Abstract
Description
One of the most common causes of death in postoperative and general intensive care medicine is sepsis or SIRS (Systemic Inflammatory Response Syndrome). In spite of Due to the rapid development in intensive care medicine, mortality is due to sepsis / SIRS sick patients in 30-70%. The reason for this is that an initially small, local limited, inflammatory reaction almost unnoticed and unfavorable Spread case irreversibly systematically across a variety of inflammatory mediators can. Cited inflammatory reactions can be without or in immediate Linked to invasive interventions and even non-invasive treatments. So far, there are no suitable therapeutic measures to combat the complex inflammatory chain reactions that result in sepsis / SIRS to MOD (Multi Organ Death) to be able to interrupt effectively.
DAB und DIN EN 285).Potentially immunostimulating or allergic reactions as part of the invasive Treatment arises not only through surgery, but also during invasive diagnostic procedures as well as consequent drug treatments. In addition to the sterile goods themselves, the cause of immune-stimulating reactions are including the active ingredients and additives used pharmaceuticals u. a. Impurities in or on the sterile items, e.g. B. from disinfectant residues Cleaning processes and heavy metals of the sterilization steam as well as organic Compounds of bacterial origin and diffusible, soluble compounds medical Plastics of packaging. Specified and recommended limit values e.g. B. for So far, pyrogens and heavy metals are aimed at widespread general Manufacturing standards (see DAB and DIN EN 285).
Indicators of the humoral and cellular mediator system inflammatory Reactions according to claim 1 include innumerable individual substances. With the multitude the groups of substances that u. a. cause increased lymphocyte activity or accompany are z. B. various cytokines, proteases, toxic oxygen species, To name lipid mediators and nitric oxide.
So far, no cross-product measurements of the humoral and cellular Mediator system used, which has a cumulative immunostimulating effect different types of treatment had been diagnosed before starting treatment, with the aim of sustainably increasing the risk of potential inflammatory malregulations limit. The test methods used so far for immune response and Allergy diagnostics have so far only covered individual groups of substances. These analyzes in However, partial areas did not allow those arising during the treatment To be able to determine interactions qualitatively and quantitatively. According to DAB (German Pharmacopoeia) and European Pharmacopoeia asked for tests regarding Pyrogens or heavy metals are therefore in no way suitable, the potential inflammatory, immunological side effects e.g. B. in treatment Sufficiently limit polytraumatized risk patients, because only with the procedures PCT / EP96 / 04666 u. a. the procedural problems in the processing of sterile goods with regard to heavy metal and pyrogen impurities.
The essentially monocausal view was therefore more inflammatory Processes in view of the previously unavoidable contamination of sterile goods Alternatives compared to a comprehensive one with a limited budget multifactorial diagnostics for an actual limitation and reduction of potential inflammatory wrong reactions would have been justified. A major advantage of today's developments in treatment technology are undoubtedly the new ones non-invasive diagnostic options (CT, magnetic resonance and ultrasound diagnostics) and the Development towards minimally invasive surgery.
In view of the currently extensive uncontrolled inflammatory potential Medical treatments are urgently needed, e.g. B. prior to an invasive Intervention in addition to knowledge of the patient's current immune status, potentially immunostimulating or allergic reactions through the intended To predict treatment type to reduce inflammatory misstimulation on one reduce medically acceptable, unavoidable mininum. It is for that in view of the associated wealth of data, a computer-based one is desirable Monitoring of the current immune status and the predictable tendency To be carried out in the case of high-risk medical care patients. So that's it possible, e.g. B. the choice of medication in terms of product, dose and time of application Interaction with the individual factors of the entire treatment complex for everyone Optimizing main, side and interactions even on a case-by-case basis. Provided If there is no increased risk potential, it is sufficient to treat one To plan a certain standardized course of treatment for the patient group. This standardized course of treatment would then contain free windows for additional measures can be used. Given the variety of interactions can also be used in postoperative intensive care for high-risk patients with regard to the liability risk of possible maintenance errors Computer-aided monitoring for individual patients, which serves as a control and Control module for the medication application also the timely documentation the course of treatment allows, not to be waived.
The primary task from the realization of claims 1 ff. Is the rapid testing for Specification of the calculation factors for standardized identification of the Medications e.g. B. with alphanumeric identifier, barcodes and magnetic strips. The comprehensive comparison of digitized anonymous patient data with the Calculation factors for medication and forms of treatment would also be one significant acceleration of the statistical recording of previously unknown Enable interactions.
The actual calculation would come from the approximate mathematical derivation the inflammatory reaction, or the main, secondary and alternating reaction for the individual patients or a group of patients as a time function. According to the invention are according to claim 1 u. ff. that for the entire concept the Coordination and inclusion of suitable indicators and their calculation factors for all medically relevant parameters e.g. B. the circulation, such as pulse, blood pressure, Body temperature, EKG as well as the various organ functions as the liver, kidney, Pancreas, lungs, pituitary gland, nervous systems are recorded with EEG, the Priority temporarily exceeded z. B. the limits for a SIRS risk would justify.
In addition to its importance for organ transplantation, the procedure also includes Control of inflammatory reactions of medical treatments that still exist Gaps to ensure treatment success for many patients.
