Abstract
Wound cleansing device with a carrier layer and a wound cleansing layer arranged thereon, designed to absorb wound exudate and/or solid wound components and having cleaning threads, wherein the carrier layer and/or the wound cleansing layer is provided with at least one storage substance that increases the absorption capacity for wound exudate.
Description
The invention relates to a wound cleaning cloth with a carrier layer and a wound exudate and / or solid wound material, such as fibrin coverings, dead tissue of the subcutaneous tissue, such as excessive horny material or dead horny cells and / or necrosis, and cleaning threads for loosening, arranged thereon the wound components from the wound cleansing layer.
Wound cleaning tissues of the type described above are used, for example, for so-called debridement. This is the process of wound bed preparation, in which substances or human material formed by the body itself, such as excess fluid (wound exudate), fibrin coverings, dead tissue of the epidermis, such as excessive horny material or dead horny cells and / or coverings from dead tissue (Necroses) are removed.
With the known wound cleaning devices, the substances to be removed can be loosened from the wound with the aid of the threads and held between the threads. When using the known wound cleaning devices, however, it has been shown that the substances to be removed are released again from the wound cleaning layer in the course of the wound cleaning and get back into the wound.
In view of these problems in the prior art, the invention is based on the object of providing wound cleansing devices with an improved cleansing effect.
According to the invention, these objects are achieved by a further development of the known wound cleaning devices, which is essentially characterized in that the carrier layer and / or the cleaning layer is provided with at least one storage substance which increases the absorption capacity for wound exudate.
The invention is based on the knowledge that the deficiencies observed when using the known wound cleansing devices are primarily due to the fact that the absorption capacity of the wound cleansing device is reached in the course of the wound cleansing process and substances removed thereafter return to the wound. According to the invention, this deficiency is solved in that suitable storage materials are used. It has surprisingly been shown that despite the volume occupied by the storage materials, an overall increased storage capacity can be achieved because, on the one hand, the available storage space of the wound cleansing device can be better utilized and / or, on the other hand, the overall capacity through suitable integration of the wound exudate using the Storage materials in the wound cleansing layer and / or carrier layer is optimized. This also prevents wound exudate from smearing onto the surrounding skin and reduces the risk of infection that this causes.
In the context of the invention, the absorption capacity of the wound cleansing device for essentially liquid substances is described by the so-called “fluid holding capacity”. To determine this “fluid holding capacity”, the wound cleansing device is first weighed. The wound cleaning device is then placed in a wound exudate solution for a period of 60 seconds. The product soaked with the wound exudate solution is then grasped with tweezers and held in a vertical orientation for a period of 30 seconds so that the solution can drip off from the wound cleansing device. The product is then placed horizontally in a bowl and weighed again. The weighings are used to determine how many milliliters of the exudate solution per gram of the wound cleansing device are retained in the wound cleansing device. This value is the wound cleansing capacity for the purposes of this invention. If the value is increased by adding storage substances, this means in the context of this invention an increase in the absorption capacity for wound exudates.
Similar to conventional wound cleansing devices, the wound cleansing threads of wound cleansing devices according to the invention can have plastic fibers, in particular polyester fibers and / or polypropylene fibers, in particular consist of polyester fibers and / or polypropylene fibers. The cleaning threads can be designed as monofilaments, that is to say from one fiber per thread, and / or as multifilaments, that is to say as optionally twisted fiber bundles. In the context of the invention, the cleaning threads can form loops in which the threads emerge from the carrier layer and, after loop formation, re-enter the carrier layer. Alternatively or additionally, the threads can have freely protruding ends, ie that they emerge from the carrier layer and end free.
In the context of the invention, it has proven to be particularly advantageous if the proportion of storage substances, based on the dry weight of the carrier layer and cleaning layer (total weight), is 0.1-25% by weight, in particular 0.15-20% by weight 0.2-17% by weight. If the proportion of storage materials is increased above the stated values, this can lead to the cleaning threads sticking together, which in turn can lead to a reduction in storage capacity. If the proportion of storage substances is reduced below the stated values, the effect of the storage substances is hardly noticeable.
It has been shown that the storage substances can also develop an effect that increases the absorption capacity within the carrier layer. It has therefore proven to be useful to relate the amount of storage substances to the total weight of the wound cleansing device. However, care must also be taken that the properties of the wound cleaning threads are not excessively impaired by the storage substances and that the available absorption capacity between the cleaning threads is not excessively reduced by the storage substances. From this point of view it has proven to be advantageous if the proportion of the storage material, based on the dry weight of the wound cleansing layer, is 0.2-30% by weight, in particular 0.25-27% by weight, preferably 0.3-25 % By weight.
In the context of the invention, it has proven to be particularly advantageous if at least the wound cleansing layer is impregnated with a preferably aqueous solution of the storage material. This prevents the cleaning threads from sticking due to dried storage materials.
In the context of the intended increase in the storage capacity, it has proven to be expedient if the concentration of the storage material in the solution used to impregnate the cleaning layer is 0.1% or greater, preferably 0.3% or greater and / or 5% or less, in particular 2.5% or less, particularly preferably 1.5% or less. An increase in the concentration causes an increased viscosity of the solution. This can make a homogeneous distribution of the storage substances in the cleaning layer more difficult. If the concentration is too low, the available capacity of the cleaning layer is already largely filled by the soaking solution, so that only a small residual capacity is then available for the wound exudate and the other substances to be removed from the wound.
In the context of the invention, it has proven to be particularly expedient if the viscosity of the solution is adjusted to 2,000 mpas or less, in particular 1,000 mpas or less, measured in accordance with DIN ISO 2555 with a Brookfield viscometer, model RTV.
The storage material used in the context of the invention can have at least one surfactant and / or at least one substance which swells on contact with water or an aqueous solution. In the context of the invention, a surfactant is understood to be a hydrophilizing substance which enables hydrophobic substances to go into solution. Surfactants or emulsifiers per se lower the energy of the phase boundary (surface or interfacial tension). This counteracts segregation. The ability of surfactants or emulsifiers to mix immiscible liquids into a finely divided mixture and to stabilize them promotes the desired increase in the absorption capacity for wound exudates in the context of wound cleaning. By reducing the surface tension with the aid of a surfactant, the penetration of the wound exudates into the spaces between the wound cleansing threads is also facilitated and thus the overall absorption capacity is increased.
If a substance swells on contact with water or an aqueous solution, the water is bound in the form of a gel. Overall, this leads to an increase in the absorption capacity because the water or wound exudate that has already bound cannot drip out of the cleaning layer again.
Ein im Rahmen der Erfindung einsetzbarer Speicherstoff kann mindestens einen Celluloseether, insbesondere Hydroxyethylcellulose; Carboxymethylcellulose; Methylcellulose; Ethylcellulose und/oder Hydroxypropylcellulose; weitere Polysaccharide, wie etwa Stärke, Dextrane, Chitosan, Carrageen und Alginate;; Milcheiweiß; Hyaluronsäure; Zuckeralkohole; Glykole; Salze und/oder Mischungen davon aufweisen.A storage material that can be used within the scope of the invention can contain at least one cellulose ether, in particular hydroxyethyl cellulose; Carboxymethyl cellulose; Methyl cellulose; Ethyl cellulose and / or hydroxypropyl cellulose; other polysaccharides such as starch, dextrans, chitosan, carrageenan and alginates ;; Milk protein; Hyaluronic acid; Sugar alcohols; Glycols; Have salts and / or mixtures thereof.
Wound healing can be further promoted if the wound cleansing layer and / or the carrier layer are mixed with anti-inflammatory and / or antimicrobial substances such as WO 3 (tungsten (VI) oxide). The antibacterial effect of WO 3 is attributed to the absorption of water by the tungsten oxide, as a result of which a thin layer enriched with H + ions is formed, which can damage bacterial membranes. It is of course possible to use other antimicrobial and / or anti-inflammatory substances in addition to or as an alternative to WO 3. It can be in the frame According to the invention, use can also be made of the anti-inflammatory / antimicrobial effect of the storage substances themselves, such as, for example, hydroxyethyl cellulose.
- In one embodiment of the invention, cellulose is used as a storage material. Cellulose is the most commonly available polysaccharide in nature with many beneficial properties. These include biodegradability, biocompatibility, lack of toxicity, low costs and good thermal and chemical stability. The combination of the properties of high biocompatibility with a lack of toxicity and a lack of immunogenic activity has proven to be particularly advantageous in the context of the invention.
The storage material, such as hydroxyethyl cellulose, is added to distilled water until the desired concentration of the storage material is obtained in the solution and continuously stirred at elevated temperatures of, for example, about 60 ° C. for one hour in order to obtain a homogeneous mixture. When using hydroxyethyl cellulose as a storage material, 0.5% citric acid is added as a crosslinking agent. A sponge-like, porous material can be obtained using citric acid as a crosslinking agent. This property is attributed to the strong hydrogen bond between the polymer chains. Because of these strong interactions, a very large capacity for absorbing large amounts of water can be obtained. The structure obtained in this way can swell considerably without being dissolved in the aqueous solution. The resulting solution is mixed for 2 hours at elevated temperatures of, for example, about 60 ° C. in order to achieve crosslinking and is poured into a glass bowl without prior cooling. The wound cleansing device is then placed in the solution with the boundary surface of the wound cleansing layer facing away from the carrier layer facing downwards, so that the solution can be absorbed evenly in the cleaning layer and / or the carrier layer, possibly using the capillary action between the cleaning threads. Additionally or alternatively, the wound cleansing device can be mixed with a storage material solution obtained without special temperature treatment and / or without the use of a crosslinker or placed in a corresponding solution.
In the context of the invention, it is contemplated first of all to dry the wound cleansing device obtained in this way before use and to moisten it again only after use. However, this can lead to the cleaning threads sticking together and the cleaning effect being impaired. For this reason, it has proven to be particularly advantageous within the scope of the invention if the wound cleaning device is in the form of a pack with a liquid-impermeable cover and a wound cleaning device, such as a wound cleaning cloth, accommodated therein. This prevents the cleaning threads from sticking together during transport and storage.
In an alternative embodiment of the invention, a kit for producing a wound cleaning device according to the invention or a wound cleaning cloth according to the invention is provided, with a dry wound cleaning device or a dry wound cleaning cloth and a preferably aqueous solution of at least one storage substance. Before using the wound cleansing device, the wound cleansing layer can be soaked with the solution provided in the kit in order to obtain the cleaning layer and / or the carrier layer with the storage material.
A kit according to the invention can be made available in the form of a so-called “click pack”. In this click pack, the wound cleansing device and the storage solution are stored in separate chambers. By kinking the pack at a predetermined point, a connection can be established between the chambers and thus the wound cleansing device can be soaked with the storage material solution.
In the context of the invention, the use of hydroxyethyl cellulose as a storage material has proven to be particularly expedient. This material is heat-resistant and is also suitable for sterilization processes using high temperatures. When using this storage material, it has proven to be particularly preferred if the wound cleansing device is moistened with an aqueous solution of the storage material and then in a liquid solution impermeable casing is stored. Hydroxyethyl cellulose can be used as an aqueous solution in a concentration range of usually between 0.1 and 1.5%. In the context of the invention, it is particularly expedient if hydroxyethyl cellulose solutions with concentrations between 0.1 and 5% are used to soak the wound cleansing device. At higher concentrations, the solution becomes very viscous and can hardly be applied.
In the context of this invention, independent protection is claimed for the use of a storage material for a wound cleansing device for wound bed preparation in which substances formed by the body itself, such as excess fluids, fibrin coatings, dead epidermis tissue, such as excess corneal material or dead corn cells, and / or coverings from dead tissue (necrosis) are removed. Within the scope of the use according to the invention, use can also be made of antimicrobial, disinfecting, wound healing-promoting, film-forming, moisturizing and / or acidic properties of the storage substance in conjunction with the wound cleansing device.
If hydroxyethyl cellulose is used as storage material, use can be made of the property of hydroxyethyl cellulose as a muco-adhesive polymer. This means that it binds to mucous membranes very well. Since the surface properties of wounds and mucous membranes can be regarded as similar, hydroxyethyl cellulose adheres just as well to wound surfaces. This promotes a corresponding film formation. The hydroxyethyl cellulose film prevents the wound from drying out or allows moisture to be retained in the wound, thus promoting wound healing.

Als weitere Tenside können im Rahmen der Erfindung eingesetzt werden Tween-20/Tween-180, Esterquats, Natriumdodecylsulfat, Betain, EHG (Ethylhexylglycerin), Carboxymethylcellulose, Methylcellulose, Ethylcellulose, Hydroxypropylcellulose.Poloxamers are block copolymers of ethylene oxide and propylene oxide. It is a cleaning and antimicrobial surfactant. Poloxamer 188 achieves increased heat stability (up to 260 ° C) through propoxylation, which makes the substance compatible with conventional heat sterilization processes. In addition to the increased heat stability, preservation, antioxidation, anti-foaming and a virus-killing effect can be achieved through propoxylation. Butylhydroxytoluene can optionally be added. Tween-20 / Tween-180, esterquats, sodium dodecyl sulfate, betaine, EHG (ethylhexylglycerol), carboxymethyl cellulose, methyl cellulose, ethyl cellulose, hydroxypropyl cellulose can be used as further surfactants in the context of the invention.
MilcheiweißMilk protein
Zur Herstellung erfindungsgemäßer Wundreinigungseinrichtungen kann beispielsweise Milchpulver (Fett/Eiweiß/Milchzucker) eingesetzt werden. Dabei wirkt Milcheiweiß ebenfalls als Emulgator. For example, milk powder (fat / protein / milk sugar) can be used to produce wound cleansing devices according to the invention. Milk protein also acts as an emulsifier.
Acids have antimicrobial properties. The natural pH value of the skin is slightly acidic with a value of 4.5 to 5.5. The reason for this is that a slightly acidic environment helps the skin kill external pathogens. In the context of the invention, acetic acid, lactic acid, citric acid; Formic acid and the like can be used.
Hyaluronic acid is moisturizing; water bridges are formed (water-binding) and the O-tension is reduced. Since hyaluronic acid is important for the exchange of information between cells, it also has a positive effect in the process of wound healing. It ensures that wounds heal without leaving any visible scars. This can be used in the context of this invention.
ZuckeralkoholeSugar alcohols
Sugar alcohols such as glycerol have water-binding properties. The water-binding effect can be used for the increased absorption of wound exudate. An antimicrobial effect has also been observed with the use of sugar alcohols, especially glycerol.
GlycoleGlycols
In the context of the invention, glycols such as PEG 500/3000 (polyethylene glycol) can also be used for the production of wound cleansing devices according to the invention. PEG is water-soluble and highly hygroscopic. The ability to absorb water can be used within the scope of the invention to increase the capacity of wound cleansing devices. In addition, PEG also shows an antimicrobial effect.
SalzeSalts
In the context of the invention, use can be made of the odor-binding, disinfecting and water-binding properties of salts. In the context of the invention, salts serve as storage materials. They can also have antimicrobial properties and neutralize the smell of wounds. In the context of the invention, a Ringer's solution consisting of sodium chloride, potassium chloride and calcium chloride, lactates (salts and esters of lactic acid), sodium acetate (sodium salt of acetic acid), common salt and salts of acids can be used as storage material. It is also possible to use mixtures of the storage substances explained above.
