der Orthopädie
und Chirurgie am Bewegungsapparat werden Implantate oft mit Knochenzement
im Knochen fixiert. Bei diesen Knochenzementen handelt es sich in
aller Regel um Polymethylmethacrylate, die aus einem Pulver und
einer Flüssigkeit bestehen
und vor Einbringen in den Knochen angesetzt und gemischt werden
und danach mit einer exothermen chemischen Reaktion aushärten. Die
Wirkung des Knochenzementes ist dabei eine doppelte: einmal schrumpft
der Zement auf das Implantat auf, wodurch ein vollständiger Kraftschluss
erreicht wird und zum anderen dringt der Knochenzement in die von
Fett und Mark ausgewaschenen Knochenwaben des Implantatlagers ein
und führt
zu einem so genannten Interlocking. Die Aushärtung des Knochenzementes ist
eine Kettenpolymerisation, die, wenn sie gestartet ist, nahezu vollständig abläuft. Bei
Ersatz des Kniegelenkes durch einen so genannten Oberflächenersatz
beispielsweise, werden die Gelenkflächen des Femur, des Tibiakopfes
und die der Kniescheibe ersetzt. In aller Regel werden die Gelenkflächen reseziert,
die künstlichen
Komponenten angepasst und danach wird eine 4og, manchmal eine 60g
Packung eines PMMA Knochenzementes angerührt. Mit diesem Ansatz werden
in aller Regel alle drei Komponenten zementiert, was zur Folge hat, dass
in der kurzen Verarbeitungszeit vom Zement (meist 3-4 Minuten) schnell
hintereinander alle Komponenten platziert und zementiert werden
müssen. Abgesehen

Krankenhaus der Grundversorgung wird einem Patienten vorgeschlagen,
das schwerst arthrotisch veränderte
Kniegelenk durch einen künstlichen
Oberflächenersatz
auszutauschen. Nach Zustimmung durch den Patienten und einer präoperativen
Planung, bei der die Grössen
der Komponenten und die Achskorrektur in der Ebene des Kniegelenkes
festgelegt werden, wird der Patient nach der üblichen Operationsvorbereitung
in Spinalanästhesie
operiert. Hierbei waren die drei artikulierenden Gelenkkörper mit
Hilfe von Schablonen zugesägt,
indem die zerschlissenen, Knorpel tragenden Anteile der Epiphysen
entfernt werden. An die so zugesägten
Gelenkenden, werden die künstlichen

eingesetzt, die mit einem pulsierenden Strahl einer adaptierten
physiologischen Lösung,
beispielsweise Ringerlösung
oder NaCl in physiologischer Konzentration, die Knochenwaben säubert. Im
Anschluss daran werden 1-1,5cm
unter dem Osteotomieniveau medial und lateral, jeweils in der anterioren bzw.
posterioren Hälfte
selbstschneidende Drainagekanülen
1,5-2cm tief parallel zur Osteotomiefläche eingeschraubt und Vakuumleitungen
angeschlossen. Die freiliegenden Knochenoberflächen werden mit Kompressen
abgedeckt, die mit H2O2 in
einer 3-5%igen Lösung
getränkt
sind. Anschliessend wird ein Vakuum angelegt und das H2O2 über
die Kanülen unter
leichtem Sog abgesaugt. Aus der Umverpackung, in der Regel ein feuchtigkeitsdichter
Aluminiumblister, welcher zwei 30g und einen 20g PMMA Pulver enthaltende
Peel-off Beutel umschliesst, wird ein 30g Beutel entnommen und mit
dem Inhalt einer 15ml Monomer enthaltenden Amphiole in der Weise gemischt,
dass das Monomer in einem Becher vorgelegt wird und das Pulver der
Flüssigkeit
zugegeben wird. Das Ganze wird danach unter Vakuum gemischt und
in eine Kartusche intrudiert, welche nach Verschluss mit einer Kappe
einer pneumatischen Zementpresse aufgesetzt wird und eine Minute
unter Druckbeaufschlagung von 3-5 Bar vorkomprimiert wird. Danach
wird der plastische PMMA Zement auf die Knochenoberfläche aufgespritzt
und beispielsweise mit einem Spatel gleich mässig über die Oberfläche verteilt.
Im Anschluss daran wird das Vakuum angeschaltet und der Zement in
die Knochenwaben eingesaugt. Simultan mit der Saugung wird die Gelenkkomponente
der Tibia aufgesetzt und von Hand oder mit einem Stempel festgedrückt. Die
Gelenkkompente wird nicht weiter gehalten, der Vakuumsog bleibt
bis der überschüssige Zement
mit dem Spatel oder scharfen Löffel
entfernt worden ist und der Zement exotherme Reaktion erkennen lässt, d.h.
seine Temperatur deutlich und schnell ansteigt. Am Femur wird in
derselben Weise vorgegangen, indem die zweite 30g Packung zu einem
PMMA Ansatz verwandt wird und im Anschluss daran wird die 20g Peel-Off
Packung geöffnet
