Abstract
An apparatus and a method for treating OSA (Obstructive Sleep Apnea) are disclosed, wherein an apparatus for Positive Airway Pressure is used, in which a supply of gases is provided, which is provided by a humidifier be directed. Since the amount of water vapor that is generated by the humidifier (8) is very small at the beginning (typically the humidifier has a water container (5) on a heating plate (7)), the pressure of the gases flowing through the device is provided, controlled so that the required pressure and the resulting flow rate are achieved at the same time in which the required moisture level is reached. A control strategy is disclosed in which the pressure is progressively adjusted to keep pace with the increase in humidity.
Description
This invention relates to health care devices and, more particularly, but not to only, PAP humidification devices (PAP = Positive Airway Pressure; Luftröh renal pressure), which is useful in the treatment of obstructive sleep apnea (obstructive sleep Apnea (OSA)) and a method of controlling such devices.
Man gels an tiefem andauerndem Schlaf.OSA is a sleep disorder that is estimated to affect at least 5% of the population in the muscles that normally hold the windpipe open relax and eventually collapse, closing the windpipe. The sleep pattern of one OSA sufferers are characterized by repeated sequences of snoring and breathing Difficulty, stopping breathing, waking up and starting up, and then back in sleep. Often the person concerned is not aware that this sleep pattern is occurring. OSA Affected people suffer from drowsiness and irritability during the day due to a man gels of deep, sustained sleep.
In an effort to treat those affected by OSA, a technique has been called Continuous Positive Airway Pressure (CPAP) (sustained positive airway pressure) is known to be found. A CPAP device has a gas supply (or a blower) with an attached Senen tube to supply a patient with pressurized gas, usually through a nasal mask supply. The compressed air that is supplied to the patient effectively supports the muscles, keeping the patient's windpipe open and eliminating the typical OSA sleep pattern gene.
The use of a CPAP system has been known to cause side effects such as the off Drying of the windpipe and nasal passages may occur, leading to rhinitis (inflammation of the Na sengänge) lead. The side effects mean that the patient is less likely consents to his or her CPAP therapy, and the therapy itself may lead to an awakening sen of the nasal resistance due to the high air flow, what the pressure level lowers which is placed on the windpipe, and thereby the effectiveness of the therapy re dues. Accordingly, a humidified CPAP system would be an improvement represent. An improvement on the standard CPAP system is shown in U.S. Patent No. 5,537,997, issued to Respironics Inc. describes in which a humidifier in the CPAP system is provided so that the patient receives humidified gases.
However, a mere combination of a known humidifier (in which gases are produced by what water vapor emerging from the surface of the water in a water humidification chamber on a heater plate rises, be guided) and a CPAP device the advantage of CPAP therapy do not maximize with gas humidification for the patient. The reason for this is that the Heiz plate needs some time to warm up, so that the patient occasionally (especially weh end of the warm-up time of the device) would be supplied with gases that were not humidified the. It should be noted that the delicate tissues of the nasal passages adhere to can swell if non-humidified gas is received for as little as 10 minutes. The accordingly it would be an advantage if the gases absorbed by the patient at any point in time would be humidified (according to the current or existing capacity of the humidifier).
It is therefore an object of this invention to provide a breathing assistance device hen that overcomes at least part of the above disadvantages or at least one provides usable alternative.

Fig. 5 is a graph of data experimentally detected versus time for a number of parameters in the CPAP system with gas humidification according to the second preferred embodiment of this invention.
In Fig. 1, a continuous-Positive Airway Pressure (CPAP) system is shown with gas humidification, in which a patient 1 humidified and compressed gases through a nasal mask 2 it holds that in a transport line for humidified gases or to a breathing tube 3 connected. It should be understood that this invention, however, is not limited to the supply of CPAP gases, but can also be applied to other types of gas supply systems, such as VPAP (Variable Positive Airway Pressure) and BiPAP (Bi-level Positive Airway Pressure) . The breathing tube 3 is connected to the outlet 4 of a humidification chamber 5 which contains a quantity of 6 water. The breathing tube 3 may have a heater or heating wires (not shown) which heat the walls of the tube in order to ensure a constant moisture profile along the tube and thereby reduce the condensation of the humidified gases in the tube. The humidification chamber S is preferably formed from a plastic material and can have a base that is highly thermally conductive (e.g. an aluminum base) which is in direct contact with a heating plate 7 of the humidifier 8 . The humidifier 8 is provided with a control or regulating device or an electronic regulator 9 , which can have a microprocessor-based control module (controller) that executes computer software commands that are stored in an associated memory.
The controller 9 receives input signals from sources, such as from the user input device or from the adjusting disk 10 , with which a user of the device z. B. a predetermined required value (preset value or setpoint) of the humidity or temperature of the gases that are supplied to the patient 1 , sets. The controller can also receive input signals from other sources, e.g. B. of temperature, humidity and / or flow rate sensors 11 and 12 via a connecting element 13 and the temperature sensor 14 of the heating plate. The controller 9 determines when (or at what level) the heating plate 7 is to be activated in order to heat the water 6 in the humidification chamber 5, depending on the humidity or temperature input values set by the setting disk 10 and the other inputs. Since the amount of water 6 is heated in the humidification chamber 5 , the water vapor begins to fill the volume of the chamber above the water surface, and flows through the outlet 4 of the humidification chamber 5 with the gas stream (z. B. air) from a gas supply device or a fan 15 is supplied and which flows into the chamber through the inlet 16 , out. It should be noted that it is possible to establish a relationship between the humidity of the gases in the humidification chamber 5 and the temperature of the heating plate 7 . Accordingly, it is possible to use the temperature of the hot plate in an algorithm or a table to determine the humidity of the gases (accordingly, the temperature of the hot plate serves as an indication of the humidity of the gases, and the two terms become interchangeable in this Description used). Exhaled gases from the patient's mouth are passed directly to the surrounding environment in FIG. 1. It should also be noted that in the preferred embodiment of this invention the temperature of the hot plate is used to represent the humidity, but a suitable humidity sensor could alternatively be used.
The fan 15 is provided with a variable pressure regulator or a variable speed fan 21 which draws air or other gases through the fan inlet 17 to produce a resultant gas flow rate. The speed of the fan 21 with variable speed is controlled by another control device or an electronic controller 18 (or alternatively, the function of the controller 18 could be performed by the controller 9 ) depending on input signals from the controller 9 and one from the user via the adjusting disk 19 set, predetermined target value (preset value) of the flow rate or the pressure or the fan speed regulated (as mentioned above with regard to the temperature of the heating plate and the humidity, it is also possible to establish a relationship between the fan speed, the gas flow rate and the gas pressure, and the three terms are therefore used interchangeably in this description).
An alternative preferred embodiment of a CPAP system with gas humidification is shown in FIG. 2, where the humidifier is included in the fan 15, so that the system comprises only a main component, the senmaske with the patient via the same tube 3 and the Na 2 connected is. In this embodiment only one controller 9 is required. This gas humidified CPAP system can be used in any embodiment of the preceding description, including the two embodiments with "warm up" mode. All reference numerals from Fig. 1 denote the same features of the inven tion.
It is possible to control the way the heating plate and / or the fan are switched on during the "Warm-up" (i.e. the time during which the heating plate is at its set or desired Temperature has not reached) is operated. The following are two preferred cases games of control methods used during this period.
During operation, a user of the CPAP system with gas humidification determines a “preset” (or setpoint) value of the gas pressure (P set ) that is to be supplied to the patient 1 by the fan 15. This set value is passed from the adjusting disk 19 to the controller 18 . The user also determines a “set” (or setpoint) value of the temperature (T set ) for the heating plate 7 , which is entered into the controller via the adjusting disk 10. The user input device for the set temperature may be named "humidity" for user convenience. The controller 9 then determines the current temperature of the heating plate 7 (T actual ) through the sensor 14 and the current gas pressure (P actual ), e.g. B. from the speed sensor 20 . It should be noted that it can take up to 30 minutes for the gases to reach their set humidity level, which is dependent on ambient conditions, flow rates and obstructions in the patient's trachea (e.g., inflammation). The current pressure value can be determined by a pressure or flow sensor in the fan 15 , the humidification chamber 5 or the pipes connecting the system, or alternatively, as has already been mentioned, the speed of the fan 21 (measured by speed sensor 20 , or alternatively, the speed command issued to the fan by controller 18 may be used as the actual fan speed) to represent the gas pressure.
The controller 9 then uses the set and the current values of the temperature (which represent the humidity) and the pressure (or the fan speed) in order to control the humidification and the pressure of the gas flow to the patient 1 . The pressure and temperature (humidity) of the gases supplied to the patient will eventually reach the values set by the user; However, to ensure that the patient is always supplied with humidifying gases that have been saturated with the maximum possible amount of water vapor (within the currently existing limits of the humidifier), the controller 9 controls the speed of the fan 21 in accordance with the Humidity of the gases (or in accordance with the temperature of the heating plate 7 ). As an example (with reference to Figures 3 and 4) the following table shows the measured (initial) and set (or required) temperature and (relative) pressure values (which correspond to the speed of the fan) during the start-up phase of the system.
In dem vorliegenden Fall:The controller 9 then determines the required change in pressure (.DELTA.P) and the required change in temperature (.DELTA.T) in order to obtain the required setpoint pressure or the setpoint temperature of the system. In the present case:
The controller 9 then determines the required average rate of pressure increase with respect to temperature by dividing ΔP by ΔT. In the present case, this calculation corresponds to 10 cm H 2 O / 30 ° C or 1/3 cm H 2 O per ° C.
Accordingly, for every 1 ° C. increase in the temperature of the heating plate 7, the controller 9 will instruct the controller 18 to increase the speed of the fan 21 in order to achieve a pressure increase of 1/3 cm H 2 O for this example. In this way the temperature and pressure of the gases supplied to the patient will reach their set values at the same time (that is, at time t s in FIGS. 3 and 4). Preferably, the heating plate is switched on before starting the CPAP system with gas humidification, and it will gradually increase its temperature up to its set temperature (as shown in Fig. 4), at which time the controller 9 continuously controls the heating plate in a suitable manner switches off and then switches the heating plate on again to maintain the set temperature. It should be noted that the controller 9 either continuously monitors the temperature of the heating plate until the set temperature is reached and continuously determines the updated target average values of the rate of rise, or the initially determined target average rate of rise could be over the entire warm-up time be used. In this way, the patient will only receive humidified gases, because in the initial phase little water vapor that is in the humidification chamber 5 is carried away by a small gas flow, while when the heating plate reaches its required set value (and thus much more water vapor is generated in the humidification chamber), the fan is instructed to generate a greater gas volume flow rate.
It should be noted that all of the numbers shown and described with reference to FIGS. 1 and 2 are also relevant to this second embodiment.
As in the previous preferred embodiment, the CPAP system with gas humidification according to this preferred embodiment has a "warm-up" function which determines the manner in which the motor speed (and thus the delivered flow rate or pressure) of the fan 21 in the Time after the device has been turned on increases. Therefore, it is desirable to increase the pressure, and hence the resulting flow rate, with the increase in humidity and thereby achieve the desired set pressure at which the device's humidity output reaches equilibrium or the required level.
The controller 9 uses a closed loop for the temperature of the heating plate 7 in order to reach the desired moisture level as early as possible. Experiments have shown that the temperature of the heating plate alone (as used in previous embodiments) is not necessarily a very good indicator of the moisture delivered to the patient, and that the water temperature in the humidification chamber 5 is a much better indicator of the moisture . It should also be noted that the increase in the water temperature is much slower than that of the heating plate 7 because water transfers energy comparatively poorly. For example, regulating the fan speed, which is based only on the temperature of the heating plate, can lead to excessive flow rates too quickly during the warm-up phase. Although the water temperature is not measured directly (it would be impractical to provide a temperature sensor in the humidification chamber 5 ), it is possible to estimate with sufficient accuracy how the temperature of the water in the humidification chamber 5 rises, as described below is.
The specific heat capacity of water describes the amount of heat (or energy) that is necessary to heat a unit mass of water by 1 ° C. When switching on the CPAP device with humidification according to this preferred embodiment of the inven tion, the controller 9 measures the actual temperature of the heating plate 7 (which is assumed to correspond to the water temperature in the steady state) and also receives the input value of the set (or target) -) Temperature of the heating plate by a user (via a user interface, such as buttons, switches or dials). The controller 9 then determines the temperature change required to achieve the set (or setpoint) Moisture value. Preferably, the controller 9 uses 90% of the required temperature change, since the water temperature in the humidification chamber 5 is always only 90% of the value of the set temperature of the heating plate 7 in the steady state it is enough. Using the value of the required temperature change (or a percentage thereof) and knowing the mass of the water in the chamber (it can be assumed that the humidification chamber 5 is filled with 400 ml of water, which corresponds to a mass of 400 g), the The amount of energy required to increase the set temperature of the water can be calculated.
Since the controller 9 also regulates the energy supply to the heating plate 7 , e.g. B. by controlling the duty cycle (time on / (time on + time off )) of the element, the amount of energy that is transferred to the water via the heating plate 7 is known. For example, with a heating plate element with 85 watts and a duty cycle of 60%, the energy of 51 joules is transferred to the water every second. Energy is extracted from the water by two dominant factors, conduction and convection.
During the warm-up phase and in the steady state, the amount of energy that is lost through conduction losses is directly proportional to the temperature gradient between the water and the environment (air temperature). If the temperature of the water and the air temperature of the environment are known (e.g. via a temperature sensor 22 for the ambient air or, more preferably, by estimating a fixed temperature of e.g. 20 ° C.), the conduction energy losses (E cond ) during the Warm-up phase can be estimated. In this embodiment the water temperature is not measured directly, but tests during the warm-up phase have shown that the difference between the average water temperature and the environment is approximately 68% of the difference between the temperature of the heating plate and the environment, ie (T water - T ambient ) = 0.68 × (T heater - T ambient ). This approximation of the water temperature was found by taking random samples of the temperature of the heating plate and the water during the warm-up phase by dividing the water temperature by the temperature of the heating plate and then determining the resulting values over the warm-up period (from the start time until the gases reached their required level Moisture).
Convection losses arise due to the air flow above the water in the humidification chamber 5 . For the average flow rate for a CPAP user, the flow rate (Q) in liters per minute is directly proportional to the speed () of the fan motor 21 in revolutions per minute (RPM) for a given restriction (where at e.g. a Standard pipe with a length of 1.83 m is assumed), and these convection losses are approximately directly proportional to the flow rate. The appropriate constants of proportionality have been determined experimentally and are included in the equations given below. Therefore the convection energy losses (E conv) can be estimated from. Accordingly, the net amount of energy (E trans ) transferred to the water in the humidification chamber 5 can be calculated at any point in time.
In operation (and with reference to Figure 5), the warm-up algorithm in accordance with the second preferred embodiment of this invention works as follows:
1. When switching on the CPAP device with gas humidification, the controller 9 reads the set or set speed of the fan and the temperature (which represents gas pressure and humidity) from the user inputs 19 and 10 and also the current temperature (T start ) of the Heating plate 7 and note that the water temperature is approximately 90% of the temperature of the heating plate 7 . The fan 21 is started at an initial low speed of e.g. B. 3000 RPM operated to an initial pressure and a resulting flow rate of z. B. 16 liters per minute (it is assumed that the flow rate (Q) in liters per minute is directly proportional to the speed of the blower motor (u) in RPM in a CPAP system, so that Q = / 186.2 ). The required amount of energy (E required ) to increase the temperature of the mass (m) of water in the humidification chamber S by an amount (ΔT = (T required - T start ) × 0.9) to its set temperature is then as is calculated as follows (where k s is the specific heat capacity of water):
Etotal required = ks × m × ΔT = 4,19 × 103 × 0,4 × (Trequired - Tstart) × 0,9E total required = k s × m × ΔT = 4.19 × 10 3 × 0.4 × (T required - T start ) × 0.9
The controller 9 then operates the heating plate 7 in such a way (z. B. a closed loop system that the duty cycle of the voltage that is applied to the heating element, adjusts) that a predetermined target temperature of the heating plate (z. B. approaches approximately 60 ° C) is reached as early as possible with minimal or no overshoot.
(%) aufweist), berechnet.
2. At predetermined time intervals (t f ), e.g. B. every 30 seconds, the net amount of energy (E transferred ) that has been transferred from the heating element 7 to the water in this interval (which has a power of P T watt and a duty cycle of D (%)) is calculated.
Etransferred = Pr × tf × D
E transferred = P r × t f × D
Etransferred = 85 × 30 × D = 2550 × D [für die Dauer von 30 Sekunden]E transferred = 85 × 30 × D = 2550 × D [for a period of 30 seconds]
chen. Am Anfang gilt also:
This value is then subtracted from the total energy required to heat the water to the set temperature, and the energy required (from this point in time) to reach the set temperature is obtained. So at the beginning:
Eneeded(start) = Etotal required - Etransferred = 4,19 × 103 × 0,4 × (Trequired - Tstart) × 0,9 - (2550 × D)
E needed (start) = E total required - E transferred = 4.19 × 10 3 × 0.4 × (T required - T start ) × 0.9 - (2550 × D)
und allgemeiner für jede folgende Iteration (i):
and more generally for each subsequent iteration (i):
Eneeded(i) = Eneeded(i-1) - (2550 × D)E needed (i) = E needed (i-1) - (2550 × D)
3. Using the net energy transferred to the water during the last 30 seconds, or rather the average rate of net energy transfer (P transfer), and the recalculated value for the energy required to reach the set temperature an estimated time (t s ) in which the set temperature should be reached can be calculated as follows:
Ptransfer = Etransferred/30 (in Watt)
P transfer = E transferred / 30 (in watts)
und
and
Ts = Eneeded/Ptransfer
T s = E needed / P transfer
am Anfang gilt daher:
at the beginning therefore applies:
ts = (30 × Eneeded(total))/Etransferred (in Sekunden)
t s = (30 × E needed (total) ) / E transferred (in seconds)
And for the subsequent iterations, an updated estimated time (t s (i) ) can be calculated as follows:
ts(i) = 30 × (Eneeded (i-1) - Etransferred(i))/Etransferred(i) (in Sekunden)t s (i) = 30 × (E needed (i-1) - E transferred (i) ) / E transferred (i) (in seconds)
As previously mentioned, however, the amount of energy that is lost in the water in the humidification chamber due to conduction and convection losses must also be taken into account. The conduction and convection losses can be calculated as follows.
Econv = kconv × Q und Q = kQ ×
E conv = k conv × Q and Q = k Q ×
folglich
consequently
Econv = kconv × kQ × = (1/186,2) × 5,79 × (für eine Dauer von 30 Sekunden)
E conv = k conv × kQ × = (1 / 186.2) × 5.79 × (for a duration of 30 seconds)
also
so
Econd = kcond × (Twater - Tambient) = 10,83 × 0,68 × (Theater-20) = 7,4 × Theater-148
E cond = k cond × (T water - T ambient ) = 10.83 × 0.68 × (T heater -20) = 7.4 × T heater -148
(in Joule während einer Dauer von 30 Sekunden während des Aufwärmens)(in joules for 30 seconds during the warm-up)
The losses are added to E needed and removed from E transferred , thus
Eneeded (actual) = Eneeded(i-1) - Etransferred(i) + Econv(i) + Econd(i)
E needed (actual) = E needed (i-1) - E transferred (i) + E conv (i) + E cond (i)
Etransferred(actual) = Etransferred(i) - Econv(i) - Econd(i)
E transferred (actual) = E transferred (i) - E conv (i) - E cond (i)
und somit
and thus
oder alternativ
or alternatively
4. Using this calculated time and the difference between the set speed and the actual speed, an average rate of speed increase (γ) can be calculated:
γ = (required - actual(i))/ts(i) (in UPM pro Sekunde)γ = ( required - actual (i) ) / t s (i) (in RPM per second)
The actual speed of the motor is then adjusted accordingly to generate the calculated acceleration.
Steps 2 to 4 are repeated every 30 seconds (the value of E totai required being reduced by the net amount of energy E transferred transferred to the water in the last 30 seconds), with the result that the time to to achieve the set temperature and the average required increase in motor speed is continuously calculated because the rate of net energy transfer to the humidification chamber changes. This step is repeated until the time to reach the set temperature (t s ) is zero and the set speed is reached, at which time the water temperature in the humidification chamber 5 should also reach its required or set temperature.
It should be noted that the algorithm described above is based on the humidification chamber containing 400 ml of water. If the chamber contains less than 400 ml, then the warm-up time will take longer because the smaller amount of water takes less time to reach the set temperature, which means that the underlying duty cycle goes to a lower level more quickly, which results in a lower rate of net energy transfer (E net = E transferred - E cond - E conv ) to the water. Under experimental conditions, the warm-up times for the humidified CPAP devices have varied between approximately 15 and 25 minutes, depending on the operating conditions.
It should also be noted that in the above preferred embodiment, the fan speed is controlled essentially as a function of an indirectly determined water temperature. However, as previously described, it is desirable to control the pressure of the gases delivered to the patient in accordance with the humidity of those gases. The pressure (P) and flow rate (Q) in the system described above (with about 1.83 m of hose length, etc.) can be approximated by the following equation:
P = 8,836 × 10-3Q2 P = 8.836 x 10 -3 Q 2
Accordingly, the pressure increases quadratically with the fan speed. However, with the above equation it is possible to use the fan speed as an indication of the pressure and to calculate the gas pressure from it. Similarly, it would be possible to set up an equation relating temperature to humidity, e.g. B. by determining the mass of water that evaporates per second and dividing this value by the current flow. Thus, although this preferred embodiment of the invention has been described with reference to fan speed and temperature, the system ensures that the gas pressure is controlled to deliver only sufficient humidified gases to the patient. The known relationships between Gebläsegeschwin speed and pressure and temperature and humidity could also be used in step 1 described above, so that the user could enter the required pressure and humidity values directly into the device. The controller 9 could then calculate the required fan speed and the temperature values and carry out the remaining steps accordingly.
The graphs shown in Fig. 5 show that the temperature of the hot plate reaches its set temperature relatively quickly and that the fan speed (RPM in Fig. 5) gradually increases in line with the increase in water temperature.
In cases where the temperature of the heating plate is close to the set temperature when the system is started (e.g. if the patient is currently using the device but has been temporarily called away and the device is switched off or in stand-by mode has brought), the controller no longer has to keep the temperature and pressure coordinated while they are increasing. In this case, the controller can first determine whether the current temperature of the heating plate is approximately or more than approximately 75% of its setpoint value or, alternatively, within 5 ° C of its set value. If this is the case, the speed of the fan 21 is regulated so that it increases linearly from a low initial value to the setpoint value over a predetermined period of time (e.g. 15 minutes). Alternatively, the controller could determine whether the current Tem temperature of the heating plate is within a range of the set temperature setpoint, e.g. B. a range of about 10 ° C, and then the speed of the fan 21 to re rules to reach the set value of the fan speed in a predetermined time.
The alternative steps mentioned above are necessary because the hot plate is already warm and will therefore soon reach its set temperature (before the patient is asleep) and therefore the full fan speed should be delayed for a set amount of time to enable the gases to be humidified within the operating limits of the humidifier and / or to allow the user to fall asleep before the maximum flow rate occurs. The predetermined period of time could be set by the manufacturer before the device is sold or, alternatively, this value could be adjustable by the user by turning the controller 9 e.g. B. adds another adjusting wheel and input.
The controller also preferably monitors the calculated value of the net energy transfer (E net ), and if it determines that this is zero or negative (e.g. for a period of 30 seconds) then it is determined that the warm-up must be finished ( because less energy was used for warming up than was calculated, after which there was consequently less water in the chamber than expected). In this situation, the fan speed is automatically increased to the set fan speed. Another feature could be the monitoring of the warm-up period, so that if the warm-up continues after about 25 minutes, the fan speed could be automatically increased to the set fan speed because the warm-up period should have ended.
A further addition could consist in the provision of a key with an input in the controller 9 . The button could be pressed by the user to start the linear increase in fan speed for the duration set above. Once the warming up has ended (temperature and pressure have reached their required values), the user can determine that he has not been able to fall asleep and therefore request that the blower speed be temporarily reduced. In this case, the user could simply press the above-mentioned button again and there is a further linear increase in fan speed, starting at a low predetermined speed and increasing linearly up to the required set speed (even if the water temperature is already there is at its required set temperature).
Accordingly, the invention provides a breathing assistance system with gas humidification before, in which the patient is supplied with useful humidified gases for the duration, in which the humidifier warms up, and also while the humidifier is in operation (and with its set temperature is working). In addition, the moisture of the gases that are at the patient are guided during these two periods within the performance limits zen of the humidifier to humidify these gases, maintained for the benefit of the patient. This is extremely beneficial to the patient because it is not even a stream of humidified or humidified insufficiently humidified gases to the patient for a short period of time (e.g. 10 minutes) can cause harmful swelling of the nasal passages and even greater inconvenience when administered orally.
