- – Outside-In-Technik: Zwei Kanülen werden von außen nach innen in das Kniegelenk vorgeschoben, so dass sie die zu fixierenden Strukturen (Meniskusbasis und Meniskusfragment) durchstoßen. Dabei ist die ventrale (bauchwärts angeordnete) Kanüle so mit einem Faden versehen, dass dieser zu einer Schlinge geöffnet werden kann. Durch die weitere, dorsal positionierte Kanüle wird ein zweiter Faden vorgeschoben und mit einer Fasszange durch die Schlinge der ventralen Kanüle geführt. Anschließend kann durch Zurückziehen der Fadenschlinge das freie Ende des zweiten Fadens durch die Kapsel nach außen transportiert werden, wo eine extra-artikuläre Verknotung der Fadenenden stattfindet. Obwohl diese Technik einfach, schnell und kostengünstig durchgeführt werden kann, ist sie aufgrund der dorsal liegenden, verletzungsanfälligen neurovaskulären Strukturen in ihrer Verwendung eingeschränkt.

Das Inside-Out-Verfahren hat sich als Standardtechnik durchgesetzt; gleichwohl ist aufgrund des zum Herausführen der Fäden erforderlichen Einschnitts im Bereich des Gelenkspalts die Gefahr einer Verletzung weiterer Strukturen gegeben, beispielsweise N. peroneus, N. tibial posterior, N. saphenus, Vena saphena und poplitea sowie Art. poplitea und geniculata.
