Anwendungsgebiet der ErfindungField of application of the invention
The invention relates to a method for producing a nonabsorbable sheet material for wound treatment, consisting of several partial layers of a polyurethane-based foam in conjunction with a biological adhesive system, in particular for covering and healing external, large-scale injuries or skin loss including burns.
Charakteristik der bekannten technischen LösungenCharacteristic of the known technical solutions
It is known that large-area external injuries or skin defects after surgery or trauma represent a therapeutic problem. The skin as a boundary layer of the organism to the outside world has a decisive role in the regulation of the electrolyte and water balances, the heat exchange and as a barrier for metabolic product and protein losses.
The ideal skin replacement is therefore the autologous skin graft. However, since the autologous removal of the skin must be limited or the graft bed often requires a longer preparation and cleaning, primary full coverage usually does not succeed.
The homologous and heterologous transplants used for this are also, for generally known reasons, not a satisfactory skin replacement.
Since it comes in the healing process especially in a burn after the exudative and resorptive-toxic phase in the final stage of shrinkage and scarring of the new epithelialized skin area, it was attempted to replicate the physiological performance of the skin in addition to the known open-sterile treatment by so-called artificial skins. Particularly noteworthy are the efforts to produce a wound dressing from totally or partially resorbable materials. The synthetic replacement skin based on polymer and copolymer de alpha-amino acids and alkyl esters described in DE-PS 2802295 should be completely absorbable at least in one of the wound-contacting layer or tissue. The disadvantage that results from this, however, lies in an early loss of the mechanical and microwave protective functions, whereby the already already heavily damaged tissue surface is required to have an additional resorptive performance. The insufficient natural adhesiveness to the tissue also leads to fluid-filled cavities with increased risk of infection. The known wound dressings based on bioabsorbable substances such as collagen, cellulose, etc. (DE-PS 3050567 and DE-PS 3037513) and in connection with crosslinkable fibrinogen protein (DE-PS 3105624 and DE-PS 3214337) with their tissue-bonding effect are above all suitable for internal use. Inadequate regulatory performance and increased risk of infection almost exclude external wound management. In addition, attempts have also been made to make synthetic, non-absorbable materials with favorable physical and mechanical properties by coupling with biological substances more tissue-compatible (DE-PS 3002038). However, since the tissue adhesion also does not seem sufficient, the repealed form-fitting of the artificial skin in question their autoregulative effect and the protective function against external agents.
Ziel der ErfindungObject of the invention
—The aim of the invention is to provide a method for producing a non-resorbable sheet for wound treatment, which consists of two sub-layers and achieved in conjunction with a biological adhesive system, a high form-fit of the existing non-absorbable foam PUR-based skin with the wound surface. -
Darlegung des Wesens der ErfindungExplanation of the essence of the invention
The object of the invention is to provide a method which makes it possible to produce a non-absorbable, biocompatible flat material consisting of two partial layers, which adheres positively to a larger wound surface by means of a biological adhesive system and microcirculation of the wound surface for the purpose of wound treatment normalized.
The object is achieved according to the invention in that a non-absorbable flat material consisting of two partial layers connected to one another in an interlocking manner and suitable for external sealing and healing of wounds whose partial layers mutually have an asymmetrical porous structure to achieve an adsorbing, vapor-permeable and outwardly protective function, wherein the wound-side part-layer has large-pore openings and the outer-side partial layer has small-pored openings, the wound-side partial layer is impregnated with an aqueous solution of highly concentrated fibrinogen and factor XIII and the outside partial layer is impregnated with an aqueous solution of thrombin and calcium and other pharmaceutical additives, frozen and lyophilized.
Als nicht resorbierbare Materialien sind allefilmbildenden und verschäumbaren nichttoxischen Polymerstoffe auf synthetischer oder natürlicher Grundlage geeignet, die durch ihre Porenstruktur die Adsorptionsleistung bei vorgegebener Durchlaßrichtung !für die verschiedenen Medien aufweisen. iIt is well known that fibrinogen and other coagulation-promoting proteins may exert a beneficial effect on wound healing and re-epithelialization, thus leaving the nonabsorbable sheet as artificial skin on the defect surface until sufficient tissue regeneration or autologous skin grafting is achieved. Suitable non-absorbable materials are all film-forming and foamable non-toxic polymer substances on a synthetic or natural basis which, by their pore structure, have the adsorption capacity at a given direction of passage for the various media. i
