Technické řešení se týká obuvi se speciální mezipodešví příznivě ovlivňující rozložení plantárních tlaků při chůzi a jiném pohybu v přední části obuvi. Tento typ mezipodešve má největší využití pro speciální profylaktickou obuv pro diabetiky.The technical solution concerns footwear with a special midsole favorably affecting the distribution of plantar pressures during walking and other movement in the front of the footwear. This type of midsole is most used for special prophylactic footwear for diabetics.

V současné době je profylaktická obuv pro diabetiky určena pro pacienty s diagnózou neuropatické, popřípadě angiopatické nohy. Je konstruována tak, aby byla schopna zabránit dalšímu nežádoucímu vývoji nevratných změn na nohou diabetiků, a je zpravidla vyráběna sériově. Hlavním účelem této obuvi je ochránit nohy diabetika před možnostmi poranění a vznikem defektu. Velikost plantámího tlaku hraje jednu z klíčových rolí při vzniku defektu na noze diabetika. Proto je jedním z nej důležitějších úkolů profýlaktické obuvi rozkládat nášlapné síly na co největší plochu chodidla, a tak snižovat vysoké lokální tlaky na plantu.Currently, prophylactic footwear for diabetics is intended for patients diagnosed with neuropathic or angiopathic foot. V současné době je problematika snižování vysokých plantámích tlaků v profýlaktické obuvi pro diabetiky řešena především vkládacími stélkami. K rozkladu nášlapných sil se snaží výrobci dosáhnout různými způsoby. Používají se stélky, které mají již předem vytvarované lůžko pro nohu, viz např. řešení podle českého patentu č. 298894, které popisuje stélku obuvi, jejíž homí strana je vytvarována do reliéfu ve tvaru otisku dolní části lidského chodidla a jejíž spodní strana má tvar dolní části lidského chodidla s hladkou prstovou částí. Principielně obdobné je také řešení podle českého užitného vzoru č. 16872, u něhož je navíc ve stélce vytvořena jamka palcového kloubu. Používají se také stélky se schopností vytvarovat se podle tvaru chodidla v průběhu nošení. Další speciální typy obsahují polstrování na bázi gelem vyplněných částí - viz např. řešení podle české patentové přihlášky PV 2009-219 spočívající v tom, že našívací stélka má na homí ploše své nosné vrstvy zakotven tvarovaný dílec, který má v oblastech s největší koncentrací tlaku chodidla, zejména pak v oblasti pod prsty, oblasti pod metatarzálními kůstkami, oblasti pod střední a zadní okrajovou částí chodidla a v oblasti pod patní kostí, situovány tvarované prohlubně. Toto vybavení obuvi však neřeší, pokud dochází ke zvýšení lokálního tlaku nerovnostmi vnějšího terénu. Příkladv provedení technického řešeníTechnical solution
Speciální mezipodešev I v příkladném provedení na obr. 1 tvarově odpovídá konstrukčnímu řešení spodku obuvi. Její základní díl je vyroben ze dvou stejných výseků spodkového materiálu, lehčené pryže o tloušťce 3 mm. V jednom z výseků je v oblasti prstních kloubů vykrojen otvor pro umístění lamel 2. Oba díly jsou slepeny a do vzniklého vymezeného prostoru je vlepena soustava tří lamel 2 tak, aby osa ohybu obuvi byla rovnoběžná s delšími hranami 3 lamel 2 a aby ležela v oblasti mezi dvěma lamelami 2.
