The invention relates to an acetabular cup prosthesis for cementless anchoring with as Thread formed support ribs with at least two cutting grooves.
Prostheses for replacing the hip joint have been known for a long time. At first only the ball was on Femoral neck replaced, later both hip joint parts followed, i. H. the pan in the basin and the ball on Femoral neck.
The biggest problem was the anchoring of the hip joint prosthesis, especially the Acetabulum, out.
Today, the anchoring is mostly done with bone cement made from methyl methacrylate. This procedure is relatively simple from an operational point of view and has the advantage that an immediate load stability is achieved can be, but it is still known because of the thermal loads during the hardening of the cement, also because of any toxic effects of the low polymer substances and the uncontrollable running and mostly harmful long-term reactions are extremely problematic.
Eine ohne Verwendung von Zement fixierbare Hüftgelenkprothese ist beispielsweise beschrieben in »Complete Replacement Arthroplasty of the Hip by the Ring Prosthesis«, (|. of Bone and Joint Surg. Vol. 50 B, No_4, Nov. 1968, pages 720-731).For this reason, the use of cement-anchored hip joint prostheses is already in some cases especially acetabular cups. One that can be fixed without the use of cement Hip joint prosthesis is described, for example, in “Complete Replacement Arthroplasty of the Hip by the Ring Prosthesis ", (|. Of Bone and Joint Surg. Vol. 50 B, No_4, Nov. 1968, pages 720-731).
The hip joint socket has a long, thin handle with a thread, but that Insertion of the threaded pin can endanger the sciatic nerve. In addition, high Surface pressures on the fine thread of the pin and stresses at the transition between the socket and the threaded pin, as the socket itself is not intimately connected to the acetabulum.
Furthermore, an acetabular cup is already available for the non-oral Anchoring has been proposed (DT-OS 23 14 175), the outer wall with as a thread trained support ribs is equipped with at least two cutting grooves. But there the wall of the pan is cylindrical, a relatively large amount of bone has to be milled out prior to implantation.
In contrast, the object of the invention is to provide a socket to be anchored without cement indicate, when using only a comparatively little bone substance has to be removed.
This object is achieved according to the invention in the aforementioned prosthesis in that the The circumference of the pan with the support ribs tapers towards its end face and that the thread profile of the Support ribs largely corresponds to that of a bone screw.
In a preferred embodiment, the joint surface of the socket consists of aluminum oxide ceramic. This As is well known, ceramics not only have excellent tissue compatibility, but also a pressure load capacity, which is even higher than that of metal. Most importantly, the surfaces can be so smooth be designed so that favorable coefficients of friction occur.
Die Erfindung wird im folgenden anhand eines in der Zeichnung dargestellten Ausführungsbeispiels erläutert, wobeiThe invention is explained below with reference to an embodiment shown in the drawing, whereby

Die Hüftgelenkpfanne 11 besitzt bei dieser Ausführungsform
einen schalenförmigen Aufbau. Zuinnerst ist die kalottenförmig gestaltete Gelenkfläche 12 aus
AI2O3-Keramik angebracht, die zur Führung des
Kugelkopfes dient.
Keramik-Gelenkoberflächen.F i g. 3 show the socket from below.
In this embodiment, the acetabulum 11 has a shell-shaped structure. At the center is the dome-shaped joint surface 12 made of Al2O3 ceramic, which is used to guide the ball head. This ceramic cap 12 is fixed by the beads 13 so that it can be snapped into place on the plastic casing 14 and has slots 15 through which the synovial fluid can penetrate and wet the joint surfaces. This plastic casing, which is fixed in the outer socket part 16 with the same snap-in mechanism 13, provides shock absorption to compensate for the very hard ceramic joint surfaces.
However, to the paving too Simplify, these ribs 17 are not designed circular, but similar to the thread profile of the Shaped bone screws. The socket part has on its circumference 18 (see FIG. 3) four grooves 19 which serve to accommodate a socket wrench, not shown. There are several more to this extent Cutting grooves 20 attached, only two being shown. This makes the pan very easy to be built in. First, a drill hole is made in the pelvic bone 23 to the core diameter pre-drilled, whereupon the pan is screwed in with the socket wrench that engages in the grooves 19 will be. Thanks to the self-cutting effect of the cutting grooves, the pan can be driven in if necessary without pre-cutting a thread

About liability to increase in the bone, the supporting ribs of the
