Abstract
Die Erfindung betrifft ein pharmazeutisches Erzeugnis für human- oder veterinärmedizinische Anwendung zur Behandlung von Augenerkrankungen, dessen Zusammensetzung eine Substanz enthält, die im oder am Auge eine Angiogenese und/oder Lymphangiogenese hemmt oder ganz ausschaltet.The invention relates to a pharmaceutical product for human or veterinary use for the treatment of eye diseases, the composition of which contains a substance which inhibits or completely eliminates angiogenesis and / or lymphangiogenesis in or on the eye.
Description
Die vorliegende Erfindung betrifft ein pharmazeutisches Erzeugnis für human- oder veterinärmedizinische Anwendung zur Behandlung von Augenerkrankungen.The present invention relates to a pharmaceutical product for human or veterinary use for the treatment of ocular diseases.
Die Zahl der direkt oder indirekt mit der Angiogenese und/oder Lymphangiogenese verknüpften Erkrankungen ist vielfältig. Auf großes Interesse stößt dabei in den letzten Jahren dieser Zusammenhang im Bereich der Augenheilkunde [siehe z. B. (a)
Die Bereiche der Indikation für eine anti(lymph)angiogene Behandlung konzentrieren sich insbesondere auf den vorderen Augenabschnitt – Hornhaut, Bindehaut und Vorderkammer – und umfassen unter anderem visusmindernde progessive korneale Neovaskularisationen, Lipidkeratopathien aus etablierten kornealen Gefäßen, Neovaskularisation nach Keratoplastiken und Limbusstammzelltransplantationen, vaskularisierende Filterkissen mit Vernarbung nach Glaukomchirurgie, maligne Melanome der Bindehaut, aggressive, rezidivierende Pterygien, sowie ein sekundäres Winkelblockglaukom bei Rubeosis iridis [siehe die o. g. beiden Literaturstellen von
Derzeit ist für die Behandlung von neovaskulären Erkrankungen der Netzhaut die antiangiogene Therapie mit Anti-VEGF-Antikörpern die Methode der Wahl. Hierbei werden zumeist aus der Therapie der altersbedingten Makuladegeneration bekannte Antikörper wie Avastin® (Bevacizumab) oder Lucentis® (Ranibizumab) als Augentropfen oder subkonjunktivale Injektion appliziert. Nachteilig bei der Verwendung von VEGF-Antagonisten ist der lediglich vorübergehende Behandlungserfolg, der durch eine zeitlich begrenzte Blockade des VEGF-Rezeptors hervorgerufen wird, wodurch sich somit der Therapiezeitraum häufig über mehrere Monate mit wiederholter Applizierung des Antikörpers erstreckt. Desweiteren wird ausschließlich eine weitergehende Gefäßsprossung inhibiert, hingegen bereits gebildete Blut- oder Lymphgefäße nicht oder nur bedingt reduziert werden. Somit kann eine neovaskuläre Erkrankung am Auge mit dieser Methode zwar behandelt, jedoch in der Regel nicht vollständig geheilt werden.At present, antiangiogenic therapy with anti-VEGF antibodies is the method of choice for the treatment of neovascular diseases of the retina. Zur Herstellung des erfindungsgemäßen pharmazeutischen Erzeugnisses war es zunächst erforderlich, bei den zahlreichen in Betracht zu ziehenden Stoffen – darunter auch solche der Naturstoffklasse der Tripyrrol-Alkaloide – deren antiangiogene bzw. antilymphangiogene Aktivität zu untersuchen. Insbesondere wurde Undecylprodigiosin (
Durch in-vitro-Vorversuche an retinalen Epithelzellen (ARPE-19) und der uvealen Melanomzelllinie 92.1 wurde zunächst die zytotoxische Aktivität des Undecylprodigiosins ermittelt. Mit Hilfe eines Sulforhodamin B-Zellassays konnte dabei eine hohe Zytotoxizität des untersuchten Wirkstoffkandidaten gefunden werden (IC50 = 18 nM für ARPE-19 bzw. IC50 = 29 nM für 92.1). Hieraus konnte eine geeignete, sehr geringe, Dosierung des Tripyrrol-Alkaloids für in-vivo-Versuche abgeschätzt werden.
