Description
Oblast technikyTechnical field
The technical solution concerns the surgical instrumentation for the transplantation of articular cartilage of the bearing joints.
Před každou operací je přitom nutno provést nezbytné předoperaČní plánování, zahrnující diagnostiku defektu a potvrzení rozio sáhu nálezu, a stanovit vlastní operační postup. Currently, articular cartilage transplants are relatively common; however, there are no special instruments available to date to perform this surgical performance as a complex operating kit. Before each operation, it is necessary to carry out the necessary pre-operative planning, including defect diagnosis and confirmation of the extent of the finding, and to establish its own surgical procedure. This is a two-stroke operation, where the first step is to clarify the size of the cartilage defect and to collect cartilage samples for autologous chondrocyte culture in the laboratory. The second step is the actual implantation of the chondrograft.
Certain special instruments, as well as methods and materials for carrying out these transplants, are known from some patent documents, however, this is only a partial solution, especially from the point of view of these inserts. Thus, for example, a method, tool and material for chondrocyte transplantation is known from WO 00/09 179, also published as CZ PV 2001-442. As for the present tool, it is merely a tool for introducing an implanted element containing chondrocytes on a support matrix to a cartilage defect site, the instrument comprising a tubular outer sheath, a telescopic element handle, and an injection channel.
From WO 1998/008 469, also published as CZ 297 248, the object of which is an implantable member for the repair of articular cartilage defects, is also a surgical tool for graft site modeling, which allows better graft material uptake. From WO 2002/083 878, also published as CZ PV 2003-3051, the subject of which is a method of autologous transplantation, an arthroscopic insertion device with at least two outwardly deflected gripping elements is known for gripping, inserting and positioning the implant at the implantation site.
Other basically dedicated devices, respectively. similar surgical instruments are further known, for example, from EP 0 528 398, EP 0 824 893 and EP 1 698 307, however, as mentioned above, a complex surgical instrumentation for performing articular cartilage transplantation is not known.
Podstata technického řešeníThe essence of the technical solution
Tyto nevýhody dosavadního stavu techniky jsou odstraněny operačním instrumentariem pro transplantaci kloubní chrupavky nosných kloubů člověka podle předkládaného technického řešení, jehož podstata spočívá v tom, že sestává z diagnosticko-odběrové sady nástrojů a transplantační sady nástrojů, přičemž transplantační sada nástrojů je tvořena jednak soupravou velikostně a tvarově rozdílných kalibračních bloků pro přípravu spodiny defektu k vlastní transplantaci chondrograftu, jakož i pro přípravu a následnou implantaci chondrograftu a jednak soupravou nárazníků a soupravou tlaček, velikostně a tvarově odpovídajícím jednotlivým kalibračním blokům.These drawbacks of the prior art are eliminated by surgical instrumentation for the articular cartilage transplantation of the human joints according to the present invention, which consists of a diagnostic-collection toolbox and a transplantation toolbox, the transplantation toolbox being made up of a kit of sizes and different calibration blocks for the preparation of the defect base for the chondrograft's own transplantation, as well as for the preparation and subsequent implantation of the chondrograft and, on the other hand, the set of buffers and the set of push-buttons, the size and shape corresponding to the individual calibration blocks.
Furthermore, the principle of the technical solution is that each calibration block is preferably formed by a connecting shaft, one end of which is provided with a calibration punch in the form of a closed loop and the other end with a handling handle, the punching edges of the calibration punch being provided with a cutting edge and a connecting shaft with a guide holder of the respective bumper or button.
Podstata technického řešení spočívá dále i v tom, že každý nárazník a každá tlačka je rovněž s výhodou tvořena vodicím dříkem pro jejich uchycení ve vodicím držáku spojovacího dříku kalib45 račního bloku, přičemž u nárazníku je na spodním konci jeho vodícího dříku vytvořena spodníFurthermore, the essence of the technical solution is that each bumper and each boss is also preferably formed by a guide shaft for holding them in the guide holder of the connecting shaft of the calibrating block, wherein a lower bumper is formed at the lower end of its guide shaft.
- 1 CZ 23235 Ul narážecí patka a na horním konci horní narážecí patka a u tlačky je na spodním konci jejího vodícího dříku vytvořena spodní výtlačná patka a na horním konci horní tlačná patka. And the upper end of the upper bead, and the lower bead at the lower end of the guide bead and the upper bead of the upper bead at the upper end.
The transplantation set of the surgical instruments according to the technical solution enables the preparation of the defect base for the chondrograft's own transplantation as well as the subsequent preparation of the chondrograft and immediately its implantation against the existing instruments. The basic steps of the whole process are that according to the measured size and shape of the defect, the appropriate size and shape of the calibration block is selected so that its edges partially extend into the healthy part of the cartilage around the defect. Calibration Block Blade its calibration punch is pressed into the cartilage around the defect so that it does not move. To the Calibration Block Guide Bracket, respectively. a bumper is then inserted into its connecting shaft, the blade is fixed to the surface of the subchondral bone by a hammer, and then the bumper is extracted. After the extraction of the bumper, the bed is treated and the surrounding healthy cartilage is protected by a calibration block. Finally, the subchondral bone is drilled with a thin Ki-wire drill.
The same calibration block used to adjust the bottom of the defect then imprints and adjusts the final size of the chondrograft, ready to be implanted in the calibration block and held by the bumper. After the chondrograft calibration block has been inserted into the defect contour, the bumper is replaced with the chondrograft fixture in the bed, the chondrograft is held down with the pressure and the calibration block is extracted from the bed approximately 1 to 2 cm from the surrounding cartilage surface, leaving the calibration block in conjunction with a presser in place of the holder. The edges of the defect are then filled with tissue glue while removing the pin from contact with the chondrograft, whereupon the tissue glue overlaps the entire chondrograft and the bead adjusts the edges of the implanted chondrograft to lie in the niveau with the surrounding healthy cartilage.
The diagnostic and sampling set of the surgical instruments includes gauges and mirrors that are used for arthroscopic inspection of the joint with a defect in the localization. The kit further includes a biopsy probe, respectively. tools to confirm operative success in subsequent sequential look arthroscopy.
The entire set of diagnostic and collection instruments of the surgical instrumentation according to the technical solution is further formed by two condylae elevators to clarify the surgical field after penetration into the joint during the invasive surgery, as well as at least one chisel, spoon and milling cutter for bed processing. The transplantation instrumentation includes a preparatory table for fixation and preparation of chondrograft.
The surgical instrumentarium according to the invention is intended for the surgical treatment of focal defects of hyaline cartilage using an elastic solid carrier of cultured autologous chondro35 cytos (chondrograft), respectively. its solid modifications, especially in the knee and ankle joints. Generally speaking, this instrumentarium is designed for miniopen surgery (mine-invasive operations) with the prospect of arthroscopic use in the indication of focal defects of hyaline cartilage of large joints in the exercise zone in the range of 2 to 8 cm 2 , Outerbridge UL or IV defect classification. degree and patient age range 10 to 60 years.
Výhodou operačního instrumentaria podle technického řešení je snadná manipulace, přehlednost, možnost přesného změření defektu pomocí měrek, možnost volby velikosti a tvaru kalibračních bloků, příprava lože a štěpu jedním nástrojem, a tím i získání přesné velikosti chondrograftu.
