Technical Field
The utility model belongs to the technical field of medical auxiliary instruments, and particularly relates to a universal fracture reduction fixing device.
Background
The traumatic orthopedic fracture internal fixation concept is developed from the AO (Arbeit FuereOsteose; associationfor the study of internal fixation) concept to the BO (Biological Osteosynthesis, biological fixation) concept which is deep in the heart, and the CO (Chinese Osteosynthesis, chinese and Western combined osteosynthesis) concept is more proposed in recent years. The basic principle of fracture treatment is fracture reduction, fixation and rehabilitation, and modern minimally invasive orthopedics advocates that fracture patients have smaller iatrogenic injury, faster healing and fewer complications. The minimally invasive, reliable reduction and internal fixation are good starts for fracture healing, so that the early functional exercise and early rehabilitation of the patient can be realized, and the original life and working positions are restored.
At present, the fracture reduction in the operation is basically realized by manual reduction of orthopedics doctors, the fracture ends are held by means of tools such as bone holding forceps, and the assistant assists traction to correct fracture displacement, so that more manpower is required. On the other hand, the fracture reduction fixator is difficult to maintain after reduction, temporary fixation is needed to be achieved through means such as clamping of a reduction forceps, a Kirschner wire or a screw, and due to the fact that the damage to blood circulation of a fracture end caused by large wounds is large, periosteum is peeled off and damaged greatly, delayed healing and even bone nonunion are easy to occur.
Prior art chinese patent literature: CN209864019U discloses a percutaneous minimally invasive fracture restorer, which adopts a hollow arc end, the hollow arc end is connected with a rod part by adopting a connecting part, the connecting part is provided with an instrument opening corresponding to a cavity of the rod part and used for extending out of the rod part by a functional piece, the instrument opening is provided with an instrument door hollow rod part for assembling the functional piece, and the functional piece comprises one or more of a top rod, a conical rod, a drill rod, a guide wire gun and a steel wire, and each functional piece can be used independently or in combination. The device has the advantages of labor-saving and simple and convenient resetting operation, less manpower use, well meeting the requirements of fracture resetting and fixation, and the bone fracture multidirectional resetting external fixator is matched for use in the minimally invasive orthopaedics operation, so that the fracture resetting operation difficulty is greatly reduced, periosteum stripping and blood transport damage can be reduced, the resetting precision is improved, the fracture healing is facilitated, the pain of a patient is relieved, the bone fracture plate is convenient to install subsequently, and meanwhile, the percutaneous minimally invasive internal fixation technology is simpler and more convenient to apply.
When the fracture reduction device is used for reduction of fracture, the fracture reduction device has strong fastening and fixing effects on two broken ends of the fracture respectively, and if other external images and reference fixing equipment are combined, a doctor can well reduce the broken ends. However, the condition of the patient is various and complex, the position, degree and section type of the fracture are various, the function of the above-mentioned reduction gear is obvious for the conventional common intermediate fracture position, and for the unconventional fracture, the adjustment space of the reduction gear is too small, and the adaptability is insufficient; in addition, when the above-mentioned restorer uses multiple types of functional parts, the problem of coaxial structure of the restorer also causes that the expandability is poor, and the use and operation are inconvenient.

Detailed Description
[ example 1 ] infrastructure
As shown in fig. 1-3, a general fracture reduction fixation device is characterized in that:
the fixing device comprises a base, a handle, a hook piece and a rod piece; the base is in a long column shape and is provided with at least two through holes which are communicated along the axial direction; regarding the structure of the base, it is referred to as "base" in this embodiment, the basic function of the protruding base is to provide an assembling support for devices such as hooks and rods, and to serve as a base structure for holding and applying force by medical staff, and thus the base is not limited to a specific shape, and may be referred to as a base as long as the above functions can be achieved.
The handle is arranged at one end of the base and is used for being held by a user, is usually arranged on the outer wall and one end of the base or is integrally constructed with the base, is usually made of high polymer materials such as rubber, plastic and the like to cover the surface, and is textured to prevent slipping; in addition, the two ends of the handle can be provided with baffle ring structures, so that the medical staff can be prevented from slipping off the hands of the medical staff when the medical staff operate the resetting fixing device.
The rod piece is integrally straight, and comprises a plurality of hollow rod pieces and a plurality of solid rod pieces, wherein the hollow rod pieces are provided with hollow holes, and the hollow rod pieces with smaller sizes penetrate into the hollow rod pieces with larger sizes through the hollow holes; the hollow rod piece comprises an outer rod and an inner rod, and the outer diameter of the inner rod is smaller than the inner diameter of the outer rod; the outer rod can pass through a through hole on the base, the through hole is a first through hole, and the outer peripheral wall of the outer rod is provided with a clamping thread or a tooth slot; a first clamping lock is arranged at a position on the base corresponding to the handle, and is provided with a clamping groove matched with the clamping thread or the tooth slot, and the clamping groove is meshed with the clamping thread or the tooth slot and used for limiting the outer rod to move along the axial direction; the rod member refers to one or more functional members such as a top rod, a conical rod, a drill rod, a guide wire gun, a steel wire and the like, wherein the functional members are used independently or in combination, the functional members can be directly placed into a fixing device for use when the functional members are of simple rod-shaped structures, and the other driving devices are required to be matched with the fixing device for use when the functional members are structures such as the guide wire, the drill rod and the like which are required to be driven by other devices, so that the functional members can exert the functions of the functional members, specifically, the following are: the ejector rod is used for being matched with the hollow arc-shaped end to clamp and fix bones, the conical rod is used for further reinforcing and fixing the bones, the drill rod is used for punching holes in the bones and screwing screws, the guide wire is matched with the guide wire gun, and the guide wire gun is used for penetrating soft materials such as steel wires from the inside of the restorer to bone parts.
The rod member comprises a plurality of hollow rod members and a plurality of solid rod members, namely the rod member is composed of hollow or solid rod-shaped structures with various specifications and various functions, and the rod members can be nested and matched with each other. As shown in fig. 1, the rod in the first through hole is a solid top rod, the outer wall of the rod is provided with threads or tooth grooves, the rod can be meshed with the clamping grooves on the first clamping lock to limit the position of the rod in the base, and when the outer wall of the rod adopts a threaded structure, the rod can be screwed in or screwed out by matching with the clamping grooves, so that the front end of the rod and the surface of the bone can form different degrees of pressure. As shown in FIG. 2, a plurality of rod pieces are arranged in the first through hole in the drawing, the rod pieces on the outermost layer are similar to the rod pieces in FIG. 1 and also have threads or tooth grooves, the rod pieces in the middle can further provide other front end functions, and the rod pieces on the innermost layer are solid rod pieces, such as drill rods, guide wires and the like, so that the working requirements of medical staff can be flexibly realized through the combined cooperation among the rod pieces. Further, in the outer, middle and inner three-layer nested structure in fig. 2, a locking structure can be further arranged between two adjacent layers of rod pieces, so that a more flexible reset fixing effect can be realized.
One end of the hook piece is straight, the other end of the hook piece is arc-shaped, and the hook piece is hollow; the hook piece can penetrate into a through hole on the base, and the through hole is a second through hole; the end of the straight part of the hook piece is provided with a locking groove, after the straight part of the hook piece penetrates into the second through hole, a second lock is arranged at the position on the base corresponding to the locking groove, the second lock is provided with a lock tongue matched with the locking groove, and the lock tongue and the locking groove are locked to limit the hook piece to be separated from the base. The locking groove is used for limiting the hook piece in the through hole after penetrating into the second through hole, so that medical staff can control the hook piece through the handle to perform fracture reduction operation. Because the reduction of the fracture is different in the position, degree and the like of the fracture, various alignment operations such as stretching, rotating, swinging, shifting and the like can be involved in the reduction of the fracture end, so the fixing device should be firm and can not rotate the fracture end, namely the hook piece does not displace and rotate relative to the base. After the locking groove is matched with the lock tongue of the second lock, the hook piece cannot be separated from the base and cannot rotate in the base. Further, when an operator applies a large force through the rear handle, the limiting effect of the locking groove may not be maintained, particularly, the rotation of the hook member may not be prevented, so that 4-6 rib structures with convex or concave outer walls of the straight section of the hook member may be further provided, and the second through hole is internally provided with a locking ring matched with the 4-6 rib structures, that is, after the hook member penetrates into the second through hole, the 4-6 rib structures of the straight section are sleeved in the locking ring, and the locking ring and the rib structures can limit the hook member to rotate in cooperation with the limiting effect of the locking groove on the hook member along the axial direction.
The hook piece comprises an arc-shaped section and a straight section, and a certain threshold angle is formed between the arc-shaped section and the straight section. The conventional hook is in a horizontal state, the arc section and the straight section are positioned in the same plane, and the hook and the fixing device are mutually vertical relative to the fracture end during use. As described above, the fixing device in a vertical relationship is inconvenient to operate when it is used to fix other specific positions, or to stretch the fractured end by a plurality of instruments, etc., depending on the fracture condition. The arc section and the straight section have a certain angle, namely, relative to a hook piece on the same plane, the plane of the arc section and the plane of the straight section (namely, the same plane of the conventional hook piece) form a certain angle state, namely, the arc section is inclined to the left or right by a certain angle compared with the arc section of the conventional hook piece, and the specific angle can be between 10 and 45 degrees, preferably 20 to 35 degrees. Further, the arcuate and straight sections of the hook are merely descriptive of structural features, rather than two separate components, and an integral structure is preferred because the hook serves as a direct contact component for securing the fractured end. According to medical requirements, a plurality of hook pieces can be arranged, namely, a plurality of hook pieces with different arc section angles, hook pieces with arc sections inclined to the same side or opposite sides, arc lengths of a plurality of arc sections, arc sizes of a plurality of arc sections and other factors are arranged and combined to form the hook pieces with various specifications so as to meet the medical requirements.
The first lock comprises a button, a rocker and a pull bolt, the pull bolt is rod-shaped and is vertical to the rod piece, a sliding groove capable of containing the pull bolt to slide in a vertical clearance is formed in the base, the bottom end of the pull bolt is fixed with the clamping groove, and the top end of the pull bolt is hinged with one end of the rocker; 1-2 wanes are arranged, a support is arranged in the middle of each wane, one end of each wane is hinged with the pull bolt, the other end of each wane is a free end, and the free end is provided with a spring; the button is arranged on the seesaw, and the two ends of the button are provided with feeler levers which are contacted with the end parts of the free ends of the seesaw.
The second lock adopts the same structure as the first lock, and the clamping groove of the first lock is replaced by the lock tongue of the second lock.
It should be noted that, the structures of the first latch and the second latch are merely illustrative, and in the practical application process, various structures capable of realizing similar functions of the same locking function with the two latches belong to the same structure. In addition, the first lock and the second lock respectively correspond to the clamping threads (or tooth grooves) on the rod piece and the locking grooves on the hook piece; corresponding to the clamping screw thread or the tooth slot is a clamping groove, if the clamping screw thread is adopted, the clamping groove is of a screw thread structure matched with the clamping screw thread, so that the rod piece can move spirally, if the tooth slot (namely, a structure similar to a rack) is adopted, the clamping groove is of a structure similar to a ratchet or a clamping pin matched with the tooth slot, and the position of the rod piece in the first through hole is controlled through the ratchet or the clamping lock.
The outer rod and/or the inner rod is/are provided with an end handle which is cylindrical and is arranged at the outer ends of the outer rod and the inner rod far away from the base;
the middle of the end handle is provided with a perforation which is communicated with the hollow holes of the outer rod and the inner rod.
Example 2 square hook
As shown in fig. 4 and 5, in the above embodiment, the hook member is in a flat square column shape, the cross section of the hook member is in a rectangular square shape, and the hook member and the second through hole are in spaced fit. The hook member is used for directly contacting and fixing the fracture end, and the hook member in the embodiment mainly has a circular section, and is characterized in that the whole instrument is small in size and beneficial to being placed into a patient, and secondary wound surface caused during operation of the fracture end is small, but the hook member also has the defects that the contact area between the inner side of the hook member and the fracture end is small, the point contact is easy, sliding is easy to cause, and larger damage is possibly caused. Therefore, the flat square hook is designed in the embodiment, and is in surface contact with the fracture end, so that the stability is better. Further, the outer side wall (back to the fracture end) of the square hook piece can be subjected to fillet treatment, so that the damage is conveniently placed in the square hook piece, the inner side wall (towards the fracture end) is subjected to fillet treatment by injecting, and point-shaped or prismatic protrusions are arranged on the inner surface of the square hook piece to play a role in anti-skid reinforcement. In addition, the square hook and the square rod which are described below do not have the problem that the hook or the rod rotates relative to the base.
The hook piece is provided with a flexible binding band matched with the hook piece, and the binding band can be penetrated by the straight section of the hook piece, penetrated out through the arc section, wound around the bone and finally penetrated back by the rod piece in the first through hole. The conventional circular hook member has a circular guide wire (circular shape refers to a cross section) penetrating therein, and the defects are clarified as above. The flexible band in the hook in this embodiment may be in a flat band shape, or may be a conventional guide wire, and which scheme is specifically adopted may be selected according to actual medical needs. Further, in order to implement the above solution, the rod and the first through hole should also be configured to be flat and square and matched with the hook, and the specific structure is as described in the following embodiments.
Embodiment 3 hollow rod square
As shown in fig. 4 and 5, based on the above embodiment, the hollow rod in this embodiment is in a flat square column shape or square shape, specifically, the outer wall sections of the outer rod and the inner rod are both in a flat rectangular shape or square shape, the inner wall section of the outer rod is in a flat rectangular shape or square shape, and the inner wall section of the inner rod is in a round shape. Therefore, the rod piece of the outer layer can be compatible with the flat band requirement of the hook piece, other rod pieces of the round shape can be matched through the rod piece of the middle layer, and the universality of the fixing device is better.
Example 4 double layer banding
As shown in fig. 6-8, on the basis of the above embodiment, the hook member is provided with an auxiliary hook, and the auxiliary hook is a structure arranged along the inner side of the hook member and towards the rod member, and is hollow;
the auxiliary hook comprises an auxiliary arc section and an auxiliary flat section, wherein the auxiliary flat section is made of a rigid hard material, the auxiliary arc section is made of a flexible material with toughness, the auxiliary arc section is detachably attached to the inner side of the arc section, the auxiliary flat section and the flat section are fixedly connected, and the auxiliary flat section and the flat section share a second through hole.
The two ends of the auxiliary arc section are made of hard materials respectively, and the end part connected with the auxiliary flat section in the two end parts is a first arc end, and the other end part is a second arc end;
the first arc end is fixedly connected with the straight section and/or the arc section of the hook piece, and the second arc end is flush with the far end of the arc section of the hook piece and is in magnetic absorption connection with the far end of the arc section of the hook piece.
The arc section of the hook piece is provided with an adsorption groove which is arranged at the inner side of the far end of the arc section, the adsorption groove is made of magnetic materials, and the second arc end is made of metal which can be magnetically attracted by the magnetic materials of the adsorption groove; the adsorption groove is provided with two side baffles, the adsorption groove and/or the side baffles are provided with ribs, the second arc end is provided with grooves matched with the ribs, and when the second arc end is placed into the adsorption groove, the ribs are matched with the grooves.
It should be noted that: in the practical application process, the bone types of fracture, the positions of fracture and the like are various, if the hook pieces and/or the rod pieces with various specifications are designed according to the thought so as to be matched with specific fracture ends, the number of the hook pieces and the rod pieces is excessive, the practical popularization and application are not facilitated, and the manufacturing cost of the product is also high. In order to solve the technical problem, in this embodiment, a flexible auxiliary hook structure attached to the conventional rigid hook is disposed on the inner side of the conventional rigid hook, and the front end of the auxiliary hook, that is, the second arc section, is detachably connected with the front end of the hook. Further, a rigging fixing device is arranged outside the first through hole and/or the rod piece, and a rigging tightening device is arranged outside the second through hole and/or the hook piece and/or the auxiliary hook to tighten rigging in the hook piece and the auxiliary hook respectively. Fig. 6 and 8 show the initial state of the secondary hook and the state of the rigging being put in; when the two rigging are respectively tightened, the rigging in the hook member can only be tightened between the front end of the hook member and the front end of the rod member due to the rigid hook member arc section, and a larger gap is reserved between the arc section and the fracture end. As shown in fig. 7, the secondary hook is a flexible member, and when the rigging in the flexible member is tightened, the secondary arc section of the secondary hook and the rigging between the front end of the secondary hook and the front end of the rod member can be attached to the fracture end as much as possible, and the gap between the secondary hook secondary plate section and the fracture end is relatively smaller, the contact area is large, and the fixing effect on the fracture end is better.
