Abstract
Gegenstand der vorliegenden Erfindung ist ein Beatmungssystem mit einem elektrochemischen Filter zum Abreichern von Alkylphenolen, insbesondere von 2,6-Diisopropylphenol, in Atemgas und die Verwendung des Filters zum Entfernen von Alkylphenolen, insbesondere von 2,6-Diisopropylphenol, aus Atemgas.
Description
- Die vorliegende Erfindung betrifft ein Beatmungssystem mit einem elektrochemischen Filter zum Abreichern von Alkylphenolen, insbesondere von 2,6-Diisopropylphenol, in Atemgas und die Verwendung des Filters zum Entfernen von Alkylphenolen, insbesondere von 2,6-Diisopropylphenol, aus Atemgas.
- Stand der Technik
- 2,6-Diisopropylphenol (auch Propofol genannt) ist ein intravenös verabreichtes Hypnotikum, das häufig zur Sedierung bzw. Bewusstseinsausschaltung während Untersuchungen und operativen Eingriffen verabreicht wird.
- Weiterhin ist bekannt, dass ein Patient während der Narkose mit einem Beatmungssystem versehen wird, das die Atmung des Patienten während der Narkose überwacht, etwa ein Rückatemsystem, wo das ausgeatmete Atemgas des Patienten nach einem Aufarbeitungsschritt, der in der Regel das Abreichern von Kohlendioxid beinhaltet, wieder zum Patienten zurückgeführt wird.
- Aufgabe
- Um die Wirksamkeit der Narkose zu überprüfen, kann es wünschenswert sein, die Propofol-Konzentration im Atemgas zu detektieren. Allerdings darf hierfür im Beatmungssystem keine Akkumulation des Hypnotikums erfolgen, das die Propofol-Konzentration verfälscht und zu vermeintlich höheren Werten führt, als real gegeben sind. Das oben geschilderte Problem findet bislang keine Erwähnung in der Literatur. Das liegt vor allem an der Tatsache, dass es keine einfache, markteingeführte Methode gibt, Propofol im Atemgas eines Patienten während einer Operation oder medizinischen Behandlung zu detektieren, insbesondere wenn Atemgeräte eingesetzt werden. Von dem Unternehmen B. BRAUN Melsungen wird allerdings derzeit eine Messapparatur für GC-IMS Messungen in den Markt eingeführt, die zum Einsatz in OP-Systemen vorgesehen ist. Das Gerät wird unter dem Namen EDMON angeboten und kann nach Angaben des Herstellers die Propofolkonzentration in der Ausatemluft von Patienten, die intravenös mittels Propofol sediert oder narkotisiert wurden, bestimmen. Es wird ersichtlich, ob die Konzentration in der Ausatemluft steigt, fällt oder gleichbleibt.
- Zusammenfassung der Erfindung
- Die Erfindung ist durch die unabhängigen Patentansprüche beschrieben. Vorteilhafte Ausführungsformen sind Gegenstand der Unteransprüche oder nachfolgend beschrieben.
- Gegenstand der Erfindung ist ein Beatmungssystem aufweisend eine elektrochemische Zelle zur Oxidation von Alkylphenolen, insbesondere ein Rückatemsystem, wo das ausgeatmete Atemgas nach einem Aufarbeitungsschritt, der das Abreichern von Kohlendioxid an einem Absorber umfasst, wieder zum Patienten zurückgeführt wird. Der Absorber umfasst z.B. eine Zusammensetzung enthaltend Calciumhydroxid wie z.B. einen Atemkalk.
- Detaillierte Beschreibung der Erfindung
- Es wurde gefunden, dass bei einer Operation oder medizinischen Untersuchung, die den intravenösen Einsatz von Propofol umfasst, Propofol im Atemgas im ppb Bereich detektiert werden kann und diese Eigenschaft dazu genutzt werden kann, die Propofol-Konzentration zu überwachen, insbesondere deren Abfallen über die Zeit. Diese Information kann dazu eingesetzt werden, das Aufwachverhalten des Patienten zu steuern bzw. zu überwachen oder andererseits eine stets ausreichende Sedierung bzw. Narkose zu gewährleisten.
- Es wurde allerdings beobachtet, dass, wenn bei der Operation oder medizinischen Untersuchung ein Rückatemsystem eingesetzt wird, das mit einem Messaufbau zur Überwachung der Propofol-Konzentration versehen ist, bei einer kontinuierlichen Absaugung des Atemgases, sowohl eingeatmetes als auch ausgeatmetes Gas durch den Messaufbau gesaugt wird. Obwohl Propofol nur in wenigen ppb im Atemgas vorkommt, wurde bei längerem Betrieb des Rückatemsystems eine Anreicherung von Propofol im Beatmungssystem beobachtet. Das führt dazu, dass das Messsignal nicht nur - wie gewünscht - von Propofol aus dem Atemgas des Patienten beeinflusst wird, sondern auch von solchem, das von der Maschine retardiert wurde und dann zurückgeatmet wird.
- Ein stabiler Messwert auf den Analyten ist unter diesen Umständen nicht zu erreichen. Versuche, das Propofol durch Physisorption auf Filtern zu binden bzw. dieses durch eine chemische Umsetzung auf Filteroberflächen zu fixieren, scheiterten. Durch die sterische Abschirmung der Isopropylgruppen ist die HydroxylGruppe nur sehr schwer irreversibel verknüpfbar. Nach einer anfänglichen Physisorption auf der Oberfläche findet mit steigender Zeit bzw. Erhöhung der Temperatur die Desorption des Propofol statt. Hinzu kommt die Anforderung, dass ein zusätzlicher Filter im Atemsystem zu keiner Erhöhung des Drucks im System führen darf, d.h. der Atemwiderstand des Filters muss möglichst klein sein. Andere Anästhetika, die häufig parallel zu Propofol verabreicht werden, dürfen von dem Filter in ihrer Konzentration weder zurückgehalten noch beeinflusst werden. Die Probennahme für die Gasmesstechnik erfolgt vorteilhafterweise auf der patientenabgewandten Seite des Patientenfilters, um eine Kontamination des Rückatemsystems mit Patientenkeimen zu vermeiden.
- Die Lösung des Problems besteht überraschenderweise darin, eine elektrochemische Zelle, die von dem Atemgas überstrichen wird, in dem Beatmungssystem vorzusehen. Beim Passieren der Elektroden der elektrochemischen Zelle wird das Propofol elektrochemisch umgesetzt und somit dem Gasstrom entnommen. Die elektrochemische Zelle wirkt wie ein Filter. Das Besondere an diesem Filter ist weiterhin, dass er elektrisch schaltbar ist, d.h. man kann den Filter bei Bedarf zuschalten (über das Anlegen einer Potentialdifferenz) und so seine Wirkung nutzen oder ihn ausgeschaltet lassen. Im zweiten Fall hat er keine Filterwirkung. Das ist zunächst eine überraschende Tatsache, da elektrochemische Elektroden, wie sie beispielsweise auch in Sensoren Verwendung finden, immer einen katalytischen Umsatz von Analyt bewirken. So ist es nicht möglich z.B. einen CO Sensor zu betreiben, ohne dass an ihm kein CO umgesetzt wird, auch wenn er „formal“ ausgeschaltet ist.
- Bei dem erfindungsgemäß eingesetzten Filter handelt es sich um eine elektrochemische Zelle, durch die der Gasweg so geleitet wird, dass das Gas über die Oberfläche einer Arbeitselektrode streicht. Die Arbeitselektrode ist z.B. über einen Leiter zweiter Ordnung mit einer Gegen- sowie einer Referenzelektrode verbunden. Die Arbeitselektrode wird von der Referenzelektrode bzw. einem mit dieser verbundenen Potentiostaten auf einem Arbeitspotential von 100-500 mV gehalten.
- Bei diesem Potential wird das Alkylphenol wie Propofol elektrochemisch oxidiert und somit dem Gasstrom entzogen. In einem Leiter zweiter Ordnung wird der elektrische Strom nicht über Elektronenbewegungen - wie in Metallen - sondern über lonenbewegungen transportiert.
- Die Elektroden können dabei jeweils eine oder mehrere Edelmetalle umfassen oder auch aus Kohlenstoff in unterschiedlichen Modifikationen bestehen. Neben Platin ist vor allem Kohlenstoff geeignet, insbesondere „glassy carbon“, DLC (diamond like carbon), MW-CNT, Aktivkohle, Graphen oder reduziertes Graphenoxid; bevorzugt wird Graphen.
- Als Elektrolyt dient eine Salzlösung, wie eine wässrige Schwefelsäure. Der Elektrolyt selbst kann flüssig, gelförmig oder ein poröser mit dem Elektrolyten getränkter Feststoff sein.
- Der Sensor ist analog zu einem typischen Dreielektroden (3E)-elektrochemischen Gassensor aufgebaut. Er besteht aus einer Arbeits-, Referenz- und Gegenelektrode. Auf der Arbeitselektrode wird das Gas oxidativ umgesetzt. An der Gegenelektrode läuft eine Reduktion ab - in diesem Fall von Sauerstoff. Die Referenzelektrode hält die Referenzspannung auf der Arbeitselektrode aufrecht, ist selbst aber nicht stromdurchflossen. Die Arbeitselektrode besteht aus katalytisch wirksamen Materialien wie sie oben näher beschrieben sind, ggf. aufgepresst auf eine offenporige hydrophobe Membran. Diese Membran bildet dann mit dem Elektrolyten eine gasberührte große Oberfläche. Auf dem hydrophilen Katalysator bildet sich ein dünner, gasdurchlässiger Elektrolytfilm, durch den Gas zum Elektrodenmaterial diffundieren kann. Das Material der Gegenelektrode ist nicht so wichtig. Die Arbeitselektrode wird durch Anlegen einer äußeren Spannung (siehe oben 100-500 mV) gegenüber der Gegenelektrode polarisiert.

- Der Filter wurde in unterschiedlichen Ausführungen getestet. In der Hülsenbauart enthält der Filter eine große Elektrode, über die der Gasstrom geleitet wird. Um die Verweilzeit des Propofols im Filter auch bei hohen Luftdurchsätzen zu gewährleisten, kann ein Absorbens mit einer großen inneren Oberfläche, an der sich das Propofol anlagern kann, zugefügt sein. Geeignet ist hier z.B. Aktivkohle oder Siliziumdioxid (Handelsname Sipernat® oder Sident® , jeweils der EVONIC SE), bevorzugt ist das Handelsprodukt Sipernat® 500 der EVONIC SE
- Die Filter können bei Raumtemperatur oder auch bei erhöhten Temperaturen betrieben werden. Der Umsatz auf dem Filter nimmt bei Temperaturerhöhung leicht zu und auch die Geschwindigkeit, mit der das Propofol abgebaut wird.
- Die Erfindung ist durch die Figuren näher erläutert, ohne auf diese beschränkt zu sein. Es zeigen:

2 : eine Ausgestaltung des Filters in Form einer atemluftdurchströmten Filterhülse, und -
3 : ein Beatmungssystem mit dem Filter. - In
1 ist eine schematische Funktionsskizze der erfindungsgemäß eingesetzten elektrochemischen Zelle gezeigt (Filter8 ), die dem eines elektrochemischen Gassensors entspricht. Insbesondere verdeutlicht sind der Aufbau der Zelle aus Membran4 , Elektrolyt-benetzter Arbeitselektrode1 , Gegenelektrode2 , Referenzelektrode3 und potentiostatischer Schaltung7 zur Beaufschlagung der Arbeitselektrode mit einer konstanten Spannung im Bereich von 100 bis 500 mV. Propofol tritt aus der Atemluft6 auf die Arbeitselektrode1 und wird dort oxidiert. Dies verursacht einen Strom. Der Sensor ist genauso aufgebaut wie der Filter aus1 , nur dass er für kleinere Gasströme ausgelegt ist und der zur Propofol-Konzentration proportionale Strom gemessen wird. - In
2 ist eine Ausgestaltung eines Filters8 aufgezeigt. Propofolhaltiges Gas6 strömt durch den Eingang9 in die Filterhülse10 ein. Die Hülse10 ist mit einem Adsorptionsmittel gefüllt 11. Das Gas6 permeiert durch die Hülse10 und trifft auf die Arbeitselektrode1 , die zylinderförmig die Filterhülse10 umgibt. Der Leiter zweiter Ordnung12 und die Referenzelektrode3 liegen zwischen der Arbeitselektrode1 und der Gegenelektrode2 . Der Filter8 ist nach außen mit einer Folie13 abgedichtet, um das Austreten des Elektrolyten zu verhindern. Die Filterhülse10 ist am Anfang14 und am Ende15 verschlossen, so dass das Gas6 über die Elektrode1 geleitet wird. Die Elektroden sind mit einem Potentiostaten kontaktiert (nicht gezeigt), der die Vorspannung für die Arbeitselektrode1 liefert. Die Verwendung des Absorptionsmittels11 führt zu einer gewissen Filterwirkung. Allerdings gewährleistet erst die Arbeitselektrode1 die Eliminierung des Propofols und verhindert damit die Wiederfreisetzung und Anreicherung des Propofols im System. Ein Vorteil dieser Bauform ist die große innere Oberfläche des Filters8 , die einen hohen Massendurchfluss durch diesen gewährleistet, ohne den Strömungswiderstand signifikant zu erhöhen. Es können auch mehrere solcher Filter8 hintereinandergeschaltet oder parallel geschaltet werden. - Durch Messen der Umsatzströme in µA kann man bei dieser Bauart verfolgen, wie der Filter arbeitet und wie groß seine Auslastung ist. Unterschiedliche Propofol-Konzentrationen im Messgas führen zu unterschiedlichen Umsatzströmen im Filter, vor allem jedoch im ersten Teilfilter, wenn mehrere Filter hintereinander geschaltet sind. Eine Propofol-Konzentration von 40-90 ppb kann sicher zurückgehalten werden.
- Im Rückatemsystem eines Beatmungsgerätes muss nur darauf geachtet werden, dass der Propofolfilter hinter dem Patientenfilter sitzt, um selbst nicht zu verkeimen, siehe
3 unten. Auch sollte der Filter besser weit weg vom Patienten bzw. vom Y-Stück liegen, um diesen bzw. das medizinische Personal nicht zu behindern. - Bei Operationen von Patienten unter Vollnarkose müssen diese in der Regel beatmet werden, da Narkotika atemdepressiv wirken. Häufig werden hierzu Beatmungsgeräte verwendet, bei denen die Atemluft des Patienten aufgearbeitet und diesem wieder zugeführt wird.
- Bei einem solchen in
3 gezeigten Kreislauf-Beatmungsgerät wird die ausgeatmete Luft des Patienten/Atmenden20 über einen exspiratorischen Drucksensor31 in den exspiratorischen Zweig des Beatmungskreislaufs geatmet: Die Flussrichtung wird durch entsprechende Rückschlagventile30 ,22 auf der exspiratorischen und der inspiratorischen Seite vorgegeben. Das PEEK-Ventil29 hält einen definierten Gegendruck zur Lunge des Patienten20 und verhindert so, dass die Lungenbläschen während der Beatmung zusammenfallen. Über einen Bypass wird der Atemluft kontinuierlich Gas entzogen und in einer Gasmessbank32 auf ihre Bestandteile untersucht. Das sind, zum einen, die Atemgase Kohlendioxid und Sauerstoff, die direkte Aussagen über die Lebensfunktionen des Patienten geben, aber auch Anästhetika. Volatile Anästhetika und Sauerstoff können auf diese Weise kontrolliert und bedarfsgerecht in den Atemstrom über den Seiteneinlass23 zurückdosiert werden. An der Messstelle25 , die dort angeordnet ist, wo die Gasmessbank32 ist, so dass der Propofol-Sensor Bestandteil der Gasmessbank32 ist, wird das injizierte Hypnotikum Propofol aus dem Patientenatem gemessen und so der Sedierungsgrad des Patienten kontrolliert. Es sind aber auch beliebige andere Messstellen möglich. - Wenn die Steuerung
28 keinen spontanen Patientenatem detektiert, sorgt der Blower24 für die externe Beatmung. Alternativ besteht immer auch die Möglichkeit, den Patienten mit einem Handbalg27 zu beatmen. Das ausgeatmete Kohlendioxid wird dem Atemgas mit Hilfe einer Atemkalkkartusche26 entzogen. Anschließend wird die Atemluft, um Sauerstoff und ggf. Anästhesiegas über den Seiteneinlass23 ergänzt, über den inspiratorischen Drucksensor21 wieder dem Patienten zugeführt. Der erfindungsgemäße elektrochemische, schaltbare Phenolsensor ist ein Filter8 und stellt eine infinite Senke für Propofol dar und verhindert so, dass sich das Hypnotikum im Beatmungssystem anreichert und so die Messung in der Gasmessbank32 und damit auch die Regelung der Patientennarkose verfälscht. Wird kein Propofol für die Narkose verwendet, kann der Filter8 ausgeschaltet bleiben und ist somit nicht mehr aktiv. - Bezugszeichenliste

Claims (13)
- Ein Beatmungssystem, das von einem Atemgas umfassend ein Alkylphenol durchströmt ist und das eine elektrochemische Zelle mit Elektroden als Filter für das Alkylphenol aufweist, wobei an die Elektroden ein elektrisches Potential angelegt ist zur Oxidation des Alkylphenols.
- Das Beatmungssystem nach
Anspruch 1 , wobei das Beatmungssystem ein Rückatemsystem ist und das ausgeatmete Atemgas nach einem Aufarbeitungsschritt, der das Abreichern von Kohlendioxid an einem Kohlendioxid-Absorber umfasst, wieder zum Atmenden zurückgeführt wird. - Das Beatmungssystem nach
Anspruch 2 , wobei der Kohlendioxid-Absorber Calciumhydroxid umfasst. - Das Beatmungssystem nach zumindest einem der vorhergehenden Ansprüche, wobei das Alkylphenol 1 bis 3 Alkyl-Substituenten aufweist und die Alkyl-Substituenten in der Summe 1 bis 12 Kohlenstoffatome, insbesondere 3 bis 8, und insbesondere Propofol ist.
- Das Beatmungssystem nach zumindest einem der vorhergehenden Ansprüche, wobei die Konzentration an Alkylphenol im Eingangsbereich des Beatmungssystems für das Atemgas 1 bis 100 ppb beträgt.
- Das Beatmungssystem nach zumindest einem der vorhergehenden Ansprüche, wobei das Beatmungssystem weiterhin einen Sensor zur Bestimmung der Konzentration des Alkylphenols aufweist.
- Das Beatmungssystem nach zumindest einem der vorhergehenden Ansprüche, wobei die elektrochemische Zelle wie ein Filter wirkt, der elektrisch ein- und ausschaltbar ist.
- Das Beatmungssystem nach zumindest einem der vorhergehenden Ansprüche, wobei die elektrochemische Zelle eine Arbeitselektrode aufweist, und das Atemgas über die Arbeitselektrode streicht, die Arbeitselektrode, über einen Leiter zweiter Ordnung mit einer Gegen- sowie einer Referenzelektrode verbunden ist und die Arbeitselektrode von der Referenzelektrode bzw. einem mit diesem verbundenen Potentiostaten auf einem Arbeitspotential von 100 bis 500 mV gehalten wird.
- Das Beatmungssystem nach zumindest einem der vorhergehenden Ansprüche, wobei die Elektroden jeweils ein oder mehrere Edelmetalle umfassen oder aus Kohlenstoff bestehen, insbesondere Graphen.
- Das Beatmungssystem nach zumindest einem der vorhergehenden Ansprüche, wobei die Elektrode, die die Arbeitselektrode ist, auf einer offenporigen hydrophoben Membran angeordnet ist und die Membran mit dem Elektrolyten eine gasberührte Oberfläche bildet.
- Das Beatmungssystem nach zumindest einem der vorhergehenden Ansprüche, wobei der Filter ein Absorbens für das Alkylphenol aufweist und dies insbesondere Aktivkohle und/oder ein Siliziumdioxid ist.
- Verwendung einer elektrochemischen Zelle mit Elektroden als Filter für ein Alkylphenol in einem Beatmungssystem, wobei an die Elektroden ein elektrisches Potential angelegt ist zur Oxidation des Alkylphenols.
- Verwendung einer elektrochemischen Zelle nach
Anspruch 12 , wobei das Beatmungssystem und das Alkylphenol unabhängig voneinander weiter wie in denAnsprüchen 2 bis11 gekennzeichnet sind.
