open, so that the blood flow is restricted but not interrupted during treatment.

Es zeigt:The drawings show embodiments of the balloon catheter according to the invention and are described in more detail below. It shows:

Schnitt A-A). Der Ballonkanal 4 mündet in Richtung des radial entfaltbaren Ballonanteils, der Arbeitskanal 5 mitten auf dem fixierten Ballonanteil 3.Fig. 1 shows the distal catheter shaft 1 with the balloon 2 deployed. In addition to the usual balloon fixation at both ends of the balloon sleeve to the catheter shaft, a strip of the balloon sleeve 3 with a width of χ oriented in the catheter's longitudinal axis adheres to the catheter shaft. The balloon can therefore only unfold in a predetermined orientation eccentrically to the catheter shaft (see section A-A). The balloon channel 4 opens in the direction of the radially deployable balloon part, the working channel 5 in the middle of the fixed balloon part 3.
Fig. 2 shows the orientation of the balloon in relation to the aneurysm. The opening of the working channel 5 points in the direction of the aneurysm 6. When the balloon 2 is inflated, the neck of the aneurysm 6 is sealed off from the blood vessel 7. However, in this embodiment, the vessel 7 itself is also completely closed, and embolic material 8 can be transported into the aneurysm 6 via the working channel 5.
Bei Balloninflation resultiert ein freier Restquerschnitt 10 (in Schnitt D-D schraffiert dargestellt) des Gefäßes 7.Fig. 3 shows an embodiment of the eccentrically deployable balloon catheter, which has all the features described in Fig. 1 and 2 and is additionally characterized by an opposite deployment limitation of the balloon envelope. This is implemented by a filament 9 located on the outside of the balloon or inserted into the balloon envelope, which limits the complete radial expansion of the balloon envelope. The filament 9 has a significantly reduced extensibility compared to the balloon envelope 2. Both ends of the filament 9 are attached to the catheter shaft 1. When the balloon is inflated, a free remaining cross-section 10 (shown hatched in section D-D) of the vessel 7 results.
