Abstract

本实用新型公开了一种双导丝远端保护器,包括自弹式骨架与带孔的滤网构成的保护器主体、保护器导丝和输送鞘管,保护器主体与导丝通过连接件相互固定连接,其特征在于,所述输送鞘管的管壁上设有两个导丝孔,第一导丝孔和第二导丝孔,所述保护器导丝穿过第二导丝孔将收起的保护器主体固定在第一导丝孔与第二导丝孔之间。本实用新型采用双导丝结构,利用输送鞘管内从第一导丝孔到远端端口的较软的空输送管,穿越病变血管;再使保护器主体穿越病变血管在介入治疗部位的远端释放,从而确保冠状动脉血管不损伤不痉挛,使冠脉介入采用远端保护装置成为可能,特别适合于心脏的介入治疗技术。

Description

双导丝远端保护器
技术领域:
本实用新型涉及一种医疗器械。具体地说是介入医疗技术中的适合于心脏冠状动脉的远端保护器。
技术背景:
现有的介入技术中,所治疗的血管病变部位存在有可能脱落的块状固体,如血栓,粥样硬化斑块等。如它们在介入术中脱落便会堵塞其下游的血管,造成下游器官的损害。如发生在颈部以上会造成脑拴塞;发生于心脏冠脉血管会使心尖部位血管堵塞造成心尖部位心埂,从而坏死停跳。若在介入治疗部位远端(即下游)放置远端保护器,便可阻止这一危险发生。保护器的作用是将血流中的块状物滤住,使其不能堵塞下游血管。介入术完成后再将保护器的滤网及滤住固体收入回收鞘共同取出体外。
现有的远端保护器的滤网固定在导丝上,用输送鞘管(外鞘管)套住。导丝,保护伞以及外鞘组合体一同送入病变远端。这种输送方式只适用于颈动脉、肾动脉等外周血管,保护器系统容易穿过这类血管的病变;而对于冠状动脉病变,此种保护器由于在导丝的前段加上保护伞,保护伞外边又套了外鞘后,使得整个保护器结构较硬,对于更细、较软而迂曲的冠状动脉血管,难以穿过,且穿过时易造成血管创伤或痉挛,因此对于心脏的冠状动脉来讲不适用。故目前远端保护器用于冠脉介入治疗的情况并不多见,因此也就增加了冠状动脉介入操作中远端血管栓塞的风险。
发明内容:
本实用新型所要解决的技术问题是设计一种专用于心脏血管以及便于穿过心脏冠状动脉血管的双导丝远端保护器,以克服现有技术存在的上述缺陷。
本实用新型解决技术问题的技术方案如下:
一种双导丝远端保护器,包括自弹式骨架与带孔的滤网构成的保护器主体、保护器导丝和输送鞘管,保护器主体与导丝通过连接件相互固定连接,所述输送鞘管的管壁上设有两个导丝孔,第一导丝孔和第二导丝孔,所述保护器导丝穿过第二导丝孔将收起的保护器主体固定在第一导丝孔与第二导丝孔之间。所述保护器主体在释放后,自弹式骨架弹开,使覆盖在骨架上的滤网展开成保护伞状。
术语:远端是指远离操作手柄部位的一端,近端是指靠近操作手柄部位的一端。
所述第一导丝孔距输送鞘管的远端端口的距离大于50mm小于输送鞘管的全长。优选的距离为100~250mm。
所述鞘管还包括固定于鞘管内的轴状导向套管,所述套管的近端端口镶嵌在第二导丝孔内,所述套管的长度小于第一导丝孔与第二导丝孔之间的间距。
所述输送鞘管为变直径鞘管,所述输送鞘管内设置导向套管一段的直径小于导向套管远端端口至鞘管远端端口所在的另一段的直径,所述套管的外壁紧靠输送鞘管的内壁,所述保护器主体固定在导向套管的远端端口外。
所述输送鞘管的远端端部设有不透射线显影标记,所述自弹式骨架上也设有不透射线显影标记。
所述自弹式骨架采用NiTi管切割而成的骨架,所述带孔滤网为聚氨酯滤网、聚四氟乙烯滤网或尼龙滤网的一种。
本实用新型是这样使用的:使用前,保护器导丝从近端的第二导丝孔穿过将保护器主体拉至两个侧孔之间。此时远端侧孔即第一侧孔的远端是一段空输送导管,这段空管子的长度100~250mm可以从冠脉口穿至心尖前的冠脉血管的各部位。使用时先用普通的导丝穿越冠脉病变区,再将装有保护器的输送器的空导管远端从导丝近端尾部穿入,从第一侧孔穿出,沿导丝穿过病变区。输送导管的远端端部有显影标记。使显影标记位于打算释放保护器滤网的位置,撤出穿入用的普通导丝。通过固定有远端保护器的保护器导丝将保护器主体推送至输送鞘管的端部待释放部位,后撤输送鞘管,保护器便脱出鞘管自动弹开,保护器主体上的显影标记系统可指示出保护器是否已释放在理想部位并已自动弹开保护伞。保护器主体弹开后将其输送鞘管撤出体外。沿固定保护器主体的保护器导丝将做介入的器械从导丝近端尾部穿入,送入病变区,实施介入治疗。介入治疗完成后将介入治疗器械沿保护器导丝撤出体外。再将保护器回收鞘沿保护器导丝送入体内,至保护器主体释放位置。将保护器连同滤住的斑块收入鞘内,随鞘同时撤出体外,即完成介入治疗的全过程。
由以上公开的技术方案可知,本实用新型与现有技术相比具有如下优点:
1.本实用新型采用双导丝结构,一个导丝用于将整个远端保护器输送到病变处,利用输送鞘管内从第一导丝孔到远端端口的较软的空输送管,穿越病变血管;再使保护器主体穿越病变血管在介入治疗部位的远端释放,从而确保冠状动脉血管不损伤不痉挛,使冠脉介入采用远端保护装置成为可能;
2.在冠脉介入操作中增加保护装置,降低了远端血管栓塞风险;
3.本结构不仅可配合支架及球囊介入术使用,而且还可配合传统溶栓、取栓治疗手术,使急性心梗治疗效果得到显著提高。
具有结构简单、使用方便、保护效果好等优点,特别适合于心脏的介入治疗技术。

具体实施方式
下面结合附图进一步说明本实用新型。
参见图1、图2和图3,本实用新型双导丝远端保护器,包括自弹式骨架101与带孔的滤网102构成的保护器主体1、保护器导丝3和输送鞘管2,保护器主体1与导丝3通过重金属管103相互固定连接,所述输送鞘管2的管壁上设有两个导丝孔,第一导丝孔201和第二导丝孔202,所述保护器导丝3穿过第二导丝孔201将收起的保护器主体1固定在输送鞘管内第一导丝孔201与第二导丝孔202之间。
作为本实用新型的进一步改进,第一导丝侧孔201距输送鞘管的远端端口A的距离为100~250mm。还包括固定于鞘管2内的轴状导向套管203,所述套管203的近端端口B镶嵌在第二导丝孔202内,所述套管203的长度小于第一导丝孔201与第二导丝孔202之间的间距。从而在撤回保护器主体过程中,对导丝起到导向的作用,从而便于保护器导丝尾端插入第二导丝孔内将保护器收起。
所述输送鞘管2为变直径鞘管,所述输送鞘管2内设置导向套管一段的直径小于导向套管远端端口至鞘管端口所在的另一段的直径,即AB段鞘管直径大于BC段鞘管直径。所述套管的外壁紧靠输送鞘管的内壁,所述收起的保护器主体1固定在导向套管的远端端口B外。采用此种变径鞘管有利于收起的保护器主体在鞘管内的定位。
实施例:
女性病人***58岁  发生急性心埂,造影检查诊断冠脉前降支被急性血栓堵塞,先用普通导丝穿过病变区,再将本实用新型保护器输送鞘管从普通导丝远端尾部穿入,从第一导丝孔穿入,送到病变部位远端,释放保护器,血管堵塞处用尿基梅作溶栓处理,急性血栓被溶为小块被保护器滤住,随同回收鞘取出体外。病人治愈。

Claims (10)

1.一种双导丝远端保护器,包括自弹式骨架与带孔的滤网构成的保护器主体、保护器导丝和输送鞘管,保护器主体与导丝通过连接件相互固定连接,其特征在于,所述输送鞘管的管壁上设有两个导丝孔,第一导丝孔和第二导丝孔,所述保护器导丝穿过第二导丝孔将收起的保护器主体固定在第一导丝孔与第二导丝孔之间。
2.根据权利要求1所述的双导丝远端保护器,其特征在于,所述第一导丝孔距输送鞘管的远端端口的距离大于50mm小于输送鞘管的全长。
3.根据权利要求1或2所述的双导丝远端保护器,其特征在于,所述鞘管还包括固定于鞘管内的轴状导向套管,所述套管的近端端口镶嵌在第二导丝孔内,所述套管的长度小于第一导丝孔与第二导丝孔之间的间距。
4.根据权利要求3所述的双导丝远端保护器,其特征在于,所述输送鞘管为变直径鞘管,所述输送鞘管内设置导向套管一段的直径小于导向套管远端端口至鞘管远端端口所在的另一段的直径,所述导向套管的外壁紧靠输送鞘管的内壁,所述保护器主体固定在导向套管的远端端口外。
