Technické řešení se týká soupravy podložek umožňujících provádění katetrizačních výkonů na srdečních a ostatních tepnách z levé radiální tepny.The technical solution refers to a set of pads enabling the performance of catheterization procedures on the heart and other arteries from the left radial artery.
Dosavadní stav technikyCurrent state of the art
Až doposud se katetrizaění výkony na srdečních a ostatních tepnách provádějí standardně, což znamená asi v 90 % případů, přístupem přes pravou femorální tepnu z třísla. Katetrizaění výkony přístupem z třísla jsou spojeny s nutností následné imobilizace pacienta a s nezanedbatelným rizikem následných krvácivých komplikací v místě vpichu do tepny. Statisticky k takovým následným závažným krvácívým komplikacím dochází ve 2 až 4 % případů.Until now, catheterization procedures on the heart and other arteries have been performed standardly, which means in about 90% of cases, through access through the right femoral artery from the groin. Ve zbylých případech se katetrizaění výkony na srdečních a ostatních tepnách provádějí přes radiální tepnu z pravého zápěstí. Výkony přístupem z pravého zápěstí riziko krvácivých komplikací minimalizují, jsou ale spojeny s následnou imobilizací pravé ruky pacienta. Přístup do tepen z pravé strany pacienta je používán na základě standardní konstrukce angiografických přístrojů, na kterých se výkony provádějí. Postavení lékaře provádějícího katetrizaění výkon je z pravé strany pacienta proti soustavě monitorů, které jsou zavěšeny vždy vlevo nad ležícím pacientem. Pro pacienty praváky, kterých je v populaci 90 %, je ale imobilizace jejich dominantní ruky bezprostředně po výkonu nevýhodná. Výhodné by proto bylo provádění většiny katetrizačních výkonů z levé radiální tepny.In the remaining cases, catheterization procedures on the heart and other arteries are performed through the radial artery from the right wrist. Pevná podložka 1 je z materiálu kopolyester Pet-G tloušťky 0,6 cm určeného k ohýbání a tvarování. Základna 2 má rozměr šířky 40 cm a délky 40 cm. Boční strana 3 má výšku 20 cm a délku 40 cm. Na boční straně 3 je vyznačen šikmý řez 7, kdy zešikmení horní části boční strany 3 umožňuje v některých případech lepší přístup k pacientovu levému zápěstí.
