Lasererzeugte Schockwellen werden in den letzten Jahren zunehmend in der
Medizintechnik bei Operationen eingesetzt. Ein wichtiges Anwendungsgebiet
dieser Technik ist dabei die Entfernung von Linsen aus menschlichen Augen,
wie sie zur Behandlung des Grauen Stars nötig ist. In einem bisher verwen
deten Verfahren wird dazu intensive gepulste Laserstrahlung mittels eines
Lichtleiters in den Innenraum einer Hohlnadel eingebracht [1, 2, 3]. Dort
trifft die Laserstrahlung auf die Innenwand der Hohlnadel. Ist diese Laser
strahlung intensiv genug, wird an einer inneren Oberfläche der Hohlnadel ein
Plasma erzeugt, welches sich gegen die in der Hohlnadel befindliche wässrige
Flüssigkeit ausdehnt und dabei Schockwellen erzeugt. Durch diese Schock
wellen werden verhärtete Bestandteile der Linse zerkleinert. Die Bruchstücke
können dann durch die Hohlnadel abgesaugt werden. Die Zuführung von
Flüssigkeit in das Auge zum Ausgleich der abgesaugten Flüssigkeit kann
zusammen mit der Absaugung in einer Hohlnadel integriert sein, oder un
abhängig von der Absaugung mit einer zweiten Nadel erfolgen. Ein relativ
großer Teil der Schockwellenenergie erreicht bei dem bekannten Verfahren
den Ausgang der Hohlnadel aber nicht und wird in Inneren der Hohlnadel
absorbiert. Der Vorteil des bekannten Verfahrens im Vergleich zu mit Ultra
schall arbeitenden Verfahren beruht aber trotzdem wesentlich darauf, daß die
Augen bei der Entfernung der Linse mit den lasererzeugten Schockwellen nur
gering erhitzt werden [4]. Die im folgenden beschriebene Erfindung hat sich
daher zum Ziel gesetzt, bei gleichbleibender oder verkleinerter Einkopplung
von Laserenergie in die Hohlnadel die Ausrichtung der Schockwellen zum
Ausgang der Hohlnadel zu verbessern, wodurch so die Hitzeeinkopplung zum
Beispiel in das Auge weiter verkleinert werden kann oder wodurch sich die

zeugt werden senkrecht zur von hinten einfallenden Laserstrahlung ausgebil
det wird, und wenn gleichzeitig der Ausgang des Lichtleiters selbst und seine
Umgebung wie vorher beschriebenen abgeschrägt wird. Abb. 1 zeigt
dazu eine schematische nicht maßstabsgetreue Darstellung der Reflexion von
Schockwellen in einer Hohlnadel. Dargestellt sind in dieser Abbildung Kom
ponenten der Schockwellen, die vom Lichtleiter zum Ausgang der Hohlnadel
reflektiert werden, andere Komponenten der Schockwellen werden von den
im Inneren der Hohlnadel angebrachten Rillen oder Kanten aus einer anderen

Laserstrahlung vom Lichtleiter ins Wasser um einen Übergang von einem
optisch dickeren in ein optisch dünneres Medium handelt, wird das Laser
licht beim diesem Übergang um etwa 6 Grad (abhängig vom Brechungsindex
des Lichtleitermaterials) vom Lot der abgeschrägten Lichtleiterfläche weg
gebrochen, wodurch die vom Laser bestrahlte Fläche um etwa 0.06 mm in
Richtung der naheliegenden äußeren Wand der Hohlnadel verschoben wird,
von der dann ebenfalls Schockwellen ausgehen können.

Schockwellen kann dabei, bis auf den bei der Reflexion absorbierten Anteil,
in Richtung des Ausgangs der Hohlnadel reflektiert werden. Divergente An
teile der Schockwelle, welche auf die Rillen oder Kanten auftreffen, werden
jedoch nur dann zum großen Teil ebenfalls zum Ausgang reflektiert, wenn
die dem Eingang zugewandte Fläche der Rillen oder Kanten parallel zu der
gegenüberliegenden, der dem Ausgang zugewandten Fläche ausgerichtet ist.
Aus geometrischen Gründen würde sonst ein großer Teil dieser divergent sich
ausbreitenden Schockwelle von der dem Ausgang näher liegenden Rille oder
Kante behindert und nicht zum Ausgang der Hohlnadel reflektiert. Bei dem
Vorhandensein einer zweiten parallen Fläche erfahren Schockwellen jedoch
durch mehrfache Reflexion an beiden Flächen eine Parallelverschiebung, wo
durch schließlich ein großer Teil dieser Schockwellen ebenfalls zum Ausgang

wird daher vorgeschlagen, die Laserenergie so in das Innere der Hohlnadel
einzukoppeln, daß dann die Schockwellen direkt den Ausgang der Hohlnadel
erreichen können. Abb. 2 zeigt eine schematische nicht maßstabsgetreue
Darstellung der Erzeugung und Ausrichtung von Schockwellen in einer Hohl
nadel bei seitlicher Auskopplung von Laserlicht aus dem Lichtleiter. Der in
der Abbildung angegebene Winkel α bezeichnet den mittleren Winkel uni den
das Laserlicht von seiner ursprünglichen Ausbreitungsrichtung im Lichtleiter

es so vorzugsweise auf die dem Lichtleiter gegenüberliegende Innenseite der
Hohlnadel auftreffen zu lassen, wo dann wieder Schockwellen erzeugt wer
den. Metallische Verspiegelungen am Ausgang des Lichtleiters sind hier zur
seitlichen Ablenkung des Laserlichts ungeeignet, weil die Zerstörschwelle be
kannter metallischer Spiegel (einigen Joule pro Quadratzentimeter) bei den
für dieses Verfahren benötigten Laserintensitäten überschritten wird. Um das
Laserlicht seitlich aus dem Lichtleiter austreten zu lassen kann jedoch die To
talreflexion von Licht beim Übergang von einem optisch dickeren Medium zu
einem optisch dünneren Medium ausgenutzt werden. Beim Übergang von
Quarzglas nach Wasser tritt etwa ab einem Winkel von 66 Grad zwischen
dem Lot auf die abgeschrägte Fläche des Lichtleiters und der Richtung der
Laserstrahlung Totalreflexion ein. Dabei sind dann die bei Gläsern wirksamen
Zerstörschwellen deutlich höher als die bei den bekannten metallischen Spie
geln. Die Anwendung der Totalreflexion des Laserlichts kann nacheinander
auch mehrfach erfolgen um den Ablenkungswinkel der Laserstrahlung zu ver
größern. Bei seitlichem Austritt aus dem Lichtleiter kann bedingt durch die
Krümmung der Außenwand des Lichtleiters beim Übergang in die wässrige
Flüssigkeit der Hohlnadel eine zylindrische Fokussierung der Laserstrahlung
erfolgen, durch welche die zur Schockwellenerzeugung benötigte Laserenergie
weiter verkleinert werden kann. Diese Fokusierung der Laserstrahlung kann
auch vermieden oder verkleinert werden, wenn der Austrittsbereich für die
Laserstrahlung an der Fiberoptik abgeflacht wird. Die Oberflächen am Ent
stehungsort der Schockwellen sollten dann vorzugsweise wie oben beschrieben
als eine oder mehrere Rillen oder Kanten ausgebildet werden. Die Flächen
normalen der dem Ausgang der Hohlnadel zugewandten Flächen der Rillen
oder Kanten sollen aber hier direkt zum Ausgangs der Hohlnadel weisen, weil
hier zumindest für den zentralen Teil der Schockwelle von einer annähernd
senkrechten Ausbreitung der Schockwellen über diesen Flächen ausgegangen
werden kann. Aus dem Lichtleiter auf diese Flächen der Rillen oder Kanten
auftreffendes Laserlicht wird dagegen gemäß den optischen Gesetzen der Re
flexion mit gleichen Einfalls- und Ausfallswinkel reflektiert. Dadurch kann der
wesentliche Teil des Laserlichtes, der nicht auf diesen Flächen zur Erzeugung
der Schockwellen in der Hohlnadel absorbiert wird, zu anderen Punkten auf
den Rillen oder Kanten oder im Inneren der Hohlnadel reflektiert werden,
wodurch ein unerwünschter Austritt von Laserlicht in das Auge verhindert
oder verkleinert werden kann. Die Ausrichtung der Flächen der Rillen oder
Kanten in der Hohlnadel kann natürlich bei herabgesetzter Wirkung durch
die Ausrichtung auch nur teilweise erfolgen. Mehrere Einzelflächen der Rillen
oder Kanten können in der Art optimiert werden, daß sie alle zum gleichen
Punkt des Ausgangs zeigen, wodurch ein Fokuspunkt der Schockwellen ge
geben ist und wodurch auch bei weiter verkleinerter Laserenergie noch die
gewünschte Zerstörung des Linsenmaterials am Ausgang der Hohlnadel er
reicht werden kann. Um die Absorption des Laserlichts auf die dem Ausgang
zugewandten Seiten der Rillen oder Kanten zu vergrößern, beziehungsweise
um die Absorption auf der dem Eingang zugewandten Seite der Rillen oder
Kanten zu verkleinern können diese Flächen besonders behandelt werden.
Dadurch kann die zur Plasmaerzeugung nötige Intensität ebenfalls herabge
setzt werden, beziehungsweise es kann Laserlicht, das sonst keinen Beitrag
zur Schockwellenerzeugung am Ausgang liefern würde doch ausgenutzt wer
den. Diese Behandlung kann in einer Aufrauhung der Oberflächen bestehen
oder in einer Dotierung der Oberflächen, die dem Ausgang der Hohlnadel
zugewandt sind und in einer Verspiegelung oder Glättung der Oberflächen,
