PACS, das Picture Archiving and Communication System, ist in der Medizin ein Bildarchivierungs- und Kommunikationssystem auf der Basis digitaler Rechner und Netzwerke. Wichtig ist dabei auch, dass an Hand der Gendatei die Auswahl der diagnostischen Bildgebung wie ohne Einschränkung der Allgemeinheit PET, PET/CT, SPECT, CT, MRT, US, VIS/NIR optimiert werden kann. Die „Software” korreliert nicht nur Daten der hGD und HDG, sondern sie erlaubt zusätzlich einen Rückgriff auf schon archivierte Daten aus PACS oder anderen Patientenkarten in digitaler Form. Da die humanen Gendateien sehr persönliche und sensible Daten enthalten, kommt dem Datenschutz ein hoher Stellenwert zu, so dass die Gendaten nicht zwingend einem PACS zugeführt werden müssen. Die „Software” des Prozessors enthält eine Suchmaschine, so dass neueste Erkenntnisse und Korrelationen zwischen Geno- und Phänotypus integriert werden. Der Prozessor ist schnell und unabhängig. Durch Anpassung oder Selbstlernprozesse der „Software” an neue Erkenntnisse und aktuelle Ausgabeformate ist der Prozessor stets auf dem neuesten Stand der Technik. Der Patient besitzt einen Datenträger mit seinem individuellen Genom (GenPat). Der Datenträger wird der Leseeinheit (L) des erfindungsgemäßen Systems zugeführt, die auf dem Datenspeicher enthaltenen Informationen werden mit den Gensequenzen des Speichers (S) verglichen. Ein Patient mit Nierenbeschwerden stellt sich in einer radiologischen Praxis vor. Wegen der potentiellen Unverträglichkeit iodhaltiger Kontrastmittel bei Patienten mit Niereninsuffizienz wird von einer CT Untersuchung abgesehen und eine Kontrastmittel unterstützte MR Untersuchung angestrebt. MR mit Gd-haltigen Kontrastmittel hat sich als Methode der Wahl auch bei Patienten mit eingeschränkter renaler Clearance herausgestellt. Einige dieser renalen Dysfunktionen lassen sich auf genetische Defekte zurückführen. Die Entscheidung gegen Iod-haltige Kontrastmittel und eine CT Untersuchung ist mehr auf physikalische Effekte (Menge des Kontrastmittels im 100 ml Bereich, Viskosität und Osmolarität) als auf spezifische Wechselwirkungen zurückzuführen, deshalb soll das geringer visköse und in geringeren Mengen applizierte Gadolinium MR Kontrastmittel (im Bereich 10–20 ml) zum Einsatz kommen. Seit Wechsel des Paradigmas bei Nieren insuffizienten Patienten – weg von Iod und CT hin zu Gd-Kontrastmittel und MR – zeigen sich ernste Zwischenfälle mit nephrogener systemischer Fibrose (NSF, nephrogeni systemic fibrosis oder nephrogenic fibrosing dermopathy). Der auslösende Faktor für die Ausbildung der NSF ist nicht bekannt. Eine Umchelatierung gerade bei offenkettigen Gd-Chelaten wird in diesem Zusammenhang als eine Triggermöglichkeit diskutiert. Die NSF tritt praktisch bei den zyklischen (und damit stabileren) Gd-Komplexen nicht auf. Gegenwärtig wird ein Gendefekt für die NSF diskutiert. Man kann davon ausgehen, dass in wenigen Jahren, dieser Zusammenhang abgesichert sein wird, so dass in Zukunft das Sicherheitssystem bei potentiellem NSF Verdacht ein Warnsignal abgeben werden kann und der Patient einem anderen Diagnoseverfahren ohne den Einsatz offenkettiger Gd-Chelate unterzogen wird. Eine Patientin mit Rückenschmerzen soll einer Computertomografie unterzogen werden. Die Patientin besitzt einen Datenträger mit seinem individuellen Genom (GenPat). Das Lesegerät (L) des erfindungsgemäßen Systems vergleicht die Gensequenz des Patienten mit dem im Speicher befindlichen Gensequenzen (Seq) und findet Übereinstimmungen bei einer Mutation des BRCA-2-Gens.
