Description
The present invention relates to a method ex vivo for classifying the risk of a living being with a persistent infection of the cervix uteri with human papilloma viruses (HPV infection) and inconspicuous cytological findings for the development of a cervical intraepithelial neoplasia (CIN). It also relates to the use of at least two biomarkers to classify the risk of a living being with a persistent HPV infection of the cervix uteri and normal cytological findings for the development of a CIN. Finally, the invention relates to a device for classifying the risk of a living being with a persistent HPV infection of the cervix uteri and inconspicuous cytological findings for the development of a CIN.
Cervical intraepithelial neoplasia (CIN), also known as cervical dysplasia, is an abnormal growth of cells on the surface of the cervix (cervix uteri) that can lead to the development of cervical cancer, also called collum cancer or cervical cancer. In particular, CIN relates to the potentially precancerous transformation of cells in the cervix.
Die CIN wird auf einer Skala von 1 bis 3 bewertet, wobei 3 die stärksten Abnormitäten aufweist.CIN most commonly occurs at the squamocolumnar junction of the cervix, a transition area between the squamous epithelium of the vagina and the columnar epithelium of the endocervix. It can also appear in the vaginal walls and vulvar epithelium. The CIN is rated on a scale of 1 to 3, with 3 having the most severe abnormalities.
The most common cause of developing CIN is infection with high-risk types of human papillomavirus (HR-HPV). However, a large number of women with such an infection will never develop CIN or cervical cancer. The infections often heal on their own. However, women with an infection that lasts for more than 1 or 2 years are at higher risk of developing CIN or cervical cancer.
With regard to the further procedure for women who are HR-HPV-positive but cytologically normal, a new test will probably be carried out after 12 months.
However, this strategy carries risks for the woman and for the cost-effectiveness of the screening. In most cases, the HR-HPV infection heals spontaneously, so there is no increased risk of developing cervical cancer. However, these women have to wait 12 months for a negative result, which represents an increased psychological burden.
In the case of an already existing persistent infection during the first HPV test, it cannot be determined how long the persistent infection has been present or whether spontaneous healing may occur. If the HR-HPV infection persists for over 24 months, there is an increased risk of developing a higher degree of CIN, which can develop into cervical cancer undetected.
The invention is therefore based on the object of providing a solution with which the disadvantages from the prior art can be avoided or at least reduced. In particular, a method is to be provided with which the risk of developing a CIN can be determined in a living being with a persistent HPV infection of the cervix uteri and normal cytological findings via a single test.
- 1) Bereitstellung einer Mikrobiom-Probe des Lebewesens;
- 2) Bestimmung der relativen Häufigkeit von zumindest zwei Biomarkern in der Probe zum Erhalt eines Häufigkeitsmusters;
- 1) providing a microbiome sample of the living being;
- 2) determining the relative frequency of at least two biomarkers in the sample to obtain a frequency pattern;
- 3) supply of the frequency pattern from
step 2 of a pattern recognition; - 4) Classification of the living being as a result of the pattern recognition in
- - a high risk group, or
- - a low risk group,
The inventors have recognized that the mucous membrane microbiome of the cervix uteri significantly influences the CIN risk both through metabolic and immunological influences and that certain factors involved are suitable predictors for predicting a CIN risk.
In a prospective study carried out by the inventors, they were able to determine that there are significant differences in the cervical microbiome of women in whom a CIN develops and those in which such a development does not take place. The inventors were able to identify biomarkers of bacterial, functional and fungal factors that enable risk stratification of patients.
According to the invention, all microbiome bacteria belonging to the genus Devosia are suitable as further biomarkers.
Microorganisms with the metabolic pathway PWY-2941, which is also referred to as the lysine biosynthesis II pathway, include a large number of species, such as, for example, members of Bacillus sp., Brevibacillus laterosporus, members of Paenibacillus sp. or Virgibacillus sp. According to the invention, all microbiome organisms are suitable as further biomarkers which have this metabolic pathway.
Thermothelomyces is a genus (genus) so far little characterized fungi that belong to the tribe of Ascomycetes. A well-known representative is the Thermothelomyces thermophila, formerly also known as Myceliophthora thermophila. According to the invention, all microbiome fungi which belong to the genus Thermothelomyces are suitable as further biomarkers.
Marssonia is a genus (genus) of mushrooms that belong to the Dermateaceae family. According to the invention, all microbiome bacteria that belong to the genus Marssonia are suitable as further biomarkers.
According to the invention, a “living being” comprises a human and animal living being, in particular a female sex, preferably a female human.
According to the invention, a “persistent HPV infection” is understood to mean one that is present in the living being to be examined for a period of at least 12, preferably at least 24 months. The terms persistent and persistent as well as persistent HPV infection and HPV persistence are used synonymously.
According to the invention, “inconspicuous cytological finding” means that the cervix uteri shows no visible signs of a pathological change, which can be determined, for example, in the course of a histological examination of the cervical mucosa.
Grades bezeichnet.According to the invention, “CIN” or cervical intraepithelial neoplasias are understood to mean those of any degree,
According to the invention, a “microbiome sample” is understood to be one of the living being to be examined that potentially contains cells of microorganisms, especially bacteria and fungi, such as a human sample of the cervical and / or vaginal mucosa from a smear.
„Zumindest zwei Biomarker“ bedeutet erfindungsgemäß 2 oder 3 oder 4 oder 5 Biomarker.According to the invention, “at least two biomarkers” means 2 or 3 or 4 or 5 biomarkers.
According to the invention, the “relative frequency” of a biomarker is understood to mean the number of the specific biomarker in the microbiome sample, recorded via its DNA sequence, in relation to all the DNA sequences recorded in the microbiome sample.
According to the invention, a “frequency pattern” is understood to mean information composed of the relative frequencies of the biomarkers.
According to the invention, “classification” means the assignment of the living being to be examined to a group with a high risk for developing a CIN (high risk group) or a group with a low risk for developing a CIN (low risk group) on the basis of the detected frequency pattern. The classification can be carried out using classification methods known to the person skilled in the art, which are also referred to as classifiers.
The object on which the invention is based is thereby completely achieved. The method according to the invention now makes it possible for the first time to stratify patients with persistent HPV infection who are cytologically normal with regard to the development of a CIN with only one test. The process can be carried out simply, quickly and inexpensively.
According to one embodiment of the invention, the pattern recognition takes place by means of a classification method, preferably by means of machine classification.
This measure has the advantage that a method is used that is special delivers reliable results. “Machine classification”, which is also referred to as automatic classification, is understood according to the invention to be a method that can be implemented as an algorithm and that carries out the risk classification of the living being on the basis of the recognized frequency pattern as a result of a machine learning process. For this purpose, the algorithm builds a statistical model that is based on training data, i.e. a large number of frequency patterns of the high-risk type and a large number of frequency patterns of the low-risk type. In this way, previously unknown frequency patterns can also be recognized, assessed and assigned to one of the two groups.
According to one embodiment of the invention, the machine classification is carried out by means of a k-nearest neighbor algorithm (KNN) and / or by means of a stochastic gradient descent algorithm (SGD).
This measure has the advantage that pattern recognition or classification methods are used which, according to the inventors' findings, deliver particularly good and reliable results. ANN is a method for pattern recognition and classification based on a nonparametric method for estimating probability density functions. The class is assigned taking into account the 'k' nearest neighbors. The SGD is an iterative method for optimizing an objective function with suitable smoothness properties.
According to one embodiment of the invention, the HPV infection is one with a high-risk strain (HR-HPV infection).
With this measure, the method according to the invention is advantageously adapted to the needs currently present in everyday clinical practice. The most common cause of the development of a cervical cancer is an infection with high-risk types of the human papillomavirus (HR-HPV). Therefore, especially in the case of a persistent infection of a patient with HR-HPV and inconspicuous histological findings, there is a need for a reliable prognosis as to whether the development of a CIN is likely or rather unlikely. The invention provides a remedy here.
According to a further embodiment of the invention, the microbiome sample is a cervical and / or vaginal swab.
With this measure, a suitable biological sample is provided which can be obtained in a minimally invasive manner using means of clinical routine.
According to one embodiment of the invention, in
This measure has the advantage that as the biomarkers to be determined increase, the accuracy of the classification is also increased. The classification accuracy is lowest for two of the identified biomarkers and highest for six of the identified biomarkers. The person skilled in the art will adapt the number of biomarkers to the accuracy required for the respective question. If, for example, only a rough assessment of the risk is to be determined, it is sufficient if the relative frequencies of a few, for example two or three, biomarkers are determined. If, on the other hand, the classification is to form the basis for therapeutic measures and, as a result, a high level of accuracy is required, the relative frequencies of many, for example four or five, biomarkers are determined.
Another object of the present invention relates to the use of at least two biomarkers to classify the risk of a living being with a persistent HPV infection of the cervix uteri and normal histological findings for the development of a CIN, the biomarkers being selected from Leisingera (Genus), Devosia (Genus), microorganisms with the metabolic pathway PWY-2941, Thermothelomyces (Genus), and Marssonia (Genus).
It is preferred if a frequency pattern formed from the relative frequencies of the at least two biomarkers in a microbiome sample of the living being is used for classification.
The properties, advantages, features and configurations of the method according to the invention apply accordingly to the use according to the invention.
- - eine Eingabeeinheit zum Eingabe von Daten zur relativen Häufigkeit von zumindest zwei Biomarkern in einer Mikrobiom-Probe des Lebewesens;

- 34 were histologically normal.
- 10 were histologically abnormal (CIN2 +).
Ergebnisse der Mikrobiom-Untersuchungen zur CIN2+ Risiko-StratifizierungResults of the microbiome studies for CIN2 + risk stratification
As a result, 6 different machine learning classification models were developed to differentiate the study population into both groups. This population still comprised 44 women. A test cohort of 20% was excluded from this population in order to independently evaluate the resulting machine learning classification models. This means that the final test is carried out on samples from women that have not previously been used to set up the machine learning classifier. This ensures that the data can be generalized.
TestanwendungTest application
The invention includes all classification methods created, predominantly machine classification models. However, the KNN and SGD models have particularly proven themselves in practice due to their high distinctive character. All models are available as pickle files.
Anyone who belongs to the risk group in this CL-HPV-P model is examined further by the second machine learning classification model ("CL-CIN Classifier") with regard to a possible development towards CIN or CIN2 + ("CL-CIN2 + Classifier") . Women who do not belong to the high risk group in this CL-CIN test are classified as “medium risk”.
Anyone who belongs to the high risk group is classified as “high risk”.
The invention is based on the determination of relative frequencies of specific biomarkers in a biological sample. The biomarkers are preferably determined at the DNA level using sequencing devices.
Spezies.The sequencing devices usually sequence the DNA of microorganisms in a sample according to a random sequence. Nonetheless, this means that the DNA of organisms that are frequently found in a sample is usually sequenced more frequently. Several million small sequences are usually generated per sequencing run. These are compared with a database. Then such a sequence is annotated. It is assessed where their origin lies. This is usually a gene (annotation of the gene function or metabolic pathway) and belonging to a microbial species (taxonomy). Now the total number of sequences is counted for each unit, e.g. species.
Example: In sample 1 there are 20 sequences from species A and 10 sequences from species B. In
In beiden Mikrobiomen kommt Spezies A gleich häufig vor.In the above example it is assumed that exactly 100 sequences were produced in sample 1 and exactly 50 in
A database and a software program are preferably used to determine the relative frequency, which realizes a comparison with the database and an annotation including a count of the sequences. According to the invention, the following databases have proven themselves: no, RefSeq. The following software is also preferably used: Kaiju.
