Description
Head mounts for Doppler probes are used diagnostically in transtemporal Doppler sonography for the detection of changes in flow (= cerebrovascular reactivity), for embolic detection as well as in brain death diagnostics. The position of the Doppler probe is then manually aligned with a cerebral artery and then this probe position is fixed.
In addition, there are recently self-adjusting Doppler probes from Delica (model 9UA). In this headgear motors are attached, with the help of which first visited a cerebral artery and then the Doppler probes is automatically aligned with this cerebral artery. If the probe position is changed, the cerebral artery is automatically retrieved. Positioning with this system usually takes longer than manual positioning. Each repositioning of the Doppler probes also alters the angle between the ultrasound and the artery so that cerebrovascular reactivities can not be determined.
It is anticipated that the demand for headgear for ultrasound probes will increase in the future. Sonothrombolysis, for example, is becoming more and more important in acute stroke therapy. The continuous supply of ultrasound energy leads to the dissolution of an arterial occlusion (= thrombus). Sonothrombolysis over two hours has demonstrated efficacy and safety in acute stroke, and the effectiveness and safety of systemic administration of Actilyse ® are comparable (Cochrane Analysis Cochrane Database Syst Rev. 2012 Oct 17; 10). Manual holding of the ultrasound probes during sonothrombolysis is time consuming and labor intensive, which is a significant obstacle to the use of this therapy. This problem is met by the helmet ClotBust ER ™ manufactured by Cerevast. The helmet contains 16 Doppler probes, so that the entire brain is irradiated with ultrasound energy. Positioning or manual holding of the probes is superfluous. ClotBust ER TM was developed for sonothrombolysis and is unsuitable for the diagnosis of brain-supplying arteries.
Duplex sonography is used more frequently in Doppler sonography, but duplex probes have not been used to date. Head mountings for Doppler probes on the market are manufacturer-specific and device-specific. For example, the models of DWL, Delica or Cerevast must each purchase the Doppler system. In addition, only trans-temporal examinations can be performed with conventional headgear.
- • No headgear for duplex probes exist.
- • The positioning of the Doppler probes is complicated by the short shape of the probes.
- • With the headgear fixation of the Doppler probes often leads to a change of the probe position and thus a new adjustment is necessary.
- • Only manufacturer-specific Doppler probes can be used with the head mounts.
- • With the headgear only transtemporal examinations are possible.
- • The lack of a user-friendly, manufacturer- and device-independent headgear for duplex and Doppler probes means that patients with acute stroke rarely receive sonothrombolysis.
Thus, the task was to develop improved headgear for duplex and Doppler probes, which can be used independently of the ultrasound system used.
Due to a simplified positioning of the ultrasound probes, previously accessible brain-based arteries (eg, the posterior cerebral artery, superior cerebellar artery) can be examined. In addition, the novel headgear also allows the examination of extracranial head and neck arteries such as the internal carotid artery or meningeal artery. The fixation of the ultrasound probes is so strong that even in troubled patients with acute stroke, the probe position is not changed.
